LEEP시술 절단면에 병변이 잔존하는 자궁경부상피내종양(CIN) 한의학적 치험 4례 (4 Cases of Traditional Korean Medicine Treatment for Patients with Surgical Margin Positive after LEEP in Cervical Intraepithelial Neoplasia)

  • The positive margins after LEEP(loop electrosurgical excision procedure) in cervical intraepithelial neoplasia are generally considered to be a risk factor for the recurrence or persistence of CIN currently. When positive margin exists, secondary LEEP or hysterectomy is performed. The aim of this study was to observe effects of Traditional Korean Medicine treatment for patients with surgical margin positive after LEEP. It was conducted retrospective chart review for 4 patients with the surgical margin positive after LEEP, who were scheduled to have secondary LEEP 3 months later. Patients were treated with herbal medicine, pharmacopuncture and herbal liquid vaginal treatment. They were followed up by cytology, colposcopy, human papillomavirus DNA test and punch-biopsy at 1, 3 and 6 months. After 3 month of treatment, three patients did not need secondary LEEP because of normal cytology, negative HPV status and normal colposcopy, while the other patient underwent secondary LEEP because of ASCUS cytology and positive high-risk HPV. After 6 month of treatment, the other patient also had normal cytology, negative HPV status and normal colposcopy and had been in fifth week of pregnancy. This study suggest that Traditional Korean Medicine treatment may be an effective to the patients with surgical margin positive after LEEP in cervical intraepithelial neoplasia.

한의학증례보고에서 가치 있는 증례는 무엇일까? (What Cases Are Worth Publishing in the Korean Medical Case Report?)

  • Objectives : This study aimed to understand the characteristics of the cases covered in the case studies on traditional Korean medicine (TKM) and furthermore, to provide basic information that can lead the discussion on 'what cases are worth reporting' in future case reports. Methods : Case reports on TKM were searched using the OASIS. The searched researches were analyzed according to the type of case, including information on disease/symptoms and intervention. Results : A total of 940 researches were searched. The most frequently reported type of case study was the report on the effectiveness of intervention. Case reports, which were only two cases in the 1970s, increased rapidly in the 2000s, and in particular, 314 cases within the last five years accounted for about 33% of the total literature. As for the number of studies by disease, the cases dealing with musculoskeletal diseases such as spine, shoulder and knee joint disorders were the most prevalent. Besides, there were many case reports related to cardiovascular, gynecological, cancer, psychiatric, and dermatological diseases. In a total of 51.9% of the included case reports, a combination of two or more Korean medical treatments such as acupuncture and herbal medicine was used at once, and western treatment was used with Korean medical treatment in 28.2% of the studies. The types of Korean medical treatments were varied, such as acupuncture, moxibustion, pharmacopuncture, electroacupuncture, Chuna, acupotomy, herbal medicine, external preparation, and psychotherapy. The main purpose of the publication of the included case reports was analyzed as a report of TKM treatment for rare diseases, or the application of TKM treatment to diseases or symptoms that are "uncommon in TKM treatment" even if it is not a rare disease. Conclusions : Case reports have the strength of generating new scientific hypotheses by detecting the basic needs and novelty of medicine. The current case studies of TKM do not seem to be sufficient to highlight these strengths. It is necessary to discuss which cases are reported as cases of patients worth publishing, and based on this, it is necessary to activate case studies of TKM by utilizing diagnostic tools and science technology.

제 4족지 원위지골 골절 지연유합 환자의 복합 한의치료 효과 (The Clinical Effects of Complex Korean Medicine Treatment in Patient with Delayed Union of the 4th Toe Distal Phalanx Fracture)

  • The purpose of this study is to report the effect in treating a patient with delayed union of 4th toe distal phalanx fracture with a combination of Korean Medicine treatment. We treated the patient with acupuncture, cupping, pharmacopuncture, moxibustion and herbal medicine, from March 3, 2018 to June 25, 2018 (total 33 times). The numeric rating scale (NRS), X-ray were applied as outcome measures. After complex Korean medicine treatment was carried out, delayed union of 4th toe distal phalanx fracture was improved and pain was reduced. This result shows that Korean medicine treatment may be an effective option for delayed union. Further clinical studies are needed to clarify the effect of Korean medicine treatment on delayed union.

