캄보디아 환자와 의사의 원격 의료에 대한 반응 조사 (Findings from the Survey Responses by Cambodian Doctors and Patients for Telemedicine)

  • 날로 증가하는 공공의료비 지출을 줄이고, 의료 취약계층에 효과적인 의료 서비스를 제공하기 위해 의사와 환자 간 원격 의료도입이 하나의 대안으로 제시되고 있으나, 의료계에서는 유효성과 안전성 등을 이유로 이에 반대하고 있다. 정부는 지난 10여년간 꾸준히 원격의료의 도입을 위해 다양한 시범사업을 진행하고 있으나, 아직 이를 실제화 시키지 못하고 있다. 이러한 상황에서 본 연구팀에서는 국내에 비해 비교적 원격의료에 대한 거부감이 덜한 캄보디아에서 원격의료 시범사업을 1년 간 진행하였다. 사업진행 후 이러한 새로운 진료방식이 앞으로 확산될 수 있을 것인지를 파악해 보고자 로저스 확산 이론에 근거한 설문을 진행하였다. 설문 분석결과, 의사와 환자 모두 원격진료의 상대적 이점에 대하여 긍정적으로 생각하고 있는 것으로 나타났다. 그러나 원격의료에 사용되는 기기의 복잡도와 가격은 보완 되어야 할 필요가 있는 것으로 나타났다. 또한 캄보디아와 같이 상대적으로 열악한 환경에서도 원격의료 서비스의 제공이 의미가 있는 것으로 분석되었다.

수산자원회복계획의 성과평가 개선방향에 관한 연구 (A Study on Establishing the Performance Evaluation System of the Fish Stock Rebuilding Plans)

  • The Fish Stock Rebuilding Plan (FSRP) is evaluated as one of the most effective fisheries policies domestically and internationally. FSRP is a comprehensive fisheries policy, including target stock size, rebuilding period, application of management measures, and participation of fishermen. The performance outcomes of FSRPs have been partially evaluated for recommendation for stock rebuilding in Korea, while they are fully evaluated in other countries that have implemented FSRPs. The performance evaluation system for FSRP is very needed to enhance its effectiveness as one of fisheries policies. That is, the system where problems can be reconsidered, achieving targets can be evaluated, and the plan can be modified should be implemented. This study classified the performance evaluation of FSRP into the economic evaluation as a quantitative analysis and the policy evaluation as a qualitative analysis. In the economic evaluation, the sustainability of fishery resources, the stability of fishing business, the efficiency of fishery production, and efficient utilization of fishery resources can be considered. As analytical methods for the economic evaluation, the bioeconomic model, efficiency estimation model, the dynamic MEY model can be utilized. The qualitative evaluation is to assess the qualitative performances of FSRPs, including interviews and surveys with fishermen and fisheries experts. In the survey, many different items can be included by evaluation factors which are related to policies such as timeliness, transparency, effectiveness, etc. In addition, the study suggested the performance evaluation system and procedures for objective and efficient evaluation of FSRPs. In order to practically apply the developed performance evaluation model, the pilot project is needed. That is, from the application of pilot project, detailed research methods, effective procedures, and evaluation factors can be investigated.

  • 최근들어 BIM 기술은 건설프로젝트의 전 생애주기동안 생산성 향상을 위하여 활용되고 있는 바, 건설의 각 분야에서 적용함에 있어 프로젝트 별 (간섭체크용, 모델해석용, 공정 및 물량산출용, 공사현장용, 협업용 등) BIM 프로세스의 정립은 필수적 요소이다. 하지만 국내에서 프로젝트 별 BIM 프로세스가 정립되어 있는 사례 및 해당 연구는 아직 마련되어 있지 않은 실정이다. 본 연구에서는 건설사업 초기단계에서의 BIM 어플리케이션 설정 방법, BIM 팀구성 방법, 패밀리설정 방법, 모델링방법 연구를 통하여 BIM 프로세스 구축을 제안하고, Pilot Study 모델을 통한 기초설정 및 모델링 작업, 물량검토, 중립적인 포맷방식인 IFC $2{\times}3$ 버전을 활용한 정보교환 등을 실시하여 정립된 BIM 프로세스의 적용능력을 검토하여 국내 BIM 가이드라인 구축의 초석을 마련하고자한다.

