Role of concomitant percutaneous pie crusting and local corticosteroid injection in lateral epicondylitis: a prospective, case control study

  • Background: Lateral epicondylitis is an increasingly debilitating condition in working population. Evidence for conservative treatment modalities has been inconclusive. Percutaneous pie crusting of the common extensor origin at the lateral epicondyle at the time of local corticosteroid injection (CSI) has been proposed sparsely. The objective of this study was to analyze if concomitant CSI and pie-crusting of the common extensor origin provides better outcome than CSI alone in lateral epicondylitis. Methods: This case-control study on 236 patients was conducted at a single center between January 1, 2020, and May 31, 2022. Patients were divided into two groups (n=118 each) based on their preference. Group A underwent CSI alone and group B underwent pie crusting along with CSI. The clinical and functional outcomes of all patients were evaluated at 2, 4, 6, and 12-week post-procedure using the visual analog scale (VAS) and Nirschl score. The mean time for return to daily activities was also compared. Results: Both groups showed significant improvement in post-procedure outcome at successive follow-ups on intragroup longitudinal analysis (VAS: F=558.384 vs. F=1,529.618, Nirschl: F=791.468 vs. F=1,284.951). On intergroup analysis, VAS of group B was superior to that of group A; however, it was statistically significant (P<0.05) only from the 6-week follow-up onwards. Nirschl score of group B was significantly better throughout the period of follow-up (P<0.05). Group B returned to daily activities faster than Group A (6.2±0.44 weeks vs. 7.18±0.76 weeks). Conclusions: Concomitant pie crusting with CSI is recommended for lateral epicondylitis as it provides significantly better results than CSI alone.

편측성 교근비대 환자에서 편측 보툴리눔 독소 주사 후 안모의 변화 (The Change of Facial Contouring after Unilateral Injection of Botulinum Toxin in Unilateral Masseter Hypertrophy Patients)

  • 교근비대 환자들은 그들의 각진 턱을 심미적으로 개선시키기 위해 교근 두께를 줄이기를 희망한다. 과거엔 수술적인 방법이 널리 알려져 왔지만 최근 양측으로 주사하는 보톡스 시술이 수술에 비해 덜 침습적이기에 그 대안으로 관심이 대두되고 있다. 이 연구의 목적은 기존의 양측성 교근비대에서 양측에 동량을 주사하는 방법과 달리 편측성 교근비대 환자에서 보툴리눔 독소를 편측으로만 주입 후 삼차원 레이저스캐너를 이용하여 안모의 부피변화를 측정하고자 하였다. 삼차원 레이저스캐너를 이용하여 술전에 편측성 교근비대 환자 10명의 하안모 부피를 측정한 후, 편측으로 보툴리눔 A형 독소 주사 25U이 주사되었다. 주사 4주, 8주, 12주 후 같은 방법으로 하안모의 부피를 측정한 뒤 술전의 삼차원 사진과 중첩하여 변화된 부피를 측정하였다. 주사한 쪽의 부피는 술전에 비해 보툴리눔 독소 주사 4주, 8주, 12주 후 현저히 줄어들었으며 통계학적으로 유의미한 결과를 보였다. 반면 주사안한 쪽의 부피는 특기할 변화가 없었다. 따라서 편측성 교근비대 환자에서 편측 보툴리눔 독소 주사 후 주사한 쪽의 부피가 현저히 감소하여 근육성 비대칭 환자에서 편측 보툴리눔 독소 주사가 심미적 개선에 도움을 줄 수 있을 것으로 사료된다.

재발성 호흡기 유두종증의 병변내 시도포비어 치료 (Intralesional Cidofovir Therapy for Recurrent Repiratory Papillomatosis)

  • Background and Objectives: Recurrent respiratory papillomatosis (RRP) is difficult to treat because of its tendency to recur and spread throughout the aerodigestive tract. We aimed to estimate the effect of intralesional injections of cidofovir in patients with RRP. Materials and Method: Within the period from January 2003 to July 2007, 13 patients aged 2 to 61 years were treated with intralesional injections of cidofovir combined with surgical excision of RRP. Cidofovir was injected intralesionaly at a concentration of 5 mg/cc after complete removal of the papilloma with $CO_2$ laser or microdebrider. We evaluated the effect of intralesional cidofovir therapy by comparing pre-treatment mean interval of recurrence with post-treatment interval of recurrence. Results: Of 13 patients, two patients showed complete response during follow up period and four patients showed partial response. Seven patients did not respond to cidofovir at all. Mean pre-treatment mean interval of recurrence was 9 months and mean post-treatment interval of recurrence was 13.1 months (p=0.039). There was a statistical significance between the injected dose of cidofovir and post-treatment interval of recurrence (p=0.009). There were no local or systemic side effects caused by cidofovir. Conculsion : Intralesional injection of cidofovir seems to have a potential of a safe and effective adjuvant therapy of RRP. There was a positive correlation between the injected dose of cidofovir and patient clinical outcomes so that administration of higher doses and more frequency of injections should be needed to reduce recurrence. Further study regarding injection therapy regimen for RRP is required.

