A Study on the Development of an Instrument for Evaluating the Quality of Nursing Care (간호의 질 평가도구 개발에 관한 일 연구)

  • Many in nursing look back on Nursing Research history and proudly point to the fact that emphasis in nursing research has changed from studying the nurse to studying nursing practice. In recent years, much emphasis has been placed on seeking a method of evaluating the quality of nursing care. In spite of these attempts, however, an instrument for evaluating the quality of nursing care that is actually applicable in the clinical area has not been found. The Purposes of this study are as follows: 1) To develop the instrument to be used in evaluating the quality of nursing care provided in the Neuro - Surgery Constant Care Unit of Severance Hospital 2) To evaluate the quality of nursing care in the clinical area. 3) To provide the necessary information for improvement of quality of nursing care. The instrument for evaluating the quality of nursing care, developed by the investigator, was composed of 7 nursing goals and divided into 65 standards of nursing performance. The 7 nursing goal are as follows : 1) Maintenance of airway 2) Maintenance of fluid at electrolyte balance 3) Maintenance of elimination 4) Personal hygiene 5) Optimum activity 6) Prevention of accidents 7) Emotional care The study population defined was composed of all the case (51) who were admitted in the Neuro- Surgery Constant Care Unit of Severance Hospital from May 7-13, 1976. The observation method was used and the data was subjected to the %, X²-test, T-test, F-test and Correlation. The results of tile study were as follows : 1. Levels of nursing performance regarding nursing goals. Seven different nursing care indices were constructed in terms of nursing goals. The index scores were grouped arbitrarily into ,j categories such as "excellent", "good", "moderate", "incomplete", and "poor"based upon the investigator′s personal judgement. a. The nursing index of maintaining airway showed that 78% of the patients fell within the "excellent" and 22% of the patients, fell within the "good" category. b. The nursing index of maintaining fluid & electrolyte balance showed that 95% of the patients fell within the "excellent" and 5 % of the patients fell within the "good" category. c. The nursing index of maintaining elimination showed that 100% of the patients fell within the "excellent" category. d. The nursing index of personal hygiene revealed that 49% of the patients fell within the "excellent" and 51% of the patients fell within the "good" category. e. The nursing index of optimum activity showed that 63% of the patients fell within the "excellent" and 32% of the patients fell within tile "good" and 5% of patients fell within the "moderate" category. f. The nursing index of prevention of accidents showed that 100% of the patients foil within the "excellent" category. g. The nursing index of emotional cart revealed that 27% of the patients fell within the "excellent", 24 % of the patients fell within tile "good", 29 % of the patients fell within the "incomplete" category. From these findings it is disclosed that the quality of nursing care provided in the Neuro- Surgery Constant Care Unit of Severance Hospital was excellent. h. There were statistically significant differences between the nursing index of physical care and emotional care. (t=8.73, D. F. =100. p<0.01) It is revealed that more physical care then emotional care was carried out by nurses. 2. Levels of nursing performance regarding general characteristics of the patients. No significant differences were observed statistically with the nursing indices of nursing goals according to the sex (t=0.084, D. F. =12, p>0.05). Age (F=0.1251, D. F. : 3.18. p 〉0.05), absence or presence of operating experiences (t=0.6032, D. F. =12, p〉0.05, levels of consciousness (F=0.31, D. F. :3. 18, p >0.05) 3. Relationship between the levels of consciousness and the nursing index of each nursing goal. There was negative correlation between the levels of consciousness and the nursing index of maintaining airway (r=-0. 5449, p<0.01) and personal hygiene (r= -0.4075, p<0.01) There was positive correlation between the levels of consciousness and the nursing index of optimum activity (r=0.3936, p <0.01) and emotional care (r=0.7819, p〈0.01). There was slight correlation between the levels of consciousness and the nursing index of maintaining fluid & electrolyte balance (r=-0.3418, 0.010.05) and preventing accidents (r=0.1441, p>0.05.

