The Effect of Balance Training using Force Platform on Postural Control and Central Somatosensory Pathway in Adults with C. N. S. Disorders (Force Platform을 이용한 평형성 훈련이 중추신경계 손상자의 자세조절 및 중추 감각신경전도로에 미치는 영향)

  • 본 연구의 목적은 균형 훈련이 중추신경계 손상자들의 자세 조절 및 중추감각신경전도로에 미치는 영향을 규명하는데 있다. 연구대상자는 중추신경계 손상자로써 실험군 10명, 통제군 10명 등 총 20명을 선정하였으며, 실험군은 본 연구의 훈련 프로그램에 따라 12주간 force platform을 이용하여 균형훈련을 실시토록하였다. 자세조절 변인의 측정은 운동처치 전, 처치 후 8주 및 12주 후에 대상자들의 동적 및 정적 자세에서의 흔들림을 Dynamic Balance System을 이용하여 측정하였고, 체성감각 유발전위의 말초신경 근위부 유발전위$(N_9)$ 잠복기, 척수 유발전위$(N_{13})$ 잠복기, 뇌 유발전위 $(N_{20})$ 잠복기는 Neurotec을 이용하여 측정 분석한 결과 다음과 같은 결론을 얻었다. 1. 정적 자세 조절 요인의 경우, 좌우 흔들림과 전후 흔들림은 실험군에서 8주 후부터 유의하게 (p<.05) 감소하였고, 실험군이 통제군에 비해 운동처치 8주 및 12주 후에 각각 유의하게(P<.05, P<.01) 흔들림이 감소하였다. 2. 전후 이동면과 전후 기울기면에서 동적 자세 조절의 변화는 전후 이동면에서 좌우 흔들림과 전후 흔들림은 실험군에서 8주 후부터 유의하게 (P<.05) 감소하였으며, 실험군이 통제군에 비해 운동처치 8주 및 12주 후에 각각 유의하게 (P<.05, p<.01) 흔들림이 감소하였다. 3. 체성감각 유발전위의 잠복기 변화는 실험군과 통제군에 있어서 말초신경 근위부 유발전위$(N_9)$ 잠복기와 척수 유발전위$(N_{13})$ 잠복기가 다소 증가하였으나 유의한 차이는 나타나지 않았으며, 실험군에 있어서 뇌 유발전위 $(N_{20})$ 잠복기는 8주 후부터 유의하게 (P<.05) 증가하였다. 이상의 결과를 종합해 볼 때, 12주의 균형 훈련은 자세 조절에 있어서 전후와 좌우의 흔들림을 감소시킴으로써 정적인 상태나 동적인 상태에서의 자세 안정성을 증가시킬 수 있음을 시사하고 있다. 이는 자세 조절에 필요한 항중력근의 긴장성 수축을 유발시킬 뿐만 아니라 근육 긴장분포를 조절할 수 있다는 것으로 신경근 조절 기능의 향상을 의미하는 것으로 사료된다. 또한 뇌 유발전위 $(N_{20})$ 잠복기의 증가는 중추신경계의 감각기능의 신경학적 회복을 의미하는 것으로 중추신경계의 감각 운동통합에도 영향을 미쳐 운동기능의 향상을 기대할 수 이을 것으로 사료된다.

Effect of Abdominal Draw In Maneuver in Sitting Position (앉은자세에서 실시하는 복부드로우인기법의 효과)

