Sex Differences of the Relationships between Cardiovascular Risk Markers and Psychosocial Factors in Community-Residing Adults (지역사회 성인의 심혈관 질환 위험 표식자와 심리사회적 요인의 성별차이)

  • Purpose: This study was undertaken to explore sex differences in the risk markers of cardiovascular disease (CVD) and psychosocial factors and the relationships among these variables for the community-residing adults. Methods: A cross-sectional study was conducted with a total of 114 participants consisting of 65 men aged 42.85 years old and 49 women aged 46.86 years old. Participants were selected by a convenient sampling of at community cultural centers and churches. They completed a structured questionnaire including demographic characteristics, physiologic risk markers of CVD; lifestyle factors; and depression, social support, and stress. Results: The male participants showed significantly higher systolic and diastolic blood pressures than their counterparts. Significantly higher percent of women were currently non-smokers, and practiced moderate alcohol intake, physical activity, and low fat diet. Among women, the higher the scores of depression and stress were and the lower that of the social support was, the higher blood pressure, waist circumference and BMI were. Among men, those who were less depressed and with higher scores of social support had more physical activity. Conclusion: The findings suggest further studies to explore sex differences of CVD risk markers and psychosocial factors, and their relationships in a larger sample, and to find the mechanism underlying these differences with a prospective design.

  • A whole body physiologically based pharmacokinetic (PBPK) model was applied to investigate absorption, distribution, and physiologic variations on pharmacokinetics of imatinib in human body. Previously published pharmacokinetic data of the drug after intravenous (i.v.) infusion and oral administration were simulated by the PBPK model. Oral dose absorption kinetics were analyzed by adopting a compartmental absorption and transit model in gut section. Tissue/plasma partition coefficients of drug after i.v. infusion were also used for oral administration. Sensitivity analysis of the PBPK model was carried out by taking parameters that were commonly subject to variation in human. Drug concentration in adipose tissue was found to be higher than those in other tissues, suggesting that adipose tissue plays a role as a storage tissue for the drug. Variations of metabolism in liver, body weight, and blood/plasma partition coefficient were found to be important factors affecting the plasma concentration profile of drug in human body.

  • After stage-two surgery, the highest incidence of failure has been attributed to implant overload. However, the biomechanical factors cited inthe literature that contribute to implant overload, such as bone type, cuspal inclination, horizontal offset, maxillary compared to mandibular arch, the inclusion of natural teeth within the prosthesis, and occlusal harmony are superimposed on physiologic variations. Following two cases, including reduction of cuspal inclination and usage of modified incisal pin, showed a method of contouring occlusal surface for reduction of unpreditable implant overload.

  • Insomnia is a disorder of initiaton and maintenance of sleep that results in daytime sleepiness and dysfunction, and arises from multiple psychological, physiologic and environmental factors. A number of stroke patients suffer from insomnia classified as one of the sleep disorders associated with physical illness and on the contrary insomnia may have profound deleterious effects on the natural course of stroke. We experiened three cases of stroke patients with insomnia. In the point of Differentiation of Syndromes, these subjects were diagnosed as Excessive heat-fire syndrome of stroke and were administrated with Hwangyeonhaedog-tang Gamibang. After treatment, both sleep patterns and other accompanied symptoms improved.

  • Advanced cancer of the oral cavity has been treated with wide excision in conjunction with mandibulectomy and neck dissection. This has resulted in significant mandibulofacial defects with functional and cosmetic significance. Therefore, proper mandibular reconstruction is very important for physiologic and esthetic restoration. The risk factors of free flap reconstruction have been reported including obesity, age, smoking, previous irradiation, and systemic vascular disease. We recently experienced a case of donor site necrosis after fibular osteocutaneous free flap in oral cavity cancer.

  • The paper is to report a case of zinc-deficient parakeratotic hyperkeratosis in a dog. In this dermatosis, although an available diagnosis of zinc-deficient dermatosis is to analyse the serum or hair zinc Bevels, exact analysis of zinc is difficult and unreliable due to contamination of zinc by various environmental, physiologic and disease-related factors. Diagnosis may be performed by history, physical examination and blood chemical analysis. Laboratory evaluation revealed hypercholesterolemia and low activities of serum alkaline photophatase and total protein. The dog showed thick crusts at the elbows joint, stifle joint and testis. Zinc sulfate is administered per oral to patient with application of salicylic acid added vaseline ointment on hyperkaratic lesions. The dog is successfully cured.