An Analysis and Evaluation of Cyber Home Study Contents for Self-directed Learning - Focused on the Earth Science Content of the Science Basic Course for the 7th grade - (사이버가정학습의 자율학습용 콘텐츠 분석 및 평가 - 중학교 1학년 과학 기본과정 지구과학영역을 중심으로 -)

  • The purpose of this study is to analyze and evaluate the self-directed learning contents of Earth science area in the basic course of the 7th grade. For this purpose, we applied the 'Cyber Home Study Content Quality Control Tool' presented in 'Elementary Secondary Education e-Learning Quality Management Guidelines (Ver.2.0)' of Korea Education & Research Information Service (2008). The results of contents analysis are as follow: First, it was presented that the study guide introduced the contents which should be studied for one class, properly. And it was not analyzed that the diagnosis assesment was not completed in the initiative study; Second, it was possible to study choosing the contents fitting the learner's level of learning in the main study, it was comprised of about 15 minutes. Third, it was performed without feedback for incorrect answers in the learning assessment, just the number of wrong questions. And the learning arrangement present the important contents learned in that class, summarizing and arranging again. The results of content evaluation are as follows: First, a big difference was not showed against the needs analysis, instructional design, interaction in each class. And the evaluation of the ethics was not included a word or sentence not suitable. The evaluation of copyright, it was analyzed that Work within the content display in compliance with international copyright Second, the evaluation of instructional design presented mainly the description of a simple picture based, the visible resources like flash card were poor. And in the evaluation of Supporting System, it was presented that the contents were installed so that it was freely available for learners. But it was analyzed that there was no memo-function learners were able to jot down something during the studying contents. And in the evaluation for evaluation, the clear valuation basis about the described content was not presented. So there were slightly differences for each class. Third, in the evaluation and analysis for learning content, it was presented that there were some big differences for each class because it was not composed of the latest information, not corrected and complementary.

A Study on Psychological Rehabilitation to Decrease Powerlessness in the Elderly Population (노인의 무력감 완화를 위한 심리 재활에 관한 연구)

  • Older people, because of the psychological and physiological changes related to the aging process are more vulnerable to experiencing powerlessness than any other age group. This self destructive cycle of depression in older people related to the experience of continued and long term powerlessness can lead even to death. The purpose of this study was to measure powerlessness and resources to increase power in older people, and to measure the effectiveness of a psychological rehabilitation program for reducing powerlessness. The research methodology used was a two step process. In the first step, a survey was done of perceived powerlessness and power resources comparing four groups of elderly people ; those living at home, those in hospital, those living in nursing homes and those attending educational programs for the elderly. The total sample size was 236. In the second step, a psychological rehabilitation program was carried out, pre and post measurements were taken related to this program. The sample consisted of 29 residents in a nursing home. The results of the study are as follows : 1. Powerlessness was classified as cognitive, emotional, activity and learning. The lowest score for powerlessness was in the area of activity, that is the people in the sample felt more power concerning their activities. The highest score was in the area of cognition where they felt they had less power. 2. When the different groups of elderly were compared, it was found that the residents of the nursing home had the highest score on perceived powerlessness and the group who were living at home had the lowest score. 3. Among the general characteristics, the factors influencing the powerlessness score were age, sex, level of education, financial resources and health status. In the interaction effects among these factors, it was found that level of education and health status were factors influencing perceived powerlessness. The elderly with lower education and poorer health status had the higher scores for perceived powerlessness. 4. The power resources could be classified into the following areas : physical strength, emotional strength, positive self-image, energy, knowledge, motivation and belief system. Belief system was given the highest score among the power resources and energy, knowledge and motivation were given low scores. 5. The group participating in an educational program for the elderly had the highest score for power resources while the group made up of residents of a nursing home had the lowest score as well as the highest score for perceived powerlessness. 6. The factors influencing the power resource scores were sex, level of education, financial resources and health status. In the analysis of the interaction effect among the factors, it was found that sex, level of education and financial resources were the factors that influenced the power resource score, that is, women, those with a low level of education and those with poor financial resources reported a lower level of power resources. 7. There was a negative correlation between perceived powerlessness and power resources in the elderly in this study. Since power resources explainded 49% of the variance for powerlessness, it can be concluded that the power resources can be used to reduce powerlessness. 8. The psychological rehabilitation program was carried out with the nursing home residents over a period of five weeks. No statistically significant difference was found in the scores on powerlessness between the pre and post tests, but there was a slight decrease in the raw scores on the post test for emotional, activity and learning powerlessness. There was a statistically significant increase in the power resource scores for emotional strength, positive self-image, energy, knowledge and motivation in the post test as compared to the pre test. In conclusion, the study indicates that a psychological rehabilitation program for the elderly could be effective in increasing power resources and this in turn could lead to a decrease in perceived powerlessness.

