Horizontal change of philtrum after orthognathic surgery in patients with facial asymmetry

  • Background: Soft tissue asymmetry such as lip canting or deviation of the philtrum is an important influencing factor for unbalanced facial appearance. Lip canting could be improved by the correction of the occlusal canting or positional change of the mentum. Although there are many studies about changes of lip canting, however, postoperative changes of philtrum deviation have not been yet reported. In this study, we investigate the positional change of the philtrum after orthognathic surgery and influencing factors. Methods: Positional change of the philtrum was evaluated in 41 patients with facial asymmetry who underwent bimaxillary surgery, in relation to other anatomical soft tissue landmarks using a frontal clinical photo. The surgical movement of the maxillary and mandibular dental midline and canting were measured in postero-anterior cephalogram before and 1 day after surgery. The same procedure was repeated in patients with more than 1.5 mm perioperative change of the mandibular dental midline after bimaxillary surgery. Results: Maxillary dental midline shifting and canting correction did not have a significant correlation with lateral movement of the philtrum midline. However, the mandibular shift had a statistically significant correlation with a lateral movement of the philtrum (p < 0.05) as well as other linear parameters and angle values. Conclusion: The horizontal change of the philtrum is influenced by lateral mandibular movement in patients with facial asymmetry, rather than maxillary lateral movement.

전치부 개방교합을 동반한 측두하악장애가 연하에 미치는 영향 (The Swallowing Pattern in TMD Patients with Anterior Open Bite)

  • The present study explored the influence of temporomandibular disorders(TMD) with anterior open bite on swallowing. Fifteen subjects with anterior open bite(group A), 9 subjects with anterior open bite and TMD(group B), and 24 subjects without malocclusion or TMD symptoms (group C) were included. BioPAK system(Bioresearch Inc., Milwaukee, WI, USA) was used to record the muscle activities of anterior temporal, masseter, sternocleidomastoid(SCM) and anterior digastric muscles during maximum clenching and swallowing. Positional change of the mandible during swallowing was also recorded using the same system. The obtained results were as follows: 1. Group A, B, and C did not show significant differences each other in the muscle activity of resting position in most of head and neck muscles. 2. Group B showed significantly lower muscle activity in maximum clenching than group C in anterior temporal(p<0.01), masseter(p<0.05), SCM(p<0.05) and digastric muscles(p<0.05). 3. Group A showed significantly lower muscle activity during swallowing than group C in anterior temporal and masseter muscles(p<0.01). Group B showed significantly lower muscle activity during swallowing than group C in anterior temporal(p<0.01), masseter(p<0.01), and SCM muscles(p<0.05). 4. Group A and B showed increased positional change of the mandible during swallowing measured from vertical, anteroposterior and lateral aspects, and prolonged swallowing(p<0.05). 5. After given instructions for normal swallowing pattern, group A and B showed increased muscle activity during swallowing in anterior temporal, masseter and SCM muscles(p<0.01). Positional change of the mandible and time elapsed for swallowing also decreased after the instruction(p<0.01). 6. Occlusal splint did not change the muscle activity during swallowing. However, vertical change(p<0.01) and swallowing time(p<0.05) were decreased with splint.

양성돌발성 체위현훈의 치료 (Treatment of Benign Paroxysmal Positional Vertigo)

  • Benign paroxysmal positional vertigo (BPPV) is one of the most common clinical entities encountered in a dizziness clinic. Treatment of this disease, canalith repositioning procedures, have been reported to be successful in 44-90%. Treatment requires only one treatment visit in most patients. However, there are significant numbers of patients who require multiple treatment visits for relief. The goal of this study is to identify variables that may be associated with these difficult to treat cases. Retrospective review was performed for the patients diagnosed as BPPV at St. Carollo dizziness center. Variables for statistical analysis included age, sex, involved canal, presence of bilateral disease, presence of recent head trauma, presence of chronic otitis media, history of middle ear surgery, history of otologic surgery, unilateral vestibular loss, underlying disease such as hypertension or diabetes, change of involved canal during treatment course and number of treatment visits. Two hundred patients with BPPV who received treatment were identified from JAN. 2006 to JUN. 2007. 87% required one treatment visit, 5% required second treatment visit, and 95% were successfully treated after three treatment visits. Variables such as bilateral disease, post-traumatic BPPV, duration of symptom before treatment and change of involved canal during treatment were significantly related with number of treatments. Patient's with bilateral BPPV or with recent head trauma or longer duration of symptom are more likely to require multiple visits for canalith repositioning.

