A case of pseudohypoaldosteronism type 1 with a mutation in the mineralocorticoid receptor gene

  • Pseudohypoaldosteronism type 1 (PHA1) is a rare form of mineralocorticoid resistance characterized in newborns by salt wasting with dehydration, hyperkalemia and failure to thrive. This disease is heterogeneous in etiology and includes autosomal dominant PHA1 owing to mutations of the NR3C2 gene encoding the mineralocorticoid receptor, autosomal recessive PHA1 due to mutations of the epithelial sodium channel (ENaC) gene, and secondary PHA1 associated with urinary tract diseases. Amongst these diseases, autosomal dominant PHA1 shows has manifestations restricted to renal tubules including a mild salt loss during infancy and that shows a gradual improvement with advancing age. Here, we report a neonatal case of PHA1 with a NR3C2 gene mutation (a heterozygous c.2146_2147insG in exon 5), in which the patient showed failure to thrive, hyponatremia, hyperkalemia, and elevated plasma renin and aldosterone levels. This is the first case of pseudohypoaldosteronism type 1 confirmed by genetic analysis in Korea.

Dopaminergic Activity가 부종상태에서 혈청 Aldosterone 분비에 미치는영향 (The Effect of Dopaminergic Activity on Aldosterone Secretion in Edematous State)

  • To evaluate the effect of dopaminergic activity on aldosterone secretion, the plasma renin activity, serum cortisol and aldosterone were measured by radioimmunoassay in 6 normal controls and 12 patients who had hyponatremia and generalized edema or ascites with possible condition with secondary aldosteronism before and after(15, 30, and 60 min) 15 mg of metoclopramide by iv bolus injection and same method with 500 mg of L-dopa by per oral in 6 normal controls and 12 patients with edema ascites. The result were as follows; 1) The basal level of PRA was higher in patients rather than normal controls but PRA was not influenced by MC or L-dopa administration on both normal controls and patients group. 2) The serum cortisol level was significantly elevated at 30 min after MC injection compared with basal level in normal controls but no significant change was noted in patients group. After L-dopa administration the serum cortisol level was not changed in both normal controls and patients group. 3) The serum aldosterone level was significantly elevated in 15, 30 and 60 min after MC injection in normal controls, and there also same tendency of aldosterone secretion was noticed in patients group. On the other hands, there was no changes in aldosterone level in both normal controls and patients group with L-dopa administration. Above result means that MC stimulate aldosterone secretion by dopaminergic antagonist and aldosterone secretion in normal subject is controlled by maximal tonic dopaminergic inhibition. In edematous patients, however, both of the dopaminergic inhibitory and stimulating effect of PRA, ACTH etc on the aldosterone secretion seems to be variable.

Antihypertensive effect of Korean Red Ginseng by enrichment of ginsenoside Rg3 and arginine-fructose

  • Background: Ginsenoside Rg3 and arginine-fructose (Arg-Fru) are known as the hypotensive compounds of Panax ginseng; however, their efficacy on antihypertension has not been reported yet to our best knowledge. Thus, hypotensive components-enriched fraction of red ginseng (HCEF-RG) was prepared from fine root concentrate (FR) and their antihypertensive effects were investigated in spontaneously hypertensive rats (SHR). Methods: Male SHRs were divided into six groups: control (Wistar Kyoto, SHR); FR 500; FR 1,000; HCEF-RG 500; and HCEF-RG 1,000; samples (mg/kg body weight) were orally administered every day for 8 wk. Blood pressure was monitored at 1 wk, 2 wk, 3 wk, 4 wk, 6 wk, and 8 wk by tail cuff method. At 8 wk after samples administration, mice were killed for the measurement of renin activity (RA), angiotensin-I converting enzyme inhibition, angiotensin II, and nitric oxide (NO) levels in plasma. Results: HCEF-RG with four-fold more Rg3 and 24-fold more Arg-Fru contents was successfully prepared from reacted mixtures of FR and persimmon vinegar (12 times against FR, v/v) at $80^{\circ}C$ for 18 h. Both FR 1,000 and HCEF-RG 1,000 showed lowered systolic blood pressure than SHR control group and HCEF-RG 1,000 group exhibited a significant decrease in diastolic blood pressure. RA was significantly lowered in all treated groups, while angiotensin II did not affect by FR and HCEF-RG treatment. However, angiotensin-I converting enzyme inhibition and NO in FR 1,000 and HCEF-RG 1,000 were significantly increased compared with SHR control group. Conclusion: HCEF-RG is more effective and useful for alleviating hypertension than FR, implying the health benefit of Rg3 and Arg-Fru.

