Self-Efficacy as a Predictor of Self-Care in Persons with Diabetes Mellitus: Meta-Analysis

  • Diabetes mellitus, a universal and prevalent chronic disease, is projected to be one of the most formidable worldwide health problems in the 21st century. For those living with diabetes, there is a need for self-care skills to manage a complex medical regimen. Self-efficacy which refers to one's belief in his/her capability to monitor and perform the daily activities required to manage diabetes has be found to be related to self-care. The concept of self-efficacy comes from social cognitive theory which maintains that cognitive mechanism mediate the performance of behavior. The literature cites several research studies which show a strong relationship between self-efficacy and self-care behavior. Meta-analysis is a technique that enables systematic review and quantitative integration of the results from multiple primary studies that are relevant to a particular research question. Therefore, this study was done using meta-analysis to quantitatively integrate the results of independent research studies to obtain numerical estimates of the overall effect of a self-efficacy with diabetic patient on self-care behaviors. The research proceeded in three stages : 1) literature search and retrieval of studies in which self-efficacy was related to self-care, 2) coding, and 3) calculation of mean effect size and data analysis. Seventeen studies which met the research criteria included study population of adults with diabetes, measures of self-care and measures of self-efficacy as a predictive variable. Computation of effect size was done on DSTAT which is a statistical computer program specifically designed for meta-analysis. To determine the effect of self-efficacy on self-care practice homogeneity tests were conducted. Pooled effect size estimates, to determine the best subvariable for composite variables, metabolic control variables and component of self-efficacy and self-care, indicated that the effect of self-efficacy composite on self-care composite was moderate to large. The weighted mean effect size of self-efficacy composite and self-care composite were +.76 and the confidence interval was from +.66 to +.86 with the number of subjects being 1,545. The total for this meta-analysis result showed that the weighted mean effect sizes ranged from +.70 to +1.81 which indicates a large effect. But since reliabilities of the instruments in the primary studies were low or not stated, caution must be applied in unconditionally accepting the results from these effect sizes. Meta-analysis is a useful took for clarifying the status of knowledge development and guiding decision making about future research and this study confirmed that there is a relationship between self-efficacy and self-care in patients with diabetes. It, thus, provides support for nurses to promote self-efficacy in their patients. While most of the studies included in this meta-analysis used social cognitive theory as a framework for the study, some studies use Fishbein & Ajzen's attitude model as a model for active self-care. Future research is needed to more fully define the concept of self-care and to determine what it is that makes patients feel competent in their self-care activities. The results of this study showed that self-efficacy can promote self-care. Future research is needed with experimental design to determine nursing interventions that will increase self-efficacy.

과거성공이 성과에 미치는 영향 : 자아효능감, 소진, 몰입의 매개역할을 중심으로 (The Effects of Past Success on Performance: The Mediating Role of Self-Efficacy, Burnout, and Engagement)

  • 본 연구는 과거성공이 성과에 미치는 긍정적 영향과정에서 자아효능감이 매개역할을 하는지를 파악하고 자아효능감이 소진을 감소시키고 몰입을 증가시킴으로써 성과에 매개적 영향을 미치는지를 확인하려고 하였다. 이론적 연구들을 토대로 매개역할에 대한 가설을 세우고 438명의 학생들을 상대로 설문지와 성적을 근거로 얻어진 자료를 토대로 독립변수와 종속변수간의 이론적인 구조방정식의 제안모형을 만들어 대안모형과 비교함으로써 가설을 검증하였다. 본 연구를 위해 구조방정식 이외에도 Sobel test와 경로계수 비교를 위한 Equality test, 쌍대모수비교법 등이 동원되었다. 그 결과 자아효능감이 과거성공과 소진/몰입 사이에서 매개역할을 한다는 사실과 동시에 자아효능감도 성과에 직접적 영향뿐만 아니라 소진과 몰입을 통한 경로를 거친다는 사실도 입증되었다. 특히 자아효능감은 소진의 감소보다는 몰입의 증가라는 긍정적 측면의 역할이 더 강하다는 것도 입증하였다. 동시에 본 연구결과는 그간 부분적으로 연구되어 온 변수간의 상호관계를 하나의 연결모델로 도출했다는 점과 직무요구-자원(JD-R)모형과 자원보존(COR)모형에서 직무자원의 종업원 well-being에 대한 영향 중 부정적 소진의 감소보다 긍정적 몰입의 증가에 의한 영향이 더 크다는 긍정심리학의 주장을 재 입증하는데 공헌하였다.

