본 연구는 전문가치면세정술 및 구강보건교육이 구강청결도와 구강보건행태에 미치는 영향을 검증하는데 목적이 있다. 연구는 교육 군별 교육효과를 비교하기 위해 대조군 (80명-구강보건교육), 실험군 (80명-구강보건교육과 전문가치면세정술)으로 분류하여 진행하였다. 잇솔질 횟수, 잇솔질 시간, 점심식사 후 잇솔질 실천율, 구강위생용품 사용률, 구강청결도의 변화를 각 교육 군별로 비교 분석한 결과는 다음과 같다. 잇솔질 횟수의 변화는 두 교육군 모두 증가하였으나 개별전문가치면세정술과 구강보건교육을 시행한 실험군에서만 통계학적으로 유의한 행태변화를 나타냈다. 점심 식사 후 잇솔질 실천율의 변화는 두 교육 군 모두 증가하였으나 개별전문가치면세정술과 구강보건교육을 시행한 실험군의 증가가 더 높았고, 구강청결도의 점수는 대조군보다 실험군에서 더 높게 조사되었다.

The purpose of this study was to identify the effect of providing toothbrushing by professionals as part of oral hygiene education for the elderly. We randomly visited 12 centers in the metropolitan city and analyzed the data of 114 participants among 310 early registrants aged >65 years, who participated in the study at all 3 time points. The subjects were categorized into an experimental group (odd-numbered visits) and a control group (even-numbered visits). Oral hygiene practices were provided for both experimental and control groups, but professional toothbrushing was performed in a different manner in the experimental group. Differences in plaque index (PI) according to the subjects' general characteristics and oral health status were assessed using the t-test, and the effect of PI difference between the experimental and control groups was assessed by repeated measure two-way analysis of variance. A stepwise multiple regression analysis was used to analyze factors affecting the PI. At baseline, the mean overall PI was 61.82. In both experimental and control groups, the PI significantly reduced from the baseline (p<0.01). At 5 weeks, the experimental group showed a decrease of 27.16 points from the baseline, and the 10-week PI was similar to the 5-week PI. The control group showed a decrease of 14.87 points from the baseline, and the PI increased by 5.74 points at 10 weeks. PI-related factors were gender and self-xerostomia. The PI was lower in the female group and the group with self- xerostomia (p<0.01). It is important to select an appropriate method to educate elderly subjects on proper removal of dental plaque and to habituate correct behavior, which requires inclusion of a direct toothbrushing intervention by a specialist.
Objectives: The purpose of this study was to provide basic data for gingivitis management programs by dental hygienists by evaluating the effect of oral health education, oil pulling, and professional toothbrushing on gingivitis. Methods: A total of 38 subjects were divided into three groups: control group (12 subjects), experimental group 1 (13 subjects), and experimental group 2 (13 subjects). The control and experimental groups were instructed to brush using the rotating method. Distilled water was provided to the control group after training. Coconut oil was provided every morning for about 10 minutes. In the experimental group 2, a professional brushing method was used at each visit. Results: There were no significant differences in oral health among the three groups, and there was homology between patient hygiene performance (PHP) index (p=0.144) and bleeding rate (p=0.213). The PHP index showed a significant interaction between the group and measurement time. The control and experimental groups showed changes in the PHP index with time (F=3.711; p=0.013). The bleeding rate showed a significant interaction between the group and measurement period. The control and experimental groups showed changes in the bleeding rate with time (F=6.707; p<0.001). Conclusions: Oral health education, professional toothbrushing, and oil pulling specialists in oral care of gingivitis were effective in managing gingivitis. It is necessary to educate people on self-management methods for oral health promotion using gingivitis management programs by dental hygienists.