  • The problem of trunk stability is a major factor in back pain.. Abdominal draw in maneuver(ADIM), One of the trunk stabilization exercises to relieve lumbar instability, is a method of inducing selective contraction of the transverse abdominis associated with anticipatory posture control among the abdominal stabilization muscles. ADIM is usually performed with a visual feedback by applying a pressure biofeedback unit(PBU) under the lumbar at the supine position, which is not functional compared to the sitting position. This study was conducted to investigate the effect of ADIM applied in supine and sitting position on 31 healthy men and women. In each posture, muscle activity was measured by rectal abdominis (RA), external oblique (EO), transverse abdominis (TrA)/internal obilique (IO) and erector spinae (ES) using wireless EMG. In the result, there was no significant difference between RA and EO between the two postures and there was a significant difference between TrA / IO and ES. In both postures, the activity of TrA/IO was higher than that of RA, and the effect of ADIM was shown to be the same, whereas TrA/IO and ES showed higher activity in sitting position. This means that the activity of the muscles involved in the postural stability and lumbar stability is increased further in the sitting position. Therefore, ADIM in sitting, which can be applied more easily in daily life, is useful for improving lumbar stability.

Study on the Improvement of Equilibrium Sense of the Elderly Using Virtual Bicycle System (가상 자전거 시스템을 이용한 고령자의 평형감각 증진에 관한 연구)

  • In this paper, a new rehabilitation training system was developed to improve equilibrium sense by combining virtual reality technology with a fixed exercise bicycle. The subjects consisted of two groups. A group of young people, was compared against a group of elderly. We measured three different running modes of virtual bicycle system with two successive sets. The parameters measured were running time, velocity, the weight movement, the degree of the deviation from the road, and the variables about the center of pressure. The repeated training, our results showed that the running capability of the elderly improve compared. In addition, it was found out that the ability of postural control and the equilibrium sense was improved with the presentation of the visual feedback information of the distribution of weight. From the results of this experiment, we showed that our newly developed system might be useful in the diagnosis of equilibrium sense or in the improvement of the sense of sight and, somatic, and vestibular sense of the elderly in the field of rehabilitation training.

Effects of 12-week balance training with visual feedback on balance and walking functions in patients with chronic stroke (12주 동안의 시각 되먹임 균형 훈련이 만성뇌졸중 환자의 균형 및 보행에 미치는 영향)

  • This study was to determine the effect of 12-week balance training with visual feedback on balance and walking functions in patients with post-stroke hemiparesis. Twenty individuals with chronic stroke volunteered to participate in this study. They were randomly allocated to either experimental group (EG) or control group (CG), with 10 subjects in each group. Subjects from the group 1 underwent 15-min balance training with visual feedback and 15-min routine-scheduled physical therapy, and subjects from the group 2 performed 30-min routine-scheduled physical therapy only, which comprised mat exercise, strengthening, postural correction, and functional practice. Assessment tools were made with the Functional Reach Test (FRT), Timed Up and Go Test (TUGT), and 6 Min Walk Test (6MWT). In within-group comparison, the EG showed significant differences in all parameters (p<0.05), while significant differences were not found for the CG (p>0.05). In between-group comparison, the change values of the FRT and 6MWT appeared to be significantly greater for the EG than those of the CG (p<0.05). These findings suggest that balance training with visual feedback may be favorably used to improve balance and walking functions in patients with chronic post-stroke hemiparesis. Further studies with larger sample and long-term follow-up period need to generalize the results of this study.

A Systematic Review of Sensory Integration Intervention for Children in Korea (아동을 대상으로 한 감각통합치료의 중재효과에 대한 체계적 고찰: 국내 연구를 중심으로)

  • Objective : The purpose of this study is to summarize the best-available intervention evidence for children's sensory integration therapy, drawn from studies published domestically in Korea over the last 10 years. Methods : The articles evaluated in this study were collected from the RISS and DBpia databases using the search terms "sensory integration," "sensory processing," and "Ayres Sensory Integration (ASI)". A total of 19 papers were analyzed. The selected studies were then assessed using the Population, Intervention, Outcomes, and Comparison method, the International Classification of Functioning, Disability and Health (ICF) method, and the modified Evidence Alert Traffic Light Grading System. Results : Development delay was the most commonly applied diagnosis for children's sensory integration therapy and individual sensory integration therapy was the most frequently used intervention method. The intervention effect was 91 percent in the body structure and function of ICF. The areas concentrated on were sensory modulation, sensory processing, fine and gross motor, body scheme, body-self concept, balance, basic movement, postural control and hand function, attention, and self-esteem. Conclusion : This simple overview of the efficacy of children's sensory integration therapy provides a basis for easy understanding and use by therapists, researchers and families with children.

