Central line-associated bloodstream infections in neonates

  • Newborn infants, including premature infants, are high-risk patients susceptible to various microorganisms. Catheter-related bloodstream infections are the most common type of nosocomial infections in this population. Regular education and training of medical staffs are most important as a preventive strategy for central line-associated bloodstream infections (CLABSIs). Bundle approaches and the use of checklists during the insertion and maintenance of central catheters are effective measures to reduce the incidence of CLABSIs. Chlorhexidine, commonly used as a skin disinfectant before catheter insertion and dressing replacement, is not approved for infants <2 months of age, but is usually used in many neonatal intensive care units due to the lack of alternatives. Chlorhexidine-impregnated dressing and bathing, recommended for adults, cannot be applied to newborns. Appropriate replacement intervals for dressing and care sets are similar to those recommended for adults. Umbilical catheters should not be used longer than 5 days for the umbilical arterial catheter and 14 days for the umbilical venous catheter. It is most important to regularly educate, train and give feedback to the medical staffs about the various preventive measures required at each stage from before insertion to removal of the catheter. Continuous efforts are needed to develop effective and safe infection control strategies for neonates and young infants.

주기적 예방보전의 최적정책에 관한 연구

  • This paper introduces models for preventive maintenance policies and considers periodic preventive maintenance policy with minimal repair when the failure of system occurs. It is assumed that minimal repairs do not change the failure rate of the system. The failure rate under prevention maintenance received an effect by a previously prevention maintenance and the slope of failure rate increases the model where it considered. Also the start point of failure rate under prevention maintenance considers the degradation of system and that it increases quotient, it assumed. Per unit time it bought an expectation cost from under this prevention maintenance policy. We obtain the optimal period time and the number for the periodic preventive maintenance by using Nakagawa's Algorithm, which minimizes the expected cost rate per unit time. Finally, it suppose that the failure time of a system has a Weibull distribution as an example and we obtain an expected cost rate per unit time the optimal period time and the number when cost of replacement and cost of minimal repair change.

단거리전용통신방식 노변기지국의 예비부품수 및 교체시기 산정 (The Estimation of the Number of Spare Parts and the Changing Time about DSRC Road Side Equipment)

  • 현재 국내 ITS는 현장장비 유지관리에 대한 연구 및 고장관련 DB가 부족하여 예비부품수 및 교체시기 산정에 대한 규정이 없는 실정이다. 이에 본 연구는 실제 고장이력자료를 갖고 신뢰성 분석을 실시하여 단거리전용통신(DSRC)방식 노변기지국(RSE)의 예비부품수 및 교체시기를 산정하였다. 전체 수집기간동안의 고장자료는 욕조곡선의 형상을 나타내어 우발고장기간의 자료로 신뢰성 분석을 실시하였으나 고장 수명 분포 중 적합되는 분포가 없었으며, 이러한 사실은 본 연구대상 노변기지국(RSE)의 경우 통계적 모형에 의한 예방 정비가 어려움을 의미한다. 따라서, 본 연구는 실증적인 방법으로 하자보수기간 $1{\sim}2$년 동안의 수리비용 및 고장빈도 이력자료를 활용하여 노변기지국(RSE)의 교체여부를 결정하는 방법 및 필요 예비부품수를 산정하는 방법을 제안하였다. 향후에는 노변기지국(RSE) 외에 다른 ITS 장비에 대한 신뢰성 분석이 필요하다.

Soybean Curd Residue (Biji) as a Dietary Fiber Source in Cake

  • The content of total dietary fiber in biji dried by hot air was 65.40%, and the percent of insoluble fiber and soluble fiber in total dietary fiber was 63.60% and 1.80%, respectively. In testing the feasibility of biji as a fiber source in cake, cakes were prepared with flour substituted with biji powder at the level of 0%, 5%, 10%, 15% and 20%. The Hunter value showed that the crumb colors of the cakes with 10%, 15% and 20% biji powder were significantly different from the control. The specific volume of the cake with 20% biji powder was significantly lower tan the other cakes. Cakes with 5%, 10% and 15% biji powder were les hard than the control after 10 days storage at 2$0^{\circ}C$. Sensory evaluations showed that their characteristics - crumb color, crumb texture, moistness, softness and overall preference - were significantly influenced by the replacement of flour with 10%, 15% and 20% biji powder. However, there was no difference in softness and overall preference of the cake prepared with 5% replacement compared to the control.

Association Between Dental Implants and Cognitive Function in Community-dwelling Older Adults in Korea

  • Objectives: This study aimed to evaluate the association between dental implants and cognitive function in community-dwelling older adults. Methods: Data were collected from the baseline survey (2016-2017) of the Korean Frailty and Aging Cohort Study. The study sample comprised 1115 community-dwelling people aged 70 years to 84 years who had 0-19 natural teeth. Dental implants and natural teeth were identified by panoramic radiography, while the cognitive function was assessed by the Korean version of the Mini-Mental State Examination (MMSE-KC). The association between dental implants and cognitive function was analyzed by multiple linear regression. Sensitivity analysis was performed to test for potential bias. Results: The mean number of natural teeth in the study population was 9.50 (standard deviation [SD], 6.42), and the mean MMSE-KC score was 24.93 (SD, 3.55). In the simple univariate analysis, tooth replacement, age, sex, smoking status, alcohol consumption, body mass index, osteoporosis, number of natural teeth, periodontitis, chewing discomfort, tooth-brushing frequency, education level, monthly household income, participation in economic activity, living alone, and marital status had a significant impact on the association. After adjusting for confounders, the association between dental implants and cognitive function remained significant (B, 0.85; standard error, 0.40; p<0.05). Age, body mass index, periodontitis, tooth-brushing frequency, and education level were also significantly associated with cognitive function. The results of the sensitivity analyses were consistent with those of the primary analysis. Conclusions: Dental implants were associated with cognitive function in older adults living in the community. Dental implants as tooth replacements may play a role in preserving cognitive function.

  • Burn-in is a widely used method to eliminate the initial failures. Preventive maintenance policy such as age replacement is often used in field operation. In this paper burn-in and maintenance policy are taken into consideration at the same time. The properties of the corresponding optimal burn-in times and optimal maintenance policy are discussed.

Ordering Policy for Planned Maintenance with Salvage Value

  • A spare ordering policy is considered for planned maintenance. Introducing the ordering, uptime, downtime, inventory costs and salvage value, we derive the expected cost effectiveness. The problem is to determine jointly the ordering time for a spare and the preventive replacement time for the operating unit which maximize the expected cost effectiveness. Some properties regarding the optimal policy are derived, and a numerical example is included to explain the proposed model.

  • Burn-in is a widely used method to eliminate initial failures. Preventive maintenance policy such as block replacement with minimal repair at failure is often used in field operation. In this paper burn-in and maintenance policy are taken into consideration at the same time. The cost of a minimal repair is assumed to be a non-decreasing function of its age. The problems of determining optimal burn-in times and optimal maintenance policy are considered.

  • An equipment is subject to shocks which occur randomly according to a Poisson process. The equipment fails when the total cumulative damage reaches a certain prescribed level. An optimal preventive replacement policy is presented when the level of cumulative damage cannot be checked. Renewal theory is used to obtain the results and a case example is presented.
