Purpose: This study evaluated the effect of an Educational Program on Palliative Care for MPM for Nurses in Japan. Program: The 5-h program consisted of lectures and care planning group work. Materials and Methods: This study used a pretest-posttest design with a single cohort of nurses and included a Difficulties in Palliative Care for Patients with MPM (DPCMPM) Scale with 15 items. The pre- and posttest scores were compared using a t-test. Results: We included 27 female nurses with a mean of 14.4 years of nursing experience. In 12 of 15 DPCMPM items, the posttest difficulty scores were lower than the pretest scores. Participants highly evaluated the program for validity, clarity, clinical usefulness, and the facilitators. The Palliative Care for MPM Handbook for Nurses was developed as an educational tool for clinical settings. Conclusions: The Educational Program on Palliative Care for MPM for Nurses was effective in reducing nursing difficulties.
Purpose: After a traffic accident, first-aid performed within the first few minutes is important for the injured in terms of survival, future health, and quality of life. Taxi drivers have more possibility of witnessing traffic accidents because they spend much time on the road. If taxi drivers are well trained and can perform first-aid in a correct manner, they will play an important role in pre-hospital emergency medical services. We investigated the effect of first-aid training on taxi drivers' willingness to perform emergency care. Methods: We provided first-aid training for 600 deluxe taxi drivers in Busan in 2012. The subjects were given a questionnaire to answer at the beginning and at the end of first-aid training. Results: 427 out of 600 deluxe taxi drivers answer our questionnaire completely. 93 out of 427 deluxe taxi drivers answered that they had first-aid training within past 3 years(21.8%). 323 taxi drivers have witnessed a traffic accident(76%). 45 out of 323 deluxe taxi drivers who witnessed a traffic accident answered that they provided first-aid to victims(14.0%). After first-aid training, taxi drivers' willingness to perform emergency care was increased compared to that before training. The failed group of taxi drivers that had same or decreased willingness after first-aid training had a low level of education than the successful group that had increased willingness after first-aid training. Conclusion: First-aid training increased taxi drivers' willingness to perform emergency care. So Taxi drivers should be encouraged more to undertake and maintain first-aid training. For more volunteering of first-aid training and improvement in the effect of first-aid training, a graded education program for taxi drivers with a low level of education should be developed, and a policy on giving credit for completing first-aid training course and for carrying out first-aid needs to be formulated.
Objectives: This study aims to assess the maternal characteristics and changes of body among patients who received Korean medicine postpartum care compared to pre-study. Methods: From January 1, 2018 to February 28, 2019, we included 31 postpartum patients who had body composition analysis at admission and discharge among 34 patients who received postpartum care in ${\square}{\square}$ University Hospital. We used SPSS 21.0 for window to test for statistical significance. Results: The average age, weight, and caesarean section rate increased compared to previous study. The average period of postpartum care has decreased. After postpartum care, abdominal circumference, body weight and Body Mass Index (BMI) was significantly decreased, but not as much as pre-study. The coefficient of determination was derived to predict the effects of postpartum care and calculated as "$Abdominal\;circumference\;change=2.745+0.593{\times}admission\;period$", "$weight\;change=0.214+0.345{\times}admission\;period$" and "$BMI\;change=0.198+0.120{\times}admission\;period$". Conclusions: Compared to previous study, as the maternal age increased and admission period decreased, the weight loss after childbirth decreased. It is necessary to provide individual treatment through coefficient of determination.

Background: Beyond its current function as a rescue therapy in acute respiratory distress syndrome (ARDS), extracorporeal membrane oxygenation (ECMO) may be applied in ARDS patients with less severe hypoxemia to facilitate lung protective ventilation. The purpose of this study was to evaluate the efficacy of extended ECMO use in ARDS patients. Methods: This study reviewed 223 adult patients who had been admitted to the intensive care units of 11 hospitals in Korea and subsequently treated using ECMO. Among them, the 62 who required ECMO for ARDS were analyzed. The patients were divided into two groups according to pre-ECMO arterial blood gas: an extended group (n=14) and a conventional group (n=48). Results: Baseline characteristics were not different between the groups. The median arterial carbon dioxide tension/fraction of inspired oxygen ($FiO_2$) ratio was higher (97 vs. 61, p<0.001) while the median $FiO_2$ was lower (0.8 vs. 1.0, p<0.001) in the extended compared to the conventional group. The 60-day mortality was 21% in the extended group and 54% in the conventional group (p=0.03). Multivariate analysis indicated that the extended use of ECMO was independently associated with reduced 60-day mortality (odds ratio, 0.10; 95% confidence interval, 0.02-0.64; p=0.02). Lower median peak inspiratory pressure and median dynamic driving pressure were observed in the extended group 24 hours after ECMO support. Conclusion: Extended indications of ECMO implementation coupled with protective ventilator settings may improve the clinical outcome of patients with ARDS.