Effect of Distribution System Materials and Water Quality on Heterotrophic Plate Counts and Biofilm Proliferation

  • The biofilms on pipe walls in water distribution systems are of interest since they can lead to chlorine demand, coliform growth, pipe corrosion, and water taste and odor problems. As such, the study described in this paper is part of an AWWARF and Tampa Bay Water tailored collaboration project to determine the effect of blending different source waters on the water quality in various distribution systems. The project was based on 18 independent pilot distribution systems (PDS), each being fed by a different water blend (7 finished waters blended in different proportions). The source waters compared were groundwater, surface water, and brackish water, which were treated in a variety of pilot distribution systems, including reverse osmosis (RO) (desalination), both membrane and chemical softening, and ozonation-biological activated carbon (BAC), resulting in a total of 7 different finished waters. The observations from this study consistently demonstrated that unlined ductile iron was more heavily colonized by a biomass than galvanized steel, lined ductile iron, and PVC (in that order) and that the fixed biomass accumulation was more influenced by the nature of the supporting material than by the water quality (including the secondary residual levels). However, although the bulk liquid water cultivable bacterial counts (i.e. heterotrophic plate counts or HPCs) did not increase with a greater biofilm accumulation, the results also suggested that high HPCs corresponded to a low disinfectant residual more than a high biofilm inventory. Furthermore, temperature was found to affect the biofilms, plus the AOC was important when the residual was between 0.6 and 2.0 mg $Cl_2/l$. An additional aspect of the current study was that the potential of the exoproteolytic activity (PEPA) technique was used along with a traditional so-called destructive technique in which the biofilm was scrapped off the coupon surface, resuspended, and cultivated on an R2A agar. Both techniques indicated similar trends and relative comparisons among the PDSs, yet the culturable biofilm values for the traditional method were several orders of magnitude lower than the PEPA values.

Influencing Women's Actions on Cervical Cancer Screening and Treatment in Karawang District, Indonesia

  • Introduction: The impact of cervical cancer prevention programs depends on persuading women to go for screening and, if needed, treatment. As part of an evaluation of a pilot project in Indonesia, qualitative research was conducted to explore the factors that influence women's decisions regarding screening and treatment and to generate practical recommendations to increase service coverage and reduce loss to follow up. Methods: Research was conducted at 7 of the 17 public health centers in Karawang District that implemented the pilot project. Interviews and focus group discussions were held with 20 women, 20 husbands, 10 doctors, 18 midwives, 3 district health officials, and 16 advocacy team members. Results: Free services and mobile outreach events encouraged women to go for screening, along with promotional efforts by community health workers, advocacy teams, and the mass media. Knowledge and perceptions were the most important barriers to screening: women were not aware of cervical cancer risks, did not know the disease was treatable, and were fatalistic. Factors facilitating treatment were social support from husbands, relatives, and friends and the encouragement and role modeling of health workers. Barriers to prompt treatment included limited access to services and the requirement for husband's consent for cryotherapy. Conclusion: As cervical cancer prevention services are scaled up throughout Indonesia, the findings suggest three strategies to expand screening coverage and ensure prompt treatment: strengthening community mobilization and advocacy activities, modifying the service delivery model to encourage a single visit approach to screening and treatment, and working to gain men's support.

3차원 국토공간정보 구축 개선방안 연구 (Improvement Scheme for 3D Land Geospatial Information Construction)

  • 우리나라는 2004년 시범사업을 시작으로 현재까지 10개 지자체에서 3차원 국토공간정보를 제작완료 하였으며, 추가적으로 많은 지자체에서 구축 중에 있다. 현 3차원 국토공간정보 구축지침은 2005년 3차원 공간정보구축 시범 사업의 연구결과를 기반으로 2006년 구축방법론과 관련 규정을 작성하였다. 그러나 지자체 3차원 국토공간정보 구축에 현 지침의 적용 결과, 3차원 공간정보 제작방법, 표현 정확도 등에서 많은 문제점이 대두되고 있다. 본 연구는 무결점 3차원 국토공간정보 구축을 위하여 기 수립된 구축지침의 문제점을 분석 도출하여 3차원 국토공간정보 구축항목의 재정립 및 제작방법의 표준화된 기준을 제시하는 것을 목적으로 수행하였다. 3차원 국토공간정보 개선방안은 표준화된 양질의 3차원 국토공간정보의 구축 및 활용에 기여할 수 있을 것이라 사료된다.

