Clinical Features and Risk Factors of Post-COVID-19 Condition in Korea

  • Objectives: Numerous studies have explored the causes and spread of outbreaks, yet there is a lack of research on post-coronavirus disease 2019 condition (PCC) in Korea. The goal of this study was to identify the various types of PCC and associated factors in discharged patients and to provide directions for the ongoing health management of confirmed patients. Methods: A telephone survey was conducted among 680 coronavirus disease 2019 (COVID-19) patients diagnosed between July 7, 2021 and August 26, 2021, in Dangjin, Chungnam, Korea. A descriptive analysis of characteristics, univariate analysis, and regression were performed using data from basic epidemiological surveys conducted at the time of diagnosis and post-discharge questionnaires. Results: Of the 585 patients who responded, 159 (27.2%) developed PCC. Of the 211 patients with no initial symptoms, 27 (12.8%) developed PCC, versus 132 (35.3%) of the 374 patients with initial symptoms. Among the initial symptoms, fever or chills, cough or sputum, loss of smell, and sore throat were associated with PCC. Compared to patients with less than 10 days of hospitalization, those with a hospitalization period of 21 days to 30 days (odds ratio [OR], 2.3; 95% confidence interval [CI], 1.0 to 5.2) and 31 days or more (OR, 5.8; 95% CI, 1.9 to 18.1) had a higher risk of PCC. Conclusions: More than a quarter of COVID-19 patients, including those who had no initial symptoms, experienced PCC in Korea. People with the initial symptoms of fever, chills, and respiratory symptoms and those who had prolonged hospital stays had a high risk of PCC.

현장 조사를 통한 가축전염병 대응 정책개선 방안 연구 (Study on Policy Improvement Measures to Respond to Infectious Diseases of Livestock through Field Investigation)

  • 연구목적: 본 연구는 가축전염병 가축사체처리 및 방역관리에 대한 특성을 분석하고, 가축전염병 방역과 사후 관리의 실효성을 제시하는 것을 목적으로 한다. 연구방법: 실제로 가축전염병이 발생하여 가축 사체를 처리한 매몰지 및 매몰 관리시설을 현장 방문하여 관리현황과 문제점을 분석하였다. 연구결과: 가축전염병 발생 가축의 매몰방식과 그에 따른 장기적인 후속 관리는 많은 인력과 예산을 투입하게 만들어 비효율적인 방역시스템임을 알 수 있었다. 결론: 2차적인 감염병 전파우려가 없고 후속 관리가 필요 없는 정부와 광역 시도 및 시군구 차원의 통합 가축전염병동물 사체처리 시설 설립 방안의 검토가 필요하다.

아동 감염성 질환관리에 대한 보육교사의 역할인식, 지식, 실천 자신감 및 교육요구도 (Child Daycare Teachers' Role Perception, Knowledge, Self-confidence and Educational Needs Regarding Infectious Disease Management in Children)

  • 본 연구는 보육시설 아동의 감염성 질환관리에 대한 보육교사의 역할인식, 지식, 실천 자신감 및 교육요구도를 알아보고, 이들 변수간의 관련성을 파악하기 위한 상관관계연구이다. 연구 대상자는 보육교사 180명이었다. 연구대상자의 78.9%는 감염성 질환관리에 대한 교육 경험이 없었으며, 감염성 질환관리에 대한 지식 정답률은 52.3%, 실천 자신감은 낮았다. 그러나 감염성 질환관리에 대한 역할인식은 전반적으로 높았으며, 감염성 질환관리에 대한 교육요구도는 높았다. 감염예방을 위한 손씻기 및 위생관리법, 감염성 질환차단방법, 감염성 질환별 관리 및 간호법, 감염성 질환별 증상에 대한 교육요구도가 높았다. 대상자들의 역할인식은 그들의 지식과 교육요구도와 유의한 상관관계가 있었다. 보육교사 대상 감염성 질환 예방 및 관리를 위한 교육 프로그램의 개발, 적용 및 평가가 요구된다. 특히 보육교사들의 감염성 질환관리에 대한 역할인식을 향상시키기 위한 노력이 요구된다.

