Objectives : Comparative review was rendered to evaluate strengths and weaknesses of two common treatment approaches of treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease. A typical symptom of headache from hypertension was chosen for substantial approach of this review. Methods : Pros and cons of two different approaches to disease were evaluated based on literatures and texts focusing treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease. Headache can be correlated with hypertension yet it can be induced by multiple other factors, and headache may/may not accompany hypertension. Hypertension is an example of treatment based on syndrome differentiation from disease and headache can be an example of treatment based on syndrome differentiation. Results : Treatment based on syndrome differentiation can give more autonomy and flexibility in approach to the disease, Intuition, experience, and traditional medical theory can be applied with ease. However, management and eradication of diseases are difficult and standardization of treatment is not easy among practitioners. Treatment based on syndrome differentiation from disease incorporated merits of both eastern and western medicines, achieving more evidence based diagnosis and treatment. Eradication and standardization of disorders are possible with the latter approach. Conclusions : Comparing two systems of treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease with emphasis on hypertension and headache yielded medical values. Treatment based on syndrome differentiation from disease appears to be superior in medical values and effectiveness, but further evaluation and interest are needed to make advancement in Korean traditional medicine.
Objective : In Oriental medicine, the concept of Preventive Treatment Disease(治未病, PTD) is intended to actively deal with changes in modern disease patterns due to increase in chronic diseases. In China, preventive health service program based on PTD Theory are being carried out aimed at improving and/or preventing people's health. Method : For the introduction of PTD program based preventive service in Korea for the promotion of people's health, I would like to consider its possibility by reviewing the PTD program being conducted in China. Results : China's preventive health service programs based on PTD Theory started in 2008 for the purpose of promoting people's health and reducing medical expenses by providing medical services tailored to individuals. Regarding the effects of PTD program based preventive health service, improvement of discomforting symptoms comprised 73.04% of responses. As to service items, health guidance, Traditional Chinese Medicinal diagnosis, and preparation of health records were answered as being important. The importance of food and internal medicines for preventing and/or curing PTD Theory are also recognized. Also, as to satisfaction level, 90.64% responded as being satisfied in the order of the level of service providers, their attitudes, service processes, items, costs, and environments. Conclusion : As shown above, according to assessments on China's PTD programs, the PTD Theory is being applied to actual public health programs with highly effective results. Hence, it can be seen that such attempt could also be implemented in Korea as part of a scheme for promoting health by means of Korean medicine health promotion programs. However, further researches into concrete implementation schemes will have to be developed in the future as medical systems are different in Korea from in China.
Objectives : The objective of this study is to examine the findings of a survey assessing the treatment and management status of individuals with diabetes and pre-diabetes, along with their inclination to participate in the Korean Medicine Chronic Disease Management Program. Methods : The survey focused on individuals with diabetes and pre-diabetes, gathering information on respondents' demographic details, diabetes diagnosis, duration of diabetes, attitudes and experiences regarding Korean medicine diabetes treatment, and their willingness to participate in the 'Korean Medicine Chronic Disease Management Program'. Results : 33.1% of participants reported prior experience with Korean medicine treatment for diabetes. The intention to actively utilize Korean medicine treatment was positively observed, with a response rate of 61.1% expressing a favorable inclination and 8.9% indicating a negative stance. Furthermore, 75.3% of patients expressed their intention to participate in the 'Korean Medicine Chronic Disease Management Program' in the future. Among those expressing this intention, 81.4% stated a preference for the 'Korean-Western medicine combined management' in the chronic disease management Program. Conclusions : Patients' willingness to participate in 'Korean Medicine Diabetes Management Program' is high, but lack of information about Korean medicine is a factor that deprives people of the opportunity to experience Korean medicine treatment, and non-medical factors such as difficulty in visiting and administrative complexity are factors that impede participation in the 'Korean Medicine Chronic Disease Management Program'. It should be considered for improvement when formulating policies.
