후방으로 전위된 소아 쇄골 원위부 골절의 수술적 치료 - 1예 보고 - (Operative Treatment of Posteriorly Displaced Fracture of the Distal Clavicle in Child - A Case Report -)

  • We presented a case of posteriorly displaced fracture of the distal clavicle in Child. The closed reduction was failed due to posteriorly displaced proximal fragment and interpositon of trapezius muscle and periosteum. We found that periosteal sleeve and coracoclaviclar ligament and acromioclavicular ligament was intact. The open reduction should be considered for the treatment of the irreducible posteriorly displaced fracture of the distal clavicle in child.

야생 멧돼지의 전염성위축성비염 소견의 1예 (A Case Report of a Feral Pig with Suspected Infectious Atrophic Rhinitis Lesions)

  • A weakened wild feral pig (a boar of about 15 kg) with snout distortion and characteristic external lesion of infectious atrophic rhinitis was captured at farm land near low mountains in Chinju, Kyeongnam province. This pig was necropsied and then the snout parts and the parenchymal organs were removed. The snout and nose were transversely sectioned at thickness of 1.5 cm interval. Grossly, the right side of the snout was shorted than that of left by reduction of right nasal turbinate length, but the nasal opening exudate was not observed. At necropsy, degeneration, adhesion, occlusion, and asymmetry of left and right sides on the meatus and turbinate were observed and findings of mild pneumonia were observed. Microscopically, the leukocyte infiltration, hyperemia and hyperplasia on the mucosa of the turbinates and septum were observed. The atrophied periosteum and osseous tissue were also observed. But Bordetella bronchiseptica was not identified in culture from nasal swabs. We expect the possibility that the snout distortion of this pig was due to infectious atrophic rhinitis according to these findings.

Life-threatening airway obstruction after flapless implant placement in the anterior mandible

  • This paper reports a patient who had an airway obstruction caused by a hemorrhage of the mouth floor that occurred after she underwent flapless implantation in the region of the mandibular anterior teeth. The hemorrhage may have been caused by iatrogenic malpositioned fixture and patient's hypertension. The lingual periosteum was not dissected during the flapless procedure. Therefore, when hemorrhage occurred, the blood did not drain easily into the oral cavity but instead drained into the deep neck region, which might have been the cause of the airway obstruction.

  • Although painful conditions of varying degrees of severity involving the soft tissues (i .e., muscles, tendons, ligaments, periosteum and peripheral nerves) occur frequently, their underlying pathogenesis is poorly understood. The term peripheral neuropathic pain has recently been suggested to embrace the combination of positive and negative symptoms in patients whose pain is due to pathological changes or dysfunction in peripheral nerves or nerve root. The spinal nerve root, because of its vulnerable position, is very easily prone to injury from pressure, stretch, angulation, and friction. Therefore, not a few of musculoskeletal chronic pains are result of nerve root dysfunction. Neuropathic changes due to nerve root dysfunction are primarily in soft tissue especially muscle, tendon and joint. It shows tenderness over muscle motor points and palpable muscle contracture bands and restricted Joint range. Careful palpation and physical examination is the important tool that, be abne to detect all of these phenomena.

  • A 7-year old Holstein bull showing marked thickening of left forelimb was diagnosed as pulmonary hypertrophic osteoarthropathy on the histological basis of the exostotic bone of left metacarphus. Thickening of the bone was due to apposition of excessive cancerous bone beneath the periosteum on the shaft. The lung lesion of the bull, chronic suppurative bronchopneumonia was possibly related to pathogenesis of the bone thickening.

Management of Alveolar Cleft

  • The alveolar cleft has not received as much attention as labial or palatal clefts, and the management of this cleft remains controversial. The management of alveolar cleft is varied, according to the timing of operation, surgical approach, and the choice of graft material. Gingivoperiosteoplasty does not yet have a clear concensus among surgeons. Primary bone graft is associated with maxillary retrusion, and because of this, secondary bone graft is the most widely adopted. However, a number of surgeons employ presurgical palatal appliance prior to primary alveolar bone graft and have found ways to minimize flap dissection, which is reported to decrease the rate of facial growth attenuation and crossbite. In this article, the authors wish to review the literature regarding various advantages and disadvantages of these approaches.

