협간극과 상악동에서 발생한 신경초종 : 증례보고 (Schwannoma in the maxillary sinus and buccal space: Case report)

  • Schwannomas are tumors which originate from the neuroectodermal Schwann cell of cranial, intraspinal, peripheral and autonomic nerve sheaths, and they are solitary, benign, slow growing and well encapsulated neoplasm. Schwannomas are usually asymptomatic. No strong gender or age predominance exists. The incidence of extracranial schwannomas in the head and the neck region varies from 25~45%. In addition, schwannomas are rare in the maxillary sinus or buccal space. In this paper, it diagnosed and treated a 54-years old female patient, who had schwannoma in the maxillary sinus derived from infraorbital nerves, the branch of the left trigeminal nerve, and a 19-years old male patient, who had schwannoma arose in the buccal space derived from the buccal branch of the right facial nerve. There was no particular complication except sensory extinction of the nerve in the female patient and paralysis by the nerve in the male patient. It is determined those two cases of schwannoma in the rare portion is valuable and herein, it reports those with literature discussions.

  • Varicella-zoster virus(VZV) becomes latent in the sensory ganglia after primary infection and emerges from latency to cause zoster in adults. After primary infection, VZV remains latent in the dorsal spinal ganglia. The mechanisms responsible for its reactivation and the clinical entity of herpes zoster are poorly understood. Reactivation of VZV is commonly known to manifest as Ramsay Hunt syndrome which is one of the VZV-associated neurologic diseases with facial paralysis, ear pain, and a characteristic herpetic auricular rash. It is now known that lesions of this syndrome can affect all cranial nerves. Central, cervical and peripheral effects of this syndrome is polyneuropathic in nature. VZV usually involves the 5th and 7th cranial nerves and less commonly the lower cranial nerves such as 9th and 10th. We report a treated case of healthy 40 years old male with VZV infection of the 5th, 9th and 10th cranial nerves. The patient typically showed herpetic vesicles in the auricle and temporal bone area without facial paralysis.

8과 1/2 증후군 2례 (Eight and a Half Syndrome: A Clinical Study of 2 Cases)

  • Objectives : The purpose of this case is to report the improvement of two patients diagnosed with eight and a half syndrome after combination therapy of oriental and western medicine. Methods : We treated the patients with combination therapy of oriental and western medicine such as acupuncture, moxibustion, herbal medication, antithrombotic agents and steroid therapy. Changes of peripheral facial paralysis were evaluated using House-Brackmann facial grading system and the degree of dizziness, tenderness and pain of upper abdomen were assessed using numerous rating scale. Changes in motor grade of upper and lower extremities were evaluated using medical research council scale. Results : We have recently experienced two cases of eight and a half syndrome - a syndrome characterized by the coexistence of one and a half syndrome, a rare ophthalmoparetic syndrome characterized by a conjugate horizontal gaze palsy in one direction and an internuclear ophthalmoplegia in the other, and cranial nerve VII palsy. The one and a half syndrome was caused by acute cerebral infarction. Two patients in this report were improved through combination therapy of oriental and western medicine. Conclusions : We report the clinical course and treatment methods of eight and a half syndrome.

Guilain-Barre Syndrome 환자에 대한 한방치료 1례 (A case report of Guillain-Barre syndrome)

  • Objective : This is clinical about the Wei syndrome(痿證)-patient diagnosed as Guillian-Barre Syndrome(GBS). GBS, What is called acute inflammatory plolyneuritis, is a disorder in which the body's immune system attacks parts of peripheral nerve system. It is characterized by the rapid onset of weakness, paralysis of the legs, arms, breathing muscles and face. But the analysis of CSF and electical tests on nerve and muscle function can be performed to confirm the diagnosis. Most cases occur shortly after a viral infection. Methods & Result : This is the clinical report about the one patient diagnosed as Guillain-Barre Syndrome. The patient, 46-year-old men had weakness in both legs and arms after divertculitis. His weakness and general condition improved after oriental medical treatment and acupuncture. As a result, symptoms were improved remarkably. Conclusion : We report that we had good effects of oriental medical treatment on Guillain-Barre Syndrome.

  • Bell's palsy is an acute facial paralysis caused by peripheral facial nerve dysfunction. The aim of this study is to assess the efficacy of Korean medicine in the treatment of three patients who were hospitalized and diagnosed with Bell's palsy within 2 weeks of coronavirus disease 2019 (COVID-19) infection. The patients were administered with Korean medicine treatments, steroids, and antiviral drugs. Moreover, the Korean medicine treatments include acupuncture, pharmacopuncture, moxibustion, physical therapy, and herbal medicine. Symptom improvement was evaluated daily using the Yanagihara facial nerve grading system, a facial function evaluation tool. Furthermore, it was suggested that the patients affected by Bell's palsy after COVID-19 infection may have a slower improvement in their treatment progress compared with those without COVID-19 infection in the acute stage.

  • Bell's palsy is an acute peripheral facial paralysis with no detectable cause. Although the prognosis of Bell's palsy is generally good, some patients experience poor recoveries and there is no established treatment for those that do not recover even after receiving the conventional treatment. Here we present two cases of refractory Bell's palsy with facial nerve enhancement in magnetic resonance imaging who showed symptomatic improvement after the late administration of high-dose intravenous methylprednisolone.

