This study was conducted to assess factors associated with the degree of performance of qualify improvement(QI) activities. A mailed questionnaire survey was conducted between September 15 and October 30, 2000, with the staffs being charge of QI at each of the hospitals with 400 beds or greater. Of the 108 hospitals eligible for inclusion in our study, 79 participated, yielding a response rate of 73.1%. After excluding 12 hospitals that did not perform any QI activities, 117 responses from 67 hospitals were used for the analysis. Using the Malcolm Baldrige National Quality Award Criteria(MBNQAC), perceived performance of QI was measured in terms of the improvement of the quality of clinical practice, clinical supporting department, administrative procedure of receiving care, customer satisfaction, efficiency and standardization of work process. Factors evaluated for the association were the extent of QI implementation, compliance to 5 QI principles, participation of hospital CEOs, budget allocation, history of QI, and bed size. Path analysis was performed to assess the relationship between QI performance and these factors. Major findings of this study are as follows. Hospitals showing higher degree of QI implementation (path coefficient=0.5967, p<0.001)) and better compliance with the basic principes of QI(0.5736, p<0.05) tended to achieve better performance. Path analysis results showed that interest and participation of hospital CEOs(0.1954, p<0.05) and compliance with the basic principes of QI(0.4028, p<0.0001) indirectly affected the outcomes of QI by influencing the intermediate variable of the level of QI implementation. This study results suggest that having employees have a good orientation of the basic concept and principes of QI through relevant training be the most important requirement to achieve better outcomes from QI activities. In addition, to educate leaders of hospitals the need of active implementation of QI is important to encourage their participation and draw strong support for QI programs.
Purpose: This study compared the importance and performance of rehabilitation nursing needs perceived by stroke patients and nurses. Methods: A total of 275 subjects were divided into two groups. One group was 136 stroke patients and the other was 139 nurses. Data were collected with the Rehabilitation Nursing Service Inventory. The data were analyzed by t-test and ANOVA using the SPSS/WIN 21.0 program. Results: The degree of importance and performance in rehabilitation nursing needs perceived by the stroke patients were $3.69{\pm}0.47$ and $3.48{\pm}0.50$, respectively, showing a significant difference (t=21.04, p<.001). The degree of importance and performance perceived by nurses were $3.84{\pm}0.42$ and $3.60{\pm}0.45$, respectively, showing a significant difference (t=26.53, p<.001). A significant difference in the degree of importance and performance in rehabilitation nursing needs was also observed between stroke patients and nurses (t=-2.78, p=.006; t=-2.12, p=.035). Conclusion: These findings showed that there was a perception gap between stroke patients and nurses. Therefore, it will necessary to identify methods for reducing this perception gap and developing rehabilitation nursing intervention considering the rehabilitation nursing needs of stroke patients.
Digital fountain codes are record-breaking codes for erasure channels. They have many potential applications in both wired and wireless communications. Most existing digital fountain codes operate over binary fields using an iterative belief-propagation (BP) decoding algorithm. In this paper, we propose a new iterative decoding algorithm for both binary and nonbinary fields. The basic form of our proposed algorithm considers both degree-1 and degree-2 check nodes (instead of only degree-1 check nodes as in the original BP decoding scheme), and has linear complexity. Extensive simulation demonstrates that it outperforms the original BP decoding scheme, especially for a small number of source packets. The enhanced form of the proposed algorithm combines the basic form of the algorithm and a guess-based algorithm to further improve the decoding performance. Simulation results demonstrate that it can provide better decoding performance than the guess-based algorithm with fewer guesses, and can achieve decoding performance close to that of the maximum likelihood decoder at a much lower decoding complexity. Last, we show that our nonbinary scheme has the potential to outperform the binary scheme when choosing suitable degree distributions, and furthermore it is insensitive to the size of the Galois field.
The major questions of this study are whether and how the consciousness of value to housework and the performance of housework are related to the family life satisfaction in young housewives. The major findings are as follows : 1. The significant variables influencing the housewives' ability to perform housework are the age of the youngest child and degree of their consciouness of value to housework. 2. The degree of young housewives' preference for housework may be influenced by the monthly income of their household, the area in which each of them lives, and the degree of their consciousness of value to housework. 3. The amount of the time that young housework perform housework can influenced by the age of their youngest child, the area in which each them lives, whether they are employed, and the degree of their consciousness of value to housework. Whether a young housewife is employed or not is epecially the most influential variable even among those significant variables stated above.