A study on the sickness distribution and mode of treatment in rural area was conducted during the period from July '75 to Aug. '75 using 1,225 households, 7,918 population (4,017 male, 3,901female) and 343 cases th at found during the period of survey who had beenlived in Nammyon, Hwasoongun, Chonnam. The summarized results were as follows : 1. Average family number per household was 6.5 and prevalence rate was 43.3 (21.2 for male, 22.1 for female). 2. General sickness distribution by classification of disease according to W.H.O. was highest in disease of the nervous system and sense organs (21.3%), and important others were disease of the digestive system (16.9%) and disease of the respiratory system(14.8%). In male, distribution was in order of downward disease of digestive system, disease of nervous system and sense organs, disease of skin, cellular tissue, bones and organs of movement, and disease of respiratory system. In female, distribution was in order of downward disease of nervou s system and sense organs, disease of respiratory system, disease of digestive system, and disease of skin, cellular tssue, bones and organs of movement. 5. Types of treatment in both sexes were showed that home and folkmedicine (41.1%), pharmacy(24.5%), admission to hospital or clinic (16.9%), out-patient clinic (10.8%) and herbmedicine (6.7%) in downward order. Hospital and clinic utility rate was 27.5% (31.5 for male, 24.0 for female) and it was highest in 0-4 age groups and lowest in 40-49 year age groups. 4. Hospital and clinic utility rate was highest in neoplasms, and the other hands, disease of the nervous system and sense organs and disease of the digestive system were the highest groups in the all types of treatment other than hospital and clinic. 5. On the results of treatment not, exactly replied answer was the highest (41.7%) and only 16.0% said complete recovery. In completely recovered cases, hospital and clinic using group was predominant (58.2%) and in aggravated cases, home and folkmedicine using group was highest.
We aimed to review the current data composition of the Korean Tuberculosis and Post-Tuberculosis Cohort, which was constructed by linking the Korean Tuberculosis Surveillance System (KNTSS; established and operated by the Korean Disease Control and Prevention Agency since 2000) and the National Health Information Database (NHID; established by the National Health Insurance Service in 2012). The following data were linked: KNTSS data pertaining to patients diagnosed with tuberculosis between 2011 and 2018, NHID data of patients with a history of tuberculosis and related diseases between 2006 and 2018, and data (obtained from the Statistics Korea database) on causes of death. Data from 300 117 tuberculosis patients (177 206 men and 122 911 women) were linked. The rate of treatment success for new cases was highest in 2015 (86.7%), with a gradual decrease thereafter. The treatment success rate for previously treated cases showed an increasing trend until 2014 (79.0%) and decreased thereafter. In total, 53 906 deaths were confirmed among tuberculosis patients included in the cohort. The Korean Tuberculosis and Post-Tuberculosis Cohort can be used to analyze different measurement variables in an integrated manner depending on the data source. Therefore, these cohort data can be used in future epidemiological studies and research on policy-effect analysis, treatment outcome analysis, and health-related behaviors such as treatment discontinuation.
Most of disabled people face hardness in caring their general oral hygiene by themselves. So that, they are once involved in dental caries or periodontal disease, they have much bigger chance of aggressive progress. Therefore preventive dentistry is more important to disabled one than non-disabled. They need to be checked with routine periodic dental examination and by that, oral disease must be found at initial stage. We selected 37 patients from newly visited 237 patients who had dental treatment at the free dental clinic, Gangnam district, Seoul, between 2000 and 2001. This study is a comparative evaluation of first 2 years' treatment records of selected 37 patients (male 28, female 9) with that of their last 2 years. 24 of 37 have mental retardation, 9 have autistic disorder, 2 have auditory disorder, 1 has brain disorder and 1 has crippled disorder. Their dental treatment records categorized by 4 level; score 4 stands for endodontic treatment, score 3 stands for general conservative treatment, score 2 stands for preventive treatment, such as sealant and score 1 stands for routine check. In first 2 years of treatment, average score was 2.85 which score means approximately general conservative treatment. And that of last 2 years was 1.44, which means routine check. In early time of their visit, they will receive the treatment due to their chief complaint. And after that early time, they can have a routine check so that they can be treated before the disease get worse. The patient's oral health can remain decent only by a simple treatment.

