Effects of Spinal Stabilization Exercises on the Cross-sectional Areas of the Lumbar Multifidus and Psoas Major Muscles of Patients with Degenerative Disc Disease

  • Purpose: To evaluate, in patients with degenerative disc disease (DDD), the efficacy of using spinal stabilizing exercises for the reversal? of atrophy of the multifidus and psoas major, reductions in pain and disability, and for increases in paraspinal muscle strength. Methods: Nineteen patients diagnosed with DDD participated for 10 weeks in a spinal stabilization exercise program. Pain and disability were measured before and after exercise using, respectively, a visual analogue scale (VAS) and the Oswestry Disability Index (ODI). Paraspinal muscular strength in four directions was evaluated using CENTAUR. Both before and after exercise we used computed tomography (CT) too measure cross-sectional areas (CSAs) of both the left and right multifidus and the psoas major at the upper & lower endplate of L4. Results: After 10 weeks of a spinal stabilization exercise program, pain was significantly decreased from $5.7{\pm}0.9$ to $2.5{\pm}0.9$ (p<0.01); the ODI score decreased from $16.7{\pm}4.9$ to $7.3{\pm}3.1$. Paraspinal muscle strength was significantly increased (p<0.01) and the CSAs of the left and right multifidus and psoas major muscles were significantly increased (p<0.01). Conclusion: Spinal stabilization exercise is effective in reversing atrophy in DDD patients, in reducing pain and disability, and in increasing paraspinal muscle strength. It is an effective treatment foro aiding rehabilitation in these cases.

Reliability and Validity of Korean-Version of Information Needs in Cardiac Rehabilitation Scale

  • Purpose: The Cardiac Rehabilitation scale (INCR scale) is a verified measure worldwide. The scale measures the extent to which cardiac rehabilitation meets the various needs of the participating patients and determines how education related to heart disease and rehabilitation satisfies their preferences. The current study applied the INCR scale to verify and develop it for use in Korea by conforming to Korean culture and language. Methods: The Korean version of the INCR scale was created after a six-step process in accordance with the international questionnaire translation law. The Korean version of the INCR scale was assessed in 45 cardiac rehabilitation patients (29 males, 16 females). Internal consistency was verified using Cronbach's alpha. Test-re-test reliability and interrater reliability were verified using the intraclass correlation coefficient (ICC). Content validity was selected to verify the validity. Results: The content validity index of the Korean version of the INCR scale was high, with an average of 0.93. Cronbach's Alpha for Internal consistency also revealed high reliability of 0.89 in all 10 categories. The interclass correlation coefficient of the test-re-test and interrater reliability scores were 0.95 and 0.81, respectively. Conclusion: Our results indicate that the Korean version of the INCR scale would be instrumental in the clinical setting to determine the knowledge, educational needs, and preferences of patients participating in cardiovascular disease and cardiac rehabilitation.

  • Most patients treated by physical therapists have suffered some neurological trauma resulting from disease or injury. The traditional teaching used to be that damage of central neurons is irreversible. However, it has been necessary to cast aside this traditional view because of accumulating evidence that the brain is endowed with remarkable plasticity. This paper reviews the literature relating to neuroplasticity within the brain and draws implications pertinent to physical therapy practice.

일부 지체장애인의 미치료 경험에 영향을 주는 결정요인 분석 (Determinants of untreated experiences among persons with physical disability)

  • Objective: This study is examined the factors affecting forms of untreated experiences in persons with physical disability. Method: The data collected from 461 persons with physical disability in community. Based on the Behavioral Model of Andersen, predisposing, enabling, and need factors are hypothesized to affect persons with physical disability's untreated experiences. The data were analyzed by statistical methods such as frequency and multiple logistic regression analysis. Result: Participation rates of untreated experiences were 26.7%. The significant factors of persons with physical disability's untreated experiences are predisposing factors (gender, partner, and religion), enabling factors (income, private insurance, information of assistive device, disability discrimination, and subjective discrimination), and need factors(subjective health status health screening and chronic disease). Also untreated experiences related to gender, subjective health status, health screening, and chronic disease factors using multiple logistic regression analysis. Conclusion: Implications of the findings were discussed and the recommendations for the improvement of health care utilization, subjective health statue. Especially, development of health education and program should be needed persons with physical disability.