본 연구는 잇솔질 방법 중에 하나인 전문가 잇솔질법(Toothpick Method)을 이용하여 치주질환의 예방효과 및 환자들의 만족도를 파악하기 위하여 2008년 1월 9일부터 2월 28일까지 치주질환으로 치과의원에 내원한 환자 33명을 대상으로 전문가잇솔질법과 설문조사를 실시하였으며, SPSS WIN 12.0 프로그램을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 치면세균막지수와 탐침시 출혈을 비교한 결과 치면 세균막지수에서는 최종검사를 시행한 후 초기치료직후와 비교한 결과 초기정밀검진 시 보다 0.16점으로 감소하였으며, 탐침 시 출혈은 2.96점으로 감소하여 통계적으로 유의한 차이를 보였다(p < .001). 2. 전문가잇솔질에 대한 환자만족도는 5점 만점에서 4.20으로 만족도가 높은 것으로 조사되었으며, 전반적인 프로그램 만족 항목에서 4.67로 가장 높게 조사되었으며, 치석제거와 비교 시 만족정도에서는 3.97로 가장 낮게 조사되었다. 일반적인 특성에 대한 환자만족도는 모두 통계적으로 유의한 차이는 없었다(p > 0.05). 3. 전문가 잇솔질 적용 후 성별에 따른 구강 내 자각인 지도를 살펴본 결과 '잇몸이 좋아졌다'가 남성의 경우 16.4%, 여성은 25.5%로 가장 높게 조사되었고, '출혈감소'는 남성은 14.5%, 여성은 10.9%로 나타났고, '입냄새 감소'는 남성이 10.9%, 여성이 7.3% 순으로 조사되었다. 전문가 잇솔질을 이용해 환자구강건강관리를 한 결과 치면세균막지수와 출혈율이 감소한 것으로 조사되었으며, 전문가잇솔질에 대한 환자만족도는 5점 만점에서 4.20으로 만족도가 비교적 높은 것으로 조사되었다. 위의 결과로 미루어 볼 때, 치과진료실 내에 환자의 치주건강을 유지할 수 있는 구강건강관리 인력의 요구와 교육이 필요하며 구강건강증진 프로그램 개발이 절실히 필요하다고 생각한다.

Purpose: The present study aimed to evaluate the clinical benefit of additional toothbrushing accompanying non-surgical periodontal treatment on oral and general health in patients with type 2 diabetes mellitus (T2DM). Methods: We conducted a doubled-blind randomized controlled trial in 60 T2DM patients between June 2013 and June 2014. The patients were randomly assigned to the scaling and root planing (SRP) group; the scaling and root planing with additional toothbrushing (SRPAT) group, in which additional toothbrushing was performed by toothpick methods; or the control group. Microbiological and oral examinations were performed for up to 12 weeks following treatment. Non-surgical treatment was conducted in the experimental groups. The SRP group received scaling and root planing and the SRPAT group received additional toothbrushing with the Watanabe method once a week from the first visit through the fifth visit. The primary outcomes were changes in haemoglobin A1c (or glycated haemoglobin; HbA1c) levels, serum endotoxin levels, and interleukin-1 beta levels. Periodontal health status was measured by periodontal pocket depth, the calculus index, and bleeding on probing (BOP). Results: Both the SRP and SRPAT groups showed improvements in periodontal health and HbA1c, but the SRPAT group showed significantly less BOP than the SRP group. Furthermore, only the SRPAT group showed a statistically significant decrease in serum endotoxin levels. Conclusions: Non-surgical periodontal treatment was effective in improving HbA1c and serum endotoxin levels in T2DM patients. Furthermore, non-surgical treatment with additional tooth brushing had a more favourable effect on gingival bleeding management. Objectives : The purpose of this study was to investigate the possible differences in elementary schools jointed to fluoride mouth rinse plans in health centers. Methods : Students from the third to sixth grades filled out the questionnaires. Data were analyzed by the statistical package SPSS 12.0. Results : The rate of a personal toothbrush holding and toothbrushing after lunch were higher in those who rinsed their mouths with fluoride than those who did not.(p<0.05). Toothbrushing by the rolling method was more prevailing in the students who rinsed their mouths with fluoride(57.0%) and having regular dental checkup(p<0.05) than in those who did not(39.8%). Conclusions : The students who had rinsed their mouths with fluoride showed the good oral health behavior. Continuing good quality of oral health care education must be provided to the students by professional dental hygienists.
