The Effects of Upper and Lower Limb Coordinated Exercise in One Leg Support or non Support on Gait Ability in Chronic Stroke Patients (한 발 지지 유무에 따른 상하지 협응운동이 만성 뇌졸중 환자의 보행능력에 미치는 영향)

  • The purpose of this study was to identify the effects of upper and lower limb coordinated exercise for gait ability in stroke patients. Upper and lower limb coordinated exercise method was conducted in two different groups; one is an one leg support group and the other is a non support group. In this study, 14 patients were participated. One leg support group was applied to 7 patients, and non support group was applied to 7 patients. Both group carried out 3 times a week for 30 minutes during 4 weeks. Data were analyzed statistically via Repeated two-way ANOVA, Mann-Whitney U test, and Friedman test. The results of the measurement analysis were summarized as follows: 1. There were significant differences in 10MWT among 2 groups after intervention(p<.05). 2. There were significant differences in F8WT, FSST among 2 groups after intervention(p<.05). According to Bonferroni test, one leg support group had significant increased from pre-intervention to post-4 week. However, there were no significant differences in nonsupport group. In conclusion, improvement of gait ability in chronic stroke patients was effect to upper and lower limb coordinated exercise.

A Study on the School Health Services in the Universities, Colleges and Junior Colleges (우리나라대학의 학교보건관리에 관한 실태조사)

  • The present study is to provide information for the improvement of school health services through research on the current condition of its organization and practice in universities, colleges and junior colleges. The scope of this study is consisted of four components including health organizations/units, school health services, environmental sanitation and health education for the 30 universities, the 20 colleges and the 32 junior colleges in Korea. The major findings are summarized as follows: (1) Among the sampled schools, around 73% of them have the health service organization/unit. When we break down health service organization/unit into the types by the level of school, around 73% of the universities have formal organization called "health center" and 20.0% of them have an informal organization called "health room". For the colleges level, 30.0% of them have the "health center" and 40.0% of them have the "health room". The figure of junior colleges is a quite different from universities and colleges, 56.3% of junior colleges have the "health room" only but the other have no service organization at all. (2) It was found that only 22.0% of 82 schools have the health committee for the school health services. It might be necessary to have a kind of expert committee to establish an annual health service program, budget and health policy in the school. (3) Approximately 29% of those schools having formal health organizations/units appointed directors as a medical persons. 13.4% of the sampled schools are appointed doctors (including the dentists) at health service organization/unit, 9.8% are appointed pharmacist and 65.9% are appointed nurses. Therefore, the data imply that the school health services are depending mainly on nurses. (4) The major activities of school health services are covering primary medical care (84.1%), health counseling (72.0%), physical examination (68.3%), vaccination (58.5%), tuberculosis control (54.9%), parasite control (29.3%) and dental health case(9.8%). Also 69.5% of the schools have the program on the environmental sanitation and the health education program. (5) In regard to health budget taking account of 34 schools, approximately 92% of them have less than 5,000 won per students and only 8.8% of them have more 10,000 won per students. At the average health budget per students is 4089.8 won in universities, 1617.1 won in colleges and 475.0 won in junior colleges. (6) The students enjoy the benifit of medical insurance at 11.0% of 82 schools surveyed. They are all universities. (7) The study found that 56 universities, colleges and junior colleges provide the annual physical examination. Only 21.4% of them have provided it for all students and school employees. (8) 64.3% of the 56 schools surveyed keep a record of the regular physical examinations. Records must be utilized as the basic data for the evaluation of the student's health condition and so the individual student is encouraged to take care of his own health. (9) At the 59 schools which practice health counseling, the main concerns of the counsellees are venereal disease, tuberculosis and psychoneurosis. This shows the need to practice health education in the area of preventive medicine. (10) 69.5% of the 82 universities, colleges and junior colleges surveyed are concerned with supervision of the environmental sanitation in their school, but non-professionals are in charge at 70.1% of them. This indicates negligence in environmental sanitation. (11) 53.7% of the 82 schools responded that they have no special instructive measure for the students' health and 54.9% are found to be negative in the use of a health education method. This reveals a problem. They are not positive to the recognition of their function as the initiative organization for the students' health. (12) The supplementary education for the faculty of the school health services is executed only at 8.5% of all the schools surveyed.