  • Purpose : To describes the important aspects of scapular movement and function used when applying PNF technique to the upper limb and scapular. Method : The scapular was a very important roles in the upper limb movement. This study summarizes the physiologic movement of scapular to the PNF upper extremity patterns or scapular patterns. Result : The shoulder joint has the most freedom of range of motion in the human body, composed of the glenohumeral joint, the subacromial joint, the acromioclavical joint, the sternoclavicular joint, the scapulothoracic joint, the costosternal joint, and the costovertebral joint. During upper limb movement, the scapular position change at the sternoclavicular joint and the acromioclavical joint. This concerted motion was characterized by scapulohumeral rhythm. In clinical situations, it is import to understand factors affect the scapulohumeral rhythm so that optimal evaluation and therapeutic intervention can be devised. Conclusions : The scapular movement depend on the proper and coordinated contraction of muscles. Physical therapists need to understand the normal scapular movement relationships of the scapulohumeral rhythm under different interventions for PNF techniques application.

Effects of Walking on Cardiovascular Risk Factors and Psychosocial Outcomes in Postmenopausal Obese Women (걷기운동이 폐경기 비만여성의 심혈관계 위험요소와 사회심리적 결과에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of a moderate-intensity, walking exercise program on the body composition, blood lipids and psychosocial outcomes in postmenopausal obese women. Methods: With a quasi-experimental pre- and post-test design, a total of 36 postmenopausal obese women was recruited in 2 metropolitan areas by convenience sampling. Sixteen women participated in 1 hour of moderate-intensity walking exercise 5 days per week for 3 months and 20 women did not. Cardiovascular risk factors include body composition and blood lipids. Body composition was measured as body mass index, % body fat, and waist/hip ratio; Blood lipids were measured with total cholesterol, triglyceride, HDL and LDL; psychosocial outcomes were evaluated by self-esteem and depression. Results: Over 3 months, the score of self-esteem increased and depression decreased in the exercise group relative to the control group. However, there were no significant differences in body composition and blood lipids. Conclusions: This study suggests that 3 months of moderate-intensity exercise training can improve psychosocial outcomes but further studies are needed to replicate walking exercise on physiologic variables among postmenopausal obese women. These findings are of public health relevance and add a new facet to the growing literature on the health benefits of moderate exercise.

Academic Stress, Coping Method, and Stress Symptoms of High School Students (일부 고등학생들의 학업스트레스와 이에 대한 대처방식 및 스트레스 증상)

  • Purpose : To investigate academic stress, coping method, and stress symptoms of high school students. Methods : Four hundreds-forty five high school students were studied from 14th July to 21th July, 2006. All subjects were in 1st, 2nd and 3rd grade of two high school in Seoul. The academic stress, coping method and stress symptoms were evaluated by questionnaire. Results : Male students showed more academic stress than female students. There was a significant difference in the academic stress according to sex. The 2nd grade students show the highest score of academic stress in the 3 factors(parents, teacher, and friend). Male students used more active coping methods to academic stress than female students. There was a significant difference in the coping methods to academic stress according to sex. There were positive correlations between the academic stress and the stress symptoms. Among the academic stress factors, parents and teacher factor influenced physiologic, psychologic, and behavioral symptom of stress symptoms. The friend factor influenced psychologic and behavioral symptom of stress symptoms. Conclusion : According to the study results, we must provide high school students more effective coping methods to academic stress at school environment.

The Extracardiac Fontan Operation in Adult -A case report- (성인에서의 심외도관 Fontan 수술 - 1예 보고 -)

  • The Fontan operation is commonly practiced for the physiologic correction of univentricular heart diseases. However, for the patients who have risk factors against this operation, it is recommended to take the initial palliative operation rather than going to the Fontan operation at once. The proper timing to the Fontan operation after palliation is decided by assessing several factors such as patient's age and other risks of maintaining palliative state, etc. Usually, the Fontan operation is done relatively early after palliation stage. Here, we report a 36 years old-adult-female with univentricular heart disease who underwent the successful Fontan operation at 17 years after unidirectional Glenn procedure.