Oral health projects that cater to the disabled should be more prevailing in order to ensure the maintenance and successful promotion of the oral health of disabled people. 70 public dental clinics that conducted oral health projects geared toward the disabled were examined to get a precise grip on their oral health projects. The findings of the study were as follows: 1. 31 out of 70 public dental clinics investigated(44.3%) were equipped with two or more dental hygienists who were professional human resources in charge of the oral health projects for the disabled. As for the age and disability type of the beneficiaries of the oral health projects, adolescents(74.3%) and people with mental retardation(87.1%) benefited most from the oral health projects. Concerning the most common implementation frequency of the projects, the projects were carried out once to three times a week(62.9%). 2. The most dominant oral disease treatment provided to disabled people was amalgam treatment and resin treatment(68.6%), which were the early dental caries treatment. The most common preventive treatment that was offered to improve their oral health was oral prophylaxis(82.9%). As for reform measures for the oral health projects, education of personnels in charge of the projects and their specialization(58.6%) were most emphasized. 3. Regarding factors related to the preventive oral health projects for the disabled, the implementation of oral prophylaxis and toothbrushing education was linked to the age of the beneficiaries. More oral prophylaxis was offered to teens, and more toothbrushing education was provided to preschoolers and adolescents. The age of the beneficiaries and the number of dental hygienists responsible for the projects had something to do with the application of fluorides. 4. Concerning the relationship of the preventive oral health projects for the disabled to the number of dental hygienists, one of the personnels in charge of the projects, the application of fluorides( 54.4%) and pit & fissure sealing(56.8%) were more prevalent when there were two or more dental hygienists. There was a statistically significant disparity in that regard(p<0.05). The above-mentioned findings illustrated that in order to boost the oral health of the disabled, dental hygienists who are responsible for the oral health projects for the disabled should put ceaseless efforts into fostering their professional knowledge and ability and offering quality service to disabled patients. Every public dental clinic should be equipped with plenty of professional personnels to enlarge the scope of treatment and ensure the efficiency of treatment and the preventive projects.
Dental caries is the first disease to cause the pathological extraction of teeth in children. The complete prevention of dental caries is not achieved by toothbrushing alone. The use of fluoride and pit and fissure sealant is regarded as key tools to prevent dental caries. Fluoride-containing tablets or multi-vitamins and community water fluoridation can be used as systemic application techniques. Professional fluoride application, fluoride iontophoresis on teeth, fluoride mouth rinsing and fluoride-containing toothpaste can be used as local application techniques. Pit and fissure sealant is mainly used to prevent dental caries on occlusal surfaces of premolars and molars. Sweeteners not to occur dental caries has been developed to substitute sucrose. Dental erosion increases according to the high consumption of acidic soft drink or beverages. The appropriate use of fluoride and pit and fissure sealant are recommended to prevent dental caries effectively and the education is required to reduce the consumption of acidic soft drinks or beverages to decrease dental erosion.
Purpose: The aim of this study was to investigate the effect of tongue cleaner-equipped manual toothbrush on tooth cleanness, tongue cleanness and malodor index. Materials and methods: 504 subjects were included in this study. At 1st visit, basic information such as age, sex, smoking amount and alcohol consumption was recorded. Self assessment by individual subjects was performed regarding satisfaction to old toothbrush and toothbrushing habit. Tooth cleanness, tongue cleanness and malodor index was assessed by professional researcher. Tongue cleaner-equipped manual toothbrush was given to each subject with proper toothbrushing instruction. After 1 month passed, self assessment and researcher assessment regarding the same index were performed and analyzed statistically by chi-square test. Results: At 1st visit subjects seem to ignore tongue cleansing and showed poor tooth cleanness index, tongue cleanness index and malodor index, however the same subjects were motivated to clean their tongue and teeth and presented statistically improved distribution pattern in tooth cleanness index, tongue cleanness index and malodor index after using tongue cleaner-equipped manual toothbrush(p<0.01). Satisfaction to tongue cleaner-equipped manual toothbrush was 98%. Conclusion: Tongue cleaner-equipped manual toothbrush would be an effective tool for maintaining good oral hygiene through improving tooth and tongue cleanness and preventing malodor formation.