  • 모사추진제로 초저온 액체질소와 기체 질소를 사용하여 핀틀의 개도 및 모사추진제 공급 조건이 초저온 분무에 끼치는 영향을 정성적으로 연구하였다. 액체 공급압력을 고정하고, 핀틀의 개도와 기체 질소의 공급 압력을 변화시켜 실험을 진행하였다. 이미지 가시화는 CCD 카메라를 이용한 Shadowgraph 기법이 사용되었으며, 이미지 후처리를 통해 실험 조건에 따른 분무의 차이를 비교하였다. 액체 질소 단독 분사의 경우 개도가 늘어날 때, 분사속도가 줄어들고 챔버에서의 벤트 압력은 커져 액체질소 시트가 중앙으로 모이는 현상이 관측되었다. 액체질소/기체질소 분무의 경우 이미지 후처리를 통해 서로 실험 조건이 다르지만 Shadowgraph 이미지가 유사하게 나타나는 경우도 분석할 수 있음을 보였다.

Effects of Injection Laryngoplasty with Hyaluronic Acid in Patients with Vocal Fold Paralysis

  • Objectives: The purpose of this study was to explore the effects of injection laryngoplasty (IL) with hyaluronic acid in patients with vocal fold paralysis (VFP). Methods: A total of 50 patients with VFP participated in this study. Pre- and post-IL assessments were performed, which included analyzing the sustained vowel /a/ phonation, and the patient reading 1 Korean sentence from the "Walk" passage that comprised 25 syllables in 10 words. To investigate the effect of IL on vocal fold function, acoustic analysis (acoustic voice quality index, cepstral peak prominence, maximum phonation time, speaking fundamental frequency) was conducted and auditory-perceptual (grade and overall severity), visual judgment (gap), and self-questionnaire (voice handicap index-10) assessments were performed. Results: The patients with VFP showed statistically significant differences between pre-and post-IL assessments for acoustic and auditory-perception, visual judgment, and self-questionnaire assessments. Conclusion: The patients with VFP showed positive change in vocal fold function between pre- and post-IL measurements. The findings showed that IL with hyaluronic acid is an effective method to improve vocal fold function in patients with VFP.

Role of vitamin C and rectal indomethacin in preventing and alleviating post-endoscopic retrograde cholangiopancreatography pancreatitis: a clinical study

  • Background/Aims: This study aimed to determine whether vitamin C in addition to indomethacin decreases the occurrence and severity of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) occurrence and severity. Methods: This randomized clinical trial included patients undergoing ERCP. The participants were administered either rectal indomethacin (100 mg) plus an injection of vitamin C (500 mg) or rectal indomethacin (100 mg) alone just before ERCP. The primary outcomes were PEP occurrence and severity. The secondary amylase and lipase levels were determined after 24 hours. Results: A total of 344 patients completed the study. Based on intention-to-treat analysis, the PEP rates were 9.9% for indomethacin plus vitamin C plus indomethacin and 15.7% for indomethacin alone. Regarding the per-protocol analysis, the PEP rates were 9.7% and 15.7% in the combination and indomethacin arms, respectively. There was a remarkable difference between the two arms in PEP occurrence and severity on intention-to-treat and per-protocol analyses (p=0.034 and p=0.031, respectively). The post-ERCP lipase and amylase concentrations were lower in the combination arm than in the indomethacin alone arm (p=0.034 and p=0.029, respectively). Conclusions: Vitamin C injection in addition to rectal indomethacin reduced PEP occurrence and severity.