Evaluation of Nutritional Improvement by Total Parenteral Nutrition Guideline in Early Malnourished Inpatients (입원초기 영양불량 환자의 TPN 지침에 따른 영양개선 평가)

  • Background: Malnutrition of inpatients has been associated with higher morbidity, mortality, cost, and longer hospital stay. Total parenteral nutrition (TPN) therapy plays an important role in decreasing morbidity and mortality among critical inpatients in hospitals, and has been commonly used to improve clinical outcomes. However, only a few studies were conducted regarding patients' nutritional improvement by TPN. Method: This study therefore evaluated the changes in nutritional parameters by TPN therapy for early malnourished inpatients. Data from early malnourished inpatients who were treated with TPN therapy between January 2012 and June 2013 at the ${\bigcirc}{\bigcirc}$ university Hospital were studied retrospectively. Information regarding sex, age, underlying diseases, division, TPN (peripheral and central), and changes in nutritional parameters were collected by reviewing electronic medical records. The criteria for evaluation of the changes in nutritional parameters were included physical marker, body mass index (BMI), and biochemical markers, including albumin (Alb), total lymphocyte count (TLC), and cholesterol. Nutritional parameters were collected three times: pre-TPN, mid-TPN and end-TPN. A total of 149 patients (peripheral, 97; central, 52) was evaluated. Results: In all patients, the malnutrition number was significantly decreased following the complete TPN therapy (peripheral patients, pre-TPN: $3.33{\pm}0.12$, mid-TPN : $3.06{\pm}0.17$, and end-TPN: $2.85{\pm}0.21$ (p < 0.05); central patients, pre-TPN: $3.38{\pm}0.11$, mid-TPN: $3.06{\pm}0.13$, and end-TPN: $2.75{\pm}0.21$ (p < 0.05). The malnutrition number means number of nutrition parameters below normal range of malnutrition. In addition, all of the four nutritional parameters (BMI, Alb, TLC and cholesterol) were increased with duration of TPN periods for all patients, and the changes in the early stage were larger than in the late stage (p < 0.05). The nutritional parameters of non-cancer patients were increased to a greater extent compared to cancer patients with longer TPN therapy, but it was not significant. The nutritional parameters of younger patients (50-60 years) were also increased more than of older patients (70-80 years), but it was not significant. Conclusion: In conclusion, the TPN therapy decreases malnutritional status and improves nutritional parameters in malnourished patients, thereby decreasing morbidity and mortality. The combined evaluation of all four nutritional parameters is more accurate for nutritional assessment than a single one.

A Study on the Changes of Outdoor Space System in Korean Rural Villages - A Comparative Study of Changes in Residential Environment Accompanied by the Rural Village Improvement Projects - (한국 농촌마을의 옥외공간체계 변화에 관한 연구 - 취락구조 개선사업에 따른 주거환경의 변화 비교를 중심으로-)

  • This study was carried out to investigate the changes of rural residential environmemts accompanied by the rural village improvement projects. The results of this study can be a guidoline for further betterment works of rural village envirnments. This sutdy is composed of the survey of two major parts, one Is the interview of the residents and the survey of existing conditions of residential environments, and the other is a questionnaire survey of the mental image, preference and satisfaction for each village residential environments. Based on preliminary investigation, five rural villages, located In Kyungsamgun., Kyungbuk province, were selected as sutdy sites. The major results are summarized as follows; 1. In socio-cultural enveronments, the ratio of compound and non - agricultural household are increased with the modernization of rural villages. On the ocher hand, the numbers of neighbor's association are considerably decreased. This means the traditional community of korean rural villages are diminishing. 2. In physical environments, the ratio of improved western type gouses are increased In modprnized villages. And the modernization is most remarkable in the palls of fences and gardens. 3. By the result of the analysis of residfnt's environmental image and preference, the image scores of urban, formal, mdoern, etc. are showed to be high. And the preference is also high in modernized villages. But in general, the Preference for the village environment is comparatively low in all villages. This means the improvememt work should be continuous and systematic. 4. By the result of stepwise regression analysis, the variables, affecting the preference of village environment. are regularity, familiarity, amount of greenery, beauty of surrounding scenery, convenience, cleanness, etc. The R2s of the perference models were 0.4486 ∼0.9395, Which are considered to be high. 5. In the satisfaction for each residences, the toilets are mostly dis - satisfying. With the modernization of vilelages, the needs for environmental quality by residents are increased. 6. In the satisfaction for village enveironments, the satisfaction level is most low in service facilities, such as public bath, barbershop. And the satisfaction for the scenic beauty, the amenity of surrondings, and the com munity of the traditional villages, are commparatively high in conventional villges. 7. The imporvement works, based on the characteristics of each rural villages, are recommended. And the tradition of Jung - ja tree spaces and harmony with surrounding landscape of traditional villages, must be conserved with good regards.

