성인의 구강건강인식과 주관적 구강증상과의 관련성 (Association between perceived oral health and perceived oral symptoms among adults in Daegu)

  • Objectives : This study was to evaluate the association between perceived oral health and perceived oral symptoms among adults in Daegu. Methods : All 437 subjects aged 18 or more selected convenience sampling were surveyed cross-sectionally via the self-administrated questionnaire. The questionnaire was measured perceived oral symptoms and perceived oral health, and also obtained socio-demographic characteristics, oral health behaviors. To assess the crude associations, bivariate analysis were applied. For the adjusted association between perceived oral health and perceived oral symptoms, multivariate linear regression multiple regression analysis was conducted. Results : 33.2% of the adults rated their perceived oral health was good, and 30.9% as poor. Older age, low education, had peridontal disease was negatively perceived their oral health(p<0.05). As oral symptoms were more frequently perceived, the perceived oral health were negative. Among the factors of perceived oral symptoms, trouble biting/chewing, poor periodontal status, trouble of appearance of teeth were positively associated with the perceived oral health after adjusting for socio-demographic characteristics, oral health behaviors in the regression model. Age, education, income, recent dental treatment, and all perceived oral symptoms showed the highest impact of association with perceived oral health in the baseline-category logit model. Conclusions : Perceived oral health are significantly associated with perceived oral symptoms among adults in Daegu. The findings of this study will be helpful to design plans of oral health promotion in welfare institutions to increase the oral health related quality of life among the adults.

초등학생의 구강건강증진행위에 영향을 미치는 요인 (The Factors to Affect the Oral Health Promotion Behavior of Elementary School Students)

  • This study set out to identify the factors to affect the oral health promotion behavior of elementary school students and to provide a framework to develop educational programs to promote their oral health promotion behavior. A survey was conducted to 729 fifth and sixth graders attending four elementary schools in Seoul. The variables were measured with a five-point Likert scale and include previous oral health related behaviors, perceived oral health status, perceived benefit, perceived barriers, self-esteem, self-efficacy, and oral health promotion behavior. First, the subjects scored relatively high 3.51 points out of 5 in oral health promotion behavior. They also scored 3.88 points in perceived benefit, 3.51 in selfesteem, 3.43 in self-efficacy, 3.28 in perceived oral health status, 2.77 in previous oral health related behaviors, and 1.79 in perceived barriers. Second, a significant difference was observed according to gender in previous oral health related behaviors and oral health promotion behavior. And a significant difference was also found according to grade in previous oral health related behaviors, perceived benefit, perceived barriers, selfesteem, self-efficacy, and oral health promotion behavior. Third, when they had an experience of visiting a dental clinic for preventive purposes, a significant difference was found according to the purposes of going to a dentist in previous oral health related behaviors, perceived benefit, and oral health promotion behavior. And fourth, multiple regression analysis was carried out with oral health promotion behavior as a dependent variable. As a result, all the research variables, which include previous oral health related behaviors, perceived oral health status, perceived benefit, perceived barriers, self-esteem, and self-efficacy, turned out to have significant influences on oral health promotion behavior. And their explanatory power was 49%. Conclusion: Those factors that were identified to affect the oral health promotion behavior of programs to promote their oral health.

노인의 삶의 질 향상을 위한 주관적 구강건강에 영향을 미치는 요인 (Influencing factors for perceived oral health for the improvement of quality of life in Korean elderly)

  • Objectives: The purpose of the study was to investigate the influencing factors of the perceived oral health for improvement of quality of life in Korean elderly. Methods: The subjects were 1,289 elderly over 65 years old from the sixth National Health and Nutrition Examination Survey 2013. The dependent variable was subjective oral health status. The independent variable was sociodemographic characteristics. Results: The perceived oral health of the unemployed elderly and those having chewing problems were 1.65-fold(95% CI=1.12=2.44) and 3.45-fold(95% CI=2.37-5.02), respectively than employed and chewable elderly. The perceived oral health of the former was 2.49-fold worse(95% CI=1.73-3.60) than the latter. Conclusions: The influencing factors of perceived oral health status included occupation, perceived health status and chewing problems. To improve the oral health-related quality of life in the elderly, continuous education and hands-on programs should be provided for the elderly in the long term care.