한의학적 치료를 활용한 성기사마귀 환자 194례의 임상 특성 연구 (Clinical Analysis of 194 Patients with Condyloma Applying Korean Medicine Treatment)

  • The purpose of this study is to assess potential effects of the effects of Korean Medicine treatments on condyloma. Retrospective study of 194 patients with diagnosed condyloma in Saengki Korean Medicine Clinic from October 2013 to September 2018 was done. Patients were treated with Korean Medicine treatments such as herbal treatment, pharmacopuncture and moxibustion. The data has been analyzed by chi-squared distribution and t-test. A total of 194 patients were 148 males and 46 females. In the aspect of diagnosis, newly diagnosed patients were 96 and recurrent patients were 98. After the treatment, in the light of size complete clearances were 110 patients, partial clearances were 63 and non responses were 21. And in the light of number complete clearances were 110 patients, partial clearances were 68 and non responses were 16. In complete clearance group, the relation among the treatment period, sex, age, and the site of wart showed statistically significant. There were 3 recurrences of warts in the 6 months of follow-up observation for 110 of clearance group. No major side effects related to the treatment were found. As the result of t-test shows, the Korean Medicine treatment we had applied revealed as statistically effective on both number(t=11.471, p<.001) and size(t=11.951, p<.001) of the wart. The result of this study suggests that additional treatment of Korean medicine such as acupuncture and moxibustion treatment could be effective on condyloma patients.

통합적인 한방치료로 호전된 삼차신경통을 동반한 말초성 안면마비 환자 1례 보고 (A Case Report of a Patient with Facial Paralysis and Accompanying Trigeminal Neuralgia Improved by Integrative Korean Medicine Treatment)

  • Objective: This study reports the efficacy of using integrative Korean medicine treatments for a patient with facial paralysis and accompanying trigeminal neuralgia. Methods: A 45-year-old male patient with left side facial palsy and facial pain due to Bell's palsy with trigeminal neuralgia was treated with herbal medicine, acupuncture, pharmacopuncture, and cupping for 18 days. The treatment effect was evaluated by measuring the scores for the numerical rating scale (NRS) of facial pain, the House-Brackmann scale, and the Yanagihara scale. Results: After the hospital treatment, the patient's facial pain NRS score was decreased, and the House-Brackmann scale and Yanagihara scale scores improved after the administration of the Korean medicine treatments. Conclusions: The integrative Korean medicine treatments appeared to be effective in treating facial nerve palsy and associated trigeminal neuralgia.

사마귀의 한방 치료 증례연구 분석 (Analysis of Case Studies on Warts Treated with Oriental Medicine)

  • Objectives : The purpose of this study is to analyze the use of herbal medicine as a treatment method in the case studies on warts in Korea, to identify trends in herbal medicines and herbal prescriptions according to the characteristics of individual patients and to systematically analyze concurrent therapies. Methods : Domestic databases (OASIS, RISS, NDSL, KTKP, KISS, KISTI, KISTI) were used to search for case studies related to the herbal medicine treatment of warts. There was no restriction on the time of publication. A total of 9 papers were selected. Results : A total of 13 prescriptions were presented 23 times in 18 cases. Among them, bojungikgi-tang gamibang(補中益氣湯加減方) was the most frequently prescribed. Gwakhyangjeonggi-san gamibang(藿香正氣散加減方) and Damsoeun-tang gagam were the next most frequently prescribed. 69 herbs were used in a total of 13 prescriptions. Herbs were commonly classified as heat-clearing medicine(淸熱藥), exterior-releasing medicine(解表藥), and tonifying and replenishing medicine(補益藥) in descending order. The herbs used more than 6 times included Coicis Semen(薏苡仁), Glycyrrhizae Radix et Rhizoma(甘草) (10 times), Bupleuri Radix(柴胡), and Scutellariae Radix(黃芩) (6 times). Concurrent therapies were performed except for 1 study, and they were acupuncture, moxibustion, pharmacopuncture, external agents, and cupping therapy. Conclusions : This is the first study to analyze the use of herbal medicine as a treatment method for warts in case studies performed in Korea. We hope that this study would give helpful information about the treatment of warts in clinical practices.

국내 한의 학술지에 게재된 여드름 관련 연구 동향- 2005년 이후 발표된 실험적 연구 논문 중심으로 - (Review on the Acne Related Articles Published in Korean Medical Journals - Focusing on Experimental Studies Published after 2005 -)