  • Lateral canal benign paroxysmal vertigo (BPPV) causing dizziness is a common cause is not found while continuing to appeal for vertigo is a typical disease. It is characterized by acute stand up, brief and rotatory vertigo attacks provoked by change in head position. Treatment requires only one treatment visit in most patients. However, there are significant numbers of patients who require multiple treatment visits for relief. The purpose of this study benign paroxysmal positional vertigo treatment of type affect is to analyze the cause. Dizziness and vertigo patient's in patients admitted to the dizziness center of lateral canal benign paroxysmal positional vertigo were classified. In patients with lateral canal benign paroxysmal positional vertigo and accompanying lateral 15 treatment affects disease were investigated. March 2008 to November 2010 lateral canal benign paroxysmal positional vertigo 166 people cure rate of patients was investigated. First time the success rate of 74.1%, twice times the success rate of 12.0%, three times the success rate of 9.6%, more than three times the success rate was 4.2%. Affecting factor treatment of benign paroxysmal positional vertigo in post-traumatic, medicine disease, headache, cerebral infarction, small vessel disease, vestibulopathy, (p<0.05). Statistical analysis using SPSS (version 12K) in coefficient measure through descriptive statistical of cross table.

  • Benign Paroxysmal Positional Vertigo(BPPV) is a common disorder that peripheral vertigo in clinic, it is provoked Paroxysmal vertigo according to positional change. A man, aged 75, suffered from attacks of vertigo when get out of bed, He complained vertigo, nausea and walking trouble So he visited our clinic Based upon the case history and position-provoked vertigo with characteristic findings on the Dix-Hallpike maneuver, we diagnosed as BPPV. Through only Shang-San-Huang(上三黃) of Dong Shi Xue(董氏穴) acupuncture therapy symptoms were vanished completely. This study was showed effect that operating only Dong-Shi acupuncture therapy without maneuver therapy and the other therapies to patient can not tolerate the Dix-Hallpike Maneuver. We need additional researches with methodological improvement and many clinical cases.

악관절증에서 과두위변화에 따른 골변화양상 분석 (Radiographic study of bone deformans on charged condylar head position in TMJ arthrosis)

  • The author analyzed the morphologic changes of bone structure from 848 radiograph is (424 joints) of 212 patients with temporomandibular joint arthrosis, which were obtained by the oblque-Iateral transcranial projection and orthopantomography. The interelation of the bone changes and condylar head positions the results were as follows: 1. In the 212 patients with TMJ arthrosis, 210 patients(99.05%) show the condylar positional changes. Among them, 187 patients 989.05%) show the bone changes. 2. In TMJ arthrosis patients with bone changes, 10% patients(57.75%) show both the condylar positional changes and bone changes. 66 patients( %) show the condylar psoitional changes bilaterally and bone changes unilaterally. On the other hand, 11 patients (5.88%) show the condylar positional changes unilaterally and bone change bilaterally. 3. The bone changes in the TMJ arthrosis patients with the condylar positional changes were as follows: There were the flatlening of articular surface in 103 cases (26.55%) the erosion in 99cases 925.57%), and the erosion in 88 cases (22.68%). There were not much differences among the three types of bone changes. And the deformity in 70 cases (18.04%), the sclerosis in 22 cases(5.67%), the marginal protiferation in 6 cases(1.55%) were seen. 4. The regions of bone changes in TMJ arthrosis patients with condylar positional changes were as follows: They occurred at the condyle head(51.04%), the articular eminence(39.20%) and the articular fossa(9.60%) in that order. The condylar positional changes and bone changes according to the regions were as follows: a) In the bone changes at the condyle head, the flatteming (34.63%) was a most frequent finding and the deformity(27.63%) the erosion(24.32%) in the order. In the condylar positional changes, the downward positioning of condyle(41.44%) was a most frequent finding in the mouth closed state and the restricted movement within the articular fossa(35.46%) in the mouth open state. b) In the bone changes at the articular eminence, the eburnation(33.26%) was a most frequent finding and the flatteming(31.16%), the erosion(28.37%) in that order. In the condylar positional changes, the downward positionirg of condyle(39.81%) was a most frequent finding in the mouth closed state and the restricted movement within the articular fossa(24.77%) in the mouth open state. c) In the bone changes at the articular fossa, the eburnation(72.90%) was amost frequent finding and theerosion(17.76%), the sclerosis(9.35%) in that arder. In the condylar positional changes, the downward positionirg of condyle(41.5%) was a most frequent finding in the mouth closed state and the mormal positionirg of condyle(27.78%) in the mouth open state.

Mixed-reality simulation for orthognathic surgery

  • Background: Mandibular motion tracking system (ManMoS) has been developed for orthognathic surgery. This article aimed to introduce the ManMoS and to examine the accuracy of this system. Methods: Skeletal and dental models are reconstructed in a virtual space from the DICOM data of three-dimensional computed tomography (3D-CT) recording and the STL data of 3D scanning, respectively. The ManMoS uniquely integrates the virtual dento-skeletal model with the real motion of the dental cast mounted on the simulator, using the reference splint. Positional change of the dental cast is tracked by using the 3D motion tracking equipment and reflects on the jaw position of the virtual model in real time, generating the mixed-reality surgical simulation. ManMoS was applied for two clinical cases having a facial asymmetry. In order to assess the accuracy of the ManMoS, the positional change of the lower dental arch was compared between the virtual and real models. Results: With the measurement data of the real lower dental cast as a reference, measurement error for the whole simulation system was less than 0.32 mm. In ManMoS, the skeletal and dental asymmetries were adequately diagnosed in three dimensions. Jaw repositioning was simulated with priority given to the skeletal correction rather than the occlusal correction. In two cases, facial asymmetry was successfully improved while a normal occlusal relationship was reconstructed. Positional change measured in the virtual model did not differ significantly from that in the real model. Conclusions: It was suggested that the accuracy of the ManMoS was good enough for a clinical use. This surgical simulation system appears to meet clinical demands well and is an important facilitator of communication between orthodontists and surgeons.

