  • In drug discovery or preclinical stages of development, potency parameters such as $IC_{50}$, $K_i$, or $K_d$ in vitro have been routinely used to predict the parameters of efficacious exposure (AUC, $C_{min}$, etc.) in humans. However, to our knowledge, the fundamental assumption that the potency in vitro is correlated with the efficacious concentration in vivo in humans has not been investigated extensively. Thus, the present review examined this assumption by comparing a wide range of published pharmacokinetic (PK) and potency data. If the drug potency in vitro and its in vivo effectiveness in humans are well correlated, the steady-state average unbound concentrations in humans [$C_{u_-ss.avg}=f_u{\cdot}F{\cdot}Dose/(CL{\cdot}{\tau})=f_u{\cdot}AUCss/{\tau}$] after treatment with approved dosage regimens should be higher than, or at least comparable to, the potency parameters assessed in vitro. We reviewed the ratios of $C_{u_-ss.avg}$/potency in vitro for a total of 54 drug entities (13 major therapeutic classes) using the dosage, PK, and in vitro potency reported in the published literature. For 54 drugs, the $C_{u_-ss.avg}$/in vitro potency ratios were < 1 for 38 (69%) and < 0.1 for 22 (34%) drugs. When the ratios were plotted against $f_u$ (unbound fraction), "ratio < 1" was predominant for drugs with high protein binding (90% of drugs with $f_u{\leq}5%$; i.e., 28 of 31 drugs). Thus, predicting the in vivo efficacious unbound concentrations in humans using only in vitro potency data and $f_u$ should be avoided, especially for molecules with high protein binding.

  • Human Carcinogenic Potency (HCP) can be estimated based on human daily exposure dose to carcinogen (Dh), body weight (Wh), 10% tumorigenic dose (TD10), and slope factor at TD10 (Q10) from 2-yr bioassay data. This approach is more relevant to humans generally exposed to low doses of carcinogens and can reduce more of extrapolation errors from high dose in animal experiments to low dose in humans than HERP (human exposure dose/rodent potency dose) proposed by Ames et al. (Science, 236, 271-280, 1987). TD50 and HERP have been routinely used to compare rodent carcinogenic potency and human carcinogenic potency, but those approaches have had limitations in extrapolation of high dose to low dose in humans. The advantages of HCP are to estimate human exposure dose (Dh) by human monitoring instead of environmental monitoring, to consider slope factor (Q10) which reflects the tendency of curve at low dose, and to use TD10 which represents much lower dose thant TD50 or HERP. HCP will be a useful parameter for the estimation of human carcinogenic potency in risk assessment and management of carcinogens.

  • Clinical effects of intramuscular injection of 250mg testosterone cypionate every 2-3 weeks were investigated in 15 impotent patients with low or low normal serum testosterone level (Hypo-gonadotrophic hypogonadism was excluded). The results were obtained as follows. 1. Among 15 patients, 7(46.7%) showed markedly improved potency, 6(40%) partially improved potency and 2(13.7%) no improvement of potency. There was no correlation between effec-tiveness of testosterone replacement and age, testicular size, serum testosterone level or LH, FSH level. 2. Among 12 patients who had showed improved potency, 8(66.7%) complained of redeveloped decrease in potency during testosterone replacement. In conclusion, testosterone cypionate for treatment of the impotent patients with low or low normal serum testosterone level was effective, but further studies are necessary to investigate cause of redeveloped decrease in potency during testosterone replacement.

  • Since the active center of digestive enzymetic preparations, while under storage, lose their activity and potency by the exposure to moisture, colorization, solidifying and other physical changes. It is more important than beautiful package form that protected packaging form from moisture to get a pharmaceutical safety preparations and to maintain a definite potency. Then, in order to get a desirable conditions of storage and packaging, we used shellac and $AEA^{\circledR}$ as a coating base, and blister package, foil and bottle container as a packaging material. Temperature were set on room temperature and $37^{\circ}C$, moisture was adjusted to 40% RH and 80 % RH as a accelerated conditions. Accelerated test was carried out 6 times. The results are as follows: 1) The effect of packaging conditions give great influence on the maintenace of the stable potency. 2) Best result was produced with bottle container package. 3) $AEA^{\circledR}$ is more useful than shellac as a coating base of prevention from moisture. 4) Absorption of moisture gave considerable effects on potency and it has a limited point. 5) Difference in potency between optimal and worst condition is 472 u/2T, and difference in effective period is about 44 months.