It was reported that nursing students had most difficulties in clinical competency due to their short of knowledge and skills in nursing care. Environmental difference between laboratory in nursing school and hospital was another difficulty to them. This implicated that more preclinical competency education is needed in fundermental nursing to achieve skills of care and the laboratory environment of nursing school had to be improved. The purpose of this study was to examine contents, total lecture and practicum house of the fundermental nursing education in nursing schools. It was also examine the difference of contents between hospital in-service education and nursing school. It's study is a descriptive study with study sample of 56 nursing professors who teach fundermental nursing in colleges and universities. Structured Questionnaires was used in collecting data. The result showed that practicum hours total 96.8 hours which was 59% of funderm nursing course. The education regar medication account for large numbers of h Aseptic technique, elimination care, vital oxygen related care were the next common contents. When compared with hospital in-service education, 76% of the education were the same. This suggested that pre-clinical education in fundermental nursing needed to be enforced in its contents & education hours to establish clinical practice centered nursing education.
Purpose: This study utilized a non-equivalent control group pre-post design to assess the effects of spiritual nursing care on loneliness and spiritual well-being of terminal cancer patients. Method: Forty-one terminal cancer patients in a general hospital, were divided into an experimental group of 20 patients and a control group of 21 patients. Those in the experimental group received four weeks of treatment three times a week for about 45 minutes each session. The treatment included nurses' spiritual care involving five instruments of spiritual nursing intervention, each of which was used according to the six types of spiritual need assessment. Data was analyzed with descriptive statistics including real number, percentage, $X^2$-test, t-test, and ANCOVA. Result: There were significant differences between the experimental and control groups in the level of loneliness and spiritual well-being. Conclusion: Spiritual nursing care was verified as an effective program that can lessen the loneliness and improve the spiritual well-being of patients with terminal cancer.
Purpose: The purpose of this study was to examine the effect of an individual education program on breast cancer patients who received chemotherapy. Methods: The quasi-experimental design was employed with a nonequivalent control group pre-post test. The study was performed from January 1 to July 30, 2010 for the patients who were firstly treated by A C (Adriamycin, Cyclophosphamide) chemotherapy in a university hospital. Control group was recruited first to prevent treatment diffusion. The experimental group was recruited and received an individual education during 30 minutes. The collected data were analyzed using SPSS 16.0 version. ${\chi}^2$-test, t-test, and the Pearson correlation were implemented in order to examine the effect of the individual education among the patients who are treated by chemotherapy. Results: The experimental group had a greater improvement in self-care knowledge (t=-5.236, p=.001), and self-care performance (t=-6.543, p=.004) than did control group. Moreover, the experimental group had significantly positive correlation between self-care knowledge and self-care performance (r=.494, p=.006) as compared to the control group. Conclusion: The individual education program on cancer patients who were treated by chemotherapy could be utilized as an effective nursing intervention.

Purpose: This study was conducted to examine the experience of nurse in comprehensive nursing care. Methods: Experiential data collected from 9 nurses through in-depth interviews. Participants were selected from nurses working in the comprehensive nursing care unit at general hospital. The main question was "Can you describe your experience in the comprehensive nursing care unit?" All interviews were recorded and transcribed, then analyzed using Colaizzi's method. Results: Nine themes were derived from the analysis: 'Practice nursing care', 'Feel thankful of the client', 'Difficulty in nursing due to absence of patients' guardian', 'Tired of over-demanding patient and distrust of guardian', 'Confusion regarding one's identity as a nurse', 'Not enough to support system', 'Insufficient pre-training for nurse and client', 'Requirement of work establishment for nurse and nurse aid', 'Concerns about low rewards and high safety accidents'. Conclusion: As a comprehensive nursing service, the nurses provided total patient care, and patient satisfaction and expression of appreciation increased. However, disadvantages were identified, such as patients' excessive needs, communication difficulties, lack of support systems, low compensation, and a high number of safety accidents. Therefore, systematic comprehensive nursing will be achieved if these shortcomings are addressed.

Purpose: This study was performed to explore the effects of an admission-education program on knowledge, self-efficacy, self-care and glucose control in type 2 diabetes patients. Method: A single group pre-post test design was used. Twenty-one patients participated in this program during 6 days which consisted of education and practice about exercise, diet, self-care and prevention of complications. Variables at baseline, 3 months, and 6 months were measured. Results: Knowledge level, self-care level and self-efficacy were significantly increased over time after the admission-education program. The $HbA_1C$ level and glucose level in urine were significantly decreased over time after the admission-education program. Conclusion: The admission-education program can increase levels of knowledge, self-care, self-efficacy and decrease glucose levels. Therefore, this admissioneducation program was shown to have a positive effect on the management of diabetes.
Purpose: This study aimed to develop a pre-hospital subarachnoid hemorrhage (SAH) prediction tool by analyzing the extant predictive factors of patients with non-traumatic SAH who visited the hospital through the 119 emergency medical services. Methods: We retrospectively reviewed pre-hospital care reports (PCRs) and electronic medical records (EMRs) of 103 patients with non-traumatic SAH who were transported to the emergency department of two national hospitals via the 119 emergency medical service from January 1, 2017 to December 31, 2020. Variables required to apply the Ottawa SAH Rule and EMERALD SAH Rule, which are early prediction tools for SAH, were extracted and applied. Results: The most common symptoms-which were found in 94.1% and 97.0% of all patients according to PCRs and EMRs, respectively-appeared in the following order: headache, altered state of consciousness, and nausea/vomiting. When the variables used for the EMERALD Rule, namely systolic blood pressure (SBP), diastolic blood pressure (DBP), and blood sugar test (BST), were applied, the sensitivities of EMR and PCRs were 99.9% and 92.2%, respectively. Conclusion: For the timely prediction of SAH at the pre-hospital phase, patient age and symptoms should be assessed, and SBP, DBP, and BST should be measured to transport the patient to an appropriate hospital.
