안태음(安胎飮) 가미방(加味方)과 한방치료를 병행한 임신 중 교통사고로 인한 경추 및 요추부 통증 환자 치험 5례 (Effects of Korean Medicinal Treatments Including Antaeeum-gamibang on Posterior Neck Pain and Lower Back Pain Caused by Traffic Accidents During Pregnancy: Five Case Reports)

  • Objectives: This study was performed to assess the effectiveness of Korean medicinal treatments including Antaeeum-gamibang on posterior neck pain and lower back pain caused by traffic accidents during pregnancy. Methods: Patients were treated by Antaeeum-gamibang, acupuncture and Chuna manual therapy. To measure treatment outcomes, Numerical rating scale (NRS), Oswestry disability index (ODI), Neck disability index (NDI), European Quality of Life-5 Dimensions (EQ-5d), and range of motion (ROM) were used. Results: After treatment, the average NRS of posterior neck pain significantly decreased from $4.20{\pm}1.30$ to $1.80{\pm}0.45$ and the average NRS of lower back pain significantly decreased from $6.40{\pm}1.14$ to $2.20{\pm}0.84$. ODI significantly decreased from $41.42{\pm}17.79$ to $28.29{\pm}14.69$, NDI significantly decreased from $34.93{\pm}9.85$ to $20.27{\pm}8.96$. EQ-5d and ROM improved at all cases. Conclusions: Korean Medicinal treatments including Antaeeum-gamibang may be effective in the treatment of posterior neck pain and lower back pain caused by traffic accidents during pregnancy. However, more case reports and studies are required to identify exactly what treatment is more efficient.

후대뇌동맥 원위부에 발생한 거대동맥류 1례 - 증례보고 - (A Case of the Giant Aneurysm in the Distal Portion of the Posterior Cerebral Artery - A Case Report -)

  • The aneurysm arising from the posterior cerebral artery is relatively uncommon, and has been reported in 0.7- 2.2% of whole cerebral aneurysm. Moreover, only a few cases of the giant aneurysm over 2.5cm in diameter arising from the distal portion of the posterior cerebral artery have been reported. Neurologic complications may occur after surgical treatment of this aneurysm due to difficult surgical approach and neurovascular complexity around the brain stem. Authors experienced a case of the thrombosed giant aneurysm with approximately 2.7cm in diameter arising from the P2 segment of the left posterior cerebral artery. Complete removal of the thrombus in the aneurysmal sac and direct neck clipping was performed via left subtemporal approach.

두경부에 발생한 모기질종(Pilomatricoma) 4예 (4 Cases of Head and Neck Philomatricoma)

  • Pilomatricoma is a benign neoplasm of the hair follicle origin which most common occur in the head and neck, particularly in the cheek and preauricular region. This neoplasm is superficially located and stony, hard consistency on palpation and sometimes shows reddish or bluish discoloration on the overlying skin. The diagnosis is confirmed by histopathologic examination. Radiologic evaluation is little diagnostic and fine needle aspiration biopsy often misdiagnose because both tissues from basaloid cell component and shadow cell component should be obtained for correct diagnosis. And Otolaryngologist should consider the possibility of philomatricoma when childhood or early adulthood persion visits with hard, supferficial mass on the parotid or cheek area. The treatment of choice is surgical excision with clear resection margin. In many instances, overlying skin excision is needed. We report 4 cases of philomatricoma arising from preauricular, cheek and posterior neck.

  • Maxillary cancer is usually detected late, and the majority of patients have advanced($T_3\;or\;T_4$) diseases at the first diagnosis. It invades outside the maxillary antrum, superiorly the orbit, ethmoid sinus and the anterior cranial base, anteriorly the facial skin. If the cancer extends through the posterior antral wall, the pterygoid plates, pterygoid muscles and infratemporal fossa are to be involved that make the conventional maxillectomy impossible to remove all the involved structures in infratemporal fossa completely. So, more extensive surgical apprdoach is necessary. We report surgical experience using infratemporal fossa approach(lateral facial approach) in four cases of maxillary cancer and one case of hard palate cancer which extends through the posterior antral wall and involving pterygoid muscles, pterygoid plates and temporalis muscle.