An Update on Accumulating Exercise and Postprandial Lipaemia: Translating Theory Into Practice

  • Over the last two decades, significant research attention has been given to the acute effect of a single bout of exercise on postprandial lipaemia. A large body of evidence supports the notion that an acute bout of aerobic exercise can reduce postprandial triacylglycerol (TAG) concentrations. However, this effect is short-lived emphasising the important role of regular physical activity for lowering TAG concentrations through an active lifestyle. In 1995, the concept of accumulating physical activity was introduced in expert recommendations with the advice that activity can be performed in several short bouts throughout the day with a minimum duration of 10 minutes per activity bout. Although the concept of accumulation has been widely publicised, there is still limited scientific evidence to support it but several studies have investigated the effects of accumulated activity on health-related outcomes to support the recommendations in physical activity guidelines. One area, which is the focus of this review, is the effect of accumulating exercise on postprandial lipaemia. We propose that accumulating exercise will provide additional physical activity options for lowering postprandial TAG concentrations relevant to individuals with limited time or exercise capacity to engage in more structured forms of exercise, or longer bouts of physical activity. The benefits of accumulated physical activity might translate to a reduced risk of cardiovascular disease in the long-term.

Education based on the health belief model to improve the level of physical activity

  • [Purpose] This study aimed to investigate the effect of education based on the health belief model on the physical activity of the staff of the University of Medical Sciences. [Methods] This semi-experimental study was conducted on 130 university staff aged 25-50 years from the Hamadan University of Medical Sciences. Inclusion criteria were having at least 1 year of work experience, lack of acute and chronic physical and mental illnesses, and not using drugs that affect physical activity. The samples were randomly divided into two groups. The experimental group received three training sessions based on the health belief model. Before and 2 months after training, the control and experimental groups were evaluated via the following questionnaires: (1) demographic information questionnaire, (2) Health Belief Model Questionnaire, and (3) International Physical Activity Questionnaire. Finally, data were analyzed statistically. [Results] The training process resulted in a significant increase in the mean scores of the health belief model constructs in the experimental group, but changes in the control group were not significant. Self-efficacy was the strongest predictor of physical activity. [Conclusion] The health belief model is a useful model for improving individuals' understanding of the benefits of physical activity.

  • This evaluation was made through 11 persons who discogenic disease and it emergency disease. There was all complains low back. with sustained buttock pain with radiating to the leg due to piriformis syndrome during the period of march 2000 to November 1999 in seoul wooridul hospital. The purpose of this study was to determine the effect of manual therapy through 11 persons. The results were as follow : 1. Right buttock pain group(n = 11). there was significant difference before manual treatment(P<.05). 2. Pain decreased before Tx then after Tx by VAS.

  • The purpose of this literature review was to identify the physiological changes with aging. The results of this renew were to follows : We have seen a wide variety of such changes in the cells and in all organ systems. 1. At least 40 percent of people over 65 will die of cardiac disease, 15 percent of cerebrovascular disease, and possibly another 5 percent of other types of vascular impairment. 2. The increase rigidity of the thoracic wall and the decreased strength of the expiratory muscles decrease the propulsive effectiveness of the cough. 3. The density of capillaries per motor units is decreased. 4. Starting before age 40 in both sexes there is a shift from an increase in bone mass to a progressive decrease. 5. Histologic studies show a lim age-correlated decrease in the number of Pacini's. Merkel's, and Meissner's corpuscles.

열충격단백질 70에 대한 연구 (A Study in Heat Shock Protein 70)

  • Heat shock protein 70(HSP70) is induced by elevated temperature and many other types of stresses in cell. HSP70 ensures cell survival under stressful condition that would lead to irreversible cell damage and ultimately to cell death. HSP70 plays essential role in the synthesis, transport, and folding of proteins and is often refferred to as molecular chaperones. Increased levels of HSPs occur after arthritis, infection, imflammation, autoimmune disease and CNS injury such as infarction, ischemia, seizure and Alzheimer's disease. Also, HSP70 increases resistance to apoptosis. The recent studies that the expression of the HSP has been processed at various field. However, they an still relatively line studied in clinically application. This review summarizes the fundamental knowledge of HSP.

A Study on the Manual Lymph Drainage and Programs for Specialist: a Literature Review

  • It is necessary to produce properly educated Manual Lymph Drainage (MLD) specialists conducted by certified instructors. This study was conducted to introduce a comprehensive understanding, application, historical background, and effect of the MLD and to present a basis for using the frequency MLD as a basic resource for training professional. MLD, a field of manual therapy, is used to treat a variety of diseases such as joint disease, circulatory system disease, lymphedema, and pain et al. However, since there is no proper education for MLD in Korea, there is a need to provide education that meets internationally accepted standards. The Physical Therapists Association and related academic societies will need to prepare a system to train specialists on MLD and to recognize medical insurance through it.