Correlation of the Berg Balance Scale and Smart Balance Master System for Chronic Hemiparetic Stroke (만성 편마비 뇌졸중 환자의 버그균형척도(Berg Balance Scale)와 Smart Balance Master System의 상관성)

  • The purpose of this study was to find correlation between the Berg Balance Scale which is most commonly used and the equilibrium scores from Smart Balance Master System which is used for the test of the balance ability for the patients with chronic hemiparetic stroke to predict their falling risk though examining the postural control. Twenty-two subjects who had first stroke participated in this study. Participants were measured the Berg Balance Scale and the Smart Balance Master System. The study found that there was a significant correlation between the Berg Balance Scale and the equilibrium scores from Smart Balance Master System. And there was no significant correlation at all "eye-closed and sway of surfaces" in the 6 conditions of Smart Balance Master System and there was a significant correlation with over median in the other 5 conditions. The study was uncovered that there was no meaningful correlation between the Berg Balance Scale and "eye-closed and sway of surfaces" because chronic hemiparetic stroke patients heavily relied on their vision to maintain their balance ability and the Berg Balance Scale did not properly exam it. Thus to predict their falling risk in clinical practice for the patients with chronic hemiparetic stroke, it would be more efficient ways for us to consider all the effects of sensory organs with more than one clinical evaluation method.

The Comparison of Kinematic Data of the Body Orientation in Sitting Position to Adapt Dynamically Changing Angle of the Base of Support in Stroke Patients and Healthy Adults (뇌졸중 환자와 정상 성인의 앉은 자세에서 지지면의 동적 각도 변화에 적응하는 신체 정위의 운동형상학적 비교)

  • This study aimed to investigate the difference of the body orientation ability in sitting position to adapt to dynamically changing angle of the base of support in stroke patients and Healthy adults. The angle between vertical and head and trunk in 12 stroke patients (6 male and 6 female) and 12 healthy adults (6 male and 6 female) were measured by video motion analysis system. The head and trunk angle between stroke patients and healthy adults in sitting position were significantly different when increase the angle of dominant side(p<.05). The head and trunk angle between stroke patients and healthy adults in sitting position were significantly different when increase the angle of non-dominant side(p<.05). The head and trunk angle between stroke patients and healthy adults in sitting position were significantly different when decrease the angle of non-dominant side(p<.05). The head angle between stroke patients and healthy adults in sitting position was significantly different(p<.05), but the trunk angle was not significantly different when decrease the angle of dominant side(p>.05), Stroke patients compared to healthy adults had more deficits in their body orientation ability in sitting position to adapt to dynamically changing angle of the base of support. This finding may help to understand postural control deficits more clearly in stroke patients in sitting position.

Study of 4 Cases with Changes of Unified Parkinson's Disease Rating Scale, Heart Rate Variability and Quality of Life in Parkinson's Disease Patients through Whole Body Gi-Hyeol Therapy (전신기혈요법 치료를 통해 Unified Parkinson's Disease Rating Scale, Heart Rate Variability 및 삶의 질이 변화된 파킨슨 환자 4례에 대한 증례보고)