Acute and Chronic Effects of Ethanol on the Cardiovascular and Hormonal Responses to Hemorrhage in Conscious Normotensive and Spontaneously Hypertensive Rats

  • Acute and chronic effects of ethanol (EOH) administration on the cardiovascular and hormonal responses to repeated hemorrhage were investigated in conscious normotensive Wistar rats and spontaneously hypertensive rats (SHR). The chronic EOH treated group received 5% EOH (vol/vol) ad libitum in the drinking water far the first week,10% for the last 2 weeks, and 20% for the last 5 weeks from the age of 6 weeks. The EOH free group received tap water. Chronic EOH and EOH free groups were randomly subdivided into acute EOH infusion and control groups. Under ether anesthesia, catheters were inserted into the femoral vein and both femoral arteries. After rats regained consciousness and their blood pressure was stabilized, responses to quick hemorrhage (5 ml/kg BW) were tested. In the acute EOH infusion group, hemorrhage was induced 20 min after EOH infusion (1.0 g/kg BW), Baroreceptor reflex sensitivity was assessed by the ratio of changes in hen.1 rate and mean arterial pressure (${\Delta}HR/{\Delta}MAP$) immediately after the hemorrhage. Chronic EOH administration elevated MAP in Wistar rats. During acute EOH infusion, MAP do- creased and HR increased in all groups. In comparison to EOH free control rats, acute or chronic EOH treated rats showed a greater reduction in MAP and a smaller elevation in heart rate in response to a hemorrhage. The degree of MAP reduction was significantly greater in SHR than in Wistar rats. Both the acute and chronic EOH administration attenuated the baroreceptor reflex and retarded MAP recovery, again the trend being much more prominent in SHR. The increase in plasma vasopressin and lenin concentrations after hemorrhage were intensified by the chronic EOH administration. SHR showed a greater vasopressin response but a smaller lenin response than Wistar rats. These results indicate that the EOH treated rats, particularly SHB, are prone to shock by a hemorrhage, which may be partly attributed to an impaired baroreceptor reflex function.

온담탕이 고혈압 백서와 인간유래 혈관내피세포주(ECV 304)에 미치는 영향 (An Experimental Study of Effect on ECV 304 Cells, Platelet Rich Plasma and Rats treated with L-NAME by Ondamtang extract)

  • Nitric oxide(NO) play an important role in normal and pathophysiological cells including as a messenger molecule, neurotransmitter, microbiocidal agent, or dilator of blood vessels and artheriosclerosis, hypertension, myocardial infarction, respectively. To investigate that Ondamtang in the potential contribution of the levels of nitric oxide generated by endothelial nitric oxide synthase (eNOS) and the mechanisms of protection against L-NAME, human ECV304 cells, which normally do not express eNOS, were expressed by L-NAME. L-NAME stimulated rat or cells were found to be resistant to injury and delayed death following the Ondam-tang. Inhibition of nitric oxide synthesis abolished the protective effect against L-NAME, thrombin and collagen exposure. Interestingly, such effects have bee observed during stimulation with agents such as KCl on L-NAME mediate rats, were damaged by the NOS inhibitor NG-nitro-L-arginine methyl ester (L-NAME). Cardiovascular diseases is one of the blood vessels and renin-angiotensin system dynfunction. So we studied on herbal medicine that have a relation of vessels endothelium necrosis. In Oriental Medicine, Ondam-tang has been used for disease in relation to cardiovascular system. We studied on the protection and inhibitory effects of cardiovascular diseases in L-NAME induced rat or ECV304 cell lines through the Cell morphological pattern, Tunel assay, LDH activity, heart rate, blood pressure and immunohistochemistric analysis by Ondam-tang. As the result of this study, In group, the anti-apoptosis and necrosis in the cardiovascular system have a potential capacity for prevented, protected and treating the diseases of cardiovascular system, against the necrosis of rat and ECV304 cells with eNOS and calpain expression by L-NAME is promoted.

