Study on the Standardization of Korean Pattern Identification for Wind Stroke (한국형 중풍변증 지표에 대한 신뢰도 연구)

  • This study aimed to develop an evaluation reliability of Korean pattern identification for wind stroke. We studied 643 patients with stroke and made a list of registry for each of them. The present study analyzed 553 cases, in which the resident’s pattern identification agreed with the specialist’s one, and the cases included five differentiation pattern: the fire-heat pattern (114), the dampness-phlegm pattern (157), the static blood pattern (11), the Yin deficiency pattern (81), and the Qi deficiency pattern (190). This study showed that none of the Cronbach's alpha reached 0.700, which is the general reliable level. The average Cronbach's alpha of each symptoms was 0.353 for the dampness-phlegm pattern, 0.571 for the fire-heat pattern, 0.443 for the Qi deficiency pattern, 0.451 for the Yin deficiency pattern, and 0.302 for the static blood pattern. This suggests the possibility that each pattern identification could be coincided with other symptoms, and it also shows the limits of pattern identification of this study that narrows the symptoms of paralysis patients into only a single pattern. Continuous compliments and researches should be done referring to this matter. However, the internal consistency analysis of all the pattern identification showed that every Cronbach's alpha were within the range of 0.670 to 0.703, and the Cronbach's alpha of the whole symptoms was evaluated as 0.692, which makes the reliability of the pattern identification as itself almost satisfactory to the general reliable level, and therefore, significant. In the future, continuous clinical research to develope this pattern identification for wind stroke actually applicable to stroke patients needs to be made through accumulating more cases, improving the objectivity.

Association of Korean Medicine Pattern Identifications and Liver Blood Markers with the Severity of Cognitive Impairment in an Elderly Population (경도인지장애와 경도치매 단계에서의 한의 변증 및 간 혈액지표 연관성 탐색 연구)

  • Objectives: This study aimed to comprehend the characteristics of Korean medicine patterns in relation to varying degrees of cognitive impairment in an elderly population. Methods: The dataset included 127 elderly individuals with cognitive impairment obtained from three Korean medicine hospitals between 2018 and 2021. The participants were categorized into two groups based on Clinical Dementia Rating-Sum of Boxes (CDR-SB) scores: those with questionable impairment (QI) and those with very mild dementia (VMD). A diagnostic framework for Korean medicine patterns encompassing Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire was employed. Liver blood markers, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), and the AST/ALT ratio, were also analyzed. Results: The scores of the cognitive assessment tools (MoCA-K, MMSE-DS, and K-IADL) of the QI group significantly differed from those of the VMD group. CDR-SB exhibited a positive correlation with the scores of each pattern of Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire, whereas ALT and AST values displayed negative correlations. Binomial logistic analysis, controlling for potential confounders, such as age, education years, body mass index, the presence of chronic disease, and the presence of medication, verified that the VMD group showed higher pattern scores and lower ALT and AST values than the QI group. Conclusions: Increases in pattern scores along with decreased AST and ALT levels might be indicative of more severe cognitive impairment.

An Study on Abdominal Pain in Childhood according to Its Symptoms (소아(小兒) 복통(腹痛)에 관한 임상적(臨床的) 고찰(考察))

  • Purpose : This Study was conducted to evaluate clinical manifestations of abdominal pain(AP) in childhood according to its symptoms and to find out characteristic of children with abdominal pain. Methods : Pacients who visited Dong-Eui Oriental Medical hospital from August to November 2001 due to abdominal pain were included. According to questionaire and history taking abdominal pain was classified by its six subtype in the Oriental Pediatic Text Book and Dong Eui Bo Gam(東醫寶鑑). Result : According to the Oriental Pediatic Text Book, among 41 children, 31.7% had symptoms of AP caused by cold in internal organs of deficiency(臟腑虛冷腹痛), 29.3% had symptoms of AP caused by diet(食積腹痛), 19.5% had symptoms of AP caused by cold(寒腹痛), 9.8% had symptoms of AP caused by both cold and diet, 7.3% had symptoms of AP caused by internal diet and external cold(內食外寒腹痛), 2.4% had symptoms of AP caused by stagnation of Ki and stasis of blood(氣滯血瘀腹痛). There is no AP caused by parasites(蟲腹痛). Otherwise, according to Dong Eui Bo Gam, 43.9% had symptoms of AP caused by cold, 26.8% had symptoms of AP caused by diet, 9.8% had symptoms of AP caused by both cold and diet, 7.3% had symptoms of AP caused by fire(熱腹痛), 7.3% had symptoms of AP caused by phlegm(痰飮腹痛), 4.9% had symptoms of AP caused by stagnated blood(瘀血腹痛). Also There is no AP caused by parasites. Conclusion : 1. The type of AP in Childhood is different from AP in adults. AP in Childhood is most occured by cold of deficiency and food, and there is few AP caused by fire. Six subtype in Dong Eui Bo Gam prefer adults to children and It is more studied that what kind of subtype is proper for AP caused by stress or drugs. So the study on subclassification and clinical Manifestations of AP in Childhood is more performed.

