고령자 주거지원을 위한 주택시설설비결함과 주거만족에 관한 연구 - 전북지역의 노인가구를 중심으로 - (Housing Defects and Housing Satisfaction for Supporting to the Aged - Emphasis on the Jeonbuk Aged Households -)

  • This paper investigates housing defects and housing satisfaction of the aged according to the personal characteristics and their housing environment. Personal interview was made on the 280 senior persons living in Jeonbuk area. Research results are following. Low levels were found in education, self privately-owned property, monthly income. Most of them live in a detached house that is outdated and obsolete. The facility defects of housing were the most serious level in case of the female senior citizen who lives alone. Their house aging was old but had the high deviation among households. Main influencing factors on facility defects of housing were perceived economic status and ownership of house, the next was age of the aged. Main influencing factors on the housing satisfaction were perceived economic status. Housing satisfaction was higher, the better economic level of the aged was, male than female, the less the age was, in case of hot-water usage in bathroom, owner of houses, no difference in floor level, larger storage space of kitchen was. According to the empirical results, we propose the supporting policy of hosing safety for the aged in order to reduce the risk and social cost. Especially the rate of population aging, the aged household, and the detached houses are higher in Jeonbuk rural areas than the other area. Housing facility defects of the elder who lives alone and house aging of theirs are serious level. Local government should have supporting system that considers local difference and characteristics of the aged housing.

주거지 재생지역의 기존주택 기능전환에 대한 노인의 반응연구 (Elderly response to alteration of existing house function in housing improvement area)

  • As an aging population has increased vastly and nuclear families have been a dominating family type in modern Korean society, the numbers of the elderly who suffer Empty Nest Syndrome and LID (Loss Isolation Depression) syndrome have been accelerated. These syndromes involve psychological instability, melancholy, and lethargy. To make the elderly get out of this phenomenon and live actively, the scheme of converting their existing house into shared housing was set as a hypothetical solution in this study. This study sets out to find out responses of the elderly to alter their own house's function in housing improvement area. A Small Workshop Panel method was used and the elderly aged 55 years upwards took part and they own a detached house in a housing improvement area that is relatively large for the elderly or the elderly couple to live alone. Through the workshop, problems that the elderly have been experiencing within their houses were looked into and responses of the elderly on altering their house to shared housing by introducing developed schematic plans. Although, the first response was negative, positive responses from the house owners were carried out when a visual support i.e. developed shared housing plan was provided. The positive response was based on the fact that subsidies from the government on house renovation and tenants matching programs are supported as shared housing could provide home owners economical support with rents and help them to feel less lonely. This is a way of relieving the elderly from being neglected and supporting them to live and age actively in their later lives. Furthermore, by providing a visual media to the elder residents in housing improvement area, their greater understandings on the development and addressing their opinions were possible. Therefore, more tools that promote elder residents' participation are needed for future housing improvement projects and an aging society.

스마트 컨트롤 밸브 개발에 관한 연구 (A Study on the Development of Smart Control Valve)

  • 우리나라는 저출산·고령화가 진행함에 따라 전체 인구의 14%를 웃도는 고령사회에 진입함으로써 1인 세대의 증가라는 사회변화에 직면해 있다. 고령사회에서 1인 세대 증가로 인한 가스화재 사고를 예방할 수 있는 효과적인 방법은 컨트롤 밸브를 설치하여 설정시간이 지나면 강제적으로 가스를 차단하는 방법이다. 본 연구에서는 밸브 핸들을 돌리지 않고 택트 스위치를 눌러 모터를 이용하여 웜기어와 헬리컬 기어로 동력을 전달하여 밸브를 좌우 회전하여 개폐 할 수 있게 개발하였고, 컨트롤러와 탈부착이 쉽게 전용 밸브를 개발함으로써 조립 및 검사가 용이하고, 부품 수 및 제품 사이즈를 줄여 원가 절감을 통해 경쟁력을 강화할 수 있도록 개발하였다. 또한 내구성 시험을 통해 12초에 한 번씩 ON/OFF 반복하여 34시간 동안 10,000번을 수행하는 동안 안정적으로 동작 할 수 있도록하여 9년 이상을 사용할 수 있도록 개발하였다. 개발 방법은 Solid works와 Altium Designer 툴을 활용하여 기구 및 PCB 설계를 진행하였고, Firmware 개발은 IAR Embedded Workbench 환경에서 개발하였다.

