Use of the Talk Test for Comparison of Exercise Intensity between Aerobic Activities with Treadmill and Bicycle

  • Purpose: The talk test (TT) is an alternative, self-reported method for prescribing and guiding exercise training in both healthy adults and patients with cardiovascular and pulmonary diseases. This study examined whether the TT is a valid tool for evaluating the exercise intensity during two different types of aerobic activity on a treadmill or stationary bicycle in a healthy population. Methods: A total of ten subjects (six males and four females) who had no medical history related to musculoskeletal, cardiovascular, and pulmonary disorders were enrolled in this study. They were evaluated using the TT, which consisted of three-level of difficulties demanding cardiac loads while performing aerobic activities on a treadmill and bicycle ergometer across two consecutive days in a counterbalanced manner. During the activities, the psychophysiological response markers were collected in terms of the heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents. Results: Statistical analyses revealed a significant difference in the between-subject variance regarding the TT level effect (p<0.05). On the other hand, no significant findings were detected on the between-group variance(p>0.05) and the TT level×group interaction (p>0.05). The independent t-test indicated no significant differences in heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents at any levels of the TT in the two groups. The TT showed a strong correlation with the rating of perceived exertion. Conclusion: This study showed that the TT is a valid and alternative tool for evaluating the aerobic exercise intensity in a healthy population. In addition, differences in the psychophysiological response markers between two aerobic activities, treadmill and bicycle ergometer, were detected in the same exercise intensity scaled with the TT. The TT can be used to evaluate and prescribe the exercise intensity of aerobic activity in cardiovascular and pulmonary physical therapy.

Effect of Different Air Hole Diameters of the Inspiratory Muscle Trainer on the Rating of Perceived Exertion and Inspiratory Muscle Activity during Breathing Exercise

  • Purpose : This study aims to investigate the rating of perceived exertion (RPE) and muscle activity of the inspiratory primary and accessory muscle during breathing exercise with different air hole diameters of the inspiratory muscle trainer (IMT). Methods : The Borg's scale and surface electromyography (EMG) was used to collect data of the RPE and muscle activity of the inspiratory primary the external intercostal (EI) and diaphragm (DIA) and accessory muscles anterior scalene (AS), sternocleidomastoid (SCM), pectoralis major (PM), and upper trapezius (UT) muscles during breathing exercise with different air hole diameters (6 mm, 4 mm, and 2 mm) of the IMT in healthy young male subjects. Results : The RPE and muscle activities of the AS, SCM, and UT are increased significantly in accordance to the decreasing diameter of air hole of air tip in IMT. However, there are no differences in the muscle activities of the PM, EI, and DIA based on differences of diameters of air hole of air tip in IMT. Conclusion : The smaller the diametr of IMT air-hole, RPE and muscle activities of AS, SCM and UT were increased. Therefore, further study would be necessary to investigate the proper intensity and relaxation posture for the exercise protocol to strengthen the inspiratory primary muscles.

RPE에 의한 운동강도 예측인자의 남녀 대학생 비교 (Gender Comparison of Ratings of Perceived Exertion (RPE) as a Predictor of Exercise Intensity in College Students)

  • 심박수(heart rate) 및 산소 소비량(oxygen consumption) 등의 생리학적 변수는 운동자각도(RPE)와 밀접한 관련이 있으며, RPE의 남녀차이에 의한 예측 운동강도의 정확성은 아직 밝혀지지 않고 있고, 본 연구는 남녀 대학생의 심장 혈관 및 신진대사의 반응을 평가하고 남녀의 차이를 밝히는 데 있다. 남자대학생($25.1{\pm}1.56$ yr)과 여자대학생($25.0{\pm}4.12$ yr) 각각 10명이 본 연구에 자발적으로 참여를 하였고, 본 연구를 위하여 설정된RPE는 Borg의 15 범주에서 결정을 하였으며, 점증적 운동부하(graded exercise testing)는 Bruce Treadmill Protocol을 이용하였다. 남녀의 평균 slope (p=0.501)와 RPE의 plotting rates와 최대산소섭취율(% $VO_2$max)은 유의한 차이가 나타나지 않았다. 각자의 상대적 최대산소섭취량은 남학생의 경우52.36${\pm}$7.35 ml/kg/min, 여학생은 41.44${\pm}$6.71 ml/kg/min로 나타나 두 그룹간에 유의하게 큰 차이가 있었으며, 절대적 최대산소섭취량은 남학생은 4.05${\pm}$0.36 l/min, 여학생은 2.53${\pm}$0.39 l/min로 나타나 두 그룹간에 큰 차이가 나타냈다. 남학생과 여학생간의 slope, y-intercept, SEE는 통계적으로 유의한 차이는 없었다. 본 연구에서 운동강도에 따른 남학생과 여학생의 RPE 의 차이는 유의하게 나타나지 않았다. 이상의 결과로 보아 RPE는 운동중 산소섭취량에 대한 운동강도의 예측에 남녀별 차이가 없어 유효한 예측인자라고 사료된다.

