  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissues and involve, essentially, the same pathological process. They are usually classified, depending on their extent and radiographic appearances, into three main groups. Periapical cemental dysplasia predominantly involves the periapical region of the anterior mandible. Florid cemento-osseous dysplasia presents with multifocal lesions in the tooth bearing or edentulous areas of the maxilla and mandible, often occurring bilaterally with symmetric involvement. Focal cemento-osseous dysplasia exhibits a single site of involvement in any tooth bearing or edentulous area of the mandible or maxilla, with the posterior mandible representing the most common site. We report the clinical, radiographic and histological findings of 3 types of cemento-osseous dysplasia with a review of literatures.

  • The cementomas are derived from the periodontal ligament and a diversified group of nonrelated lesions producing cementum-like material. The diagnostic term, Cementoma, has encompassed several unrelated lesions : periapical cemental dysplasia, benign (true) cementoblastoma, cementifying fibroma and familial multifle (gigantiform) cementoma. The authors treated one case of benign cementoblastoma & the other of cementifying fibroma by conservative enucleation & curettage. By follow up check of the patient, We obtained of good result without any signs of recurrence of the lesions.

Common pitfall of plastic surgeon for diagnosing cutaneous odontogenic sinus

  • Dental origins are a common cause of facial cutaneous sinus tracts. However, it can be easily overlooked or misdiagnosed if not suspected by a surgeon who is not familiar with dental origins. Cutaneous odontogenic sinuses are typically nodulocystic lesions with discharge and are most frequently located on the chin or jaw. This article presents two cases of unusual cutaneous odontogenic sinus presentations, as deep dimpling at the middle of the cheek. The patients were undergone surgical excision of sinus tract and dimpling immediate before and after treatment of causal teeth and the lesions resolved without recurrence. Surgeons should consider dental origins of facial dimpling lesions with discharge and provide appropriate treatment.

THE RELATIONSHIP OF RADIOGRAPHIC LESION SIZE AND CHARACTERISTICS TO DIAGNOSIS OF PERIAPICAL CYSTS AND GRANULOMAS (방사선학적 치근단병소의 크기와 특징에 대한 치근단낭종 및 육아종의 상관관계연구)

  • The purpose of this study was to find out the relationship of radiographic lesion size, gender, age of patients and radiographic character to the diagnosis of periapical cyst and granuloma. The data was collected from 187 periapical lesions of 167 patients who undergone apical surgery at Department of Conservative Dentistry, Seoul National University Dental Hospital from 2003 to 2005. The lesion were surgically removed and send for biopsy to the Oral Pathology Laboratory. From the initial radiograph, lesion size was calculated using $PiViewSTAR^{(R)}$ (INFINITT, Korea) program. The obtained data were statistically evaluated using SPSS (p<0.05). The result were as followings: 1. From 187 biopsy samples, the incidence of periapical cyst was 28.34% and granuloma was 65.24%. 2. There was a significant correlation between periapical cyst and the size of radiographic lesion (p<0.01). 3. There were no significant correlations between age, gender, location of lesion and the final diagnosis (p>0.05). 4. There was a significant correlation between the non-demarcation of the lesion and the incidence of periapical granuloma (p<0.01).

Determining the reliability of diagnosis and treatment using artificial intelligence software with panoramic radiographs

  • Purpose: The objective of this study was to evaluate the accuracy and effectiveness of an artificial intelligence (AI) program in identifying dental conditions using panoramic radiographs(PRs), as well as to assess the appropriateness of its treatment recommendations. Materials and Methods: PRs from 100 patients(representing 4497 teeth) with known clinical examination findings were randomly selected from a university database. Three dentomaxillofacial radiologists and the Diagnocat AI software evaluated these PRs. The evaluations were focused on various dental conditions and treatments, including canal filling, caries, cast post and core, dental calculus, fillings, furcation lesions, implants, lack of interproximal tooth contact, open margins, overhangs, periapical lesions, periodontal bone loss, short fillings, voids in root fillings, overfillings, pontics, root fragments, impacted teeth, artificial crowns, missing teeth, and healthy teeth. Results: The AI demonstrated almost perfect agreement (exceeding 0.81) in most of the assessments when compared to the ground truth. The sensitivity was very high (above 0.8) for the evaluation of healthy teeth, artificial crowns, dental calculus, missing teeth, fillings, lack of interproximal contact, periodontal bone loss, and implants. However, the sensitivity was low for the assessment of caries, periapical lesions, pontic voids in the root canal, and overhangs. Conclusion: Despite the limitations of this study, the synthesized data suggest that AI-based decision support systems can serve as a valuable tool in detecting dental conditions, when used with PR for clinical dental applications.

  • This is a case report of first branchial cleft cyst in 56 year old male patient, which was tentatively diagnosed as acute right submandibular abscess resulted from the periapical lesion of the lower right second molar. The results are as follows, 1. The accompanying ipsilateral inflammatory swelling resulted from the periapical lesion of lower right second molar tooth makes the diagnosis difficult. 2. The onset of this case was very late in comparison to the mean discovering age of branchial cleft cysts. 3. The plain radiography using contrast media is helpful for the diagnosis of cystic lesions within soft tissues. 4. This case in a first branchial cleft cyst(Type I) which occurs less than 1% of all branchial cleft anomalies.