도담탕(導痰湯)이 뇌손상(腦損傷) 및 고혈압(高血壓)에 미치는 영향(影響) (The Effect of Dodamtang(DDT) on Brain damage and Hypertension)

  • Objective : This study was carried out to investigate the effects of DDT on the brain damage and hypertension. Methods : We observed the effect of Dodamtang(DDT) extract on KCN-induced coma, focal brain ischemia by MCA occlusion, cytotoxicity and protection of PC12 cells and B103 cells induced by amyloid ${\beta}$ protein(25-35). To prove the effect of DDT as a blood pressure depressant, we measured aldosterone, renin activity, catecholamine, sodium and NO density using the seperated blood plasma. Results : DDT showed a protective effect on cytotoxicity of PC12 cells and B103 cells induced by amyloid ${\beta}$ protein(25-35) in a dose dependent manner and proved the significant abridgement of brain ischemic area and edema induced by MCA occlusion, a critical decrease of neurologic deficitic grade in the fore-limbs. DDT didn't reduce the duration of KCN(1.87mg/kg iv.)-induced coma and prolonged the survival rate in the case of KCN(3.0mg/kg iv.)-induced coma by the ratio of 20%. While DDT increased the value of NO in SHR, it significantly decreased the blood pressure of SHR and the value of aldosterone& epinephrine in SHR. Conclusions : These results suggested that DDT might be usefully applied for treatment of hypertension, cerebral infarction, and brain damage.

  • 전신형 PHA 1은 ENaC의 ${\alpha}$ (SCNN1A), ${\beta}$ (SCNN1B), ${\gamma}$ (SCNN1G) 아단위를 암호화하는 유전자의 변이로 경상피나트륨 수송의 결함으로 발생하게 되며 신생아기에 생명을 위협하는 염분 소실, 고칼륨혈증, 대사성 산증이 발생하게 된다. 또한 신장 뿐 아니라 대장, 침샘, 땀샘 및 호흡기상피 등의 다양한 표적 기간에서 전신적으로 알도스테론에 대한 저항성이 나타난다. 최근 전신형 PHA 1의 임상상과 유전자형 및 이환된 환아들의 장기 추적 결과에 대한 보고가 되고 있으나 질환의 희소성으로 임상 표현형을 설명하기는 어려운 실정이다. 저자들은 사망을 초래할 수 있는 심각한 전해질 이상을 보인 환아에서 PHA를 의심하여 염분 및 양이온 교환수지를 투여하여 효과적으로 전해질 교정이 되어 추적관찰 중이며, 유전자 검사를 통해 missense mutation을 나타낸 전신형 PHA1을 경험하였기에 보고하는 바이다.

갈근 열수 추출물이 알루미늄을 투여한 흰쥐의 혈청 효소 활성도와 호르몬에 미치는 영향 (Effects of Water in Extracts of Pueraria Radix on Serum Enzymes Activities and Hormone in Aluminum-Administeredrats)

