Effects of Breast Massage on Breast Pain, Breast-milk Sodium, and Newborn Suckling in Early Postpartum Mothers (유방마사지가 산욕초기 어머니의 유방 통증, 유즙 소디움, 신생아 흡유 횟수에 미치는 효과)

  • Purpose: In this study the effects of breast massage on breast pain, breast-milk sodium, and newborn suckling in early postpartum mothers were investigated. Methods: The design was a non-synchronized nonequivalent control group pretest-post-test design. Sixty postpartum mothers who were admitted to a postpartum care center and had problems with breastfeeding were recruited. Of these mothers, 44 were assigned to the intervention group and received two 30-minute breast massages within 10 days of postpartum period. The others were assigned control group and received only routine care. Breast pain was measured using a numeric pain scale and number of times newborns suckled was observed throughout breastfeeding. Breast milk was self-collected to evaluate breast-milk sodium. Results: Mean age of postpartum mothers was 30 years old. Compared to the control group, women in the intervention group reported significant decreases in breast pain (p<.001), increases in number of times newborns suckled after the first and second massage (p<.001), and a decrease in breast-milk sodium after the first massage (p=.034). Conclusion: Breast massage may have effects on relieving breast pain, decreasing breast-milk sodium, and improving newborn suckling. Breast massage can be used to solve breast problems. Further research is needed to validate our findings.

The trend of national health insurance service use among pregnant and postpartum women aged 35 years and older (35세 이상 고령 임산부 진료실적 추이에 관한 연구)

  • This study was conducted to examine the trend of national health insurance service use with relation to pregnancy, childbirth, and the puerperium among pregnant and postpartum women older than 35 over the last decade. A descriptive analysis was conducted, using the data which were drawn from the "nationwide claim database of Korean National Health Insurance Corporation(NHIC)". Data were composed of the total cases related to pregnancy, childbirth, and the puerperium (International Classification of Disease, $10^{th}$revision [ICD-10] codes O00-O99) from 2001 to 2008. During 2001-2008, the number of pregnant and postpartum women older than 35 had continuously increased and the percentage of them also had increased in both hospital and ambulatory care. There are similar trends in their total use of national health insurance service and total expenditure. According to demographic characteristics, there was the biggest increase of the percentage in residents in large cities, self-employed workers, ones in the highest income level. According to ICD-10 codes, there was the biggest increase of the percentage in O10-O16 (oedema, proteinuria and hypertensive disorders in pregnancy, childbirth and the puerperium). According to the major prevalent disease, there was the biggest increase of the percentage in O60 (preterm labor and delivery). Throughout the past decade, the necessity has been emphasized of supporting pregnant and postpartum women older than 35. But in maternal and child health care, they are in an early stage of development. The findings of this study would be helpful in developing the support programs for the aged pregnant and postpartum women.

Effects of Foot-Reflexology Massage on Fatigue, Stress and Postpartum Depression in Postpartum Women (발 반사마사지가 산욕기 산모의 피로, 스트레스, 산후 우울에 미치는 효과)

  • Purpose: To identify the effects of foot reflexology massage on fatigue, stress and depression of postpartum women. Methods: A nonequivalent control group pre-post design was used. A total of 70 women in a postpartum care center were recruited and were assigned to the experimental group (35) or control group (35). Foot reflexology massage was provided to the experimental group once a day for three days. Data were collected before and after the intervention program which was carried out from December, 2013 to February, 2014. Data were analyzed using Chi-square test, Fisher's exact test, and t-test. Results: The level of fatigue in the experimental group was significantly lower than the control group (t= - 2.74, p=.008). The level of cortisol in the urine of women in the experimental group was significantly lower than the control group (t= - 2.19, p=.032). The level of depression in the experimental group was significantly lower than the control group (t= - 3.00, p=.004). Conclusion: The results show that the foot reflexology massage is an effective nursing intervention to relieve fatigue, stress, and depression for postpartum women.

The Predictors of Postpartum Depression (산후우울 예측요인)

  • Purpose: This study was designed to determine the predictors of postpartum depression. Method: One hundred-sixty one women within one year after delivery from one public health center located in the northern area of Seoul were used in this study. The instruments were a survey of general characteristics, the Edinburgh Postnatal Depression Scale, recent life events index, perceived social support from family, Quality of marriage index, parenting stress index, and Rosenberg's self-esteem inventory. Data was analysed using descriptive statistics, Pearson correlation coefficients, and logistic regression. Result: The average item score of the EPDS was 6.67. 12.4% of respondents, who scored above a threshold 12, were likely to be suffering from a depression of varying severity. The fitness of the model for explaining postpartum depression from six variables, plan for pregnancy, family support, quality of marital relation, perceived social support, life events, childcare stress, and self-esteem, was statistically significant and the predictive power of these variables was 90.9%. The significant predictors of postpartum depression were family support and child care stress. Conclusion: Further research is needed to identify the prevalence rate of postpartum depression using more reliable sampling methods from a large general population. Nursing interventions need to be developed for promoting family support and reducing childcare stress.