Objectives: Crohn's disease is a chronic gastrointestinal disease that belongs to inflammatory bowel disease. This systematic review aims to assess the level of evidence in randomized controlled trials (RCTs) on the effects of acupuncture for Crohn's disease. Methods: We searched 12 databases from the date of the establishment of each database up to May, 2023 for relevant RCTs. The risk of bias of each study was assessed independently by three reviewers. The level of evidence of meta-analysis was assessed using GRADE (Grading of Recommendations, Assessment, Development, and Evaluation). Results: A total of 12 studies were included. The effective rate (odds ratio [OR] 3.23, 95% confidence interval [CI] 1.43, 7.30) for mild to moderate Crohn's disease patients showed a significant difference between the acupuncture with moxibustion group and the sham-acupuncture with sham-moxibustion group. CDAI change (mean difference [MD] -74.15, 95% CI -93.28, -55.01) for mild to moderate Crohn's disease showed a significant difference between the acupuncture with moxibustion group and the sham-acupuncture with sham-moxibustion group. Conclusion: Although acupuncture with moxibustion showed significant effects compared to sham-acupuncture with sham-moxibustion, the effect of acupuncture alone is inconclusive. Moreover, only the effect of acupuncture treatment on mild to moderate Crohn's disease patients was derived as a remarkable result. To confirm the effectiveness of acupuncture treatment for Crohn's disease, studies using only acupuncture for intervention or more RCTs targeting various Crohn's disease patients according to the CDAI are required.

In recent years, the numbers of the orthodontic dental patients have been rapidly increased in adolescent or young adult aged generation. It has been well known that it would be very hard to control the oral hygiene cares for orthodontic applied dental patient because of the complexity of the appliance. So the caries prevalence of the orthodontic dental patients would be higher than non-appliance persons, and it might be easy to cause the dental caries especially on the labial or buccal surface of the tooth through equipping the fixed type appliance with a long period, even though the alignment of the teeth would be arranged well. So, the massive preventive program for preventive dentistry should be needed for the dental patients for orthodontic treatment, in order to protect the dental caries and the periodontal disease for them. But, lots of the dentists or dental hygienists sometimes neglect of this point for preventive dental cares orthodontic dental patients, or do not know the importance and how to manage the skill for the preventive dental works in clinical. In this article, it will be introduced the basic theories and skills for preventive cares as tooth-brushing instruction, fluoride topical application and pit and fissure sealant, scaling and professional mechanical tooth cleansing and the diet control, for the dental patients with the fixed type of the orthodontic appliance, in case by case.
The disabled population is a vulnerable group, having very complex medical conditions, but little is known about differences in the level of access by type of disability. This study was performed to investigate the differences of health care utilization by the type of disability. The database was constructed from registry of the disabled and health insurance and medical aid claims data submitted to the Korea Health Insurance Cooperation during in the year 2003. The disability classified three groups according to the Disabled Welfare Act; physically disability with external dysfunction, physically disability with organic disease, and mentally disability. There were huge differences in health care utilization by the type of disability. For the inpatient care, those with a mental disability were more likely to utilize health care services in terms of average visit number of medical facilities and visit days per case, but the treatment amount per case was the highest in physically disabled with organic disease. For the outpatient care, those who the physically disabled with organic disease were more likely to utilize health care services in terms of average visit number of medical facilities, treatment amount per case, and the treatment days per case. Also, those who physically disabled with organic disease were more likely to utilize general hospital for both inpatient and outpatient care, and spent more out-of-pocket expenditure. As the number of persons with disabilities rises, the need to consider new approaches to protecting their health grows increasingly. Especially, Korean health care system should be refined to be more responsive to the needs of the type of disability.
This thesis shows about the meaning of treatment rate increasing, the current treated level and the reason of low treatment rate and increasing methods. 1. Treatment rate incresing means high treat level within short time, keeping treatment effect for a long time as well as raising treatment rate. 2. The current by diseases each others completed treatment rate of oriental medicine is 14.0% to 89.7%$(mean:\;{\pm}40.0%)$. Therefore the rate is show too low. 3. The reasons of low treatment rate; low academic level of oriental, academic limitation, clinic and prevention problem of oriental medicine, lack of medical approch suitable for current diseases and symptoms, mostly incurrable diseases using oriental medicine, lack of preventive education, disappropriate medical service and nonspecialty of the treatment, etc. 4. The next methods for incresing the treatment rate must be improved; such as accurate establishment of process that diagnosis symptoms and treats them, system research of microdiagnosis, positive treatment with medicine and nonmedicine method at the same time, appropriate subdivision and actualization of clinical basic research, research of dose and response, diversity of treatment methods and forms, development of treatment service and prevention based on health level, enormous change as cure medicine and opening-up of new disease field, specialization of medical examination, reinforcement of public medical part and herbal drugs use with same origin, mental and pysical stability of patients, accurate extract and oral drinking ways, etc.