The Effect of Placing Biomembrane cover following Microfracture on Cartilage Repair: Comparison with Conventional Microfracture Technique in a Prospective Randomized Trial (미세골절술 후 생체막 덮개가 연골 재생에 미치는 영향 : 고식적인 미세골절술과의 전향적 비교 연구)

  • Purpose: Microfracture has been used as a first-line treatment to repair articular cartilage defects. In this study, a new technique using an extracelluar matrix biomembrane to cover the cartilage lesions after microfracture was evaluated in terms of cartilage repairability and clinical outcome compared with conventional microfracture technique in a prospective randomized trial. Materials and Methods: A total of 53 patients (59 cases) without osteoarthritis who had focal full thickness articular cartilage lesions were randomly assigned in two group. Seventeen patients (17 cases) underwent conventional microfracture procedure (control group) and thirty-six patients (42 cases) received microfracture and placing biomembrane cover (ArtiFilm$^{TM}$) concomitantly (experimental group). Clinical assessment was done through 6 months postoperatively using the subjective International Knee Documentation Committee IKDC questionnaire, and visual analog scale (VAS) for pain and satisfaction. Magnetic resonance imaging (MRI) was performed at 6 months after the operation in all patients. Results: In clinical outcomes, the significant difference was observed between both groups in IKDC, but not in VAS for pain and for satisfaction (final outcomes of IKDC, p=0.001; VAS for pain, p=0.074; VAS for satisfaction, p=0.194). The MRI showed good to complete defect fill (67 to 100%) in 33 patients (78.6%) of experimental group and 4 patients (23.5%) of control group, respectively. In control group, 9 of 17 patients (52.9%) showed poor defect fill (less than 33%), whereas 5 (11.9%) in experimental group (p=0.001). Assessment of peripheral integration revealed no gap formation in 35 patients (83.3%) in experimental group and 6 patients (35.3%) in control group (p=0.001). No serious complications or adverse effects related to the biomembrane were found. Conclusion: Good short-term follow-up clinical results were obtained in the group whose cartilage defects in the knee joint were covered with biomembrane after the microfracture, with the MRI findings confirming the excellent regeneration of the defective cartilage area. This suggests that the surgery to cover the defective area with biomembrane (ArtiFilm$^{TM}$) after the microfracture procedure is a safe, more effective treatment to induce cartilage regeneration.

Fruit Quality and Storability by Harvest Time at 'Fuji'/M.9 Apple Orchard Located in the Area with a High Air Temperature during the Fall Season (가을철 기온이 높은 지역에 위치한 '후지'/M.9 사과원의 수확시기에 따른 과실품질과 저장성)