  • Extraskeletal osteosarcoma (ESOS) is a malignant soft tissue neoplasm producing osteoid, without any continuity with the bone or periosteum. Primary ESOS presenting in the mediastinum is an extremely rare, yet aggressive malignant tumor associated with a poor prognosis. We report a case of primary ESOS arising from the thymus in a 63-year-old male patient.

Atypical periosteal reaction and unusual bone involvement of ameloblastoma: A case report with 8-year follow-up

  • Unusual radiographic findings of intraosseous ameloblastoma have been reported and discussed. In the case discussed herein, cone-beam computed tomography (CBCT) clearly showed many radiographic features that were ambiguous on conventional radiographs, including an ill-defined periphery, extensive superficial buccal extension with minimal lingual extension, obvious bucco-crestal expansion, and multiple triangular (Codman's triangle-like) areas of periosteal reaction. Based on the above-mentioned findings, the differential diagnosis was a long-term infected benign or low-grade malignant lesion. An incisional biopsy was performed, and the histopathologic diagnosis was acanthomatous ameloblastoma. Recurrence of the lesion was clearly detected on CBCT images at 4 and 8 years after surgery. These unusual radiographic findings have never been reported to be associated with ameloblastoma, and thus may contribute to novel concepts in radiographic interpretation in the future. This report also underscores the important role played by CBCT as a comprehensive diagnostic tool and for definite confirmation of recurrence.

Diagnostic imaging features of traumatic subgaleal hematoma in a dog: a case report

  • Subgaleal hematomas are accumulation of blood between periosteum and galea aponeurosis. A 2-year-old male Chihuahua was presented with a severe head swelling after trauma. Radiography and computed tomography (CT) showed a massive swelling encircling the entire calvarial vault, extending toward the cervical neck and crossing the suture line. It was heterogeneously, mild hyperdense fluid to soft tissue attenuating with contrast enhancement on CT images. On day 4, physical and imaging examination showed resolution of the calvarial swelling. Subgaleal hematoma should be considered as a differential diagnosis when there is a massive soft tissue swelling over the skull on physical and imaging examinations.

하악지 길이증가를 위한 수술방법들간의 회귀현상에 관한 실험적 연구 (EXPERIMENTAL STUDY ON RELAPSE AFTER RAMAL LENGTHENING IN DIFFERENT SURGICAL METHODS - RADIOGRAPHIC EVALUATION)

  • Facial asymmetry is the most frequent disease in craniofacial deformities. And the primary causing area of that is mostly placing in mandible. That is to say, it is known that primarily, mandible grows excessively or deficiently, and other facial region involving maxilla undergoes compensatory growth secondarily, so asymmetric face develops. In facial asymmetry, the surgical correction of undergrowth is more difficult than that of overgrowth and the reason of it is the postoperative relapse caused by stress of surrounding soft tissues. It means the stress of surrounding soft tissues occurring after bone lengthening and reducing above stress is the same meaning with reducing postoperative relapse. Among various areas, mandibular ramus is the most difficult area to lengthen vertically and maintain its length. The reason of it is considered by many authors as the stress of surrounding pterygomasseteric sling which is enveloping lower border of mandible and interrupting elongation of ramal height. So we applied two different surgical procedures in which pterygomasseteric slings have different stress respectively to monkeys which have similar masticatory function and anatomy to human being and compared relapse by radiographic film and observed periodically the histochemical change of masseteric muscle fiber. So we could see the following results. The relapse was less in EVRO group in which we separated pterygomasseric sling in inferior border and didn't approximate muscle sling after vertical lengthening to minimize the stress of soft tissues than IVRO group in which we elongated ramal height preserving pterygomassetric sling. Of course, we could see a problem in EVRO group such as bone resorption in inferior border caused by uncovering the periosteum of inferior border. But we expect that such problem will be solved by developing periosteum substitutes for covering the exposed bone and minimizing the surgical trauma. In histochemical study of masseteric muscle fiber, the fiber constituents of EVRO group in which we minimized soft tissue stress was changed immediately after operation and maintained it for 1 year, whereas that of IVRO group in which we preserved soft tissue stress was changed in more portion after operation and recovered it by 1 year. By the histochemical results, we can see that the recovery of fiber constituents reflect the recovery of muscle stress and it is closely related with relapse phenomenon.
