노인의 주관적 건강인식과 의료기관 이용에 있어 건강증진행위의 매개효과 (Mediating Effects of Health-Promotion Behaviors on the Correlation between Self-Perceived Health and Medical Care Utilization among Older Adults)

  • Objectives: This study aimed to investigate the mediating effects of health promotion behaviors on the correlation between self-perceived health and medical care utilization among older adults. Methods: This study used data from a. Data were analyzed with frequency analysis, descriptive statistics, Pearson's correlation coefficient, and hierarchical multiple regression. Mediation analysis was performed according to the Sobel test. Results: The self-perceived health and some health promotion behaviors in older adults had a statistically significant effect on medical care utilization. Only the nutritional management of health promotion behaviors had a mediating effect on the relation between the self-perceived health and medical care utilization of older adults. Conclusions: The nutritional management of older adults was confirmed as an important factor to health promotion and prevention. This study was meaningful in that it identified the political directions for establishing health care policy for the older adults.

요양보호사의 노인인권의식과 서비스 질에 대한 인식 수준의 관련성: 인권교육 조절효과 중심으로 (Association between Caregiver's Awareness of Human Rights and Quality of Service: Focused on Human Right Education)

  • Background: Long-term care insurance for the elderly has been stably established along with the quantitative expansion of long-term care facilities. Indeed, the need for a paradigm about human rights-based service approach is being raised throughout society from a service perspective. Therefore, this study aimed to analyze the association between elderly human rights awareness and quality of service by considering human rights education as a moderate variable. Methods: This study conducted surveys with 138 caregivers working in long-term care facilities located in Seoul and Gangwon. General characteristics, awareness of human rights, and the level of service quality were examined using descriptive statistics, frequency analysis, and correlation analysis. And multi-variable linear regression with a hierarchical framework was employed. These analyses were performed using IBM SPSS ver. 25.0. Results: Of the 138 caregivers, 97.1% were female, 87.7% were more than 50 years old, and most of their education level was high-school graduates. Their length of employment ranged from more than 5 years to less than 10 years. The level of awareness regarding elderly human rights of the elderly was below normal (mean=2.21), but the quality of service was high (mean=4.21), and the need for human rights education was also high (mean=4.28). Among the general characteristics, the length of employment was significantly associated with awareness of elderly human rights. Moreover, political rights awareness, included as sub-domains of human rights, was positively associated with quality of service. However, the moderating variable, human rights education, was not significantly associated with the quality of service. Conclusion: In this study, human rights education, as a moderating variable, did not have a statistically significant effect on caregivers' human rights awareness in relation to service quality. This finding is inconsistent with previous research results. These results can be explained by the fact that the frequency of education in long-term care facilities was a significant factor in the practice of protecting the human rights of the elderly. Therefore ongoing encouragement for the frequency of current human rights education and improvements in the educational approach appear to be necessary. In addition, these findings reveal the need for strength of education policies and effective in-depth research about human rights and quality of service to respect the human rights of the elderly.

성년후견제도에 대한 성년후견수요자의 부모와 서비스 제공자의 인식 비교 연구 (A Comparative Study on perception of Consumer parents and Service Provider on the Adult Guardship System)

  • 성년후견제도는 2013년 7월에 시행될 예정이다. 본 연구는 성공적 시행을 위해 성년후견제도에 대한 인지도를 알아보고 후견인 중 시민후견인의 활성화를 위해 장애인 성년후견수요자의 부모와 서비스 제공자의 인식을 비교함으로서 정책적 함의를 도출하는데 목적이 있다. 연구 결과, 첫째, 성년후견제도의 인지도는 높으나(78.9%), 이해도는 낮았다(35.2%). 둘째, 후견수요자의 욕구는 재산관리 보다 병원 이용과 사회보장급여 관리에 대한 욕구가 높다고 볼 수 있으며, 향후 제도 이용 의향은 63.3%였다. 마지막으로 시민후견인에 대한 부모집단과 서비스제공자집단은 시민후견의 유형, 비용, 비용부담 주체와 부담액, 시민후견인 양성 프로그램 내용에 대해 유사한 인식 수준을 가지고 있었다.