융·복합 시대의 자기효능감이 목표설정이론에 미치는 영향에 관한 연구 - 자아조절자원의 고갈과 비고갈 집단을 중심으로 - (A Study on the influence of Self-Efficacy to Goal Setting in the age of Convergence - Focusing on the Self-regulatory depletion and non-depletion group)

  • 본 연구는 개인이 구체적인 목표를 달성해 가는 과정에서 자기효능감이 미치는 영향을 알아보고, 나아가 목표를 달성해 나가는 과정에서 개인이 자아를 통제하는 힘인 자아조절자원이 어떠한 역할을 하는지 알아보고자 하였다. 구체적으로 자기효능감과 목표설정이론 간의 관계를 자아조절자원을 조건으로 구분하여 분석하고자 한다. 이를 위해 대학생들을 대상으로 자아조절자원의 고갈 및 비고갈 집단을 조작하여, 자기효능감과 목표설정에 따른 성과를 측정하였다. 분석 결과, 자기효능감과 성과에 대한 목표설정의 매개효과가 확인되었으며, 자기효능감이 높을수록 자아조절자원이 강화되는 것을 일부 확인하였다. 따라서 자기효능감의 증대는 자아조절자원의 강화를 통해 꾸준히 높은 성과를 올릴 수 있는 토대가 된다는 사실을 확인할 수 있다. 이를 통해 기업에서는 종업원의 장기적인 고성과를 원하는 경우에 참고할만한 가이드라인을 제공할 수 있을 것으로 판단된다.

아동의 자기-효능감 발달에 대한 이론적 고찰 (Theoretical Review for The Development of Self-efficacy in Children)

  • This article introduces the conceptual framework of self-efficacy and reviewes recent research articles which are related to development of self-efficacy in children. Self-efficacy means personal judgement about one's capability to organize his behavior in a specific situation. Perceived efficacy can have diverse effects on behavior, thought patterns and affective arousal. People acquire their efficacy information from their performance accomplishments, vicarious experiences, verbal persuasion and their physiological states. In this paper, I argue that two factors, psychological and environmental factor, contribute to the development of children's self-efficacy. Specifically reward, attributional feedback, social comparision and goal setting are classified to psychological factor and home environment, school environment and social environment belong to environmental factor. Since not many studies have been done in this area, in this paper I also suggest some directions for future research.

웹기반 한국형 중증도 분류 체계 학습프로그램이 응급실간호사의 중증도 분류에 대한 자기효능감 및 수행능력에 미치는 효과 (Effects of a Web-Based Korean Triage and Acuity Scale Learning Program on Triage Self-Efficacy and Triage Performance Ability for Nurses in Emergency Department)

  • Purpose: The Korean Triage and Acuity Scale (KTAS) is a tool used to classify the severity and urgency of emergency department (ED) patients, focusing on their symptoms. In consideration of the importance of the KTAS, a web-based learning program has emerged as a new mode of education; it enables ED triage nurses to access it anytime and anywhere, and according to their own learning abilities. This study aimed to develop a web-based KTAS learning program and evaluate its effects on self-efficacy and triage performance ability in ED nurses. Methods: A quasi-experimental design with a non-equivalent control group pretest-posttest was used. The conceptual framework was Bandura's self-efficacy theory. There were 30 participants in the experimental group and 29 in the control group. The experimental group attended an orientation and 4 sessions of a web-based KTAS learning program. The learning program lasted 280 minutes over five weeks, consisting of 40 minutes of orientation and four 60-minute sessions. Results: The scores of self-efficacy, triage performance ability in KTAS level, and chief complaints significantly increased in the experimental group compared to the control group. In addition, the numbers of under-triage in KTAS significantly decreased in the experimental group in comparison to the control group. Conclusion: The results suggest that the learning program was effective in improving ED nurses' level of self-efficacy and triage performance ability (KTAS level and KTAS chief complaint). Accordingly, the web-based KTAS learning program can be applied as an education intervention to improve ED nurses' triage skill.

