족부 X선 검사에서 주상골 관찰에 용이한 Tibia-Foot angle과 X-ray tube 각도에 대한 연구 (Study of Suitable Angle of Tibia-Foot and X-ray Tube for Navicular in Foot X-ray Examination)

  • 본 연구는 영상의학과에 내원한 족부환자를 대상으로 족부 X선 검사 시 주상골(navicular)의 관찰이 어려운 점을 바탕으로 환자의 position과 X선관 각도의 변화를 주어 어떠한 position과 X선관 각도에서 주상골의 관찰이 용이한지를 알아보고자 하였다. 주상골 관찰을 위해 실험대상자의 position은 Foot AP, internal oblique, external Oblique position의 세 가지로 하였다. T-F angle(Tibia-Foot angle)은 $90^{\circ}$$135^{\circ}$로 정의하였고, X선관 각도는 $0^{\circ}$, $5^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$, $25^{\circ}$로 정의하여 실험한 후 획득한 영상을 비교 평가하였다. 실험결과, Foot AP position에서 T-F angle이 $90^{\circ}$인 경우 X선관 각도가 $15^{\circ}$일 때 설상골과 주상골의 겹침 정도는 3%이었고 블라인드 테스트 결과는 4.89점으로 골절의 판독 용이성이 가장 높았으며, T-F angle이 $135^{\circ}$경우에는 X선관 각도가 $15^{\circ}$일 때 설상골과 주상골의 겹침 정도는 5%이었고 블라인드 테스트 결과는 4.30점으로 판독 용이성이 가장 높았다. Foot internal oblique position에서는 T-F angle이 $90^{\circ}$인 경우 X선관 각도가 $0^{\circ}$일 때 설상골과 주상골의 겹침 정도는 4%이었고 블라인드 테스트 결과는 4.70점으로 가장 높았으며, T-F angle이 $135^{\circ}$경우에는 X선관 각도가 $0^{\circ}$일 때 그 겹침 정도는 5%이었고 블라인드 테스트 결과는 4.55점으로 가장 높게 나타났다. Foot external oblique position에서 T-F angle이 $90^{\circ}$인 경우 X선관 각도가 $15^{\circ}$일 때 설상골과 주상골의 겹침 정도는 4%이었고 블라인드 테스트 결과는 4.85점으로 가장 높았으며, T-F angle이 $135^{\circ}$경우에는 X선관 각도가 $15^{\circ}$일 때 그 겹침 정도는 5%이었고 블라인드 테스트 결과는 4.75점으로 가장 높게 나타났다. 결론적으로, 본 연구를 통하여 X 선 족부검사에서 각 position에 해당하는 주상골 관찰에 용이한 T-F 각도와 X선관 각도를 확인할 수 있었으며, 향후 본 연구 결과를 바탕으로 주상골 골절 판독에 유용한 참고자료가 될 수 있을 것으로 여겨진다.

  • This study was intended to evaluate condyle position and the relationship of condyle position change and post surgical relapse following the sagittal split ramus osteotomy for mandible setback in 25 patients by paired t-test and multiple regression analysis. We used oblique transcranial and cephalometric radiographs taken before operation, immediate after operation, and at least 6 months post operatively. 1. In oblique transcranial view, posterior joint space was decreased immediate after operation and increased 6 months after operation. To compare the measurement before and 6 months after operation, there was no statistically significant change in over all joint spaces(P>0.05). 2. The joint spaces changed under the 0.2mm were 30%, 0.2mm to 1.0mm were 60.7%, above 1.0mm were 9.3%. This result reveals that condyle position was relatively reproduced to pre-operative state. 3. Statistically, the amount of mandible set back didn't influence the post operative relapse(P>0.05). 4. Statistically, the amount of mandible set back didn't influence the condylar displacement(P>0.05), and the amount of joint space change didn't influence the post operative relapse.(P>0.05) The changes in joint space is in the standard tracing error or within the adaptive capacity of the individual, it was too small to influence the stability of surgery.