This study was aimed for effective nursing performance of the nurse to be able dedicate to the development of professional nursing work by understanding correlation between the leadership style of head nurse and the nursing performance of the nurse. The materials have been collected through structured questionnaires for 18 days from Feb. 2, 1999 to Feb. 20 from 234 nurses presently working at 4 hospitals Circumstances aptitude theory of Fleishman (1973) were used as the device of measuring leadership style, and staff nurses nursing performance were done by head nurses utilizing questionnaire adopted from Park. Data was analyzed by frequency, mean, ANOVA, ANCOVA, and Pearson's correlation coefficiency by using SPSS program. The results are as follows ; 1. Head nurse leadership was classified in the following manner : duty and human relationships oriented style of head nurse leadership(3.76), human relationship oriented style(3.30), duty leadership oriented style(3.12), and non-interference leadership style(2.77). Duty and human relationships showed highest score in this category. 2. The mean of nursing performance was 3.66, dependent nursing duty was 3.88, interpersonal relationship 3.78, and independent nursing was 3.34. 3. An education course of nursing performance by general characteristics was statistically significant. Job satisfaction, for seen working periods, and motivation for choosing a nursing job of nursing job characteristics was statistically siginficant. 4. As the degree of independence nursing performance increases, the degree and of dependance and interpersonal relationship of nursing performance increases. 5. The relationship between nurse leadership style and nursing performance were statistically different. The degree of duty and human relationship style of the head nurse were highest in dependent and independent nursing performance. Therefore, it is desirable that the head nurse should demonstrate the leadership style high in the duty and human relationships for effective nursing performance.
The purpose of this study was to explore the degree of stress in caregivers caring for CVA patients and the level of daily living performance of CVA patients. The subjects for the study were caregivers of 112 CVA patients who enter a hospital or out-patient-department (OPD) at two Oriental medical hospital in Jeonbuk province. The survey instruments used in this study were Kang's ADL check list for daily living performance of patients and Choi's 4 sore scale for stress of caregivers. The survey was conducted from July 4th to August 30th in 1999. The survey results were analyzed with the Statistical Package for Social Science(SPSS) program and can be summarized as follows: 1. The level of daily living performance for the CVA patients was: 1) complete dependence (M=14.9, 13.1%), 2) complete independence (M=23.6, 20.9%), 3) incomplete independence (M=23.9, 21.0%), 4) incomplete dependence (M=26.6, 25%), 5) dependence and independence (M=23.0, 20.0%). The items for with there was a high level daily living performance were: 1) drinking (M=3.62), 2) eating(M=3.25). 3) position returning (M=3.18) : and the items for which there was a low level of daily living performance were: 1) ascending and descending stairs (M=2.08), 2) walking (M=2.47), 3) dressing and undressing trousers (M=2.55). 2. Degree of caregiver stress was: Mean=2.39 at 40 score. The items for which was a high level caregiver stress were: 1) medical fee (M=3.25), 2) being handicapped or recurrence (M=3.02) : and the items for which there was a low level of caregiver stress were: 1) discontinuity of patient's treatment (M = 1.98). 2) change of home atmosphere caused by patient's disease (M = 1.98), 3) desire of patient's knowing about disease (M= 1.99). 3. There was statistically significant difference in the degree of caregiver stress according to the following caregiver's demographic characteristics: education level (F=3.52, P=0.03). change of caregiver (F=5.41. P=0.02). 4. There was a statistically signifiant difference in the level of daily living performance according to the CVA patients demographic characteristics: patient's paralytic status (F=4.48, P=0.01), duration of disease (t=2.76, P=0.03). 5. There was significant difference in degree of caregiver stress according to the CVA patient's demographic characteristics: CVA status (F=4.75, P=0.01). 6. There was statistically significant difference in the degree of caregiver stress according to the level of daily living performance in CVA patients(r=-0.482, P<0.00).
본 연구는 지방의료원의 경쟁 수준의 정도를 측정하여 재무성과와의 관련성 분석 후 효과적인 경영전략을 수립할 수 있는 기초자료를 제공하는데 있다. 조사기간은 지방의료원의 HHI(허핀달-허쉬만)지수와 경영성과를 알 수 있는 2010년부터 2012년까지 3년간을 평가기준으로 하였으며, 총 31개의 의료원을 조사대상으로 선정하였다. 결론을 보면, 집중시장일수록 재무성과가 높은 것으로 나타났으며, 지역 내 경쟁정도와 재무지표 간의 유의한 상관관계가 나타났다. 지방의료원은 지역 내 경쟁력과 공공성을 확보하기 위한 정책수립과 실행을 위해서는 지역 의료시장의 경쟁정도와 그에 따른 추이를 파악이 요구된다. 지방의료원이 타 의료기관관의 경쟁력의 우위를 선점하기 위해서는 의료서비스 제공의 향상과 자본 투자 등의 선별적인 전략이 고려되어야 할 것이다.