  • 알루미늄 용액과 갈근 열수 추출액의 급여가 흰쥐의 Aldosterone, rennin의 호르몬과 GOT, GPT. LDH의 효소 활성도에 미치는 영향을 조사하였다. 식이섭취량은 각 실험군간에 유의성은 나타나지 않았으며 체중증가량은 농도를 달리한 알루미늄 단독 급여군에 비하여 갈근과 알루미늄의 병합 급여군이 유의적으로 감소하였다. 신장조직은 다론 조직에 비하여 농도를 달리한 알루미늄 단독 급여군은 $1.78{\sim}1.83g$이었으나 알루미늄과 갈근 병합 급여군은 $2.10{\sim}2.19g$으로 유의적으로 증가하였다 rennin 활성도에서 각각 농도를 달리한 알루미늄 급여군은 $399.98{\sim}464.58ngAl/ml/hr$에 비하여 알루미늄과 갈근 병합 급여군이 $289.74{\sim}88ngAl/ml/hr$으로 알루미늄 단독 급여군에 비하여 유의적으로 감소하였다. aldosterone 활성도는 각각 농도를 달리한 알루미늄 병합 급여군은 1351.60{\sim}1394.83pg/ml$ 알루미늄과 갈근 열수 추출물 병합 급여군이 $954.54(\sim}9632.57pg/ml$으로 알루미늄 병합 급여군에 비하여 감소하였다. GOT는 농도를 달리한 알루미늄 단독 급여군은 $258.60{\sim}395.80IU/L$인데 비하여 알루미늄과 갈근 열수 추출물 병합 급여군이 $314.20{\sim}320.20IU/L$으로 유의적으로 감소하였다. GPT는 농도를 달리한 알루미늄 급여군은 $100.40{\sim}108.20IU/L$인데 비하여 갈근 열수 추출액 급여군과 알루미늄 병합 급여군은 $70.80{\sim}81.00IU/L$으로 유의적으로 감소하였다. LDH는 농도를 달리한 알루미늄 급여군은 $3157.20{\sim}3726.20IU/L$인데 비하여 갈근 열수 추출액 급여와 알루미늄 동시 병합 급여군은 $2846.60{\sim}2993.20IU/L$으로 유의적으로 감소하여 갈근 열수 추출액 급여군에 의한 알루미늄 중독 완화 효과를 볼 수 있었다.

2-kidney 1 clip 신혈관성 고혈압흰쥐에서의 심방이뇨??????타이드의 혈관이완작용의 기전 (The Vasodilating Mechanism of Atrial Natriuretic Peptide in 2-kidney 1 Clip Renovascular Hypertensive Rats)

  • 본 연구의 목적은 2-kidney 1 clip (2K-1C) 신혈관성 고혈압흰쥐에서 심방이뇨펩이드의 혈관 이완작용의 기전을 규명하고 정상혈압흰쥐에서의 혈관이완작용과 비교하는 것이다. 2K-1C 신혈관성 고혈압흰쥐는 정상혈압흰쥐에 비하여 평균동맥압의 유의한 상승과 혈장레닌활성의 증가가 관찰되었다. 2K-1C 신혈관성 고혈압흰쥐의 대동맥에서 norepinephrine (NE)의 수축력의 감수성 및 최대 수축력이 정상혈압흰쥐의 대동맥보다 증가하였다. 심방이뇨펩타이드는 NE에 의한 수축을 농도-의존적으로 각각의 혈압군에서 억제하였다. 그러나, 2K-1C 신혈관성 고혈압흰쥐에서 심방이뇨펩타이드의 NE 억제 작용은 전체적으로 정상혈압흰쥐에서 보다 감소되었다. 그러나 최대 용량의 심방이뇨펩타이드의 NE 이완작용은 양 고혈압군에서 차이가 없었다. 2K-1C 신혈관성 고혈압흰쥐에서 NE에 의하여 $Ca^{2+}$의 유입이 유의하게 증가하였고, 심방이뇨펩타이드는 이 증가를 억제하였다. 정상혈압흰쥐에서도 심방이뇨펩타이드는 NE에 의하여 유의하게 증가된 $Ca^{2+}$의 유입을 억제하였다. 이상과 같은 결과로 볼 때 정상혈압흰쥐와 2K-1C 신혈관성 고혈압흰쥐의 심방이뇨펩타이드의 이완작용의 기전에는 $Ca^{2+}$의 유입 차단이 관여할 것으로 추측되며, 이때 심방이뇨펩타이드의 NE 수축억제에 대한 potency는 2K-1C 신혈관성 고혈압 흰쥐에서 감소하였으나 efficacy는 정상혈압흰쥐와 비교하여 변함이 없음이 관찰되었다.