Relationships of Musculoskeletal Disorder Symptoms and Perceived Workload among Hospital Workers

  • Objective: This study aims to survey the prevalence of musculoskeletal disorders(MSD) among Korean hospital workers and to analyze the relationship of MSD symptoms and workload perceived by workers. Background: Despite of high exposure to the MSD risk factors and high MSD symptom prevalence among health care workers, there were not enough studies of MSD prevalence among the hospital workers. Method: This study designed a survey based on Nordic questionnaire to obtain MSD symptoms and the degree of four perceived workloads: work repetition, urgency, physical exertion and satisfaction. In this survey, 1,846 workers in a hospital participated. The prevalence of MSD was analyzed for each body part, and MSD cases, which were predetermined in this study, were identified. The relationship between the MSD cases and each perceived workload was analyzed using chi-square test. Results: The pain in the shoulder was the most prevalent among the workers as 52%, and the low back(37%) and leg discomfort(36%) followed. The MSD cases, in which degree of pain was more than severe, were also the most prevalent in the shoulder(13%). Female workers had higher rate of MSD cases than the males. Among the four workload variables, the physical exertion was the statistically related to MSD cases for all the body parts. In addition, the others also had significant relation to MSD cases except one or two body parts. Conclusion: This study found that Korean hospital workers had MSD symptoms mainly in the shoulder, low back and legs in order, and the perceived workload surveyed in this study was highly correlated with MSD symptoms. Application: This study provides another evidence that subjective physical exertion perceived by workers is an important factor to explain MSD cases as same as the objective one.

항만하역 근로자들의 직무 스트레스에 관한 연구 (A Study on Job Stress of Container Termainal Workers)

  • Objectives: The objective of this study is to evaluate the job characteristics of container terminal workers by applying the Job Strain model, and to assess the relationship among the general characteristics, job characteristics and psychosocial distress. Methods: A self-administrated questionnaire survey was performed to the container terminal workers in Pusan. Among the 200 male workers who answered the questionnaires, white-collar workers and blue-collar workers were 100, respectively. Karaseks Job Content Questionnaire was utilized to evaluate the job characteristics and Psychosocial well-being index (PWI) was applied to measure the extent of their psychosocial stress. Results: In white-collar workers, the skill discretion, created skill, decision-making authority, decision-making latitude, psychological job demand, and supervisor support of the job characteristics were significantly high, while in blue-collar workers physical exertion was significantly high. The result of Psychosocial well-being index (PWI) reveals that blue-collar workers were more stressed than white-collar workers, especially, the indices of depression (factor 2), sleeping disturbance and anxiety (factor 3), General well-being and vitality (factor 4) were significantly increased; whereas, in white-collar workers, only the index of social performance and self-confidence (factor 1) was significantly increased. And PWI scores were significantly increased in the lower social support and psychological job demand. By the multiple logistic regression analysis for PWI, blue-collar workers had increased odds ratio of 2.66(95% CI;1.11-6.41) compared with white-collar workers. The unmarried workers increased odds ratio of 3.54(95% CI;1.18-10.62) compared with married workers. And workers who have not own house increased odds ratio of 2.35(95% CI;1.15-4.79) compared with workers who have own house. Particularly, odds ratio of work-shift in blue-collar workers was 11.10(2.14-57.64). Conclusion: Skill discretion, created skill, decision-making authority, decision-making latitude, psychological job demand, and supervisor support were increased in white-collar workers. Decreased skill discretion and increased physical exertion were found in blue-collar workers, which is supported the Job Strain model. Job stress of blue-collar workers was comparatively higher than that of white-collar workers, especially, skill discretion, decision-making authority, decision-making latitude, job insecurity, physical exertion were noticeable factors. Especially, sleeping, smoking, and work shifting turned out to be a main cause that increases stress. Therefore, in order to decrease the job stress, a health promotion program to change the health behaviors should be activated and an organized job stress management program should be introduced. Especially, working condition for blue-collar such as physical exertion and work-shift should be improved.

A Study on the Maximum Torque Exertion Capabilities of Korean

  • A laboratory experiment was conducted to investigate the effects of body posture and of different types of common non-powered hand tools on maximum volitional torque exertion capabilities of Korean. Fifteen males and 15 females were participated in the experiment. Each subject exerted maximum volitional torque in 15 different body postures while using five different common non-powered hand tools. Results of the data analysis showed that, for both males and females, the magnitude of torque exertion is significantly affected by the type of tool and posture. Also the result indicated that females exerted only about 51.5% of torque when compared to that of males. This result is different with the general findings in the field of ergonomics that the muscular strength of female is usually 65% to 75% of male. It suggests that application of same profile of muscular strength of female as in the western population should be carefully considered in oriental countries since females in this region may possess less physical capability compare to that of western females in terms of percentage muscular strength of males. Profiles of maximum torque capabilities of Koreans are provided.