Institutions and Women Entrepreneurship: The Mediating Role of Women Entrepreneurial Self Efficacy and Ethical Decision Making

  • Women entrepreneurs play a vital role in employment creation, economic development, and growth. Women entrepreneurship is deep-rooted in the social and cultural norms and values of society. Women's entrepreneurship contribution is still invisible and needs to be properly investigated. The current research study explores "how institutions affect women's entrepreneurial performance in Pakistan" by using institutional and social cognitive theories. Focusing on the Formal and informal institutions, this research examines how institutions are affecting women's entrepreneurial performance by taking the mediating role of women's entrepreneurial self-efficacy and ethical decision making. A 7-point Likert scale research questionnaire is used to collect primary data. Data on active entrepreneurs are collected from the Peshawar, Mardan, and Abbottabad divisions of KPK's Women Chambers of Commerce. The data is empirically tested through the path analysis technique of structural equation modeling (SEM) through SMART PLS 3. The results indicated that women's entrepreneurial self-efficacy and ethical decision-making strongly mediate both institutions and significantly affect women's entrepreneurial performance. The study suggests that government and concerned departments should pay due attention to determinants like informal institutions and social constraints to boost women's entrepreneurial performance.

재가 뇌졸중 노인환자에서 자기효능증진 전략을 이용한 자조관리프로그램의 효과 (The Effects of Community-Based Self-Help Management Program by Strengthening Self-Efficacy of Post Stroke Elderly Patients)

  • The purpose of this study was to determine the effects of self-help management program which strengthens self-efficacy for post stroke elders who attend day care center in Chongju. One group pretest-posttest design was used, and the subjects were 7 post stroke elders with hemiparesis. The self-help management program consisted of 5 sessions and each session included group education on stroke, ROM exercise, recreation and self-care strategies. Those strategies for encouraging and reinforcing self-efficacy provided verbal persuasion, performance accomplishment and vicarious experience. All subjects were questioned before and after the program about ADLs, IADLs, self-efficacy, self-care behavior and depression. The analysis of data revealed that the program was effective to increase the IADLs, self-efficacy and self-care behavior. However, there was no significant difference between pre and post ADLs and depression. The results indicated that this program may be adopted to improve the physical and psychological function of post stroke elderly patients in community settings.

The Effects of E-Brochure Functions and Attitudes to E-Brochures on Self-Efficacy and Salespeople Job Satisfaction in Pharmaceutical Companies

  • Today, companies are making efforts to improve the performance of workers by utilizing various IT-based mobile and internet devices. In pharmaceutical companies, salespeople are using the e-brochure to search for diverse expertise in real time. Through the e-brochure, pharmacists and doctors can be provided with the information they need, thereby increasing confidence in pharmaceuticals and salespeople. Salespeople can also use e-brochures to improve their work performance and to be more satisfied with their jobs. This study examines which functions of e-brochures satisfy salespeople and what attitudes to the e-brochures they need to have. This paper explains the effect of satisfaction and attitude to the e-brochures on job satisfaction through self-efficacy with statistical analysis. As a statistical result, the functions of e-brochures (professional knowledge, massive amount of data, easy searching, information updates, and the reflection of feedback) and attitudes to the e-brochures (importance, intention to use, belief in improvement, efficacy to use, and negative cognition) influence on self-efficacy of salespeople. Further, self-efficacy has mediating effects on the relationship between the functions of e-brochures / attitudes to e-brochures and job satisfaction. Exceptionally, the mediating effect of self-efficacy was not significant in relation to information updates / reflection of feedback and job satisfaction. These results will explain what functions should be focused for the future development of e-brochures. It will also suggest what attitudes the salespeople should have about e-brochures. Through these efforts, salespeople will be able to utilize new technology of e-brochures to satisfy their jobs and improve their performance.