In this study, Aimed to develop technology to ensure the safety of firefighters responding to hydrogen incidents and to review the performance of protective super absorbent polymer (SAP) that could help maintain the thermal protection performance of equipment with protective properties. Tests were conducted, including bench-scale and full-scale thermal exposure tests, to review the protective performance of SAP using firefighting garments commonly used by firefighters. The results showed that without SAP application, 2nd degree burn areas were measured at 9.4%, and 3rd degree burn areas at 7.7%. In contrast, when SAP was applied, the percentage of 2nd degree burn areas decreased to 7% on the lower body, and there was no temperature rise causing 3rd degree burns. Therefore, it is expected that by applying SAP to the outer surface of firefighter garments, even under temporarily high temperature conditions such as hydrogen jet flames, thermal damage to firefighters could be protected for a certain period.
The change in health care environment increases the importance of Visiting Nursing Services Program. It has been performed by nurses of district health centers in Seoul since 1991. The Achievement of Visiting Nursing Services Program will be dependent upon their activities. The purpose of this study was to identify the Performance of Visiting Nurses and Job satisfaction of district health centers in Seoul. Therefore, it was to provide the fundamental data development of Visiting Nursing Services Program. The subjects were 214 Visiting Nurses of district health centers in Seoul. The data was collected by self reporting questionnaire from April 15 to April 30, 1997. Their performances and various supportive factors were measured with the instruments developed by the researcher. Job satisfaction were also measured by the instrument developed by Slavitt et al. (1978) was used. The data were, analyzed by Cronbach Alpha, mean, standard deviation, percentage, t -test, ANOVA Duncan test, Correlation Coefficient, and Stepwise Multiple Regression with SAS program. The results of this study were as follows: 1. The average of budget of Visiting Nursing Services Program of district health centers was 0.9% and the average of visiting nursing services personnel of district health centers was 10.1%. 2. With regard to the job satisfaction of Visiting Nurses the mean score was 2.92 out of 5. While the level of Job prestige / status presented as a mean score of 3.48 which was the largest among the 7 components of job satisfaction, the level of administration was the lowest showing 2.57 scores respectively. There were significant differences in the job satisfaction by age, working career of health centers(p<0.01, 0.001). 3. The average of the performance level of Visiting Nurses variables was 2.29; The variable with highest degree of performance was the teaching & consultation, establishment of performance plan, whereas the on with the lowest degree was the directive nursing services. The significant difference was found in performance level according to age, structure type of visiting nursing services, working career of health centers and working career of visiting nursing services(p<0.05). 4. With regard to the perception of the performance expertise by the Visiting Nurses the mean score was 2.37 : The variable with highest degree of performance expertise was the teaching & consultation, establishment of performance plan, whereas the on with the lowest degree was management of home-environment. The significant difference was found in performance expertise according to working career outside of health centers(p<0.05). 5. With regard to the perception of the performance necessity by the Visiting Nurses the mean was 2. 40 : the variable with highest degree of performance necessity was the teaching & consultation, establishment of performance plan, whereas the on with the lowest degree was directive nursing services. The significant difference was found in performance necessity according to working career of visiting nursing services(p<0.05). 6. A positive correlation was found between job satisfaction and performance level(r=.3731, P<0.001). Also, a weak positive correlation was found between the components of job satisfaction and performance level. 7. Stepwise multiple regression analysis revealed that the most powerful predictor was the variance of job satisfaction(R=.3557, $R^2$=.1265). Structure type of visiting nursing services and working career of visiting nursing services accounted for 19.0% of the variance in performance level in nurses. In conclusion, Job satisfaction, Structure type of visiting nursing services and Working career of visiting nursing services variables had influenced on performance level in health centers. Further research is required to confirm these findings.

A blockchain is an underlying technology and basic infrastructure of the Bitcoin system. At present, blockchains and their applications are developing rapidly. However, the basic research of blockchain technology is still in the early stages. The efficiency and reliability of blockchain communication is one of the research problems that urgently need to be studied and addressed. Existing algorithms may be less feasible for blockchain-based communication because they only consider a single communication factor (node communication capability or node trust degree) and only focus on a single communication performance parameter(communication time or communication reliability). In this paper, to shorten the validation time of blockchain transactions and improve the reliability of blockchain-based communication, we first establish a multi-link concurrent communication model based on trust degree, and then we propose a novel integrated factor communication tree algorithm (IFT). This algorithm comprehensively considers the node communication link number and the node trust degree and selects several nodes with powerful communication capacity and high trust as the communication sources to improve the concurrency and communication efficiency. Simulation results indicate that the IFT algorithm outperforms existing algorithms. A blockchain communication routing scheme based on the IFT algorithm can increase communication efficiency by ensuring communication reliability.