임플랜트-지대주의 내측연결 시스템에서 하중의 위치 및 경사에 따른 임플랜트 보철의 유한요소 응력분석 (Finite Element Stress Analysis of Implant Prosthesis of Internal Connection System According to Position and Direction of Load)

  • The purpose of this study was to assess the loading distributing characteristics of implant prosthesis of internal connection system(ITI system) according to position and direction of load, under vertical and inclined loading using finite element analysis (FEA). The finite element model of a synOcta implant and a solid abutment with $8^{\circ}$ internal conical joint used by the ITI implant was constructed. The gold crown for mandibular first molar was made on solid abutment. Each three-dimensional finite element model was created with the physical properties of the implant and surrounding bone. This study simulated loads of 200N at the central fossa in a vertical direction (loading condition A), 200N at the outside point of the central fossa with resin filling into screw hole in a vertical direction (loading condition B), 200N at the centric cusp in a $15^{\circ}$ inward oblique direction (loading condition C), 200N at the in a $30^{\circ}$ inward oblique direction (loading condition D) or 200N at the centric cusp in a $30^{\circ}$ outward oblique direction (loading condition E) individually. Von Mises stresses were recorded and compared in the supporting bone, fixture, and abutment. The following results have been made based on this study: 1. Stresses were concentrated mainly at the ridge crest around implant under both vertical and oblique loading but stresses in the cancellous bone were low under both vertical and oblique loading. 2. Bending moments resulting from non-axial loading of dental implants caused stress concentrations on cortical bone. The magnitude of the stress was greater with the oblique loading than with the vertical loading. 3. An offset of the vertical occlusal force in the buccolingual direction relative to the implant axis gave rise to increased bending of the implant. So, the relative positions of the resultant line of force from occlusal contact and the center of rotation seems to be more important. 4. In this internal conical joint, vertical and oblique loads were resisted mainly by the implant-abutment joint at the screw level and by the implant collar. Conclusively, It seems to be more important that how long the distance is from center of rotation of the implant itself to the resultant line of force from occlusal contact (leverage). In a morse taper implant, vertical and oblique loads are resisted mainly by the implant-abutment joint at the screw level and by the implant collar. This type of implant-abutment connection can also distribute forces deeper within the implant and shield the retention screw from excessive loading. Lateral forces are transmitted directly to the walls of the implant and the implant abutment mating bevels, providing greater resistance to interface opening.

Effects of Bridge Exercise with Pelvic Compression Belt on Electromyographic Activities of Selected Lumbopelvic Muscles in Young Adults with Lumbar Instability

  • PURPOSE: This study assessed the effect of the pelvic compression belt on the electromyographic activity of erector spinae (ES), internal oblique (IO), rectus femoris (RF), and biceps femoris (BF) after bridge exercise with pelvic belt compression in subjects with lumbar instability. METHODS: Forty subjects with lumbar instability volunteered for this study. We asked them to perform the bridge exercise while wearing a pelvic belt compression for 30 minutes five times weekly over a six week period. The pelvic compression belt was adjusted just below the anterior superior iliac spines with stabilizing pressure using elastic compression bands during bridge position. Surface electromyographic data were collected from the erector spinae (ES), internal oblique (IO), rectus femoris (RF) and biceps femoris (BF). RESULTS: After the six week intervention, the experimental group improved significantly. Muscle activation significantly decreased in the erector spinae, rectus femoris, and biceps femoris, and increased in the internal oblique muscle in bridge position while wearing a PCB (P <.01). CONCLUSION: Our findings suggest that the bridge exercise with pelvic belt compression is helpful to reduce activation in superficial muscles and lower extremity muscles such as erector spinae (ES), rectus femoris (RF), biceps femoris (BF) and increase activation in deep trunk muscle such as the internal oblique (IO).