뇌졸중 후 우울증의 약침치료에 대한 중의학 임상논문 연구동향 (Trend Review of Traditional Chinese Medicine Studies on Pharmacopuncture Treatment for Post-Stroke Depression)

  • Objectives: To provide clinical evidence of pharmacopuncture treatment for post-stroke depression by reviewing randomized controlled trials on Traditional Chinese medicine pharmacopuncture for post-stroke depression. Methods: Randomized controlled trials on pharmacopuncture treatment for post-stroke depression were searched from the China National Knowledge Infrastructure (CNKI). Literature searches of English and Chinese databases were performed. Selected literature was assessed with the Jadad scale. Results: Twelve papers were selected from 20 studies. The following results were obtained: 1) Pharmacopuncture was used for post-stroke depression; 2) Dengzhan-Xixin was the most used injection medicine for post-stroke depression; 3) BaiHui (GV20) was the most used in pharmacopuncture treatment; 4) Based on the Hamilton Depression Rating Scale (HAMD), it was confirmed that all studies using pharmacopunture had significant efficacy. Conclusions: For the treatment of post-stroke depression, Dengzhans-Xixin was commonly used as an injectable medication. Additional research studies on pharmacopuncture including its practical use are needed.

녹내장 환자에서 성상신경절 차단에 의한 안압의 변화 (The Changes of Intraocular Pressure by Stellate Ganglion Block in Glaucoma Patients)

  • Stellate ganglion block was performed in 15 patients with intractable glaucoma by injection of 8 ml of 1y lidocaine. The followings are the changes of intraocular pressure(mmHg): Pre -block $32.26{\pm}4.44$ 10 minutes after block $36.80{\pm}4.60$ 20 minutes after block $32.61{\pm}4.41$ 30 minutes after block $28.87{\pm}4.30$ 60 minutes after block $24.73{\pm}4.37$ 120 minutes after block $26.87{\pm}3.18$ 180 minutes after block $31.40{\pm}3.76$ The pressure rose up to 10 minutes post block and significant pressure descent was noticed from 30 minutes post-block. Maximum descent was seen by 60 minutes post-block and pre-block pressure was restored by 180 minutes post-block. For the purpose of cheeking the treatment effect, similar blocks were performed in 15 patients, one time daily for 5~8 days 3 times every othor day.

Bronchoscopic Ethanolamine Injection Therapy in Patients with Persistent Air Leak from Chest Tube Drainage

  • Background: Chest tube drainage (CTD) is an indication for the treatment of pneumothorax, hemothroax and is used after a thoracic surgery. But, in the case of incomplete lung expansion, and/or persistent air leak from CTD, medical or surgical thoracoscopy or, if that is unavailable, limited thoracotomy, should be considered. We evaluate the efficacy of bronchoscopic injection of ethanolamine to control the persistent air leak in patients with CTD. Methods: Patients who had persistent or prolonged air leak from CTD were included, consecutively. We directly injected 1.0 mL solution of 5% ethanolamine oleate into a subsegmental or its distal bronchus, where it is a probable air leakage site, 1 to 21 times using an injection needle through a fiberoptic bronchoscope. Results: A total of 15 patients were enrolled; 14 cases of spontaneous pneumothorax [idiopathic 9, chronic obstructive pulmonary disease (COPD) 3, post-tuberculosis 2] and one case of empyema associated with broncho-pleural fistula. Of these, five were patients with persistent air leak from CTD, just after a surgical therapy, wedge resection with plication for blebs or bullae. With an ethanolamine injection therapy, 12 were successful but three (idiopathic, COPD and post-tuberculosis) failed, and were followed by a surgery (2 cases) or pleurodesis (1 case). Some adverse reactions, such as fever, chest pain and increased radiographic opacities occurred transiently, but resolved without any further events. With success, the time from the procedure to discharge was about 3 days (median). Conclusion: Bronchoscopic ethanolamine injection therapy may be partially useful in controlling air leakage, and reducing the hospital stay in patients with persistent air leak from CTD.

Temporary blindness caused by corneal edema after a local anesthetic injection in the eyebrow region: a case report

  • Plastic surgery around the eyes is usually performed under local anesthesia, using a mixture of lidocaine and epinephrine. Blindness is a rare but devastating complication after the injection of local anesthesia in this region. Most cases reported to date have been caused by occlusion of the ophthalmic artery or central retinal artery. In this case report, however, we present a highly unusual case of blindness caused by corneal edema after a local anesthetic injection. A patient visited the emergency room with a laceration on the eyebrow, and local anesthesia was injected before suturing. Immediately after the injection, severe corneal edema developed, making it impossible to observe the structures in the anterior chamber in detail or check the light reflex and visual acuity of the naked eye. An antibiotic (moxifloxacin hydrochloride) and high-concentration steroid eyedrops were promptly applied. High-concentration steroids were also administered orally. On day 13 post-injury, the visual acuity of the naked eye improved to 1.0, and no recurrence of corneal lesions was observed. Although the cause of corneal edema after the local injection could not be conclusively identified, we hope that this report will help raise clinicians' awareness of this complication and appropriate treatment methods.