강원도 일부 초등학생의 자가구강건강인식과 구강건강상태의 관련성 (Association between Perceived Oral Health and Oral Health Status of Elementary School Students in Gangwon Province, South Korea)

  • Objectives: The purpose of this study was to clarify the relationship between perceived oral health and oral health status, oral health symptoms in elementary school students. Methods: Oral examination and oral hygiene survey were performed on 446 students in 5th grade from 4 elementary schools in Wonju, Gangwon province. Oral examination assessed decayed teeth, filled teeth, missing teeth from decay. Then, oral hygiene status was evaluated by O'Leary index and self-reported questionnaires were surveyed. We analysed the collected data using SPSS program ver. 20.0, significant difference level was p<0.05. Results: 38.4% of the subjects rated their oral health as poor, 61.6% as good, female students was worried about their appearances than male students. Their decayed teeth was 1.01, missed teeth was 0.01, filled teeth was 1.75, O'Leary index was 64.49, oral hygiene status of them was poor. Perceived oral health was associated to decayed teeth, DMFT, oral health symptoms except tooth fracture and pain of mucosa (p<0.05). However, there is no correlation between perceived oral health and oral hygiene status (p>0.05) Conclusion: Self-rated oral health status of the elementary school students was related to decayed teeth and oral health symptoms, but it was not related to oral hygiene status. Therefore it was needed to improve the oral hygiene status in elementary students by devise effective way to motivate them.

노인의 구강자각증상에 따른 구강건강행태 (Oral health behavior according to perceived oral symptoms in the elderly)

  • Objectives: The purpose of the study was to investigate the oral health behavior according to perceived oral symptoms in the elderly. Methods: The data were obtained from the 6th Korean National Health and Nutrition Examination Survey (2013). Data were analyzed by complex sample frequency t test, one-way ANOVA, ${\chi}^2$ test, and general analysis. The questionnaire consisted of general characteristics of the subjects and perceived oral symptoms. Results: Those who had higher education tended to brush teeth regularly. The use of oral hygiene product was also high in higher educated elderly and higher income. Those with good oral health perception showed regular toothbrushing habit. Those with good chewing and mastication used oral hygiene products. Male tended to have regular dental checkup. Those having spouse, higher income, younger age, regular dental checkup had good chewing ability. Poor oral health perception, toothache, and chewing difficulty were the main cause of dental visit (p<0.05). Conclusions: The elderly with poor perceived oral symptoms showed lower level of oral hygiene care. They tended to visit dental clinics only when they had oral symptoms.

사회경제적 상태에 따른 주관적 구강건강과 구강건강행태 기여요인에 관한 융합연구 (Socioeconomic Status in Perceived Oral Health and Contribution of Oral Health Behavioral Factors on Convergence)

  • 본 연구는 주관적 구강건강상태와 구강건강행태 요인들의 기여도에 대한 사회경제적 변화를 조사하였다. 연구자료는 2015년 국민건강영양조사 자료를 이용하였으며, 이분형 로지스틱 회귀분석을 이용하였다. 연구결과 주관적 구강건강상태는 사회경제적 수준이 낮은 경우에서 낮게 나타났다. 이러한 연관성은 인구사회학적 요인들과 구강건강행태를 보정했을 때 연관성은 증가하였다. 또한 각각의 구강 건강 행태요인들을 보정했을 때, 흡연, 칫솔질횟수, 구강위생용품사용, 그리고 구강검진은 사회경제적 구강건강상태의 많은 부분을 설명하였다. 주관적 구강불건강은 사회경제적 상태에서 비롯되지만 이런 차이는 구강건강행태 요인들에 따라 증가되었다. 따라서, 본 연구결과에서 주관적 구강건강상태에서의 사회경제적 불평등은 구강건강증진 행태에 따라 더욱 효과적으로 교정될 수 있음을 시사한다고 볼 수 있다.