  • Parkinson's disease is one of the typical neurodegenerative disease and it is caused by the destruction of substantia nigra in brain leading to lack of dopamine secretion, and it presents 4 major motor symptoms such as tremor, bradykinesia, stiffness, postural instability. Furthermore, it causes many non-motor symptoms such as anosmia, REM sleep conduct disorder, orthostatic hypotension, dementia and autonomic ataxia such as lack of adjusting blood pressure, hyperhydrosis, constipation. Dopaminergic therapy is the most commonly used strategy, but long term treatment of levodopa induce various adverse effects. Thus, many people are focusing on new therapies other than established therapies, and there are many tries and approaches with paradigm shift. Our medical team was able to get 4 cases of PD patients who are hospitalized in our hospital, treated by Whole Body Gi-Hyeol Therapy consisting of acupuncture therapy, herbal therapy, and mental therapy, and their conditions improved in perspective of Unified Parkinson's Disease Rating Scale(UPDRS), Heart Rate Variability(HRV), and Quality of life. Among all 4 cases, UPDRS score and quality of life score is gotton better, and among 2 cases SDNN, RMS-SD, TP, LF, HF scores are finely increased. And PDQ-39 score which shows quality of life is also improved. However, in spite of these improvements and positive results, there were no meaningful improvement in a hurt from a fall which is important to the aged, muscular atrophy which causes bone fracture and SMI(Skeletal Muscle Mass Index) which is indicator of osteoporosis. Thus, supplementary treatment about Whole Body Gi-Hyeol Therapy such as more active nutrition intervention, safe and effective kinesitherapy is needed, and from now on continuous case reports and systematic clinical research which has control group must be carried out.

Evaluation of the Efficiency of Use of Fixation Instruments in Computed Tomography-Guided Biopsy of Lung Lesions (전산화단층촬영 유도하 폐 병소의 생검시 고정기구 사용의 효용성 평가)

  • Minimizing patient movement during CT-guided lung biopsy is an important factor in the procedure. To minimize movement, a vacuum cushion was used to evaluate its effectiveness. The subjects of this study were 116 patients aged 40 years or older who had good coordination with postural fixation and breathing control. Posture measurements were performed in the supine position, prone position, oblique position, and lateral position according to each position of the lung lesion biopsy lesion. Measurement positions were measured in the anterior, posterior, right, and left positions based on the anatomical posture. In the prone position, the mean difference between the non-use and the use of the posterior was 1.7905, and t=2.913 (p<0.01), and the mean difference between the non-use/use was statistically significant. The difference between the unused and used averages of left was 2.4105, and the difference between the left averages was also significant with t=3.684 (p<0.01). The difference between the unused and used averages of the right was 2.3263, with t=3.791 (p<0.01). The mean difference between unused and used is statistically significant. As a result of statistical analysis, the biopsy of the lung lesion using a fixation device showed less movement in all postures. It is considered that it is meaningful in that it is possible to conduct a more accurate biopsy procedure and minimize the patient's posture movement by using a fixation device during the CT-guided biopsy of the lung lesion.

A Systematic Review of Occupational Therapy Interventions for Children With Cerebral Palsy: Focus on Single-Subject Research Design (뇌성마비 아동을 위한 작업치료 중재에 대한 체계적 고찰: 국내 단일대상연구를 중심으로)

  • Objective : The purpose of this study was to identify the characteristics of a single-subject research study and qualitative levels in which occupational therapy interventions were performed on children with cerebral palsy. Methods : This study targeted papers, published in Research Information Sharing Service (RISS), National Digital Science Library (NDSL), Koreanstudies Information Service System (KISS), and E-article from May 20 to 29, 2022. The search terms were 'cerebral palsy' AND 'single subject research design' OR 'individual subject study'. Eleven papers, were finally selected and analyzed. Results : Most of the studies were medium in methodological quality, and the subjects were pre-school age and spastic hemiplegia. Among the single-subject designs, intervention-removal designs were the most common, and among them, ABA designs were the most common. Interventions included assistive devices, constraint-induced therapy, neurodevelopmental therapy, and sensory integration therapy were 2, and upper extremity exercise, interactive metronome, and CO-OP were 1. Dependent variables were measured with 2 to 4 measurement tools, Significant improvements were found in postural control ability, gait and balance, hand function, and upper extremity function. Conclusion : This study confirmed that it is helpful to apply cerebral palsy occupational therapy by presenting the characteristics of cerebral palsy, intervention sessions and effects, measurement tools and methodological quality levels.