호스피스완화 간호사의 섬망 관련 지식, 자기효능감 및 간호수행도의 관계 (Hospice-Palliative Care Nurses' Knowledge of Delirium, Self-Efficacy and Nursing Performance on Delirium)

  • 목적: 호스피스완화 간호사의 섬망에 대한 지식, 자기효능감 및 간호수행도 간의 관계를 파악하기 위함이다. 방법: 전국 19개 호스피스 완화의료 센터에 근무하는 간호사 174명을 대상으로 구조화된 설문지를 이용하였다. 결과: 첫째, 호스피스완화 간호사의 섬망에 대한 지식 정도는 45점 만점 중 평균 32.83점으로 73%의 정답률을 보였다. 섬망에 대한 지식은 4년제 대학 졸업 이하 군, 종교가 없는 군, 호스피스 경력이 59개월 미만 군에서 유의하게 낮았다. 둘째, 호스피스완화 간호사의 자기효능감 정도는 10점 만점 중 평균 7.08점이었다. 하위영역에서는 간호중재에 대한 자기효능감이 7.36점으로 가장 높았고, 간호사정에 대한 자기효능감이 6.79점으로 가장 낮았다. 자기효능감은 40세 이하, 미혼, 4년제 대학 졸업 군, 종교가 없는 군, 일반간호사 군, 임상경력이 120개월 미만 군과 호스피스 경력이 59개월 미만 군, 가족의 생애말기 돌봄 경험이 없는 군에서 유의하게 낮았다. 셋째, 호스피스 완화 간호사의 섬망에 대한 간호수행도는 4점 만점에 평균 2.95점이었다. 간호수행도는 23~29세, 미혼, 4년제 대학 졸업 군, 종교가 없는 군, 임상경력이 59개월 미만 군, 호스피스 경력이 낮은 군, 가족의 생애말기 돌봄 경험이 없는 군에서 유의하게 낮았다. 넷째, 섬망에 대한 간호수행도는 지식(r=0.28) 및 자기효능감(r=0.51)과 양의 상관관계를 보였다. 또한 지식은 자기효능감과 양의 상관관계를 나타내었다(r=0.24). 결론: 호스피스완화 간호사의 섬망에 대한 지식과 자기효능감, 간호수행도는 비교적 양호한 편으로 나타났지만 4년제 대학 졸업 이하군과 종교가 없는 군, 호스피스 경력이 5년 미만 군에서는 낮았다. 따라서 호스피스완화 간호에서 섬망 간호의 질적 향상을 위해서는 간호사의 지식 정도 및 요구를 선택적으로 반영한 양질의 섬망 교육프로그램을 개발하고 적용하여야 하며 섬망 간호 지식 및 수행도가 낮은 집단에 대한 중재전략 개발을 위한 연구가 필요할 것으로 사료된다.

의학전문대학원생의 스트레스, 자아탄력성, 자기효능감과 임상수행능력과의 관계 (Relation amang Stress, Ego-Resilience, Self-Efficacy, Clinical performance in graduate medical student)

  • 최근 의학교육은 의과대학에서 의학전문대학원으로의 체제변화와 의사국가고시 실기시험 도입에 따른 변화에 대처해야하는 과제를 안고 있다. 따라서 의학전문대학원 학생들의 특성을 고려하고 국가고시 실기시험을 대비한 임상수행능력향상을 위한 교육이 필요하다. 본 연구는 의대생들의 스트레스, 자아탄력성, 자기효능감에 따른 임상수행능력과의 관계를 알아보기 위한 서술적 상관관계 연구로 D시에 소재한 일 의학전문대학원생 97명을 대상으로 2011년 8월19부터 20일까지 설문조사를 실시하였다. 연구결과는 스트레스는 자아탄력성과는 부적상관관계를 나타냈었고 자기 효능감의 하부영역인 자신감과는 정적상관관계를 보였다. 자아탄력성은 임상실기수행능력의 하부영역인 CPX와는 정적상관관계를 나타내었다. 자기효능감의 하부영역인 과제난이도 선호는 OSCE와 부적상관관계를 나타내었고, 임상실기수행능력의 하부영역인 OSCE와 CPX는 정적상관관계를 나타내었다. 따라서 학생들의 스트레스 감소는 자아탄력성을 향상시킬 수 있으며 궁극적으로 임상실기수행능력을 향상시킬 수 있다고 본다. 이상의 결과로 볼 때, 임상실기수행능력 향상을 위해서는 학생 개인의 특성을 파악하여 자아탄력성 향상을 위한 교육프로그램을 개발하고 교육과정에 반영하여야 할 것이다.
