Household Out-of-Pocket Payments and Trend in Korea (가계직접부담 비용의 현황과 추이)

  • After the announcement of Moon Jae-in Government's plan (Moon's Care) for Benefit Expansion in National Health Insurance in August 2017, it is necessary to monitor the effects of the policy, especially household out-of-pocket payments (OOP). This paper aims to observe the current status and trend of OOP in Korea. Current health expenditure (CHE) was 144.4 trillion won in 2018, which accounts for 8.1% of gross domestic product (GDP) increased 9.7% from the previous year. Although GDP's share of CHE has been close to the average of the Organization for Economic Cooperation and Development (OECD) countries, the public fund's share was 59.8% of the total in 2018, which was lower than the OECD average of 73.5%. OOP's share was 32.9% in 2018, which decreased from 37.4% in 2008. The share of OOP of non-covered services was 20.0% in 2018, which decreased from 22.9% in 2008. The share of cost-sharing with third-party payers was 12.9% in 2018, which decreased from 14.5% in 2008. The OOP of non-covered services was significantly decreased in hospital and inpatient curative care, but the OOP of non-covered services was significantly increased in the medical clinic. The effect of Moon's Care was not showed in OOP through the results of 2017 and 2018, but further monitoring is needed because the Moon's Care is progressing and the observational period is short.

  • Exposure of the cardiac pacemaker is still a difficult problem associated with high morbidity. Grossly infected pacemaker systems had been required removal and insertion of a new system. It would highly desirable to develop a new method that could salvage an exposed well functioning unit because cardiac pacemaker is very expensive. We have experienced one patient with exposed well functioning cardiac pacemaker, which was relocated to contralateral subpectoral pocket and healed without complication. This relocation of the pacemaker to subpectoral pocket is less simple, expensive and should be considered as an alternative to complete replacement of unit.

  • For high performance transistor in the 0.14um generation, high speed sram is using a weak region of SCE(Short Channel Effect). It causes serious SCE problem (Vth Roll-Off and Punch-Through etc). This paper shows improvement of Vth roll-off and Ion/Ioff characteristics through high concentration Pocket implant, LDD(Light Dopped Dram) and low energy Implant to reduce S/D Extension resistance. We achieve stabilized Vth and Improved transistor Ion/Ioff performance of 10%.

  • This study was to identify factors influencing drug compliance based on the subjects' interview regarding community pharmacy utilization for 2 weeks, in 2005 KNHANES. Good compliance was regarded as important factors in improving the effectiveness and minimizing adverse drug reaction, resulting in reducing the medical costs. 83% of total 11,208 pharmacy visits in 7,066 subjects showed good compliance. Good satisfaction for pharmacist's medication counseling (OR=2.23, 95% CI 1.92-2.58), higher out-of-pocket money (OR=1.32, 95% CI 1.14-1.54), and users of prescription drugs than non-prescription (OR=2.21, 95% CI 1.91-2.57) drugs were significant factors for better compliance. Disease of nervous system and mental and behavioral disorders showed lower drug compliance.

Tooth loss in aggressive periodontitis patients: retrospective study with follow-up of 3 to 8 years

  • The purpose of this study was to assess the clinical outcomes of active periodontal therapy (APT) and supportive periodontal therapy (SPT) through loss of teeth in the Korean patients with aggressive periodontitis. The number of missing and residual teeth, probing pocket depth were examined in 33 patients diagnosed with aggressive periodontitis at the baseline and who had participated in APT and SPT for 3 years or more. A 20 and 3 teeth were lost during the APT and SPT, respectively. The mean tooth loss rate of 0.13 per patient and year. There was no significant statistical difference in clinical improvement between the smoker and non-smoker groups. Within the limitation of this study, APT combined with supportive therapy could decrease the risk of tooth loss in patients suffering from aggressive periodontitis.

  • Fine blanking is a high-precision process combining principles of metal stamping and cold forming. Unlike conventional metal stamping, fine blanking uses a special triple action such as V-ring force, counter force, shearing force. This study performed the effect of pocket-shaped compression molding on the mold life of the fine blanking using the 7.4mm thick SM45C material. In order to determine the lifespan of the punch and die in the fine blanking molds, a trial mold was manufactured and various punch materials were selected to perform the mold life test. A study on the life of a fine blanking mold by applying a thick plate material was experimentally performed through a mold test.

Design of Haebaragi Park (해바라기 공원설계)

  • The purpose of designing Haebaragi park, legally designated as children park, is to make a place for children including residents performing outdoor recreation, various social interactions, and cultural activities. Design concept for space plan have twofold; the one is a positive and creative playing space and facilities for children, escaped from a monotonous playground, and the other is a cultural and social space for neighboring communities. The site having the area of 1,316.7 square meters, located in Nowon-Dong, Buk-Gu, Daegu metropolitan City, is just like a vest pocket park. The adjacent area had been developed a slum area with mixed ad visually conflicting land use patterns and low income groups. The children and residents living in the area do not have any public space suitable for playing and/or rest. After analyzing such locational characteristic as accessibility, land use of the communities, and potentials for park development, and such design concept as arrangement of facilities, efficient use of site, and functional allocation of park space, We have mad a plan for composition of spaces for various activities, provision of facilities based on estimation of user-demand and activities, and planting. In the design process, we have tried to harmonize functional spaces with facilities, and to organize all the functions as a whole. To improve urbanity and aesthetic shape of park design, we have introduced a central plaza, design of a pave floor, a torrent, large trees for shade, colonnades and so on. From this design project, we can develop the site as a children park for increasing creativities and various playing opportunities, and as a resident space for rest, cultural activities. In the future, it is required that many attempt to design and develop urban small space as a park for children and residents.