  • Objectives: This study was performed to analyze the trend of acne-related experimental studies in Korean medicine. Methods: We searched 17 articles related with experimental study of acne, published Korean medical journals from 2005 to 2014. We classified all articles according to the journal and checked on the kind of herbs. All articles were categorized into two types: cell experimental study and animal experimental study. Results: 1. Total 17 papers were searched; 7 articles (41.2%) were published in the Journal of Korean Medical Ophthalmology, Otolaryngology, Dermatology, 4 articles (23.5%) were published in the Journal of Korean Institute of Pharmacopuncture, 3 articles (17.6%) were published in Korean Journal of Oriental Physiology and Pathology, 2 articles (11.8%) were published in Korean Journal of Herbology, and 1 article (5.9%) was published in the Journal of Korean Medicine. 2. Total 17 kind of herbs were used in the studies; 10 articles (58.8%) used single herb, 7 articles (41.2%) used complex herbs. 3. Total 17 articles were categorized 2 types; 16 cell experimental studies (94.1%), 1 animal experimental study (5.9%). 4. Experimental researches were mainly investigated using Raw 264.7 cell line and P. acnes. The levels of NO, iNOS, $TNF-{\alpha}$, PGE, COX-2, $IL-1{\beta}$, IL-6, NF-kB were analyzed to confirm of herbs' antibacterial, anti-inflammatory, antioxidant activity. Conclusions: Searching for natural antibiotics, it is needed to do experimental studies continuously, and also needed to diversify the kind herbs and experimental bacteria.

한의표준임상진료지침 개발을 위한 수족냉증에 대한 한의사의 인식과 치료현황 (A Survey on Korean Medicine Doctors' Recognition and Treatment for Developing Korean Medicine Clinical Practice Guideline of Coldness of Hands and Feet)

  • Objectives: The purpose of this study were to researched a Korean medicine doctors' recognition about coldness of hands and feet, and developing of korean medicine clinical practice guidelines (CPG) for coldness of hands and feet. Methods: We conducted a questionnaire survey targeting 399 Korean medicine doctors belonging to the Association of Korean Medicine by e-mail and analyzed the answers. Results: 1. 86.86% of the respondents agreed about the necessity of CPG for coldness of hands and feet. 2. 84.2% of respondents wanted coding of Korean Standard Classification of Diseases (KCD) on coldness of hands and feet. 3. To diagnosis a coldness of hands and feet, the respondents used a Subjective symptoms (98.5%), Infrared thermographic imaging device (DITI) (26.32%) Heart rate variablity test (HRV) (17.04%), Thermometer (9.77%), Cold stress test (2.76%) 4. Causing of coldness of hands and feet, the respondents considered a constitution or heredity (84.71%), stress (73.66%), lack of exercise (64.91%), irregular eating habits (51.63%), Cold meals (32.83%), depression (31.33%), etc. 5. Treating coldness of hands and feet, the respondents used a herbal medicine (66.85%), acupuncture (70.7%) Pharmacopuncture (23.85%) and moxibustion (60.08%) for $10.91{\pm}8.03week$. Conclusions: We researched a Korean Medicine doctors' recognition of CPG, clinical diagnosis, treatment on a coldness of hands and feet, and policy they required.

항염증 효능을 가진 단미 한약의 2001년 이후 실험 연구 결과에 대한 고찰 (A Review of Experimental Study for Herbal Simplexes of Anti-inflammatory Effects Since 2001)

  • Objective : The purpose of this study is to survey the experimental studies for herbal simplexes of Anti-inflammatory Effects by reviewing oriental medicine journals since 2001. Methods : We collected 65 research papers on Oriental herbal simplexes medicines (herbs and Pharmacopunctures) of Anti-inflammatory effects in the internet site OASIS using the keyword 'Anti-inflammation'. Then we analyzed them according to published journals, years, and research methods, etc. Results : 65 papers in relation to Oriental herbal medicines of anti-inflammatory effects have been published in many kinds of journals and shows a tendency to increase recently. There are 60 cases of using herbal simplexes and 5 cases of Pharmacopuncture. Conclusions : This result will provide useful information of field of Oriental herbal medicine for the development and clinical use of anti-inflammatory related herb simplexes. In the future, we have to perform more studies such as clinical trial and research papers on developed design and herb-combined prescription study for anti-inflammatory effects.

녹용건비탕을 비롯한 한방치료로 호전된 허로 환자 1례 (Consumptive Disease and Chronic Fatigue Improved by Nokyonggunbi-tang and Other Korean Medical Treatments: A Case Report)

  • Objective: This study investigated the efficacy of using Nokyonggunbi-tang and other Korean medical treatments for a patient with consumptive disease and chronic fatigue. Methods: A 59-year-old female patient with fatigue, headache, dizziness, and neck pain due to consumptive disease and chronic fatigue was treated with herbal medicine (Nokyonggunbi-tang), together with acupuncture, pharmacopuncture, cupping, and chuna manual therapy, for 22 days. The treatment effects on the pain were evaluated using the scores for the Fatigue Severity Scale (FSS), European Quality of Life Five Dimensions (EQ-5d) Scale, Neck Disability Index (NDI), and Numerical Rating Scale (NRS). Results: Following treatment, the patient showed a decrease in the FSS, NDI, and NRS scores and an improvement in the EQ-5d score. Conclusions: Nokyonggunbi-tang and Korean medical treatment significantly improved fatigue symptoms in a patient with no substrate disease.