골격성 3급 부정교합 환자에서 하악지시상분할골절단술 후 3D CT 영상을 이용한 하악과두 위치변화 분석 (THE EVALUATION OF THE POSITIONAL CHANGE OF THE MANDIBULAR CONDYLE AFTER BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY USING THREE DIMENSIONAL COMPUTED TOMOGRAPHY IN SKELETAL CLASS III PATIENTS)

  • Purpose: This study was performed to evaluate three-dimensional positional change of the condyle using 3D CT after bilateral sagittal split ramus osteotomy (BSSRO) in skeletal class III patients. Patients and methods: Nine patients who underwent BSSRO for mandibular set-back in skeletal class III malocclusion without facial asymmetry were examined. Miniplates were used for the fixation after BSSRO. 3-D CT was taken before, immediately after, and 6 months after undergoing BSSRO. After creating 3D-CT images using V-works $4.0^{TM}$ program, axial plane, coronal plane, & sagittal plane were configured. Three dimensional positional change, from each plane to the condyle, of the nine patients was measured before, immediately after, and 6 months after undergoing BSSRO. Results: 1. The mean value of mandibular set-back for nine mandibular prognathism patients was 7.36 mm (${\pm}\;2.42\;mm$). 2. In the axial view, condyle is rotated inward immediately after BSSRO (p < 0.05), comparing with preoperative but outward 6 months after BSSRO comparing with postoperative (p < 0.05). 3. In the axial view, condyle is moved laterally immediately after BSSRO (p < 0.05), comparing with preoperative but regressed 6 months after BSSRO comparing with preoperative (p > 0.05). 4. In the frontal & coronal view, there is changed immediately after and 6 months after BSSRO, comparing with preoperative but no statistical difference. Conclusion: These results indicate that three-dimensional positional change of the condyle in skeletal class III patients is observed lateral displacement & inward rotation immediate after BSSRO, but the condyle in 6 months after BSSRO tends to regress to preoperative position.

교정치료에 따른 미맹출 하악 제3대구치의 위치 변화에 관한 연구 (A STUDY ON THE PRE-ERUPTIVE POSITIONAL CHANCE OF THE LOWER THIRD MOLAR FOLLOWING ORTHODONTIC TREATMENT)

  • 임상교정에서의 원활한 치아배열을 도모하기 위하여 진단과 치료계획의 수립 방향에 따라 제1소구치 혹은 제2소구치 발치 또는 제2대구치 발치 또는 비발치로 치료가 진행된다. 이때 나타나는 하악제3대구치의 위치적 변화 특히 근원심, 협설측 경사도의 변화도를 알아보고자 163명의 교정환자 (비발치군 77례, 제1소구치 발거군 78례, 제2대구치 발거군 8례)의 치료전후 Pantomogram상을 분석하였다. 결과적으로 하악 제2대구치 발거군에서 치료전후 근원심 경사도의 변화가 컸으며, 하악 제3대구치의 초기 근심 경사도는 치료 전후의 근심경사도와 높은 상관성을 보였으며, 치료전 후의 근심경사도 변화량과도 높은 상관성을 보였다. 비발치군과 제1소구치 발거군에서 치료전후의 협설경사도 변화의 차이가 인정되었다. 치료전후의 협설경사도 변화는 치료전의 협설 경사도, 치료후의 협설경사도, 치료후의 근심경사도, 치료전후의 근심경사도 변화량과 높은 상관성을 보였다.

서해 남부와 남해 서부의 한 정점에서 수온 및 염분과 멸치 어군의 특징의 관련성 시각화 (Acoustic characteristics of Anchovy schools, and visualization of their connection with water temperature and salinity in the Southwestern Sea and the Westsouthern Sea of South Korea)

  • Morphological and positional characteristics of anchovy aggregations, confirmed by trawling, were examined in two locations of the southern part of theWest Sea (T1) and the western side of South Sea (T11) of South Korea. Morphological characteristics (mean length, height and area) of the anchovy aggregations at T1 were smaller than those at T11, however the positional characteristics (distributional depth and bottom depth) of the aggregations at T1 were larger than those at T11. Diverse dataset such as the ship's cruise track, the cruse map, and interpolated three-dimensional-like water temperature were visualized in multiple dimensions. For a comprehensive understanding of the anchovy aggregations within their surrounding circumstances, the interpolated water temperature transferred to the location of anchovy aggregations at both stations were visualized based on geospatial information. Using quantitative investigation, the overall range of change in water temperature and salinity of anchovy aggregations at stations was considerably small. However, the water temperature and salinity of anchovy aggregations at T11 were somewhat higher than those at T1.