정신병적 장애로 첫 내원한 환자들의 임상 특징과 투약 순응도 및 의료 이용 추이: 예비 연구 (Clinical Characteristics, Drug Adherence to Antipsychotics and Medical Use Trends in Patients First Diagnosed with Psychotic Disorder: A Preliminary Study)

  • Objectives: In this preliminary study, we investigated the clinical characteristics of patients who were first diagnosed with psychotic disorder and explored the impact of the adherence to antipsychotics on long-term medical use. Methods: All national health insurance claims related to psychotic disorders including gender, age, income, and drug compliance, from January 1, 2008 to December 31, 2015, were examined. With trend test using Medication Possession Ratio (MPR), we compared the medical use between the compliant group (MRP≥0.8) and the comparative non-compliant group (0.2≤MPR<0.8). Results: Among 28,095 participants in total, 16,239 patients (57.8%) were diagnosed as schizophrenia; the 30s were the most common (n=7,151, 25.5%). Drug compliance was generally low regardless of the diagnosis and was the lowest among 20s with the 40-60% range of income. The compliant group showed lower psychiatric and medical use than the comparative group in the following years (p<0.0001). Conclusion: These findings suggest that patients in the 20s and 30s with the 40-60% range of income, who are diagnosed with schizophrenia at the first psychiatric visit, may need more clinical and political attention. The results also emphasize the importance of initial drug adherence to antipsychotics in reducing long-term psychiatric costs.

1930년대 중국 문서당안 행정개혁론의 이해 (The Records and Archives Administrative Reform in China in 1930s)

  • Historical interest in China in 1930s has been mostly focused on political characteristic of the National Government(國民政府) which was established by the KMT(中國國民黨) as a result of national unification. It is certain that China had a chance to construct a modern country by the establishment of the very unified revolutionary government. But, it was the time of expanding national crises that threatened the existence of the country such as the Manchurian Incident and the Chinese-Japanese War as well as the chaos of the domestic situation, too. So it has a good reason to examine the characteristic and pattern of the response of the political powers of those days. But, as shown in the recent studies, the manifestation method of political power by the revolutionary regime catches our attention through the understanding of internal operating system. Though this writing started from the fact that the Nationalist Government executed the administrative reform which aimed at "administrative efficiency" in the middle of 1930s, but it put stress on the seriousness of the problem and its solution rather than political background or results. "Committee on Administrative Efficiency(行政效率委員會)", the center of administrative reform movement which was established in 1934, examined the plan to execute the reform through legislation by the Executive Council(行政院) on the basis of the results of relevant studies. They claimed that the construction of a modern country should be performed by not political revolution anymore but by gradual improvement and daily reform, and that the operation of the government should become modern, scientific and efficient. There were many fields of administrative reform subjects, but especially, the field of records and archives adminstration(文書檔案行政) was studied intensively from the initial stage because that subject had already been discussed intensively. They recognized that records and archives were the basic tool of work performance and general activity but an inefficient field in spite of many input staff members, and most of all, archival reform bring about less conflicts than the fields of finance, organization and personnel. When it comes to the field of records adminstration, the key subjects that records should be written simply, the process of record treatment should be clear and the delay of that should be prevented were already presented in a records administrative meeting in 1922. That is, the unified law about record management was not established, so each government organization followed a conventional custom or performed independent improvement. It was through the other records administrative workshop of the Nationalist Government in 1933 when the new trend was appeared as the unified system improvement. They decided to unify the format of official records, to use marker and section, to unify the registration of receipt records and dispatch records and to strengthen the examination of records treatment. But, the method of records treatment was not unified yet, so the key point of records administrative reform was to establish a unified and standard record management system for preventing repetition by simplifying the treatment procedure and for intensive treatment by exclusive organizations. From the foundation of the Republic of China to 1930s, there was not big change in the field of archives administration, and archives management methods were prescribed differently even in the same section as well as same department. Therefore, the point at issue was to centralize scattered management systems that were performed in each section, to establish unified standard about filing and retention period allowance and to improve searching system through classification and proper number allowance. Especially, the problem was that each number system and classification system bring about different result due to dual operation of record registration and archives registration, and that strict management through mutual contrast, searching and application are impossible. Besides, various problems such as filing tools, arrangement method, preservation facilities & equipment, lending service and use method were raised also. In the process this study for the system improvement of records and archives management, they recognized that records and archives are the identical thing and reached to create a successive management method of records and archives called "Records and Archives Chain Management Method(文書檔案連鎖法)" as a potential alternative. Several principles that records and archives management should be performed unitedly in each organization by the general record recipient section and the general archives section under the principle of task centralization, a consistent classification system should be used by classification method decided in advance according to organizational constitution and work functions and an identical number system should be used in the process of record management stage and archive management stage by using a card-type register were established. Though, this "Records and Archives Chain Management Method" was developed to the stage of test application in several organizations, but it was not adopted as a regular system and discontinued. That was because the administrative reform of the Nationalist Government was discontinued by the outbreak of the Chinese-Japanese War. Even though the administrative reform in the middle of 1930s didn't produce practical results but merely an experimentation, it was verified that the reform against tradition and custom conducted by the Nationalist Government that aimed for the construction of a modern country was not only a field of politics, but on the other hand, the weak basis of the government operation became the obstacle to the realization of the political power of the revolutionary regime. Though the subject of records and archives administrative reform was postponed to the future, it should be understood that the consciousness of modern records and archives administration and overall studies began through this examination of administrative reform.

