Sleep Stage Analysis by using Polysomnogram and Spindle Wave (다원수면검사와 방추파에 의한 수면단계 분석)

  • SAC 847 컴퓨터를 통해서 뇌파를 기본으로 턱과 다리의 근전도, 몸의 뒤척임, 심전도, 혈중 산소 농 도, 안전도 등을 동시에 기록하는 다원수면검사(polysomnogram)를 전자기가 차폐된 수면실에서 실시하 였고 수면단계기록 국제기준에 의한 수면단계와 최근 새롭게 제시되고 있는 수면의 경과에 따른 수면방 추파(sleep spindle)의 변동추적에 의한 수면단계 판정방법을 비교 분석하였다. 또한, 수면경과에 따른 .beta. .alpha. .theta. .delta. 파형의 발생빈도를 제시하고 평가하였다. 이러한 수면단계 분석은 종합 수면생리신호의 일환으로 인간공학적인 쾌적침대개발에 활용될 수 있을 것이다.

A Case of Narcolepsy by Excessive Daytime Sleepiness (주간졸음증 환자로부터 기면증 1예 보고)

  • We report a case of narcolepsy. A 25-year-old man has had excessive daytime sleepiness of about 10 years durations. He awakens daily feeling exhausted and continually falls asleep during the day while engaged in such situation like reading and watching television. He has exhibited cataplexy, a sudden loss of muscular tone, brought on by emotion, usually laughter. Polysomnogram revealed increased sleep stage 1, 2 and decreased deep sleep. Multiple sleep latency test (MSLT) showed that sleep latency was 1.33 minutes and there were 3 noted sleep onset rapid eye movement (SOREM) on 5 trials. The epworth sleepiness scale (ESS) was 17/24. Typing of HLA haplotype that was positive for the $DQB1^{\ast}0602$ allele, and hypocretin-1 (orexin A) could not be detected in cerebrospinal fluid (CSF). Brain MRI showed normal image. We diagnosed his case as narcolepsy based on history of cataplexy, and three occurances of SOREM, and positive of HLA haplotype.

  • Sleep Apnea Syndrome is characterized by hypersomnolence, snoring, and sleep apnea. The symptoms of hypothyroidism include apathy, somnolence, lethargy, personality change, and intellectual deterioration and may be related to hypothyroid-related sleep disorders. Central, obstructive, and mixed patterns of sleep apnea may be observed in hypothyroidism. A 60-yr-old man was admitted due to hypersomnolence, snoring and generalized edema. Polysomnogram showed obstructive pattern of sleep apnea syndrome. Neck CT scan revealed narrowing of upper airway which is consistent with obstructive sleep apnea syndrome. Physical examination and hormonal study also disclosed the evidence of hypothyroidism. We report a case of sleep apnea syndrome due to primary hypothyroidism with review of literatures.

  • The Authors report a case of sleep paralysis patient who is 25-years old man. He complained that he has been experiencing episode "being suffocated" during sleep and could not move by himself while semiconsciousness state. He was a only son of divorced parents and could not attach anybody from yong childhood. His symptoms was began intermittently after his early teens. Medical history and physical examination revealed no significant finding. A nocturnal polysomnogram showed loss of muscle tone during terminal REM period and sudden hyperventilation attack followed by wake-up. Psychosocial stress and insufficient sleep were presumed to be an etiological factors. Imipramine 50mg per day almost completely relieved symptoms.

  • The purpose of this study was to find out the relationship between sleep physiological signals data and subjective feeling of sleep quality. Sixteen subjective were investigated and they slept on both comfortable mattress and uncomfortable mattress. Information of sleep stage is one of the most important clues for sleep quality. Polysomnography is basically the recording of sleep. The several channels of brain waves (EEG), eyes (EOG), chin movements (EMG) and heart (ECG) were monitored. Sixteen subjects spent 6 days and nights in the laboratory and the data of sleeping 7h for each of 3 nights was analyzed. Percentage of deep sleep (III and IV, sleep efficiency, WASO, stage 1 and subjective feeling of sleep quality were significantly affected with mattress types (comfortable and uncomfortable mattress). When subjects slept on comfortable beds, percentage of deep sleep and sleep efficiency were higher than those of uncomfortable bed. The percentages of wake after sleep onset and stage 1 were lower when subject slept in a comfortable bed. The subjective feeling of sleep quality agreed with the recorded sleep data also.

