관절함몰형 종골 골절에 대한 다공성 하이드록시 아파타이트를 이용한 수술적 치료 (The Operative Treatment using Porous Hydroxyapatite for Intraarticular Calcaneal Fractures of Joint Depression Type)

  • Purpose: This study was performed to evaluate the clinical outcomes of operative treatment using porous hydroxyapatite for intraarticular calcaneal fracture of joint depression type. Materials and Methods: Twenty patients with intraarticular calcaneal fracture were followed up for more than 1 year. The period to union was calculated to evaluate the osteoconductivity of porous hydroxyapatite used as bone graft substitute. The measurement of Bohler angle, Gissane angle and the degree of articular surface depression was performed through preoperative and postoperative radiographs. The clinical evaluation was performed according to hindfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS) and scale of the Creighton-Nebraska health foundation (CNHF). Results: Bohler angle and Gissane angle had improved significantly from preoperative average $10.4^{\circ}$, $117.8^{\circ}$ to average $22.6^{\circ}$, $113.5^{\circ}$ immediate postoperatively, and had maintained to average $21.2^{\circ}$ and $114.4^{\circ}$ at the last follow-up. The degree of articular surface depression had improved significantly from preoperative average 4.8 mm to 1.5 mm at the last follow-up. All cases achieved bone union, and the interval to union was average 12.8 weeks. AOFAS score was average 85.2 points at last follow-up. There were 7 excellent, 10 good, and 3 fair results according to the CNHF scale. Therefore, 17 cases (85%) achieved satisfactory results. Conclusion: Plate fixation using porous hydroxyapatite seems to be one of effective treatment methods for intraarticular calcaneal fracture of joint depression type, because of supporting the reduction of subtalar articulation by augmenting bony defect and facilitating bone formation. Further evaluation about long-term radiological changes and histological analysis on hydroxyapatite implantation site should be required.

  • Spherical Ti-6Al-4V powders in the size range of 250 and 300 ${\mu}m$ were uniformly doped with nano-sized hydroxyapatite (HAp) powders by Spex milling process. A single pulse of 0.75-2.0 kJ/0.7 g of the Ti-6Al-4V powders doped with HAp from 300 mF capacitor was applied to produce fully porous and porous-surfaced Ti-6Al-4V implant compact by electro-discharge-sintering (EDS). The solid core was automatically formed in the center of the compact after discharge and porous layer consisted of particles connected in three dimensions by necks. The solid core increased with an increase in input energy. The compressive yield strength was in a range of 41 to 215 MPa and significantly depended on input energy. X-ray photoelectron spectroscopy and energy dispersive x-ray spectrometer were used to investigate the surface characteristics of the Ti-6Al-4V compact. Ti and O were the main constituents, with smaller amount of Ca and P. It was thus concluded that the porous-surfaced Ti-6Al-4V implant compacts doped with HAp can be efficiently produced by manipulating the milling and electro-discharge-sintering processes.

소 뼈로부터 제조한 다공형 하이드록시아파타이트 세라믹스의 생체친화 특성 (Biocompatibility of porous hydroxyapatite ceramics prepared from bovine bones)

  • 소뼈를 하소하여 천연 하이드록시아파타이트 분말을 얻은 다음, $1100^{\circ}C$$1200^{\circ}C$로 상압 소결하여 기공율이 각각 35 % 및 18 %인 다공성 하이드록시아파타이트를 제조한 후 이들 시편의 생체친화성을 평가하였다. 골아세포의 성장속도과 세포독성의 유무에 따라 평가되는 생체친화성은 MG-63 세포의 부착능과 분화되어 증식되는 세포의 수로 측정하였다. in vitro에서 세포 반응을 관찰한 결과 소뼈로부터 제조된 하이드록시아파타이트 시편은 독성이 없고 생체 친화성이 좋은 시편이었다. 세포배양 1일 후부터 하이드록시아파타이트 표면에 세포가 부착되었고, 세포배양 후 3일 후에는 핵 주위로 세포질이 퍼져나갔으며, 5일 후에는 부착된 세포의 수가 크게 증가하였다. 배양 초기에는 밀도에 관계 없이 세포의 부착능이 유사하였지만, 세포 배양 시간이 증가하면서 기공율이 높은 시편에서 세포의 부착 및 성장이 더 활발하게 진행되었다.

흡수성차단막과 골이식재가 성견 치주질환 치조골재생에 미치는 영향 (THE EFFECTS OF RESORBABLE MEMBRANE IN CONJUNCTION WITH OSSEOUS GRAFTS ON THE PERIODONTAL HEALING IN DOGS)