Analyzing clinical and genetic aspects of axonal Charcot-Marie-Tooth disease

  • Charcot-Marie-Tooth disease (CMT) is the most common hereditary motor and sensory peripheral neuropathy. CMT is usually classified into two categories based on pathology: demyelinating CMT type 1 (CMT1) and axonal CMT type 2 (CMT2) neuropathy. CMT1 can be distinguished by assessing the median motor nerve conduction velocity as greater than 38 m/s. The main clinical features of axonal CMT2 neuropathy are distal muscle weakness and loss of sensory and areflexia. In addition, they showed unusual clinical features, including delayed development, hearing loss, pyramidal signs, vocal cord paralysis, optic atrophy, and abnormal pupillary reactions. Recently, customized treatments for genetic diseases have been developed, and pregnancy diagnosis can enable the birth of a normal child when the causative gene mutation is found in CMT2. Therefore, accurate diagnosis based on genotype/phenotypic correlations is becoming more important. In this review, we describe the latest findings on the phenotypic characteristics of axonal CMT2 neuropathy. We hope that this review will be useful for clinicians in regard to the diagnosis and treatment of CMT.

가토의 좌골신경에 Morphine, Meperidine, Pentazocine을 주사한 후 미세형태학적 및 신경생리학적 변화 (Ultrastructural and Neurophysiological Changes Observed Following Injection of Morphine, Meperidine and Pentazocine in the Sciatic Nerves of Rabbits)

  • Morphine, meperidine 및 pentazocine을 가토의 좌골신경에 주입한 후 마약제의 신경차단 유무와 약제 주입 후 4시간, 24시간 및 1주에 좌골 신경을 절취하여 신경조직학적 변화를 관찰하였다. 좌골신경에 약제를 주입한 후 신경자극에 의한 반응과 뒷다리 운동을 관찰한 결과, morphine군은 신경차단 효과가 없었고 meperidine군과 pentazocine군은 약제주입 5분 후부터 근육이완이 시작되어 10분 후부터 근육수축이 나타나지 않았으며 뒷다리에 마비증상은 약제주입 60분 후부터 부분적으로 회복되기 시작하여 90분 후에는 정상으로 회복되는 양상의 신경차단 효과가 있었다. 광학 현미경적 소견으로는 모두 4시간부터 1주까지의 표본에 특기할만한 변화가 없었으며, 전자 현미경적 소견에서 morphine군은 1주 후 소견에서 유수신경섬유와 무수신경섬유에 경미한 수포양을 보였다. Meperidine군은 4시간 후 소견으로 유수신경섬유의 축삭돌기에 경미한 수포양이 있었고 무수신경의 마이엘린화되는 소견이 있었으며, 24시간 후 유수신경섬유에 경미한 수포양이 있었고 무수신경섬유가 정상으로 되었으며 1주 후 특기할 만한 변화가 없었다. Pentazocine군은 약제주입 4시간 후 유수신경섬유에 경미한 수포양을 보였으며 24시간 후 유수신경 섬유와 무수신경섬유에 중등도의 수포양이 나타났으며 1주 후 경미한 수포양을 나타내었다. 주입된 약제중 morphine이 가장 수포양이 적었으며 pentazocine이 심한 변화를 나타내었고, 전단계 쥐 실험에서 나타났던 meperidine주입 1주 후의 심한 신경조직 손상은 본 실험에서 나타나지 않았다.

Bell's Palsy 환자의 건측-환측 안면부 피부온도차이에 관한 연구 (Differences Between Facial Skin Temperature of the Paralyzed Side and Those of the Normal Side in Bell's Palsy Patients)

  • Objectives : Bell's palsy in an acute peripheral facial nerve paralysis that usually affects only one side of the face. The seventh cranial nerve carries predominantly motor fibers, but also supplies some autonomic innervation, sensation to park of the ear, and taste to the anterior two thirds of the tongue. The aim of this study is to provide evidence of differences between facial skin temperature of the paralyzed side and normal side in Bell's palsy patients. Methods : the author studied 68 patients with Bell's palsy whose facial nerve function had been documented by the House-Brackmann grading system. We measured skin temperature of the forehead, zygoma, lower lip, temple and lower jaw area of the paralyzed side and those of the normal side. Results : there were significant facial skin temperature differences between the forehead area of paralyzed side and that of normal side of GrII(P<0.05) and III(P<0.05). The result also showed that the facial skin temperature difference according to the aflection period vanished when air temperature was calibrated (F=1.700, P=0.178). Conclusions : Thermography is a useful diagnostic tool in Bell's palsy if the air temperature is low enough to cool the facial skin temperature and the forehead area is evaluated as the sampling zone.

Comparison between Subjective Scoring and Computer-Based Asymmetry Assessment in Facial Nerve Palsy

  • Background and Objectives: The aim of the present study was to assess the feasibility of a PC-based facial asymmetry assessment program (PC-FAAP) and to compare the results of PC-FAAP with subjective regional scoring by raters in acute unilateral peripheral facial nerve paralysis (FNP). Subjects and Methods: Participants were divided into 3 groups with 8 participants per group: group I, normal; group II, mild to moderate FNP; and group III, severe FNP. Using the PC-FAAP, the mouth asymmetry ratio (MAR), eyebrow asymmetry ratio (EAR), and complete eye closure asymmetry ratio (CAR) were calculated by comparing the movement of tracking points on both sides. The FNP grading scale (FGS) integrated each score, and the scores were weighted with a ratio of 5:3:2 (MAR:CAR:EAR). Subjective regional scoring was measured on a 0-100 scale score by three otologists. PC-FAAP and subjective scoring were compared in each group regarding the consistency of the results. Results: The mean scores of the MAR, EAR, CAR, and FGS of each group were significantly different. PC-FAAP showed significant differences between the three groups in terms of MAR, EAC, CAR, and FGS. PC-FAAP showed more consistent results than subjective assessment (p<0.001). The PC-FAAP was significantly more consistent in group I and group III (p<0.001 and p=0.002, respectively). FGS in group III was the only parameter that showed a more consistent result in PC-FAAP than the subjective scoring (p=0.008). Conclusions: An FNP grading system using a PC-based program may provide more consistent results, especially for severe forms.