  • In order to investigate different and original theories on pathology of eyes from the past main etiology of fire and heat, two texts of Si-sheng-xin-yuan written by Huang Yuan Yu and Eui-gam-jung-ma written by Lee Gyu Jun are selected and analysed in terms of pathology and prescription. Huang explained that diseases of the eyes are usually born of functional disorders of spleen and stomach(脾胃升降失調) accompanied with turbidity change of energy and blood(氣血淸濁變化). In the meantime, Lee described that the diseases are made from disorders of activities of essence, spirit, energy and blood stored in 5 viscera. So following them, the main point of treatment for the diseases of eyes is to restore and clarify the stagnated gastrointestinal(GI) function, or to supply the clear essence and blood to eyes respectively. Also they have same opinions that the fire and heat are the secondary symptoms of the optical diseases. Therefore Huang focused on cleaning the phlegm and leaking the moisture of GI tract to treat those symptoms, and Lee emphasized on nourishing essential energy of kidney and liver on the other hand. Although they preferred to use radical therapy than symptomatic one. But it can be deduced that Huang's theory is more positive and direct therapy and Lee's one is more basic but indirect treatment.

  • Objectives : This study was aimed to investigate the methods for acupuncture and moxibustional treatment to insomnia Methods : The insomnia is classified by seven cause at main subject. According to this, arrange the acupuncture and moxibustional treatment for classical books, and explain eight meridian related to insomnia. Results : 1. Insomnia means deficiency of sleeping and it is the word generally used when a short period of sleeping, difficulty for deep sleeping and difficulty for recovery of original vital energy comes out. In oriental medicine, they understand that uneasiness condition occurred by abnormal operation of the internal organs as heart, liver, gallbladder, spleen, stomach, kidney, etc. caused by external affection or internal injury becomes Insomnia. 2. Cause of insomnia can be classified majorly as fire-transformation of liver and gallbladder , inner shaking of phlegm-heat , insufficiency of both the heart and the spleen, disharmony between heat and kidney, the dysfunction of the stomach, timidity of heart and gallbladder, the deficiency of Qi of the lung. They make insomnia with bad influence upon body and spirit. 3. The meridian system such as Heart Meridian of Hand Soeum, Pericardium Meridian of Hand Gworeum, Stomach Meridian of Foot Yangmyeong, Spleen Meridian of Foot Taeeum, Bladder Meridian of Foot Taeyang, Kidney Meridian of Foot Soeum, Gallbladder Meridian of Foot Soyang, Liver Meridian of Foot Gworeum and Lung Meridian of Hand Taeeum are used to treat insomnia. Sinmun acupoint and Naegwan acupoint are often used in particular because the they are good for calmming the spirit, the heart and purging the heart of (pathogenic) fire. 4. Especialy, Back-Su points of Bladder Meridian of Foot Taeyang was used by each causes. The Back-su Points was mainly used for heal the insomnia because the Back-Su points has good competent to control the ability of internal organs by direct effect to it. It is because the vitality flows through around back-Su. Conclusions : It comes to a conclusion as follows with research for relevancy of the main cause of insomnia and meridian system includes meridian point.