  • Microorganisms and their by-products are considered to be the major causes of pulpal and periapical pathosis. The role of microorganisms in endodotic infection has been studied regarding the prevalence of particular organisms found in root canal and periapical lesions. The aim of this study was to investigate the effect of culture supernatants of several oral microorganims isolated from infected root canals on the viability of cultured cell lines using colorimetirc assay. S. simulans, S. sciuri, E. faecium, S. intermedius, S. mitis, S. sanguis and S. uberis were incubated in Todd-Hewitt broth for 16 hours. 20 and 100ul of filtered bacterial cell culture supernatants were added to MK and Hep-2 cells. Cell viability was measured using MIT colorimetric assay. 20ul and 100u1 of S. sanguis supernatant showed significant cytotoxicity compared to control on MK cells. 100ul of S. sanguis supernatant significantly depressed viability of HEp-2 cells. E. faecium and S. intermedius did not affect the viability of MK and HEp-2 cells.

THE EFFECT OF PDGF-BB AND IGF-I COMBINATION ON THE HEALING OF ARTIFICIAL PERIAPICAL LESIONS IN BEAGLE DOGS (PDGF-BB와 IGF-I 혼합 투여가 비글견 인공 치근단 병소의 치유에 미치는 영향에 관한 연구)

  • It is difficult to treat the endodontic apical perforation successfully. In this study, we hypothesized that the application of PDGF-BB and IGF-I into periapical perforation site may accelerate periapical healing and lead to bone deposition. And the specificity of osteonectin in periapical healing was investigated. The experiments were performed on the upper and lower 51 premolar teeth of 4 beagle dogs. The pulp chamber of each tooth was opened and the dental plaque was inserted into the canal for developing the periapical lesion for 5 weeks. Then, the roots were artificially perforated at the apex with the number 4 profile of .06 taper. In each step, standard periapical radiographs were taken to compare the size of lesion each other. The radiographs were scanned and analyzed by image analysis system. The mean and standard deviation of periradicular radiolucency ratios were calculated in each group. ANOVA was used for comparison. 51 premolars were grouped into 3 groups; control group, calcium hydroxide-treated group and calcium hydroxide plus growth factors-treated group. In the control group, the apical perforations were not sealed and obturated with gutta-percha and ZOE sealer by lateral condensation technique. In the experimental groups, the apical perforation were sealed with calcium hydroxide and with/without $4{\mu}g$ of PDGF-BB & IGF-I in cellulose gel and obturated by lateral condensation technique. Fluorescent bone markers were used to measure new bone formation. Following 2, 4, 12 weeks after experiment the dogs were sacrificed and histologic sections were prepared. Each tooth block including periapical lesion was sectioned mesiodistally. One half of the sections were decalcified with 6% nitric acid and processed by standard paraffin embedding technique. The sections were stained by hematoxylin and eosin, and immunostained for osteonectin. Histomorphometrical measurement of neoformed bone was performed using a light microscope. And the other half of the sections were prepared by undecalcified preparation, and confocal laser scanning microscopic investigations were done.

A Study on the Visual System of the Digital Periapical Images (디지털 치근단방사선영상에 관한 시각 특성 연구)

  • Objectives: The purpose of this study was to evaluate the optimal distance and angle of observers by modulation transfer functions(MTFs) and receiver operating characteristics(ROCs), Material and Methods: Digital periapical radiograms were taken from 43 patients who have dental diseases(19 patients : dental caries. 12 patients : periapical lesions, 12 patients : periodontal diseases). Segmental images(4×4cm) were evaluated by 4 MTFs and ROC analysis. Results: The optimal distance(magnification) using MTF by Mannos & Sakrison was 12.97. and those by Nill. Ngan and Rao were 8.39, 4.78. 5.84 respectively. The optimal distance obtained from 4 radiologists by ROC analysis was 32cm(Az value: 0.89). and it was 40cm(Az value: 0.78) from 4 non-radiologists. There were significant differences of Az values between 4 radiologists and 4 non-radiologists at 24. 32 and 40cm (P<0.05). No significant differences of optimal distances were observed using 4 MTFs among +20, +10, 0, -10, -20 degrees(P>0.05). The optimal angle obtained from 4 radiologists by ROC analysis was +20 degree(Az value: 0.91). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). The optimal angle obtained from 4 non-radiologists by ROC analysis was 0 degree(Az value : 0.81). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). And there was significant difference of Az value between 4 radiologists and 4 non-radiologists at +20 degree(P<0.05). but no significant differences of Az values were observed among +10, 0, -10 and -20 degrees(P>0.05).

  • The effect of primary tooth infection on permanent teeth was reported by many previous study. It is seen histologic change in the enamel forming organ and in the surrounding sac, thus produce the defect as either hypoplasia or hypocalcification and alter the eruption of the permanent teeth. The periapical inflammation cause permanent tooth displacement in various direction and can be classified into the three categories : rotation, deflection inversion. This study reported case which displacement of succedaneous teeth may be caused by periapical infection of primary dentition and concluded that treatment procedure of infected primary tooth is selected and performed by importance to retain the tooth for space maintain, possibility to successfully restored, evidence of abnormal development of the succedaneous tooth.