Center Body 위치와 Cone 각도에 따른 Center Body Diffuser의 유동특성 및 시동에 대한 수치적 연구 (A Numerical Study of the Flow Characteristics and Starting Pressure of a Center Body Diffuser According to the Center Body Position and Cone Angle)

  • Center Body Diffuser (CBD)의 Center Body (CB) 형상에 의한 유동 특성 및 시동압력을 해석하였다. CB 위치와 Cone 각도의 변경을 통해 초음속 유동 특성 및 시동압력을 비교하였다. CB 위치를 변수로 하여 Diffuser 해석한 결과, 유동의 모멘텀의 형성에 따라 강한 경사충격파의 발생위치가 달라지는 것을 확인하였다. 또한 경사충격파가 발생할 경우, 초음속 유동의 방향이 디퓨저 벽면으로 유도되는 것을 확인하였다. Cone 각도 변화에 따른 경사충격파의 각도 차이로 인하여, CBD의 시동압력이 영향을 받는 것을 확인하였다.

  • Turbulent flow characteristics of a two - dimensional oblique plate impinging jet(OPIJ) were experimentally investigated. The jet mean velocity and turbulent intensity profIles were also measured along the plate. The jet Reynolds numbers(Re, based on the nozzle width)ranged from 10, 000 to 35, 000, the nozzle - to - plate distance(H/B) from 2 to 16, and the oblique angle (a) from 60 to 90 degree. It has been found that the stagnation point shifted toward the minor flow region as the oblique angle decreases and the position of the stagnation point nearly coin¬cided with that of the maximum turbulent intensity.

Effect of Hip Adduction Position on the Vastus Medialis Oblique and Vastus Lateralis During Closed Kinetic Chain Exercise in Sitting Posture

  • Background: Several studies have discussed diverse exercise methods considered to be useful for the selective contraction of the vastus medialis oblique (VMO) muscle for the treatment of patellofemoral pain syndrome. Some studies have reported that exercise methods, including hip adduction, in closed kinetic chain exercises are more effective in terms of the muscle activation of the VMO and the timing of the muscle's initial contraction. We focused on isometric contraction during a closed kinetic chain exercise with hip adduction. Objects: The purpose of this study was to examine muscle activation in the VMO and the vastus lateralis (VL) and the onset time difference between their initial contractions via closed kinetic chain isometric quadriceps femoris exercises including hip adduction. Methods: In total, 36 healthy subjects adopted two hip positions during isometric contraction of the quadriceps femoris in a closed kinetic chain exercise (hip neutral and hip adduction position). Statistical analyses were conducted using a paired t-test (${\alpha}=.05$). Results: Isometric contraction of the quadriceps femoris in a closed kinetic chain exercise caused a greater increase in VMO muscle activity in the hip adduction position [$52.68{\pm}22.21$ percentage of maximal voluntary isometric contraction (%MVIC)]than the hip neutral position ($43.43{\pm}19.85%MVIC$). The onset time difference (VL-VMO) decreased more in the hip adduction position ($-82.14{\pm}34.2ms$) than the hip neutral position ($73.94{\pm}2.94ms$). Conclusion: We recommend this exercise as a clinically useful therapeutic method for patients with patellofemoral pain syndrome due to weakening of the VMO muscle and lateral inclination of the patella.

Effect of Shoulder Abduction Angles on EMG Activity of the Abdominal Muscles during Single Leg Holding on the Foam Roller

  • Purpose: This study examined the electromyography (EMG) activity of the abdominal muscles and the ratio of the oblique abdominal muscle activity to the rectus abdominis muscle during a single-leg holding position with isometric shoulder abduction (SLHISA) in the supine position on a foam roller. Methods: Nineteen healthy males were recruited to the study. Each subject was asked to lay on a round foam roller and perform singleleg (nondominant) holding with contralateral shoulder abduction to one of three angles (45°, 90°, or 135°) in random order. The surface EMG signals of the bilateral rectus abdominis (RA), external oblique abdominis (EO), and combined internal oblique abdominis (IO) and transverse abdominis (TrA) muscle were collected during the tasks. The EO/RA and (IO & TrA)/RA ratios were determined using surface EMG. One way repeated measure ANOVA with three SLHISAs was used to assess the significant abdominal muscle EMG activity and the ratio of the oblique abdominal muscles activity to the RA muscle. The statistical significance level was p<0.05. Results: The results were as follows. The SLHISA 135° showed significantly higher EMG activity of both RAs, left EO, and right IO & TrA muscles (p<0.05). The right EO and left IO and TrA muscles/RA were significantly different among the SLHISA angles. The SLHISA 45° showed a significantly greater ratio of right EO/RA and left IO & TrA/RA (p<0.05). Conclusion: SLHISA on a foam roller is useful for lumbopelvic stabilization exercise by increasing the activity and recruiting a specific pattern of the oblique abdominal muscle.