Case Report on Three Patients with Cervical Myofascial Pain Syndrome Showing Improvement with Ultrasound-Guided Pharmacopuncture: Proposal of the 'SLS Triangle' Treatment Protocol (경추근막통증증후군에 대한 초음파 유도하 약침 치료 3례 증례보고)

  • Objectives : Cervical myofascial pain syndrome (C-MPS), a key aspect of Visual Display Terminals (VDT) Syndrome, leads to acute and chronic pain due to muscle shortening, potentially causing headaches and movement disorders. This study examines the effectiveness of Pharmacoacupuncture at the intersection of the Splenius Capitis (SC), Levator Scapulae (LS), and Serratus Posterior Superior(SPS) muscles under ultrasound guidance, known as the "SLS triangle" in treating C-MPS. Methods : Three patients (aged 30 to 37) with acute C-MPS presented at two Korean medicine clinics with severe neck pain and restricted cervical rotation. The evaluation of symptoms before treatment, immediately after treatment, and one day after treatment was conducted using the cervical rotation Range of motion (ROM) and the Numeric Rating Scale (NRS) for cervical pain. Treatments included Acupuncture, Cupping therapy, Chuna manipulation, Physical Therapy, and Pharmacopuncture (Jaha-geo extract, 2cc at the SLS triangle). Results : All three patients showed significant improvement post-treatment. Immediately after treatment, Patient 1's NRS improved from 7 to 2, Patient 2's NRS improved from 4 to 1, and Patient 3's NRS improved from 6 to 2. Directly after the procedure, there were no significant adverse reactions except for one patient experiencing temporary dizziness. When the patients were followed up by phone 24 hours later, no adverse reactions were reported. Conclusions : This study demonstrates that ultrasound-guided pharmacopuncture can significantly reduce pain and improve cervical rotation in patients with cervical myofascial pain syndrome. The findings suggest this non-invasive treatment is both safe and effective, warranting further research through larger-scale clinical trials to validate its efficacy and establish standardized protocols.

  • The current traffic accident reduction procedure in economic feasibility study does not consider the characteristics of road and V/C ratio. For solving this problem, this paper suggests methods to be able to evaluate safety of each road in construction and improvement through developing accident Prediction model in reflecting V/C ratio Per road types and traffic characters. In this paper as primary process, model is made by tke object of urban roads. Most of all, factor effecting on accident relying on road types is selected. At this point, selecting criteria chooses data obtained from road planning procedure, traffic volume, existence or non-existence of median barrier, and the number of crossing point, of connecting road. and of traffic signals. As a result of analyzing between each factor and accident. all appear to have relatives at a significant level of statistics. In this research, models are classified as 4-categorized classes according to roads and V/C ratio and each of models draws accident predicting model through Poisson regression along with verifying real situation data. The results of verifying models come out relatively satisfactory estimation against real traffic data. In this paper, traffic accident prediction is possible caused by road's physical characters by developing accident predicting model per road types resulted in V/C ratio and this result is inferred to be used on predicting accident cost when road construction and improvement are performed. Because data using this paper are limited in only province of Jeollabuk-Do, this paper has a limitation of revealing standards of all regions (nation).

Effect of Plant Educational Programs Elementary School using Vertical Garden (수직정원을 활용한 초등학교 식물교육프로그램 운영 효과 분석)

  • In the modern era, the elementary schools in the Republic of Korea are advanced with the competitive world. Thus advancement in technology, and other factors in the elementary schools are releases some kind of particulate matter (PM), which causes deleterious effects on school students health, academic program, growth and development. The school students are susceptible to PM particularly, PM10 and PM2.5. Based on this hazardous effect of PM on school students we conducted the research on the elementary schools class rooms by introducing vertical gardening system to get the schools "ever green". The main aim of this study is to investigate the environmental factors in the educational effect program in the class rooms and indoor air quality changes. For the educational effect, the 4th operation program was applied to 2 schools for 2 hours once a month from September to December 2019. Each school conducted a survey on satisfaction, plant friendliness, and environmental sensibilities, targeting classes that participated in the green school program and those who did not. The environmental effects were monitored by installing indoor air quality facility in the program participating classes and the non-participating classes. From November 2019 to December 2019, three factors were measured: PM10, PM2.5, and humidity. The results were analyzed by T-test using the SPSS 24.0 software program. As a result of the analysis, student's overall satisfaction with the program and their intention to re-engage were high in terms of educational effect. It was also found to be effective in emotional, educational, social, and physical aspects. In particular, it was found that there was a statistically significant effect on improving plant friendliness in terms of plant friendliness and environmental sensitivity. In terms of environmental effects, it was found that there was a significant difference between the program-applied and non-programmed classes in PM10, PM2.5, and humidity. Through this study, it was confirmed that the plant education program using vertical gardens is effective in both the educational effect and the improvement of indoor air quality in the classroom. In conclusion, vertical gardening system in the elementary schools should implement for the beneficial of young generation development and back bone to the nation.