특공무술의 개발과 발전에 관한 경호사(警護史)적 함의 (The Development and Expansion of Teukgongmusul focused on Security Service History)

  • 특공무술은 은폐된 역사로 인해서 진실이 왜곡되어 있다. 이러한 정황을 찾기 위해서 특공무술 관계자들의 증언을 토대로 한 구술사(口述史)를 연구방법으로 적용했고, 대외비(對外秘)로 발간된 대통령경호실 육군본부 제27부대 제5특전여단 등의 문헌자료들도 정보공개 과정을 거쳐서 수집했다. 첫째, 박정희 대통령의 지시로 1978년 6월 1일에 창설된 특전사 소속 제606부대는, 1978년 7월 2일 대통령경호실에 배속되어 국내 최초의 항공기 대테러를 목적으로 비밀리에 운용되었다. 그리고 제606부대에서는 기존 무술들의 장점을 합한 실전 종합무술로서 특공무술을 체계화하였다. 둘째, 특공무술은 경호무도의 특성을 가지고 개발되었다. 제606부대원들은 대통령경호실 소속 특공부대의 일원으로 특공무술을 개발했고, 이에 특공무술은 개발단계부터 경호무도로서의 특성을 포함하게 되었다. 셋째, 제606부대의 특공무술은 군과 경호실로 나뉘어 발전했다. 군에서는 제5공수특전여단에 의해 새롭게 구성한 국방무술로 보급되었고, 대통령경호실에서는 기존에 수련하던 합기도를 특공무술로 교체하여 보급되었다. 넷째, 특공무술의 개발과 발전은 정치적인 배경에 많은 영향을 받았다. 특공무술을 개발한 부대는 당시 정치적인 권력이 막강했던 경호실의 통제 하에 있었으므로, 이후 부대의 명칭이나 활동의 변화도 한국 내 정치적인 상황과 밀접하게 연관되었다. 이 같은 점은 제606부대가 제27부대로 개칭한 뒤에 차기 대통령의 경호를 담당하게 되면서부터 본격적인 경호업무로 전환했다는 점에서도 나타난다.

텍스트마이닝을 활용한 메타버스 서비스의 경험 품질 평가의 이해: 로블록스 사례 연구 (Understanding the Evaluation of Quality of Experience for Metaverse Services Utilizing Text Mining: A Case Study on Roblox)

  • 메타버스는 정치, 경제, 사회, 문화적 활동이 가능한 가상의 환경을 나타내는데, 이를 통해 현실과 디지털이 공존하여 사람들은 색다른 일상을 경험할 수 있다. 최근 메타버스의 발전으로, 기존의 서비스 경험 방식이 변화하고 있다. 기존의 선행연구는 주로 메타버스 서비스의 기술 발전에 중점을 두었지만, 최근의 연구들은 고객 관점에서 메타버스 서비스의 경험 품질을 평가하는 것에 중점을 두고 있다. 고객 관점에서 서비스 품질을 결정하는 서비스 특성을 정확히 이해하고 분석해야, 성공적인 메타버스 서비스를 설계할 수 있기 때문이다. 그러나, 선행연구들은 이러한 중요성만을 강조하고 있을 뿐, 평가를 위한 보편적이고 체계적인 개념과 관련된 연구는 부족한 실정이다. 본 연구는 이러한 한계를 극복하기 위해 텍스트마이닝을 활용한 온라인 리뷰 분석을 수행하였다. 특히, 로블록스 서비스의 온라인 리뷰 227,332건을 분석하고, 분석 결과를 기반으로 로블록스 서비스의 개선 방향을 모색하였다. 분석을 위해 토픽 모델링, 감성 분석, 로지스틱 회귀 분석 등의 텍스트마이닝 및 기계학습 알고리즘을 활용하였으며, 서비스 개선 방향을 모색하기 위해 중요도-실행도 분석을 수행하였다. 연구 결과, 메타버스 서비스의 경험 품질 평가에 활용 가능한 9개 서비스 특징을 도출하였으며, 이들과 서비스 만족도 간의 관계 분석을 통해 특징별 중요도를 추정하였다. 마지막으로 중요도-실행도 분석을 통해 메타버스 서비스를 가능케하는 기술적 요소보다 서비스 경험을 강화하는 방향의 서비스 개선 전략이 필요함을 파악하였다. 본 논문의 결과물은 메타버스 서비스에 관심이 있는 기업들에게 중요한 시사점을 제공하며, 기업은 이러한 서비스 특징을 활용하여 자사의 강점 및 약점을 파악하여, 변화하는 메타버스 서비스의 환경에서 우위를 차지하는데 유용한 통찰력을 제공할 것이라 기대한다.