Effect of Electrical Muscle Stimulation Belt for Abdominal Muscles Activation

  • Objective: The purpose of this study is to observe the change in the thickness of abdominal muscles when electrical muscle stimulation (EMS) is applied to the abdomen during rest and abdominal muscle exercise to investigate the effect of EMS applied to the abdomen on the superficial and deep muscles thickness. Design: Cross sectional design. Methods: Twenty healthy subjects participated in this study. Subjects were performed resting position, resting position with EMS, curl-up and curl-up with EMS. The electrode of the EMS belt is attached to the abdominal wall between the 12th rib and iliac crest. The thickness of abdominal muscles including rectus abdominis (RA), external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) were captured in each position by ultrasound image during expiration. All subjects were performed four positions randomly. Data were analyzed using repeated ANOVA with the level of significance set at 𝛼=0.05. Results: The muscle thickness of RA, EO, IO and TrA were significantly different at each position (p<0.05). The thickness of all abdominal muscles increased significantly when curl-up than curl-up with EMS. Both RA and EO thickness were significantly increased at resting position than resting position and EMS were combined(p<0.05). But IO and TrA thickness were decreased at resting position when EMS were combined. Conclusions: The results suggest that EMS activates superficial abdominal muscles RA and EO. Therefore, abdominal strengthening exercise combined EMS can activate abdominal muscles and can be applied to various patients and rehabilitation in clinical practice.

요부 안정화 운동에 따른 몸통 근육들의 근활성도 비교 (The Comparison of Trunk Muscle Activities During Sling and Mat Exercise)

  • The purpose of this study was to verify the most effective spinal stabilization exercises program by comparing the activities of muscles contributing to spinal stabilization during four types of exercises using a sling and a mat. Twenty healthy males were recruited and each subjects performed four types of exercises. Exercise 1 was performed in a quadruped position with the subjects lifting the left arm and the opposite leg on the mat. Exercise 2 was performed in a prone position while holding a sling with the right hand and the left knee was fully extended while lifting the left arm and right leg. Exercise 3 was performed in quadruped position while holding a sling with one the right hand and lifting the opposite arm and leg. In exercise 4, subjects were instructed to maintain a balance push-up position while holding slings with both hands in 10 cm forward reaching with extended elbows. Electromyographic(EMG) activities were recorded from the multifidus, external oblique, internal oblique, abdominal rectus, and erector spinalis muscles during the exercises. The EMG amplitude of each muscle was normalized to the amplitude in the maximal voluntary isometric contraction (MVIC) of each muscle. Repeated ANOVA and Bonferroni's tests were used to compare the differences in the muscle activity according to the types of exercise. The EMG amplitudes of all the muscles were significantly different according to the types of exercises (p<.05). The highest EMG activities of each muscle was as follow; multifidus was 73.38%MVIC in exercise 3, the erector spinalis was 40.03%MVIC in exercise 3, the external oblique was 135.88%MVIC in exercise 4, the internal oblique was 128.60%MVIC in exercise 4, and the rectus abdominalis was 95.24%MVIC in Exercise 4. The types of exercises showed a significant difference in composition rate of EMG amplitudes of each muscle (p<.05). EMG composition rate of the multifidus was high in exercise 1 and 3. However, EMG composition rates of the external oblique, internal oblique, and the rectus abdominals were high in exercise 2 and 4. These results showed differences in EMG activities of muscles contributing to trunk stabilization during different therapeutic exercises. Therefore, the type of exercise should be carefully selected to effectively strengthen a specific trunk stabilizer.