The Research Trends in Fundamental Nursing - Based on the Analysis of Journal of Korean Academy of Fundamental Nursing - (기본간호학 연구동향 - 기본간호학회지 논문분석을 기반으로 -)

  • Purpose: This study was to analyze the trends and content of all the research (171 research papers) published in the Journal of Fundamentals of Nursing from 1994, when it began to 2000 using an objective tool developed by the researches, and to identify the direction for the future research in Fundamentals of Nursing. Method: Research published in the Journal of Fundamentals of Nursing from 1994 were reviewed. Result: Most of research (94.2%) was non-degree based and funded research was very limited. The average number of researchers per research study was 2.2 and collaborative research has been gradually increasing. The total number of key words was 331 and the average was 2.6 per research. The major key words were not different from other nursing departments and included home care nursing (10 times), health promotion (8 times), self-care (7 times). self-efficacy (7 times), and homo-dialysis (6 times). Eighty seven percent of the research was quantitative research. and there were very few qualitative studies. Considering theory level, it was found that 40% were factor related research For study design, non-experimental studies were most frequent (66%), and the rate, especially of surveys, has remarkably decreased. Selection of subjects by convenience sampling, was most frequent and there were very few studies that provided the rationale for the calculation of sample size. The major subjects of study were patients (44.8%) with various diagnoses. The subjects usually gave oral consent to take part in that study. Giving information (46.9%) and exercise (26.5%) were common nursing interventions, and physiological indices (16.5%). vital signs (10.3%), physical functioning (8.2%), level of knowledge/skill (7.2%) and level of activity (6.2%) were frequently measured as outcome. Variables questionnaires were the major approach used to collect data, and 57.8% of the research provided the Cronbach alpha to guarantee internal consistency of study instruments. Data were analyzed with computerized statistical packages using, ANOVA (42.0%), T test(39.5%), and chi-square test. For the last seven years, nursing research in Fundamentals of Nursing has gradually improved in both quantify and quality. Conclusion: It was difficult to find any uniqueness or difference compared to other departments of nursing. In fact, because the history of the Journal of Fundamentals of Nursing is rather short, we can expect that there will be further improvement in qualify and content in the future.

A Study on Improvement Plan of Web Accessibility for the Disabled (장애인 웹 접근성 향상방안 연구)

  • Owing to recent advanced techniques in information and communication technology(ICT), our daily life has been changing very quickly and our life style has also been affected greatly. In the current knowledge and information society, the disabled can have a right to enjoy benefits of advanced ICT technologies. However, there is a barrier called digital divide so that the disabled have some difficulties to communicate to the world with ICT technologies. In this sense, our job is to close digital divide and let the disabled enjoy benefits of ICT technologies. Recently there have been some research works to close digital divide between the disabled and the non-disabled. Among various attempts to close digital divide, improving web accessibility for the disabled is the biggest concern since web is a still representative tool among ICT tools. The purpose of this paper is to propose some practical guidelines for improving web accessibility for the disabled. The proposed improvement guidelines are developed based on the existing web accessibility guidelines and are upgraded for more adaptable to the disabled. Our principles are based on the following philosophy. First, any input transmission must be more confirmed. Second, prevent any instant request. Third, allow the disabled to record or store the contents if necessary. Fourth, reduce physical movement for the disabled. Fifth, provide more interaction tools for the disabled.

A Study on the Application of Transfer Equipment Pooling Systems for Enhancing Productivity at Container Terminals (컨테이너터미널에서 생산성 향상을 위한 이송장비 풀링시스템 적용방안에 관한 연구)

  • Due to the increase of container terminals, as the volume of terminals become distributed, the competition of preserving existing volume and inviting new volume are becoming fierce, and various ways for processing terminal volume and inviting volume are being sought. Container terminal efforts to maximize efficiency in order to improve the volume handling capability and productivity by both expansion of the latest equipment and development of the latest terminal system. There are a variety of factors that influence the improvement of productivity at container terminals. Among them, in the case of yard transfer equipment, if it were to convert from the method of a Yard Tractor(YT) being fixed allocated to a certain Gantry Cranes(GC) to a Pooling System that processes in a method that properly distributes and allocates a Yard Tractor(YT) to multiple Gantry Cranes(GC), the terminal productivity and the fusibility of YT may be increased. The KPI which is an indicator for the productivity at container terminals is GC productivity and since GC productivity cannot exceed the speed of physical GC operations, a Pooling System is applied to increase productivity which its meaning and effect is massive. Here in the Report, we produce the Pooling Algorithm system to improve the efficiency of the transported equipments in container terminal which is actually applying for this method and have compared Non pooling system with Pooling system in the fields. By introducing a transfer equipment pooling system and enhancing the productivity compared to other terminals, it may become an essential factor for increasing the continuous service quality and profitability in terms of terminal business.