  • This study was conducted for three years (2007, 2009, and 2010) to investigate the changes in fruit quality during maturation, and the quality and storage ability of fruits harvested at different times of 'Fuji' apple in Daegu region with a high air temperature during the fall season. Changes in apple fruit quality during the maturation period were investigated from 120-135 days to 183-198 days after full bloom. In comparing quality and storage ability of fruits harvested at different times, fruits harvested more than 180 days after full bloom were used. During the maturation period, poor coloring was the problem for 'Fuji' apple in Daegu region by the high air temperature about $20^{\circ}C$. In comparing quality of fruits harvested at different times, the soluble solid contents and hunter a value were increased by the extended harvest time. Fruit weight during harvest was not affected by different harvest time, while the fruit firmness and titratable acidity during harvest were decreased critically when the freezing damage happened. Ethylene production, fruit firmness, and titratable acidity during cold storage for twenty weeks did not differ according to the different harvest time. Soluble solid contents of fruits harvested at 216 days after full bloom in 2009 were similar at the time of harvest and cold storage. For fruits harvested at 201 days after full bloom, soluble solid content during cold storage was higher than during harvest time. However fruit firmness, soluble solid content, and titratable acidity after cold storage of fruit harvested after freezing damage was lower than those of the fruit harvested before freezing damage. The results show that the extended harvest time of 'Fuji' apples about 2-4 weeks from 180-200 days after full bloom in area with above-air temperature during fall season was seemed to be beneficial to enhancing soluble solid contents and fruit red color, but harvesting after the middle of November was dangerous because minimum air temperature began to fall under $-3.0^{\circ}C$.

Corporate Bond Rating Using Various Multiclass Support Vector Machines (다양한 다분류 SVM을 적용한 기업채권평가)

  • Corporate credit rating is a very important factor in the market for corporate debt. Information concerning corporate operations is often disseminated to market participants through the changes in credit ratings that are published by professional rating agencies, such as Standard and Poor's (S&P) and Moody's Investor Service. Since these agencies generally require a large fee for the service, and the periodically provided ratings sometimes do not reflect the default risk of the company at the time, it may be advantageous for bond-market participants to be able to classify credit ratings before the agencies actually publish them. As a result, it is very important for companies (especially, financial companies) to develop a proper model of credit rating. From a technical perspective, the credit rating constitutes a typical, multiclass, classification problem because rating agencies generally have ten or more categories of ratings. For example, S&P's ratings range from AAA for the highest-quality bonds to D for the lowest-quality bonds. The professional rating agencies emphasize the importance of analysts' subjective judgments in the determination of credit ratings. However, in practice, a mathematical model that uses the financial variables of companies plays an important role in determining credit ratings, since it is convenient to apply and cost efficient. These financial variables include the ratios that represent a company's leverage status, liquidity status, and profitability status. Several statistical and artificial intelligence (AI) techniques have been applied as tools for predicting credit ratings. Among them, artificial neural networks are most prevalent in the area of finance because of their broad applicability to many business problems and their preeminent ability to adapt. However, artificial neural networks also have many defects, including the difficulty in determining the values of the control parameters and the number of processing elements in the layer as well as the risk of over-fitting. Of late, because of their robustness and high accuracy, support vector machines (SVMs) have become popular as a solution for problems with generating accurate prediction. An SVM's solution may be globally optimal because SVMs seek to minimize structural risk. On the other hand, artificial neural network models may tend to find locally optimal solutions because they seek to minimize empirical risk. In addition, no parameters need to be tuned in SVMs, barring the upper bound for non-separable cases in linear SVMs. Since SVMs were originally devised for binary classification, however they are not intrinsically geared for multiclass classifications as in credit ratings. Thus, researchers have tried to extend the original SVM to multiclass classification. Hitherto, a variety of techniques to extend standard SVMs to multiclass SVMs (MSVMs) has been proposed in the literature Only a few types of MSVM are, however, tested using prior studies that apply MSVMs to credit ratings studies. In this study, we examined six different techniques of MSVMs: (1) One-Against-One, (2) One-Against-AIL (3) DAGSVM, (4) ECOC, (5) Method of Weston and Watkins, and (6) Method of Crammer and Singer. In addition, we examined the prediction accuracy of some modified version of conventional MSVM techniques. To find the most appropriate technique of MSVMs for corporate bond rating, we applied all the techniques of MSVMs to a real-world case of credit rating in Korea. The best application is in corporate bond rating, which is the most frequently studied area of credit rating for specific debt issues or other financial obligations. For our study the research data were collected from National Information and Credit Evaluation, Inc., a major bond-rating company in Korea. The data set is comprised of the bond-ratings for the year 2002 and various financial variables for 1,295 companies from the manufacturing industry in Korea. We compared the results of these techniques with one another, and with those of traditional methods for credit ratings, such as multiple discriminant analysis (MDA), multinomial logistic regression (MLOGIT), and artificial neural networks (ANNs). As a result, we found that DAGSVM with an ordered list was the best approach for the prediction of bond rating. In addition, we found that the modified version of ECOC approach can yield higher prediction accuracy for the cases showing clear patterns.

