COVID-19 팬데믹 동안 간호사의 불안, 회복탄력성, 자기효능감이 전문직 역량에 미치는 영향 (Effects of Anxiety, Resilience, and Self-efficacy on the Professional Competence of Nurses during the COVID-19 Pandemic)

  • 본 연구는 COVID-19 팬데믹 상황에서 COVID-19 환자가 입원한 병원에서 근무하는 간호사들의 불안, 회복력, 자기효능감 및 전문직 역량 간의 관계를 파악하고, 간호사의 전문직 역량에 미치는 요인을 파악하고자 시행되었다. 2021년 2월 9일부터 19일까지 코로나 환자가 입원한 병원에서 환자간호에 참여하는 간호사들을 대상으로 온라인 설문 조사하였으며 총 120명이 참여하였다. 자료분석은 Pearson's correlation과 stepwise multiple regression을 사용하였다. 연구결과 간호사의 전문직 역량은 자기효능감(r=.58, p<.001) 및 회복탄력성(r=.56, p<.001)과 양의 상관관계가 있었다. 전문직 역량에 미치는 요인은 자기효능감(β=.36, p=.004) 과 회복탄력성(β=28, p=.021)이었으며 이 모형의 설명력은 전문직 역량 분산의 약 35%를 설명하는 것으로 나타났다. COVID-19 팬데믹 동안 간호사의 전문직 역량을 제대로 발휘할 수 있도록 병원 기반 프로그램을 개발하고 적용하여 자기효능감과 회복탄력성을 향상시켜야 한다. 예상보다 긴 COVID-19 팬데믹 상황에서 간호사의 소진을 예방하기 위해서는 간호사의 자기효능감과 회복탄력성을 체계적으로 관리하고 모니터링할 수 있는 간호조직 차원의 노력과 정책이 필요하다.

신규간호사의 전환충격 측정도구 개발 (Development of the Transition Shock Scale for Newly Graduated Nurses)

  • Purpose: The purpose of this study was to develop a scale to assess the transition shock experienced by newly graduated nurses, and test the validity and reliability of the scale. Methods: The initial items were identified through a review of literature and in-depth interviews with nine newly graduated nurses. Content validation of the items was evaluated by five nurse professors and three nurses. Participants were 269 newly graduated nurses who worked at six acute care hospitals in Busan, Ulsan, and Yangsan, South Korea. Data were analyzed using item analysis, exploratory and confirmatory factor analysis, criterion related validity, and internal consistency. Results: The final scale consisted of 18 items and six factors (conflict between theory and practice, overwhelming workload, loss of social support, shrinking relationship with co-workers, confusion in professional nursing values, and incongruity in work and personal life), which explained 71.3% of the total variance. The six subscale model was validated by confirmatory factor analysis. Cronbach's alpha for the total items was. 89. Convergent validity was evaluated by analyzing total correlation with burnout (r=.71, p<.001) and turnover intention (t=5.84, p<.001). Conclusion: This scale can be used in the development of nursing interventions to reduce the transition shock experienced by newly graduated nurses.

병원 간호사가 경험하는 무례함과 전문직 삶의 질 관계에서 조직시민행동의 매개효과에 관한 융합연구 (Convergence Study on the Mediating Effects of Organizational Citizenship Behavior in the Relationship between Incivility and Professional Quality of Life by Hospital Nurses)

  • 본 연구의 목적은 간호사가 경험하는 무례함과 전문직 삶의 질 간의 관계에서 조직시민행동의 매개효과를 파악하는 것이었다. D시에 소재한 3개 종합병원 간호사 250명을 대상으로 2018년 6월 8일에서 6월 29일까지 설문조사를 실시하였다. 자료분석은 t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, 위계적 회귀분석, Sobel test로 하였다. 연구결과 조직시민행동은 간호사가 경험하는 무례함과 공감만족 간의 관계에서 완전 매개효과를, 간호사가 경험하는 무례함과 소진 간의 관계에서 부분 매개 효과를 나타냈다. 연구결과 무례함을 경험하는 간호사의 전문직 삶의 질을 향상시키기 위해 조직시민행동을 촉진할 수 있는 교육과 전략을 개발할 필요가 있을 것이다.