Transferred newborn from oriental hospital postpartum care center; symptoms and high risk factor (한방병원 산후조리센터에서 전원된 신생아에 대한 연구)

  • Objective : To find risk factor and symptoms for transferred newborn from postpartum care center Method : We performed a follow-up study using medical record. Subjects comprised 188 neonates(include 4 twins) and 184 women in childbed who were hospitalized in oriental postpartum care center from January, 1, 2002 to December, 31, 2002. Result: Transferred newborn was total 24, N.V.D. 17, C.S. 7, male and female were 12 separately. The symptom of transfer was fever(16), diarrhea(4), jaundice(3), convulsion(1) Transferred number and rate was male 12(10.7%), female 12(15.8%). In delivery method, total normal vaginal delivery(N.V.D.) was 136 and transferred 17(12.5%), total cesarean section(C.S.) was 52 and transferred 7(13.5%). Transferred rank was N.V.D. female(17.9%). C.S. male(15.6%), C.S. female(10%), N.V.D.(8.6%). Of the 188 neonates. immature infants were 2 and transferred 1(50%), term infants were 184, transferred 23(12.5%), post-term infants were 2 and transferred none(0%). In age of women in childbed, transferred ratio was 15.4%(over 35), 15.2%(25-29), 10.5%(30-34). In fever,. C.S. infants were more transferred than N.V.D. and male infant had a tendency to transfer in later period of hospitalization and female in early period. It seems that transfer was related to abortion. In diarrhea, all was N.V.D. and their diagnosis was rotavirus. The average age of women in childbed was yonger(28.5). In jaundice, all was 1st, and average birth weight was lighter(3153g). The average age of women in childbed was older(30). In convulsion, birth weight was lighter(3153g), and age of women in childbed was older(30). Conclusion : There's any relation with sex and delivery method, birth weight and mother's age in transfer. But we cannot find any statistically significance. Further prospective studies are needed to find risk factors in transfer.

Postpartum Depression and Maternal Role Confidence, Parenting Stress, and Infant Temperament in Mothers of Young Infants (영아기 어머니의 산후 우울 정도와 모성역할 자신감, 양육 스트레스, 영아기질에 관한 연구)

  • Purpose: The purposes of this descriptive survey study were to describe postpartum depression level and to identify the influence of postpartum depression on maternal role confidence, parenting stress and infant temperament, in order to develop a base for nursing intervention programs for mothers who have postpartum depression. Method: Data were collected from 183 mothers of infants who visited one public health center in Goyang city. The instrument used for this study was a self-report questionnaire to identify the postpartum depression (BDI), maternal role confidence, parenting stress (PSI) and infant temperament (WBL). Results: Of the mothers 42.1% were in the normal range for postpartum depression, 33.3% in the mild group and 24.6% were in the moderate to severe group. There were significant differences in parenting stress, and infant temperament according to level of postpartum depression. There were correlations between maternal role confidence (r=-0.13), infant temperament (r=-0.20), parenting stress (r=0.51) and postpartum depression, and weak negative correlation between parenting stress and infant temperament (r=-0.30). Conclusion: Postpartum depression in the infant period when the mother's role is very important and the effect is not only on the mother's role but also on the infant's growth. It is essential to assess and provide immediate care to mothers who have postpartum depression.

  • The aim of this study was to identify the performance and requirements of the visiting nursing care using Omaha system in public health center. The highest performance were 'personal hygiene', 'pain', 'medication regimen', 'nutrition', 'physical activity', 'sanitation', 'sleep and rest patterns', 'oral hygiene', 'mental health' in order. The lowest performance were 'sexuality', 'postpartum', 'income', 'family planning', 'pregnancy', 'spirituality', 'abuse', 'reproductive function', 'neglect' in order. Problems such as 'postpartum', 'pregnancy' and 'family planning' need to strengthen the role of visiting nurses according to the region. this result will be the basis for visitung nursing care.

Development and Evaluation of Breastfeeding Promotion Program for Mothers with Breast Engorgement following Cesarean Birth (유방울혈이 있는 제왕절개분만 산모를 위한 모유수유 증진 프로그램의 개발 및 평가)

  • Purpose: This study was to develop a breastfeeding promotion program and to test effects of the program on levels of breast discomfort, breast size, sodium in breast milk, and type of feeding in mothers with breast engorgement following cesarean birth. Methods: A non-synchronized non-equivalent control group pretest-posttest design was used in this study. The participants were 70 postpartum mothers who were admitted to a postpartum care center and experienced breast engorgement following cesarean birth. The planned nursing intervention was the breastfeeding promotion program consisting of breast massage and 1:1 breastfeeding education, counseling, and support focusing on individualized problem solving provided for 10 days. Fifty-three women completed the program (experimental group 26, control group, 27). Measurements were level of breast discomfort, breast size, sodium in breast milk and type of feeding at pre and posttest. Results: Women who participated in the program experienced lower scores for breast discomfort, greater decrease in breast size, lower levels of sodium in breast milk, and practiced breastfeeding more than those in the control group. Conclusion: Results indicate that this breastfeeding promotion program is effective in reducing breast engorgement and improving breastfeeding practices, and is therefore recommended to enhance breastfeeding promotion practice in postpartum care centers.

Comparisons of Maternal Perceptions between Rooming-in and Non Rooming-in Postpartum Women (신생아간호 운영체계에 대한 산모의 인식과 이용에 따른 차이)

  • Purpose: The purpose of this study was to compare maternal perceptions between two groups of postpartum women, women who chose to have their babies room-in and women who did not (non rooming-in group). Methods: Data collection was conducted in 37 hospitals from August 10 to September 20, 2008. The participants were 209 mothers opting for rooming-in and 128 mothers for non rooming-in. The women completed a questionnaire which included the Edinburgh postpartum depression scale, maternal attachment inventory, and postpartum self-evaluation questionnaire. Results: There were significant differences in education level, income and antenatal education between the rooming-in and non rooming-in groups. The rooming-in group also showed higher levels of satisfaction with medical services. More important, the rate of breast feeding for the rooming-in group was higher than for the non rooming-in group. There was no difference between the two groups for postpartum depression, maternal attachment and mothers' confidence with infant care tasks. Conclusion: These findings suggest that rooming-in system has more advantages compared to non rooming-in and that it can help to promote breast feeding. The authors recommend that the rooming-in system be expanded nationwide. The results of this research can be used to assist the development of future rooming-in system expansion strategy.