  • 구개인두성형술이 코골음과 수면무호흡증에 유용한 수술적 방법으로 널리 이용되고 있으나 코골음과 수면무호흡증의 유발기전이 어리기지 복합적인 원인에 의하므로 구개인두성형술의 적응증을 결정하는데 많은 어려움이 있어, 수술전에 이 방법의 적응이 되는 환자를 보다 정확히 선택함이 중요하므로 많은 사람들에 의하여 주된 병변부위를 검시하는 방법들이 시행되어 왔다. 예를 들면 Mueller maneuver 중 내시경검사, Cephalometrics, polysomnogram, Multiple sleep latency test, CT scan 등의 방법이 환자선택에 이용되었다. 저자들은 비교적 쉽고 경비가 들지 않는 술전 검사법으로 양 후구치봉선(retromolar raphe)을 연결하는 선상에서 구인두궁 (soft palate arch)의 길이를 비교하는 방법을 총 89례에 적용 시행하여 그중 추적관찰이 가능한 46례에서 코골음은 86%, 수면무호흡증은 82%에서 자각적 증상의 개선이 있었기에 구개인두성형술을 시행함에 있어 양 후구치봉선거리와 구인두궁 거리를 비교하는 방법이 간단하고 경제적인 술전 진단방법으로 사료되어 문헌고찰과 함께 보고하는 바이다.

Polysomnographic Assessment of Nocturnal Enuresis in Adults: A Case Study of Parasomnia Overlap Syndrome With Obstructive Sleep Apnea

  • Enuresis nocturia is more prevalent in children than in adults. Using polysomnography (PSG), we investigated the causes of adult enuresis nocturia in a 20-year-old female patient with nighttime bedwetting. In spite of normal urological examinations, her detailed medical history disclosed frequent sleep paralysis and urination during dreams. During PSG, two electromyograms were attached to her anus to assess the tone of her bladder's sphincter while she slept. During REM sleep, the EMG tone of the mandible decreased, but the anal and bladder sphincter tones did not. The polysomnogram revealed moderate obstructive sleep apnea. Consequently, she was diagnosed with adult parasomnia (nocturnal enuresis) overlap syndrome with OSA. This study demonstrates the value of PSG with simultaneous anal tone EMG for diagnosing NREM parasomnia and nocturnal enuresis.

Attention Deficits and Characteristics of Polysomnograms in Patients with Obstructive Sleep Apnea (폐쇄성 수면무호흡증 환자의 주의력 결함 및 수면다원검사 특징)

  • This study tried to examine the characteristics of attention deficits in patients with Obstructive Sleep Apenea(OSA) with different age levels, and to examine which indices of polysomnograms might be related to the indices of attention deficits in OSAs. Two age-level groups and a normal control group were subjected to two computerized attention tests, including a continuous performance test(CPT) and a change blindness task(CBT). In addition, the three groups were subjected to a Polysomnography to extract several sub-indicators of polysomnogram, and an Epworth Sleepiness Scale which measures subjective sleepiness. As results, the OSAs showed significantly more omission and commission errors in CPT, and they showed lower accuracy in CBT compared to the normal group. The results of a correlational analysis showed that attention deficits in OSA are significantly correlated with arterial oxygen saturation among sub-indicators of polysomnograms. In conclusion, OSAs seems to be less attentive, having difficulties in response inhibition, and having deficiencies in noticing important environmental changes. Age seems to make these deficiencies even worse. Especially, the relationship between attention deficiency and hypoxia which could cause irreversible cerebrum damage has an implication in cognitive impairment prevention through early treatment.

A CASE OF NARCOLEPSY IN A 11 YEAR-OLD BOY (소아 기면증 1예)

  • Narcolepsy's age at onset is reported to be relatively homogeneous, occuring usually after the onset of puberty, although most cases are diagnosed when the patients are in their late teens to late 20s. It is very unusual for a patient to develop narcolepsy before 15 years of age or after 30 years of age. A 11-year old boy who has developed excessive daytime sleepiness since age of 7 and has all the four major features of narcolepsy by the time of evaluation is presented. On polysomnographic examination, the patient showed two sleep onset REM periods in the three latency test of the multiple sleep latency test and the nocturnal polysomnogram. In addition, the findings of typing HLA class I and II of the patient's family are presented. Reports of pediatric narcolepsy previously reported are reviewed.

  • The authors report a case of REM sleep behavior disorder which occurred in a 69-year-old business man. He began experiencing episodic "acting out" behavior of his dream at the age of 66. The episodes tended to be associated with dream contents, mainly being chased or threatened. Before that, his sleep was relatively quiet despite occasional nightmares, midsleep arousal, and shallowness. Injuries resulted from leaping out of bed, jumping onto furnitures, and grabbing and biting the spouse's arm. Most recent dream-incurred laceration of chin required 5 sutures. Medical and psychiatric history revealed no significant findings except that he had been overanxious all his life within himself with others' reputation of himself as pleasant and easy-going. A nocturnal polysomnogram showed repeatedly intermittent increase of chin and/or leg muscle tones during otherwise characteristic REM sleep period. The overnight video recording revealed head lifting and limb movements during REM sleep periods. Brain MRI and EEG were normal. Job-related stress was presumed to be an etiological possibility. Clonazepam 0.25-0.5mg nightly almost completely relieved the symptoms.