간호조직문화가 간호사의 전문직 삶의 질에 미치는 융합적 영향요인 (The Convergence Factors of Nursing Organizational Culture on the Nurses' Professional Quality of Life)

  • 병원간호사가 인식하는 간호조직문화의 유형을 파악하고, 이러한 간호조직 문화와 전문적 삶의 질과의 관계를 확인하기 위함이다. 대상자는 충북의 C시에 소재한 일개 대학병원에 근무하는 간호사 216명이다. 자료 분석은 SPSS/WIN22.0 프로그램을 이용하여 분석하였다. 전문직 삶의 질의 하위영역인 공감만족에 미치는 영향요인으로는 혁신지향문화, 결혼상태, 관계지향문화가 유의한 예측변인으로 나타났고, 이차성 외상 스트레스는 연령, 업무지향문화, 위계지향문화가 유의한 예측변인으로 나타났으며, 소진은 관계지향문화, 임상경력, 위계지향문화가 유의한 예측변인으로 나타났다. 이 같은 결과를 토대로 조직 내 혁신지향문화 및 관계지향문화 형성을 위한 간호사들의 인식 전환 및 개선을 위한 중재프로그램 개발 및 임상에서의 효과를 검정하는 연구가 필요하며, 숙련되고 임상경험이 풍부한 우수한 간호사를 확보하기 위해 조직차원에서의 근무환경 및 처우 개선이 이루어져야 한다.

임상간호사의 공감피로, 신체화 증상 및 침묵반응의 관계: 침묵반응에 대한 매개효과를 중심으로 (A Study of the Relationship between Compassion Fatigue, Somatization and Silencing Response among Hospital Nurses: Focusing on the Mediating Effects of Silencing Response)

  • Purpose: The purpose of this study was to identify Compassion Fatigue (CF), Somatization, and Silencing Response (SR) among nurses and understand intermediate effects between the variables. Methods: The sample of 240 nurses who were working three shifts in medical and surgical wards, and emergency room were recruited in three hospitals with over 700 beds. A structured questionnaire was used which included CF, Somatization and SR scales. The data were analyzed using descriptive statistics, ANOVA, Pearson's correlation coefficients and stepwise multiple regression. Results: There were statistically significant differences in CF, Somatization and SR depending on perceived personal health condition, experience of turnover, co-worker support. There were significant correlations between those study variables. The result also indicated that burnout (${\beta}$=.81, p<.001) which is a part of Secondary Traumatic Stress and Somatization (${\beta}$=.79, p<.001) have the role of partial mediator in the relationship between Secondary Traumatic Stress and Silencing response. Conclusion: The results of study show that an intermediary role by Burnout and Somatization in Silencing response of nurses is important for effective human resource management in hospital nursing staffs. Effective human resource management which includes mentoring and social support system can enhance the professional quality of life of nurses, which will eventually contribute to the quality of care by those care providers and counselors.

Supporting Resilience and the Management of Grief and Loss among Nurses: Qualitative Themes from a Continuing Education Program

  • Caring for patients with cancer is highly stimulating and rewarding, attracting health professionals to the field who enjoy the challenge of managing a complex illness. Health professionals often form close bonds with their patients as they confront ongoing disease or treatment impacts, which may be associated with multiple losses involving function and/or eventual loss of life. Ongoing exposure to patient loss, along with a challenging work setting, may pose significant stress and impact health professionals' well-being. The prevalence rates of burnout and compassion fatigue (CF) are significant, yet health professionals have little knowledge on these topics. A 6-week continuing education program consisting of weekly small-group video-conferencing sessions, case-based learning, and an online community of practice was delivered to health care providers providing oncology care. Program content included personal, organization and team-related risk and protective factors associated with CF, grief models, and strategies to mitigate against CF. Content analysis was completed as part of the program evaluation. In total, 189 participants (93% nurses) completed the program, which was associated with significant improvements in confidence and knowledge of CF and strategies to support self and team resilience. Qualitative themes and vignettes from experiences with the program are presented. Key themes included knowledge gaps, a lack of support related to CF and strategies to support resilience, organization-and team-based factors that can inhibit expression about the impacts of clinical work, the health professional as a "person" in caregiving, and the role of personal variables, self-skill practices, and recommendations for education and support for self and teams.

저손실형 몰드 단권변압기 개발 (A study on the Development of Low-loss Type Mold Autotransformers)

  • The autotransformer currently used on the electric railway system is made of class A insulation material and uses the paper insulation method. As a power converter supplying power to the trolley wire, the autotransformer is one of critical equipment in the railway system. In the autotransformer, load irregularly changes and overload often occurs. These cause overheating of the autotransformer and facilitate deterioration of the autotransformer resulting in burnout accidents due to insulation breakdown. Also, the current autotransformer has poor insolation and short-circuit strength which often badly affect the service life of the transformer, and needs to improve its quality urgently. To overcome one of existing shortcomings of the mold transformer, manufacturers use epoxy resins that have superior flame retardancy to get rid of fro and explosion possibilities during accidents. Currently, new mold transformers are used in indoor distribution facilities with fire-fighting equipments. Coils molded in epoxy resins do not have their insulation performance compromised by humidity, dust, etc enabling easy inspection and maintenance. Comparing to the oil immersed transformer, the mold transformer does not have any concern about environmental pollutions by oil leak or replacement Therefore, to reduce breakdowns and improve reliability of the autotransformer, it is necessary to develop a new mold autotransformer with low loss suitable for our environment to suppress breakdowns of the autotransformer and improve the reliability. This study is about development of a low-loss mold autotransformer necessitated by reasons mentioned earlier.