  • Objectives : The objective of this paper is to examine the pathology and treatment methods of tinnitus and deafness. Methods : Contents on the ears, pathology and treatment methods of tinnitus and deafness in the Donguibogam were examined. Results & Conclusions : Findings indicate that Kidney jing deficiency and problems of qi metabolism of the Kidney are at the root of tinnitus and deafness pathology. In treatment, once the primary symptoms are managed, the root, which is Kidney deficiency, needs to be improved, together with life style management. In the case of Heart-Kidney disconnection, the Heart must be collected while nurturing jing; in the case of yin deficiency and flaring, ministerial fire must be managed while tonifying jing and blood. If the cause lies in the Lungs and Kidney, they must be tonified, while in cases due to problematic water fluid metabolism, phlegm-fire or exterior pathogens, the Kidney needs to be dealt with even after treating the aforementioned causes.

The correlation between average temperature & precipitation and Pattern Identification of the patients with stroke (중풍환자의 변증(辨證)과 기온 및 강수여부와의 상관성에 관한 연구)

  • Objectives : This study was to investigated the correlation between average temperature & precipitation and pattern identification of the patients with stroke. Methods : 115 stroke patients within 30days of onset were registered during the study period. Pattern identification of stroke was classified into Fire and heat, Turbid phlegm and others. We took the mean of average temperature according to pattern identification and analyzed pattern identification into two groups according to climatological normal of average temperature in Seoul. We also analyzed pattern identification into two groups according to existence of precipitation. Results : Heat and fire, and other type of pattern identification had high frequency when average temperature was higher than 12.2℃. And there was no correlation between precipitation and pattern identification of stroke.

  • In this paper, when a physician make a diagnosis of the pattern identification (PI) in Korean stroke patients, the development methods of the PI classification function is considered by diagnostic questionnaire of the PI for stroke patients. Clinical data collected from 1,502 stroke patients who was identically diagnosed for the PI subtypes diagnosed by two physicians with more than 3 years experiences in 13 oriental medical hospitals. In order to develop the classification function into PI using Korean Stroke Syndrome Differentiation Standard was consist of the 44 items (Fire heat(19), Qi deficiency(11), Yin deficiency(7), Dampness-phlegm(7)). Using the 44 items, we took diagnostic and prediction accuracy rate through of discriminant model. The overall diagnostic and prediction accuracy rate of the PI subtypes for discriminant model was 74.37%, 70.88% respectively.

  • Objectives: Literary investigation of existing data was conducted to verify effects of Bezoar Bovis and its herbal acupuncture, and determine quality management through component analysis. Results: Following results were obtained through literary investigation. 1. Bezoar Bovis is dried cystic stone from a cattle. Its characteristics are cool, no toxicity, and bitter taste. Known actions are: quells heat and detoxifies Fire Poison, extinguishes internal movement of Liver Wind and stops convulsion, vaporizes phlegm, and opens orifice. It is mainly used for treating tremor, stroke, delirium, sore throat, oral furuncle, boil, and others. 2. Bezoar Bovis is effective for eliminating liver toxicity, protecting against brain damage, and has anti-microbial activities. 3. Bezoar Bovis is mixed with bear gall bladder and deer musk to be used as herbal acupuncture, and this mixture is effective is invigorating liver functions as well as treating arthritis, headache, and etc. 4. Principal components of Bezoar Bovis are bilirubin-type pigments and cholic acids. The amount of bilirubin can be used as a standard to determine the quality of Bezoar Bovis.

  • xWe studied about the causes of hypertension in the occidental and oriental medical records and the results are as follows. 1. The causes of hypertension in oriental medicine can be divided into wind, fire, phlegm and deficiency of Qi in our body, and broadly into three categories as emotions in excess, improper diet and damages of endogenous etiological factors. 2. Hypertension is the disease of undermining of fundamental aspects while prevailing incidental aspects and liver, heart and kidneys are the organs mainly related to hypertension. 3. The main pathological factors of hypertension are as follows; the disorder of Yin and Yang makes the situation of the deficiency of the vital essence of kidneys and the exuberance of the vital essence of liver and they are the fundamentals to be taken hypertension and finally make the state of the deficiency of both Yin and Yang. 4. In the point of occidental medical view, the essential hypertension is a disease of unknown etiology and we think it occurs not on the only one factor but on the various factors and secondary hypertension occurs from the diseases of the substances and the vessels of kidneys.