빅데이터 분석을 통한 신종감염병 중요 요인 도출 (A Study on deduction of important factors for new infectious diseases through big data analysis)

  • 본 연구는 신종감염병에 대한 텍스트 데이터를 수집하고, 이를 분석하여 신종감염병에서 중요한 요인을 도출하고자 하였다. 이를 위해 네이버 뉴스 데이터베이스의 기사를 직접 크롤링하고, 이를 전처리 하여, 데이터 분석에 활용하였다. 또한 빅카인즈를 활용하여 추가적인 분석을 실시하였다. 우선순위 분석결과, 코로나, 전염병, 방역, 백신, 발생, 바이러스, 감염, 개발 순으로 그 중요도가 나타났다. 근접중심성 분석 결과 정부, 사망, 계획 순으로 그 중요도가 나타났으며, 빅카인즈 분석결과는 코로나 19, 질병관리 본부 등이 중요한 것으로 나타났다. 본 연구의 결과를 토대로 신종감염병에 대한 대국민 인식 제고 및 방역, 백신 및 치료제 개발 등에 정부의 정책적인 지원이 필요하다고 할 수 있다.

대학생들의 신종감염병 예방행동의도에 영향을 미친 요인: 건강신념 모형을 중심으로 (Factors Influence the Preventive Behavior Intent against Emerging Infectious Diseases of University Students: Focusing on the Health Belief Model)

  • 본 연구는 COVID-19의 세계 대유행 상황에서 대학생들의 신종감염병 예방행동의도영향 요인을 살펴본 연구로, 주요 연구 결과는 다음과 같다. 첫째, 대학생들의 일반적 특성에 따른 조사 결과 2학년 학생들에서 예방행동의도가 4.11점으로 유의한 차이를 보였으며(p<.05), 건강행태 결과에서는 섭식상태에서 예방행동의도에 영향을 미친 것으로 나타났다(p<.05). 둘째, '심각성' 요인은 예방행동의도에 긍정적인 영향을 주는 것으로 나타났으며(β.164) 셋째, '민감성' 요인은 예방행동의도에 영향을 미치지 않는 것으로 조사되었다. HBM연구에서 민감성은 중요요인으로 예방행동으로 인해 발생할 수 있는 이익이 높을수록 반대로 예방행동으로 인한 손실이 낮을수록 예방행동의도가 높게 나타나 예방행동의도 형성에 도움이 될 것으로 해석한다. 그러나 본 연구에서는 민감성이 예방행동의도에 영향을 미치지 않았다는 결과는 감염병에 대한 안전불감증이 존재하는 것으로 여겨지며 '부정적 사건이 일어나지 않을 것' 이라는 인지적 경향 보여진다. 넷째, '자기효능감'은 주요 변수에서 가장 큰 설명력을 보였다(β.640).

Imported human babesiosis in the Republic of Korea, 2019: two case reports

  • Human babesiosis is a tick-borne disease induced by the genus Babesia and has been significantly reported in the Republic of Korea. This report shows the cases of 2 patients with human babesiosis who traveled to the USA in 2019. The 2 patients experienced fever and had travel histories to babesiosis-endemic regions. The diagnoses of both cases were verified by the identification of Babesia-infected red blood cells on blood smears. One patient was found to be infected with Babesia microti using polymerase chain reaction (PCR) for 18S rRNA, which discovered the phylogenetic link to the B. microti strain endemic in the USA. The 2 patients recovered from fever with subsequent hemoparasite clearance. Babesiosis could be diagnosed in anyone with histories of travel to babesiosis-endemic countries and tick bites. Furthermore, Babesia-specific PCR is required for determining geno-and phenotypic characteristics.