한국 장수도(長壽度) 변화의 공간적 특성 (Spatial Characteristics of Longevity Degree in Korea)

  • 본 연구의 목적은 빠른 속도로 고령화과정을 겪고 있는 한국의 장수 현상 및 장수지역의 공간적 특성을 파악하는 것이다. 이를 위해 1966년부터 2000년까지의 인구 자료를 이용하여 각 지역의 고령화 현상과 장수도 변화를 파악하였다. 또한 장수의 지역요인을 알아보기 위해 자연환경 및 대도시에 이르는 거리와의 관련성을 분석하였다. 연구 결과 첫째, 호남권 농어촌지역이 전국에서 가장 높은 고령화 수준을 나타내고 있었다. 둘째, 70년대 남부지역 섬과 해안을 중심으로 높은 장수도를 나타내던 것이 최근 내륙의 소백산맥 산간지역으로 이동 확장되어 장수지역의 공간적 확산이 이루어졌다. 셋째, 지역환경요인 분석 결과 기온보다는 강수량과 평균표고가 장수도와 관련성이 높은 것으로 나타났다. 넷째, 대도시의 경우 자연환경요인이 아닌 다른 사회 경제적 요인들 중에서 장수도 인자를 찾아야 함을 시사하는 결과를 얻었다.

  • 노화가 진행되고 있는 중, 장년층의 교정치료시에 통상적인 교정치료의 방법과 함께 다음과 같은 사항을 고려하여야 한다. 1. 치주질환 존재시 교정치료전 치주치료를 선행하여야 한다. 2. 발치 치료가 필요할 경우 치주 상태 및 치아 상태를 고려하여 발치할 치아를 선정하여야 한다. 3. 기존 보철물 존재시 보철물의 상태, 고정원으로 사용여부, 크기 조절 필요 여부, 환자의 경제적 상황 등을 고려햐여 처치한다. 4. 치료 종료 후 가철식 유지장치보다는 고정식 보정장치를 이용하여 치아의 유지력을 보강하는 것이 권장된다.

휴먼 케어 콘텐츠 기반의 재활 훈련 장비의 사용성 평가 연구 (A Study on Usability Evaluation for Human Care Contents based Rehabilitation Training Equipment)

  • 경제적 발전과 의학의 발달로 인해 고령인구의 비율이 높아지는 고령 사회가 도래하고 있다. 우리나라의 고령화 진입 속도는 다른 선진국들에 비해 매우 빠른 편이다. 고령으로 인한 노인들의 신체적 능력은 점점 더 악화되어 가고 있으며, 이를 예방하기 위해 신체적, 정신적 장애를 위한 보조 시스템의 개발이 요구되고 있다. 이러한 보조 시스템은 노약자의 삶의 질 향상에 크게 기여할 수 있으며, 대표적인 시스템으로 재활 훈련 장비를 들 수 있다. 특히, 근력 강화, 인지능력 강화, 그리고 균형 능력을 강화시킬 수 있는 기능이 포함된 장비는 대부분의 노약자들에게 도움이 된다. 본 논문에서는 이들 기능을 강화시키는 휴먼 케어 콘텐츠 기반의 재활 훈련 장비의 개발을 소개하고, 본 장비의 사용성 평가를 제시한다. 사용성 평가는 예비 사용자는 물론 재활전문의 등의 전문가를 대상으로 실시하고, 그 결과를 분석하였다.

5단계 가압 맥파측정에 의한 연령별 혈관 경화도 분석 (Analysis of Arterial Stiffness by Age Using Pulse Waveform Measurement of 5-levels Graded Pressure)

  • Objectives : The aim of this study is to measure pulse waveforms by applying 5-level graded pressure, and selecting optimum pulse waveforms. Also to proposing the possibility of using AW(Area of the 1/3 upper height of h1) rate in respect to AT(Total Area) for risk assessment of hypertension or arteriosclerosis is another aim of the study. Methods : Pulse waveforms of normotensive were measured by 5-level graded pressure. The pulse waveforms well reflecting properties of blood vessel(having the largest h1) were selected for optimum pulse waveforms. Various parameters(h-parameter, t-parameter, and others) of optimum pulse waveforms were analyzed. AIx(Augmentation index) was calculated by height-parameters to assess arterial stiffness. The area rate of the 1/3 upper height for h1 in respect to total area was analyzed according to aging. Results : According to aging 1. in height-parameter, h2 and h3 were increased but h5 was decreased. 2. In time-parameter, t2, t3, and t5 were getting short. 3. Area of systolic period was increased, and that of diastolic period decreased. 4. AIx rose by aging. 5. AW was significantly increased despite no changes in AT. Conclusions : By analyzing optimum pulse waveforms of 5-level graded pressure method, we could complement weakness of single graded pressure method. Also, possibility of applying the AW rate to risk assessment of hypertension or arteriosclerosis was confirmed in normotensive population which might not be assessed by AIx.