  • There has been many attempts to develop a method that can regenerate periodontal tissues that were lost due to periodontal diseasd, but none of them was completely successful. This study was designed to investigate the healing and regeneration of periodontal tissue when bone substitutes such as porous replamineform hydroxyapatite and porous resorbable calcium carbonate were used in combination with oxidized cellulose membrane and collagen absorbable hemostat, compared to a control where only oxidized cellulose membrane or collagen absorbable hemostat were used. Chronic periodontitis was induced on mandibular premolars of and adult dog by placing orthodontic elastic ligatures for 10 weeks. After flap operation, the control group were received oxidized cellulose membrane (control- I )or collagen absorbable hemostat (control- II) only, while one experimental group was given either porous replamineform hydroxyapatite or porous resorbable calcium carbonate in addition to oxidized cellulose membrane (Experimental I-A, I-B), and another experimental group was treated by using either porous replamineform hydroxyapatite or porous resorbable calcium carbonate in addition to collagen absorbable hemostat. (Experimental II-A, II-B) After 56 weeks, healing was histologically analyzed with the following results. 1. Apical migration of junctional epithelium was observed only in areas coronal to the notch for both control and experimental group. 2. Inflammatory cell infiltration was not observed in any groups. 3. Oxidized cellulose membrane and collagen absorbable hemostat were completely resorbed. 4. Newly-formed cementum was observed up to the level where junctional epithelium was located, for both control and experimental groups. 5. Bone formation was limited of the middle portion of the notch in the control group, where as experimental groups showed bone formation up to the level of implant materials coronal to the notch. 6. Minute resorption of apically located portions of implanted materials was observed in experimental group I-B and II-B only.

참치 뼈로부터 수산화아파타이트 분말의 제조 및 소결 특성 (Preparation of Hydroxyapatite Powder Derived from Tuna Bone and Its Sintering Property)

  • Hydroxyapatite(HA) was prepared from waste tuna bone, and its sintering property and dissolution behavior were investigated. Tuna bone derived-HA powder consisted of mainly HA and small amount of MgO. Porous HA ceramics with sintered density of 79% was obtained by pressureless sintering at $1200^{\circ}C$. Meanwhile, HA ceramics prepared by hot pressing at $1000^{\circ}C$ showed dense microstructure with sintered density of 95%. Immersion test revealed that both porous and dense HA ceramics were stable in liquid environment without distinct evidence of surface dissolution. It may be assumed that the presence of Mg in tuna bone-derived HA may improve dissolution resistance of HA.

  • Although most strains of escherichia coli (E. coli) are harmless, some serotypes can cause serious food poisoning in humans. It is very difficult to eliminate E. coli from our lives. Here we show that E. coli can be eliminated by hydroxyapatite (HAp). Because HAp has a positive charge, the material and E. coli are attracted through electrostatic interactions. Additionally, because the surface of HAp is porous, it enters the pores of the HAp surface removing them from the environment. The amount of adsorption was observed to increase over time, and the zeta potential value of the material tended to be similar to that of E. coli. This phenomenon is thought to have zeta potential similar to that of E. coli as it is adsorbed onto the HAp surface over time. E. coli stained with crystal violet was spread on a glass slide and HAp porous sol powder was dropped to remove the E. coli. We expect that this analysis will open a new direction for antibacterial materials.

폴리머 스펀지법을 이용한 다공성 수산화아파타이트 지지체 제조 시 MgO 첨가량에 따른 영향 (The Effect of MgO Content on the Preparation of Porous Hydroxyapaite Scaffolds by Polymer Sponge Method)

  • Porous hydroxyapatite (HAp) scaffolds have been prepared by using the slurry including HAp and magnesia based on the replication of polymer sponge substrate. The influence of MgO content in slurry on the pore morphology and size, density, porosity, and mechanical strength of porous HAp scaffolds was investigated. The obtained scaffolds with average pore sizes ranging 150 to 300 mm had open, relatively uniform, and interconnected porous structure regardless of MgO content. As the MgO content increased, the pore network frame of scaffolds became to be relatively stronger, even though the pore size was not much changed. The compressive strength of the scaffolds increased rapidly with the increase of MgO content because of increasing the pore wall thickness and density of the scaffolds. As a result, the porosity, density, and compressive strength of the porous HAp scaffolds prepared by the sponge method were significantly affected by the addition of MgO.

산호 유래 실리콘 함유 다공성 수산화아파타이트의 합성 및 특성 분석 (Preparation and Characterization of Porous Hydroxyapatite Containing Silicon Derived from Natural Coral)

  • 생체친화성 인공골 개발을 위하여 해면골과 매우 유사한 기공구조를 갖는 천연 산호(CaCO$_3$, aragonite phase)를 수열처리하여 수산화아파타이트 다공체를 제조하였다. 생체 친화성을 증진시키기 위하여 산호를 수열 처리하는 과정에서 실리콘을 치환시키는 실험을 하였다. 수열합성과 solvothermal 방법을 반복적으로 사용하여 실리콘이 치환된 수산화아파타이트를 합성하였으며 이들의 각 온도에 따른 상변화 및 실리콘 농도를 XRD, ICP, EDS 등을 통하여 분석하였다. 분석결과로부터 실리콘이 수산화아파타이트 골격 내에 균일하게 분산되어 있음을 확인하였다.

다공성 티타늄 임플란트의 생체적합성 증진을 위한 복합 표면처리에 관한 연구 (A Study of Multi-Surface Treatments on the Porous Ti Implant for the Enhancement of Bioactivity)

  • Porous Ti implant samples were fabricated by the sintering of spherical Ti powders in a high vacuum furnace. To increase their surface area and biocompatibility, anodic oxidation and a hydrothermal treatment were then applied. Electrolytes in a mixture of glycerophosphate and calcium acetate were used for the anodizing treatment. The resulting oxide layer was found to have precipitated in the phase form of anatase $TiO_2$ and nano-scaled hydroxyapatite on the porous Ti implant surface. The porous Ti implant can be modified via an anodic oxidation method and a hydrothermal treatment for the enhancement of the bioactivity, and current multi-surface treatments can be applied for use in a dental implant system.