  • As the main aim of infectious disease epidemiology is to prevent a pathogen transmission, traditional epidemiological studies have focused on the evaluation of individual-level risk factors. But group-level factors as well as individual-level are important in understanding and controlling transmission of infectious diseases, especially sex-transmitted infectious diseases. Multi-level analysis (MLA) is known as a powerful analytical tool for investigating both levels simultaneously. While new cases of HIV/AIDS in Korea are increasing annually, it is urgently needed to establish research system for MLA led by Korea Centers for Disease Control & Prevention.

Proteomic Analysis of Protein Expression in Streptococcus pneumoniae in Response to Temperature Shift

  • From its initial colonization to causation of disease, Streptococcus pneumoniae has evolved strategies to cope with a number of stressful in vivo environmental conditions. In order to analyze a global view of this organism's response to heat shock, we established a 2-D electrophoresis proteome map of the S. pneumoniae D39 soluble proteins under in vitro culture conditions and performed the comparative proteome analysis to a 37 to $42^{\circ}C$ temperature up-shift in S. pneumoniae. When the temperature of an exponentially growing S. pneumoniae D39 culture was raised to $42^{\circ}C$, the expression level of 25 proteins showed changes when compared to the control. Among these 25 proteins, 12 were identified by MALDI-TOF and LC-coupled ESI MS/MS. The identified proteins were shown to be involved in the general stress response, energy metabolism, nucleotide biosynthesis pathways, and purine metabolism. These results provide clues for understanding the mechanism of adaptation to heat shock by S. pneumoniae and may facilitate the assessment of a possible role for these proteins in the physiology and pathogenesis of this pathogen.

제주도 결핵 신환자 발생의 역학적 특성, 2011~2018 (Epidemiological characteristics on new cases of notified tuberculosis in Jeju Province, Korea, 2011~2018)

  • While the notified crude incidence rates(per 100,000 persons, CIR) of new tuberculosis between 2016 and 2017 decreased in nationwide and 16 provinces, except Jeju Province, Korea. The aim of this study was to compare CIRs of Jeju Province and nationwide by calendar years and age groups in order to interpret the increasing trend of CIR in Jeju Province. The data source was the Integrated Diseases and Health Control System of the Korea Centers for Disease Control and Prevention. The selection criteria were the notified new cases as tuberculosis, who had the address as Jeju Province at diagnosis, between 2011 and 2018. The 95% confidence interval of CIR was estimated based on Poisson distribution. There was not statistically significant difference of CIRs between Nationwide and Jeju Province of 2011 and 2018, except 2016. Especially the CIR of group aged over 65 years in Jeju Province, 2016 was significantly lower compared to Nationwide CIR. But, the CIR of group aged 30~50 between 2017 and 2018 showed significantly higher compared to Nationwide CIR. Based on the results, the fact of increasing CIR from 2016 to 2017 in Jeju Province might be interpreted as an introduction of new tuberculosis screening project rather than real increased CIRs.

감염병 예방 및 관리를 위한 한의사의 역할과 정책적 과제: 혼합연구방법론을 적용하여 (The Role and Policy of Korean Medicine Doctor for the Prevention and Management of Infectious Diseases: A Mixed-Methods Approach)

  • Objectives: This study investigated the role of Korean medicine doctors in the national quarantine system and the status of their duties through questionnaire surveys and in-depth interviews with Korean medicine staff who participated in COVID-19 work. Methods: We developed a questionnaire survey to investigate the status and perception of Korean medicine doctors' participation in responding to infectious diseases, and the results were derived by conducting frequency analysis and T-test using SPSS program. The measurement variables of this study were the status of COVID-19 related work participation and perception of Korean medicine doctors' role in infectious diseases, and the maximum significance level was 0.05. Results: There were 300 respondents to the survey, and as for the COVID-19 related work, Korean medicine treatment for COVID-19 patients accounted for the most(52.7%), and various preventive and treatment tasks such as epidemiological investigations and specimen collection were conducted. Conclusion: According to this study, it was confirmed that there are sufficient legal and administrative grounds for Korean medicine doctors to perform specimen collection and epidemiological investigations. Therefore, it is necessary to prepare a policy framework to solidify the status of Korean medicine doctors in national infectious diseases in the future.