A Comparative Analysis of Mental Status, Dietary Life and Nutritional Status among Senior Citizens With or Without Leisure Activities in Urban Area (수도권 거주 노인들의 여가활동 유무에 따른 정신건강 수준, 식생활 실태 및 영양상태 비교 분석)

  • The study aims to examine that regular leisure activity of the elderly has a beneficial effect to enhance their health condition. The elderly subjects of 164 (82 of elderly who do leisure and 82 of elderly who do not leisure) aged over 60 years in Suwon area were employed. Survey questionnaires were administered to investigate general characteristics, life style, mental status, dietary life and nutritional status. From the findings, the elderly who do leisure were highly educated, better in living status and higher employment rate. The mental status of the elderly without leisure activity was very much inferior to that of the elderly with leisure activity, representing higher prevalence rate of depression. Furthermore, the elderly who do not leisure showed higher drinking and smoking rates, irregular eating habits, poor teeth condition and chewing style as well as having eating obstacles. Mean caloric intake of both the elderly was also lower than that of RDA. The elderly who do not leisure have shown lower intakes in most of essential nutrients in contrast to the elderly who do leisure. In conclusion, the elderly with regular leisure activity seemed to have better life style and mental status, dietary life and nutritional status. Therefore, it is critically important to establish the health program combined with various leisure activity for the seniors to promote and energize the later years of life.

Epidemiologic and Clinical Features of Indigenous Vivax Malaria in Children in Kyonggi-do Province Area (경기지역 소아에서 발생된 삼일열 말라리아의 역학 및 임상적 특성)

  • Purpose : In Korea, vivax malaria has been reemerged since 1993 after being abscent for more than 10 years. There are several possibilities of casuality of recent epidemic, although it is still unclear. The epidemiologic studies including case analysis and entomological reseach have been undertaken for a successful control measure. But, unfortunately those studies have been rarely dealt with cases of children. Therefore, this study was designed to figure out the characteristics of epidemiolgic and clinical features in children with indigenous vivax malaria. Methods : The study 21 cases below 15 years of age, who were diagnosed as vivax malaria and resided in kyounggi-do province area during 1998. 9~1999. 8. We retrospectively analyzed epidemiologic data concernig with occurrence of vivax, and clinical manifestations, abnormal laboratory findings and outcomes including therapeutic responses. Results : All cases were inhabitants of the endemic areas for vivax malaria in northwestern part of Kyonggi-do or western Kangwon-do, and Paju-gun was the most prevalent. Indigenous malaria cases of this study were more prevalent in children above 10 years old age, and in male. Seasonally, vivax malaria in children occurred throughout the year except January, March and November, and the incidence was the highest in July. Clinical manifestations revealed that 48 hour cyclic fever pattern was the major fever pattern, and other symptoms such as headache, vomiting, poor appetites, chilling, abdominal pain and diarrhea were concomitantly developed. And splenomegaly revealed the main abnormal findings on physical examination, and anemia was the most frequent abnormal finding in laboratory examinations. Young trophozoite was frequently observed on peripheral blood smears. The therapeutic responses of chlorquine were very good in all cases, and no recurrence developed in follow up cases. Conclusion : Geographical and seasonal occurrence distributions of indigenous vivax malaria cases in children were very similar to those of adults as followings; Inhabitants of the endemic region, more prevalent in male, and more common during the summer season. Clinically, 48 hour cyclic fever pattern, splenomegaly and anemia were most frequent and important manifestations in children cases, and clinical courses were not serious. On blood smears, young trophozoite was most dominantly examined in children. Generally, the therapeutic outcomes were excellent, and recurrences were not observed.