교대근무 여자간호사 이직의도 구조모형: 상급종합병원을 중심으로 (Shift Work Female Nurse Turnover Intention Structural Equation Modelling: Focused on Tertiary Hospitals)

  • 본 연구는 이직인과모델 기반 상급종합병원 교대근무 여자간호사의 이직의도 구조방정식 모형구축을 위한 연구이다. 자료수집은 2015년 11월 11일부터 12월 20일까지 진행되었으며, 총 283명의 자료가 최종분석에 포함되었다. 연구결과 최종 수정모형에 대한 적합도는 권장수준에 비교적 충족하였으며, 최종 수정모형의 19개 가설 중 12개의 가설이 지지되었다. 또한 외부취업 기회, 간호전문직관, 간호조직문화(관계지향, 혁신지향), 직무스트레스, 직무만족, 조직몰입, 소진은 상급종합병원 교대근무 간호사의 이직의도를 47.8% 설명하는 것으로 확인되었다. 세부적으로 이직의도에는 소진(+), 조직몰입(-)이 통계적으로 유의한 직접효과를 나타냈으며, 간호전문직관, 관계지향 간호조직문화와 직무스트레스는 통계적으로 유의한 간접효과를 나타냈다. 이에 본 연구결과를 기반하여 상급종합병원 교대근무 여자간호사의 이직 의도 감소를 위해 소진을 감소시키고, 조직몰입과, 직무만족을 높이는 것이 필요하다고 판단되며, 이를 위해 관계지향적 간호조직문화와 간호전문직관을 강화하는 방안 요구된다. 따라서 병원인력관리 차원에서 탄력적으로 근무시간을 조정할 수 있도록 하고, 상호 존중하는 관계중심의 조직문화를 형성하며, 간호전문직관을 강화하는 교육 및 정책적 지원, 그리고 자긍심을 가지고 일할 수 있는 병원의 제도적 장치가 이루어져야 할 것이다.

간호사의 공감피로에 관한 국내연구동향 (Research Trends on Compassion Fatigue in Korea Nurses)

  • Purpose: The purpose of the study were to analyze the research trends of compassion fatigue in Korea nurses and to suggest future research directions. Methods: For this study, RISS, KISTI, KISS, National Assembly Library databases were searched using the key words 'compassion fatigue', 'secondary traumatic stress', 'professional quality of life' that contains compassion fatigue as a sub-element, and 'nurses' for this study. A total of 124 articles published in June 2014 were searched and analyzed 19 articles for the final analysis. Results: The most frequently used study design was descriptive (N=17, 89.5%). High compassion fatigue was reported in this study. Factors affecting compassion fatigue were personal factors (e.g., age, health state, and sleep hours), work-related factors (e.g., experience with traumatized patients), and psychological factors (e.g., job stress, anxiety, excessive empathy, social support, and coping strategy). Finally, Consequence factors of compassion fatigue was reported burnout, silencing response and turnover intention. Conclusion: There was a few studies on compassion fatigue. Further research on compassion fatigue needs to use a variety of study designs instruments suitable for Korean nurses, and to develop management programs on compassion fatigue in nurses.

의과대학생의 정신건강 증진을 위한 지원의 틀과 방안 (A Comprehensive Framework and Approaches for Enhancing Mental Health in Medical Students)

  • Research suggests that medical students frequently experience mental health problems such as stress, burnout, and depression, which may, in turn, affect suicidal ideation and behaviors. Since mental health problems profoundly impact academic achievement and professionalism, it is vital to understand factors influencing students' mental health and identify strategies to provide the necessary support. Some relevant influencing factors range from the personal level, including gender, personality traits, perfectionism, and social support, to the environmental level, including the grading system, educational phases, exposure to patients' death, mistreatment, and culture of medicine. In this regard, a comprehensive mental health support system that encompasses environmental interventions, as well as personal-level support, is needed. Simultaneously, proactive approaches that address the improvement of self-care and alleviation of systemic burdens are essential, together with the predominant reactive approaches focusing on problems and deficits. Altogether, we proposed a framework for enhancing mental health constructed by four categories (personal-reactive, environmental-reactive, personal-proactive, environmental-proactive) based on the intervention level and goal of support. All four categories have important implications, and one cannot replace the other, but expanding environmental-proactive support will allow more students to learn how to pursue health independently. We expect that this comprehensive framework for enhancing mental health could expand support systems for medical students' personal and professional development.