성인의 본인 인지 구강건강상태와 객관적 구강건강상태의 연관성 (Correlation of Self-Perceived Oral Health Status and Objective Oral Health Status of Adults)

  • 본 연구는 성인을 대상으로 전문가가 평가한 구강건강지표와 본인 인지 구강건강상태가 상호 관련이 있는지 분석하고 구강보건사업에서 본인 인지 구강건강상태를 반영하는 것이 유효한 지 알아보기 위하여 국민건강영양조사 제 6기 2차년도(2014) 원시자료를 분석하여 수행하였다. 전체 대상자 중에서 만 19세 이상의 성인을 연구대상으로 하여 다음과 같은 결과를 얻었다. 객관적 구강건강지표인 DMFT Index에 따른 각 연령군의 본인 인지 구강건강상태는 모든 연령군에서 통계적으로 유의한 차이가 나타났다(p<0.001). 모든 연령군에서 DMFT Index가 높을수록 본인 인지 구강건강상태가 나쁘다고 평가하였다. 각 연령군별 본인 인지 구강건강 상태에 영향을 미치는 요인에서 청년층의 경우 우식경험영구치지수, 1년간 구강검진 여부, 씹기 문제, 교육 수준이 통계적으로 유의하였고(p<0.005), 장년층에서는 우식경험영구치지수, 씹기 문제에서 통계적으로 유의하였다(p<0.005). 노년층에서는 우식경험영구치지수, 1년간 구강검진 여부, 씹기 문제, 말하기 문제에서 통계적으로 유의하게 나타났다(p<0.005). 이상의 연구결과를 토대로 전문가가 평가한 구강건강지표와 본인 인지 구강건강상태가 상호관련이 있음을 확인하였다. 이는 본인 인지 구강건강상태가 '건강하다' 라고 인지하면 객관적 구강건강지표도 높게 될 가능성이 있으므로, 삶의 질 향상을 위한 구강보건사업에서 주관적 구강건강을 반영해야 할 것으로 사료된다.

노인의 구강위생행태와 주관적 구강건강 형태에 대한 분석 (Oral hygiene form and the subjective health analysis of the elderly)

  • Purpose: This study was conducted to prepare the fundamental data on oral hygiene of the elderly and to assess the behavior of oral hygiene, subjective oral health, oral health problem among the elderly, and to contribute to successful enhancement of life in their declining years. Methods: The author surveyed the behavior of oral hygiene, subjective oral health, oral health problem to 192 elderly at a health center of Busan using structured self- administered questionnaire from Nov. 2nd 2009 to Feb. 10th 2010. Collected data were analysed by SPSS WIN 18.0 statistical program for frequency, percentage production, ${\chi}^2$ (Chi-square) verification, t-test, One-way ANOVA, Logistic Regression. Results: The distribution of total subjects by oral health behaviors were 56.8% for "have ever trained how to tooth brushing", 76.6% for "correctly performed tooth brushing by himself ", 71.8% for "tooth brushing a day is two times and less", 87.5% for "it doesn't tooth brushing in meals and after" and 53.6% for "it doesn't use the facility for oral health", respectively. The distribution of total subjects by the state of perceived oral health were 51.0% for "feel the oral discomfort", 51.6% for "feel the comfortable chewing on starchy food", respectively. The mean point of perceived oral health by general characteristics of total subjects were statistically significant difference in sex, type of medical insurance, smoke, exercise, and chronic disease. The mean point of perceived oral health by oral health behaviors of total subjects were statistically significant difference in educational experience on tooth brushing, recognition for tooth brushing by himself, tooth brushing a day, and tongue cleaning. The risk factors of perceived oral health were type of medical insurance and chronic disease in general characteristics, tooth brushing in meals and after and tooth brushing a day, and tongue cleaning in oral health behaviors. Conclusion: The author recommend to the prevention of chronic disease, carry out the tooth brushing in meals and after, three times and over tooth brushing a day and tongue cleaning for oral health. and the author consider that it need to prepare the active countermeasure to oral health such as reeducation for oral health and supply to the tongue cleaner.