Effects of Active Movement with Skin Mobilization on Range of Motion, Pain, RPE on Patients with Axillary Web Syndrome: A Case Study

  • Objective: This study was conducted to investigate the effect of active movement with skin mobilization on range of motion, pain, and rating of perceived exertion in patients diagnosed with axillary web syndrome after axillary lymph node dissection. Design: A Case report Methods: It was performed on 7 patients diagnosed with axillary web syndrome after lymph node dissection. The subjects experienced a decrease in the range of joint motion and pain in movement when raising their arms in their daily lives, and complained of discomfort. The active range of motion, numeric rating scale, and modified Borg scale of shoulder joint flexion were measured, and the differences after active movement with skin mobilization were compared. Results: All subjects increased by 24.9 degree on average in active range of motion after active movement with skin mobilization intervention. There was no pain in the maximum range of joint motion measured before intervention, and rating of perceived exertion was significantly reduced. Conclusions: Active movement with skin mobilization can be a very useful way to help improve and treat axillary web syndrome, and it is recommended for improving the function and quality of life of axillary web syndrome patients. and It is also believed that it can be used steadily at home through the education of patients and families.

Comparison of the effects of different-sized airways in inspiratory trainers on maximal inspiratory pressure and rating of perceived exertion scale in healthy young people

  • Objective: This study aimed to investigate the effect of different-sized airways of the inspiratory muscle trainer (IMT) on maximal inspiratory pressure (MIP) and the rating of perceived exertion, as measured by the modified Borg scale (m-Borg). Design: Cross-sectional study. Methods: Twenty healthy subjects (10 men, 10 women) volunteered for the study. The spirometry was used to measure MIP. The trial order of the three spirometry conditions was chosen randomly. After measuring the MIP and before taking the final break, each of the conditions were immediately measured using the m-Borg. All subjects used the IMT with an airway diameter of 5-, 6-, and 7-mm. Results: The MIP significantly decreased with the decreasing airway diameter of the IMT (p<0.001), and the differences in all three conditions (7- and 6-mm, 6- and 5-mm, and 7- and 5-mm airways) were significant (p<0.05). The RPE significantly increased with the decreasing airway diameter of the IMT (p<0.001), and the differences in all three conditions (7- and 6-mm, 6- and 5-mm, and 7- and 5-mm airways) were significant (p<0.05). Conclusions: Decreasing the airway diameter of the IMT decreased the MIP and increased the m-Borg. In regards to physical exercise within the clinical setting, the m-Borg could be a useful as a prior monitoring method to prevent dyspnea for patients with narrowed airways (blocked with secretion or sputum in pulmonary disease) and the different-sized airways of the IMT could be applied as a useful tool to improve MIP for prevention of pulmonary disease.

  • In 1989, We experienced 2 cases of T.A.P.V.D. corrected successfully under cardiopulmonary bypass. The first case was 27 years old male with mild dyspnea on exertion and easy fatigability. All clinical symptoms, physical examinations, laboratory findings, echocardiogram, cardiac catheterization and angiogram revealed supracardiac type T.A.P.V.D. drained through left innominate vein. Operation was done by manner of modified Schumacker procedure. The other case was 6 years old male with cardiac type drained to right atrium through coronary sinus. Two patients were well in postoperative 15 and 8 months.

보행용 MBT신발과 일반 신발 보행 시 에너지 소비량과 근 활성도의 비교 분석 (Comparative Analysis Between MBT Shoes(Functional walking shoes) and General Sporting Shoes for the Muscle Activity and Energy Consumption)

  • This study was to find out whether MBT shoes were more effective than general sporting shoes on the kinematic physiology. We comparatively analyzed energy consumption, heart rates, rating of perceived exertion, lactic acid density in blood and muscle activity with EMG. The subjects were 10 regular students of K Univ.(5 male and 5 female). They wore the general sporting shoes for stage 1(group 1), the MBT walking shoes for stage 2(group2) under the same conditions. There was one week interval between stages. 1. There was no significant difference in the energy consumption and the heart rates between two groups. Group 1 showed a higher energy consumption and heart rates than Group 2 under the speed of 4.0 mph. On the other hand, Group 2 showed higher ones over the speed of 4.0 mph. 2. There was no significant difference in the rating of perceived exertion and lactic acid density in blood between two groups. But Group 2 showed a lower fatigue strength than Group 1. 3. Group 2 showed a higher muscle activity with EMG than group 1, especially at the speed of 2.0, 2.5, 3.0, 3.5 mph in quadratus lumborum muscle and at 4.0mph in tibialis anterior muscle. In conclusion, we are less tired in MBT shoes than in general sporting shoes. Also, we can dorsiflex better in the ankle joint, straighten the lumbar region well and walk erect in MBT shoes. Specially, when it walks slowly, it is favorable in an erection walk and when it walks quickly, we have advantage of obesity management.