Spatial distribution of Benthic Polychaetous Communities in Deugryang Bay, Southern Coast of Korea (득량만 저서다모류군집의 공간분포)

  • This study was carried out to investigate the composition and the distribution of the benthic polychaetous communities in Deugryang Bay, semi-enclosed bays, on the southern coast of Korea and to deduce temporal changes in community with the comparison of the past studies. In Deugryang Bay, benthic polychaetous community structure was investigated on the base of the samples from 98 stations in 1996 and 1997. The main facies of surface sediment was clayey silt. The overall benthic macrofaunal density was 871 ind./m$^{2}$. The density was highest in the middle part of the bay because Musculus senhousia (Bivalvia) and cumaceans (Crustacea) had their highest densities in some stations. Benthic polychaetes were comprised of 100 species with a mean density of 138 ind./m$^{2}$. Their abundances were higher in the inner bay, in the middle bay, and in the mouth of bay, but poor community structures were established in the whole bay. The dominant species over 1.0 percentage were composed of the total 21 species, and they occupied 78.3% of the total abundance of the benthic polychaetes. The most dominant species was Lumbrineris longifolia (9.3%), followed by Eteone longa (7.3%), Heteromastus filifomis (7.1%), Sternaspis scutata (6.1%). From the cluster analysis, the study area could be divided into three station groups. Station group AI was located in the inner bay and in the shallow coastal region, and its most dominant species was Heteromastus filiformis. At the station group AII in the mouth of bay and in some channel region, its most dominant species were Lumbrineris longifolia and Eteone longa. And at the station group B located in middle part of the bay, the most dominant specis was Sternaspis scutata. In comparison with previous studies, the benthic polychaetous community experienced great change in the view of species number, density and dominant species. The dominant species were Sternaspis scutata and Eteone longa, but their densities declined greatly. Instead of these species, Lumbrineris longifolia and Heteromastus filiformis, known as the potential organic enrichment indicator species, appeared to the new dominant species even if their low densities. These facts mean that Deugryang Bay was maintained yet as little organic enriched area compared to other bays on the coast of Korea, but needed some caution of marine environmental management.

Geochemical Characteristics of Black Slate and Coaly Slate from the Uranium Deposit in Deokpyeong Area (덕평리 지역 우라늄광상의 흑색점판암과 탄질점판암의 지구화학적 특성)

  • Geochemical study was performed on black slates and interbedded U-bearing coaly slates in Deokpyeongri area, the representative uranium mineralized district of the Ogcheon Metamorphic Belt, to discuss the genetic environments of the uranium deposit. REE concentration (254 ppm) of the black slates is higher than that (169 ppm) of the coaly slates and NASC-normalized REE patterns of the coaly slates show remarkable positive Eu anomaly. l11e redox-sensitive trace elements such as V, Cr, Co, Ni, Mo and U in the coaly slates are highly enriched compared to the black slates, especially for V of 24 times, Mo of 62 times, and U of 60 times. In additions, Pd and Pt are also enriched in the coaly slates. Positive Eu anomaly and the noticeable enrichment of the elements listed above compared to those of NASC indicate that those elements were not derived from common seawater but deposited under high temperature and reducing environment of submarine hydrothermal activities. Wide compositional ranges of major elements ($SiO_2/Al_2O_3$: 3.98~11.88, $Al_2O_3/Na_2O$: 25.6~139.06, $K_2O/Na_2O$: 6.80~46.85) also suggest that the source rocks of the sediments are mixtures of sedimentary rocks and igneous rocks. Higher sulfur contents in the coaly slates, 2.6 wt.%, than those in the black slates, 0.6 wt.% also indicates that the former was influenced by hydrothermal activities containing much sulfur. These geochemical characteristics are similar to the genetic environments of South China type PGE deposits (Mo-Ni-Zn-PGE) which is geotectonically correlated with the Ogcheon Metamorphic Belt and is known as sedimentary-exhalative deposits. In conclusions, the uranium and other metallic elements mineralization seems to have occurred in the sedimentary basin that was affected by submarine hydrothermal activities and rich in organic materials under oxygen-poor environments as well.