경추견인법 (Cervical Traction)

  • Cervical traction effects its benefits by immobilizing the neck when it is used in a continuous manner from a reclining position. when used intermittently traction functions by elongating the neck and straightening the cervical Lordosis. This position of slight flexion opens the posterior articulations, widens the intervetebral foramina, disengages the facet surfaces, and elongates the posterior muscular tissues and Ligaments. The duration of traction is arbitary but the amount of traction is that which is tolerated by the patient and benefits the patient's problem. Application of traction in slingh flexion accomplishes the same separation with Less force and thus with Less discomfort experienced by the patient. But the effect of traction on the disks is debated.

비인강암의 임상적 특성 (Clinical Characteristics of Nasopharyngeal Cancer)

  • We studied the clinical charcteristics of 265 cases of nasopharyngeal carcinomas diagnosed at Korea Cancer Center Hospital over a span of 8 years from Jan. 1987. Male were 187 and Female were 78 and male: female ratio was 2.4 : 1. The age distribution ranged from 2nd decade to 9th decade evenly and mean age was 46.1 years old. Histopathologically squamous cell carcinoma (WHO type 1, 2, 60.8%) were 161 cases and undifferentiated carcinoma (WHO type 3, 39.2%) were 104 cases. Main symptoms and signs were neck mass 199 cases (75.1%), ear symptoms 126(47.5%), nasal symptom 101 (38.1%). The distribution of anatomical subsites were posterior wall 75 (24.7%), lateral wall 175 (72.8%), Inferior wall 15 (2.5%). Tumor staging by AJCC classification, 1992, distributed with stage I 3 cases (1.1%), stage II 5 cases (1.9%), stage III 24 cases (9.1%), stage IV 233 cases (87.9%).

설배부에 발생한 국한성 혈관각화종 1예 (A Case of Angiokeratoma Circumscriptum of the Dorsal Tongue)

  • Angiokeratomas represent vascular lesions and are histologically characterized by superficial dermal vascular ectasia with overlying epidermal hyperkeratosis. Angiokeratoma circumscriptum is one of five types in the group of the angiokeratomas. The lesion is present from birth but may appear in childhood or adulthood. Angiokeratoma circumscriptum is typically hyperkeratotic papules or nodules situated unilaterally on the foot or distal aspect of the leg, but can occur elsewhere on the body. Isolated tongue involvement is very rare. We report a 12-year-old boy with angiokeratoma circumscriptum of the posterior dorsal tongue with a review of the literatures.

박동성 이명으로 발현된 외이도 혈관종 1예 (A Case of Hemangioma in the External Auditory Canal with Pulsatile Tinnitus)

  • Hemangiomas are relatively common in the head and neck region, but their occurrence in the temporal bone are extremely rare. The hemangioma of the external auditory canal (EAC) is a rare otologic entity as only 22 cases of hemangioma of the EAC have been reported worldwide. Here we report a patient presenting with pulsatile tinnitus, which turned out to be caused by capillary hemangioma arising from the posterior medial portion of external auditory canal touching the tympanic membrane. Surgical excision of the tumor was successful. The clinical manifestations and management of benign vascular lesions of the EAC are discussed with a review of literature.

경부에 발생한 점액성 지방육종 1예 (A Case of Myxoid Liposarcoma of Neck)

  • Liposarcoma is a very rare malignant tumor affecting the head and neck area. Since it is an invasive and life-threatening disease, active treatments is necessary. However, differential it from lipoma, its corresponding benign counterpart, may be difficult. Herein, we report a rare case of posterior neck myxoid liposarcoma in a 72-year-old man who was initially misdiagnosed with lipoma, including a literature review of differential diagnosis between lipoma and liposarcoma.