산림경관서비스 지불제 도입을 위한 시범사업 설계 (Pilot-Project Design on Introduction of Payment of Forest Landscape Service)

  • Payment of Forest Landscape Service (PFLS) is based on the value of landscape conservation and is a positive forest policy inducing the owners of mountains to improve environmental service quality with economic incentives. The purpose of this study is to test the feasibility of PFLS and find out the elements related to PFLS such as associated statutes, target applications, eligible owner's requirements, and applicable environmental services. Research sites were selected in designated reserved forests by law and surveys were carried out with 28 professional forestry engineers and 10 owners of reserved forests located in Chungnam Province in November, 2008. As a result, the owners are willing to participate pilot-project of PFLS if they could have tax incentives. Preferred activities in their forestry are eco-tourism and carbon emission trading as PFLS business model. Although they expect low economic benefit from the PFLS, respondents answered introducing PFLS will give good opportunities for owners of a reserved forest to enhance willingness to manage their forestry properly for the landscape conservation. In this study, PFLS evaluation indicators and policy directions are established and recommends the strategies to cope with changing needs of forestry conservation by inducing the owners' active participation in the sustainable forest landscape management.

중해상도 위성영상을 이용한 지도제작 시험연구 (A Pilot Project on Producing Topographic Map Using Medium Resolution Satellite Image)

  • 본 연구에서는 중해상도 위성영상으로 대표적인 SPOT, 아리랑, IRS-1C위성영상에 대하여 각기 하나의 입체모델을 이용한 지도제작 실험을 수행하였으며, 각각에 대한 위치정확도, 판독 및 묘사가능한 지형지물의 분석, 기존 수치지도와의 비교 등을 통하여, 제작가능한 지도축척의 범위에 대한 기술적인 검토 및 경제성 검토를 하였다. 도화작업에 있어서, SPOT 영상은 수치입체도화기를 사용한 3차원 도화를 하였으며, 아리랑 및 IRS-lC 위성영상은 데이터에 대한 시스템 지원상의 문제가 있어서 영상정합 방법으로 수치표고모형을 생성한 후 정사보정영상을 제작하여 head-up digitizing에 의한 2차원 도화를 실시하였다. 실험 결과 중해상도 영상으로 큰 도로, 하천, 수애선 등 일부 항목에 대하여 판독과 묘사가 가능하였지만, 불가능한 항목이 많았다. 지도제작 측면에서 SPOT은 1/50,000지형도 수정용, 아리랑 및 IRS-lC는 1/25,000지형도의 수정용으로 활용이 가능하다고 판단되었다.

공동체의식이 온라인 주민참여에 미치는 영향 - 서울시 '맑은 아파트 만들기' 사례를 중심으로 - (A Study on the Effect of Sense of Community on Residents' Online Participation - Focused on the pilot project for Seoul Clean Apartment -)

  • 본 연구는 공동체의식이 온라인 주민참여에 미치는 영향을 서울시 '맑은 아파트 만들기' 사례를 중심으로 실증분석하였다. 지방정부는 소통의 채널이자 운영자로 그 중요성이 강조되고 있으며, 이때 주민참여와 공동체의식은 일종의 해결기제로 관심이 증대되고 있다. 이와 관련 온라인투표는 주민참여를 확대시키고자 서울시에서 시행하고 있는 사업이다. 이에 본 연구는 공동체의식이 주민참여에 미치는 영향을 살펴보고자 서울시 공동주택 온라인투표 시범실시단지를 중심으로 설문조사를 실시하여 통계 분석하여 온라인 주민참여에 영향을 주는 요인을 밝혀내고자 한다. 분석 결과 공동체의식의 구성요인 중 상호영향의식과 공유된 감정적 연계가 온라인 주민 참여에 유의미한 영향을 주는 것으로 나타났으며, 특히 공유된 감정적 연계가 주민참여의 활성화를 위해 강조되어야 할 것으로 나타났다.

Current Status of the Pediatric Palliative Care Pilot Project in South Korea Based on the Experience of a Single Center

  • Purpose: We evaluated the status of patients enrolled in South Korea's pediatric palliative care pilot project based on the experience of a single center. This study examined factors related to end-of-life services and differences in medical costs. Methods: The medical records of 120 patients referred by a pediatric palliative care team were analyzed retrospectively. Data from July 1 to February 28, 2022 were collected and analyzed using the chi-square test and the Mann-Whitney U test. Results: Volunteer programs and psychological support (100%), family support and education (99.2%), and financial support through institutional linkage (62.5%) were provided to the participants. In the deceased group, there were no significant differences in general characteristics, which included age, gender, primary disease, religion, duration of hospitalization in an intensive care unit (ICU) and non-intensive care unit (non-ICU). However, the ICU group had fewer opportunities to access individual pain and physical symptom management than the non-ICU group and there were limitations in linking with external resources. Medical expenses were significantly different for the ICU group, with a 3-times higher average cost than the non-ICU group. Conclusion: Although an individualized approach is needed for each patient in pediatric palliative care, psychosocial care is essential. In addition, if early intervention for end-of-life pediatric patients is available from a palliative care team, the cost burden of medical care for patients and their families should be minimal.