Comparison of Factors Affecting Perceived and Objective Dental Needs

  • Background: With increased interest in oral health, several efforts have been made to improve oral health conditions. To achieve this, needs for oral health must be precisely determined and accurately measured. Therefore, factors influencing both objective unmet dental needs, which were determined by experts, and perceived unmet dental needs, which were determined by patients, were examined in this study. Methods: Responses of 17,735 respondents aged greater than 19 years from the Korean National Health and Nutrition Survey collected using the fifth (2010~2012) rotation sample survey were analyzed. Based on the information collected from the survey and dental examination, we determined the associations between the independent (sex and socioeconomic level) and dependent variables using a chi-squared test. Moreover, ordinal logistic regression analyses on multiple categorical values were performed using perceived and objective dental needs as the dependent variables. Results: Generally, factors influencing both perceived and objective dental needs were similar. These included sex, household income, educational level, private insurance, and subjective oral health status. However, the high-income groups had lesser perceived and objective dental needs compared to the low-income groups. Furthermore, factors such as sex, educational level, and marital status had different influence on both needs. Conclusion: Generally, factors that affect perceived and objective dental needs were similar. To minimize unmet dental needs, factors influencing both perceived and objective dental needs should be examined for a broad dental insurance coverage, and efforts to prevent oral diseases are also required.

근로자의 구강건강관련 삶의 질에 영향을 미치는 요인 (Factors Which Affect the Oral Health-Related Quality of Life of Workers)

  • 본 연구는 근로자들의 구강보건 지식, 태도, 행위와 구강 자각증상 및 구강건강관련 삶의 질(OHIP-14)을 조사하고 구강건강관련 삶의 질 향상에 영향을 미치는 요인과 영향관계를 파악함으로써, 근로자 구강건강증진사업을 위한 기초 자료를 제공하고자 하였다. 편의표본추출법을 적용하여 서울, 경기, 인천지역에서 근무하는 근로자 398명을 대상으로 2013년 3월 26일부터 4월 30일까지 자기기입식에 의한 설문조사를 실시하였다. 구강보건지식, 태도, 행위 및 구강자각증상이 구강건강관련 삶의 질의 수준에 미치는 영향력과 이들 변수의 영향관계를 분석하기 위하여 경로분석을 실시한 결과 OHIP-14에 가장 큰 영향을 미치는 요인은 구강자각증상이었고, 구강보건행위는 간접효과가 있었다. 구강자각증상에 영향을 미치는 요인은 구강보건행위였고, 구강보건지식과 태도는 간접적인 영향이 있는 것으로 나타났다. 구강보건행위에 영향을 미치는 요인은 구강보건지식 및 태도였고, 구강보건지식은 태도에 영향을 미쳤다. 이상의 결과를 종합해 볼 때 근로자의 구강건강관련 삶의 질을 향상시키기 위해서는 우선적으로 구강자각증상을 줄이기 위한 방안을 마련해야 하며, 구강건강관련 삶의 질에 대해 간접효과가 확인된 구강보건행위를 향상시키기 위한 노력이 필요하다. 따라서 근로자의 구강자각증상을 줄이기 위한 구강검진 및 진료사업과 구강보건행위 향상을 위한 구강보건교육사업이 포함된 근로자 구강건강증진사업을 개발하여 적용할 필요가 있다고 생각된다.