Associations of alcohol consumption and alcohol flush reaction with leukocyte telomere length in Korean adults

  • BACKGROUND/OBJECTIVES: Telomere length is a useful biomarker for determining general aging status. Some studies have reported an association between alcohol consumption and telomere length in a general population; however, it is unclear whether the alcohol flush reaction, which is an alcohol-related trait predominantly due to acetaldehyde dehydrogenase deficiency, is associated with telomere length. This cross-sectional study aimed to evaluate the associations of alcohol consumption and alcohol flush reaction with leukocyte telomere length (LTL). SUBJECTS/METHODS: The study included 1,803 Korean adults. Participants provided blood specimens for LTL measurement assay and reported their alcohol drinking status and the presence of an alcohol flush reaction via a questionnaire-based interview. Relative LTL was determined by using a quantitative polymerase chain reaction. Statistical analysis used multiple linear regression models stratified by sex and age groups, and potential confounding factors were considered. RESULTS: Age-specific analyses showed that heavy alcohol consumption (> 30 g/day) was strongly associated with a reduced LTL in participants aged ${\geq}65years$ (P < 0.001) but not in younger participants. Similarly, the alcohol flush reaction was associated with a reduced LTL only in older participants who consumed > 15 g/day of alcohol (P < 0.01). No significant alcohol consumption or alcohol flush reaction associations with LTL were observed in the sex-specific analyses. CONCLUSIONS: The results suggest that older alcohol drinkers, particularly those with the alcohol flush reaction, may have an accelerated aging process.

한·일 노인복지법의 비교법적 연구 (A Comparative Study on the Elderly Welfare Act between the Korea and Japanese)

  • '고령화' 및 '고령사회'라는 표현은 20세기 말부터 우리사회에 회자되어 온 용어라 하겠다. 즉, 65세 이상의 인구가 차지하는 비율이 점점 높아짐에 따른 결과이다. 무엇보다 인구의 고령화는 사회경제적 차원의 많은 문제를 초래하기에 복지문제차원의 국가적인 문제로 대두되어 왔다. 이미 고령화를 일찍 경험한 선진국은 보건의료서비스, 소득보장 및 장기요양보호문제 등의 노후복지서비스 제공을 위한 국가 재정과 법제도적 차원의 대응방안을 모색하지 않을 수 없게 되었다. 이에 본 연구에서는 노인복지법이라는 법제적 차원의 선진 일본과의 비교법적 고찰을 실시하였다. 무엇보다 노인복지를 포함한 사회복지법제의 내용은 각 국가의 사회, 정치, 경제, 문화등과 밀접한 관련을 가진다. 또한 고령사회로의 진입에 따른 우리나라의 현실에서 법제도적 차원의 노인복지법에 대한 진단이 필요하다. 즉, 노인이라는 사회적 존재 및 역할에 대한 다양한 이슈는 바로 법제도적 차원의 진단에서 출발한다 하겠다. 따라서, 본 연구에서는 노령화의 선발국가인 일본의 노인복지법을 바탕으로 향후 우리가 당면해야 할 고령 및 초고령사회를 위한 법제도적 장치에의 시사점을 제언하고자 한다.

Barthel's Index: A Better Predictor for COVID-19 Mortality Than Comorbidities

  • Background: The most consistently identified mortality determinants for the new coronavirus 2019 (COVID-19) infection are aging, male sex, cardiovascular/respiratory diseases, and cancer. They were determined from heterogeneous cohorts that included patients with different disease severity and previous conditions. The main goal of this study was to determine if activities of daily living (ADL) dependence measured by Barthel's index could be a predictor for COVID-19 mortality. Methods: A prospective cohort study was performed with a consecutive sample of 340 COVID-19 patients representing patients from all over the northern region of Portugal from October 2020 to March 2021. Mortality risk factors were determined after controlling for demographics, ADL dependence, admission time, comorbidities, clinical manifestations, and delay-time for diagnosis. Central tendency measures were used to analyze continuous variables and absolute numbers (proportions) for categorical variables. For univariable analysis, we used t test, chi-square test, or Fisher exact test as appropriate (α=0.05). Multivariable analysis was performed using logistic regression. IBM SPSS version 27 statistical software was used for data analysis. Results: The cohort included 340 patients (55.3% females) with a mean age of 80.6±11.0 years. The mortality rate was 19.7%. Univariate analysis revealed that aging, ADL dependence, pneumonia, and dementia were associated with mortality and that dyslipidemia and obesity were associated with survival. In multivariable analysis, dyslipidemia (odds ratio [OR], 0.35; 95% confidence interval [CI], 0.17-0.71) was independently associated with survival. Age ≥86 years (pooled OR, 2.239; 95% CI, 1.100-4.559), pneumonia (pooled OR, 3.00; 95% CI, 1.362-6.606), and ADL dependence (pooled OR, 6.296; 95% CI, 1.795-22.088) were significantly related to mortality (receiver operating characteristic area under the curve, 82.1%; p<0.001). Conclusion: ADL dependence, aging, and pneumonia are three main predictors for COVID-19 mortality in an elderly population.