이용자 중심(User-Centered) 서비스 경험에 관한 현상학적 연구 -노인돌봄종합서비스 이용자를 대상으로- (A Phenomenological Study of the Experience of User-Centered Services - Focusing on the Users of Comprehensive Care Service for the Aged -)

  • 본 연구는 노인돌봄종합서비스 이용자들이 이용자 중심 서비스를 어떻게 경험하고 있는지 이해하는데 그 목적이 있다. 이를 위해 이용자 중심서비스의 실천적 원칙으로 강조되는 이용자들의 선택의 의미는 무엇이며 어떠한 맥락에서 과연 어떻게 이루어지고 있는지에 대한 서비스 이용 경험의 본질적 측면에 초점을 두었다. 자료 분석은 현상학적 연구에서 Giorgi가 제시한 연구방법을 사용하였으며 서울시내 65세 이상 이용자 10명을 대상으로 심층면접을 진행하였다. 분석결과, 참여자들의 서비스에 대한 이용자로서의 경험은 "제한된 서비스 선택", "강화되지 못한 이용자로서의 권한", "서비스에 대한 모순된 감정", "돌보미와의 관계 중요성 인식" 이라는 4개의 구성요소로 도출되었으며, 이들의 서비스 경험의 본질적 현상은 "제한된 선택권 안에서 관계의 중요성"으로 나타났다. 이 연구를 통해 이용자 중심으로서의 서비스 이용경험은 여전히 이용자로서의 선택은 제한적으로 이루어지고 있었으며 이용자들은 자신에게 주어진 서비스 선택권보다 돌보미와의 긍정적 관계를 더욱 중요하게 인식하고 있음이 발견되었다. 이러한 연구결과를 바탕으로 이용자 중심 서비스가 정착될 수 있는 정책적 함의를 제시하였다.

전자정부 서비스 사용에 있어 온라인 신뢰에 관한 연구 (A Study on Antecedents of Online Trust in the Context of e-Government Services)

  • 시민들의 전자정부 서비스 활용에 있어 신뢰는 가장 중요한 요소로 평가되고 있다. 본 연구는 정보프라이버시, 서비스 사용과정상의 상호작용, 주관적 규범, 구전효과 등의 요인이 신뢰에 미치는 영향 정도와 서비스 신뢰가 정부정책에 대한 신뢰와 정치적 효능감 제고로 이어지는 연구가설을 제시하였다. 가설검증을 위해 서비스 사용자들에 대한 설문조사를 실시하였으며 PLS(Partial Least Square) 기법을 이용하여 통계적 분석을 시도하였다. PLS는 측정 구조모형에 대해 실증 평가를 동시에 할 수 있게 지원하는 기법으로 주요 요인 간의 인과관계를 분석하는데 효과적이다. 설문자료의 분석결과, 구전효과와 상호작용 및 주관적 규범은 서비스 신뢰에 영향을 미치며, 서비스 신뢰는 다시 정부에 대한 신뢰와 정치적 효능감 제고로 이어진다는 결론이 도출되었다. 다만 정보 프라이버시 침해 우려는 서비스 신뢰에 영향을 미치지 않는다는 예상과는 다른 결론이 도출되었다.

국가이미지가 중국의료시장 진출에 미치는 영향에 관한 연구 (Country Image and Its Impacts on the Entry into the Medical Services Market in China)

  • This study is focused on the medical services market in china which would be the largest one in the world sooner or later. An empirical research has been performed on the country images and related buying attitudes of the Chinese potential consumers for foreign medical services of more higher level. Upon the basis of this research results, the components of a country image has been restructured and the country image effects on the process of a purchasing decision of the advanced foreign medical services in China has been investigated and analyzed. This research shows that the forming process and the dimensions of a country image in Chinese consumers are rather simplified than the former researches of the same kind in any other countries. In China the expectation and buying intension for foreign medical services is found to be affected directly by a country image. Furthermore among various components of a country image the expected service quality level of the Chinese is found to be mostly dependent on the social stability and safety rather than on the degree of economic developments. Recently breaking through the domestic medical market crisis, more and more hospitals consider to advance into Chinese medical market. This research shows that the reexamination and political concerns on the country image of Korea are needed in the level of government's public relations. Especially the proactive policy making and propaganda of political, social and economic stability and safety in Korea are thought to be more important for successful entry in Chinese medical services market.
