  • Early childhood caries prevalence has increased significantly in children ages 2~5 years. ECC disproportionately affects lower socioeconomic and minority groups, is a predictor for future decay, but is preventable and manageable. Caries risk assessment systematically derives a patient's caries risk and is important during an infant oral health visit beginning at age 1. Information obtained through a risk assessment can guide a disease management care path tailored to an individual's age and risk to effectively treat and manage one's caries disease process.

Reasons for primary teeth extraction in children : Retrospective study (유치 발거 원인에 대한 후향적 연구)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

  • Crohn disease is a chronic transmural inflammatory disease that may involve any portion of the gastrointestinal tract. An increased incidence of Crohn disease in the general population has been reported, along with a greater than threefold increase of Crohn disease in children under the age of 16 years noted in a recent study. Crohn disease may be seen as early as infancy, but the most common pediatric age of onset is during the teenage period. We experienced a case of Crohn disease in 6 year old male child complained abdominal pain, oral aphthous ulcers, arthralgia, anorexia, and growth failure. A brief review of related literature is also presented.

THE EFFECT OF EARLY CHILDHOOD CARIES ON HEIGHT AND BODY WEIGHT OF CHILDREN (유아기 우식증이 어린이의 신장 및 체중에 미치는 영향)

  • Early childhood caries (ECC) is a comprehensive terminology that includes nursing bottle caries and rampant dental caries occurred in infants and children. In previous studies, ECC was thought to affect body growth of children negatively. The purpose of this study was to evaluate the effect of ECC on body growth of children in respect of their chronologic age and degree of dental caries. Height and body weight were used as means for physical growth measurements. Children, who visited the pediatric department of Yonsei University Dental Hospital, received oral and physical examinations, and they were divided into the control and ECC groups. Then, each group was subdivided according to their age and gender. Two-sample T test was used to compare the mean height and body weight of the control and ECC groups, and Likelihood Ratio Chi-square test was used to compare their growth percentile distribution. When the mean height and weight were compared, there was a common tendency observed even though statistical significance was not found in all cases. Before the age of 3-4, the mean height and weight tended to be greater in the ECC groups compared to the control groups, whereas after the age of 3-4, the mean height and weight of the ECC group tended to be less compared to the control group. In addition, in groups with age equal or greater than 3-4, which presented significant difference in height and body weight, the percentage of children showing less than 3 percentile growth was greater in the ECC group than the control group. These results imply the negative effects of the ECC on physical growth of the infants and children, and its effects on physical growth may present different characteristics according to chronologic age of the patients.

COMPARISON OF THE CHRONOLOGY OF ROOT RESORPTION OF DECIDUOUS TEETH BETWEEN EARLY 1990S AND EARLY 2000S (1990년대 초와 2000년대 초의 유치 치근 흡수의 연령 비교)

  • It is essential to have adequate knowledge of the timing of root resorption of deciduous teeth for diagnosis and treatment planning in pediatric and orthodontic dentistry. Dental development is also influenced by hereditary characteristics, environmental factors, race, sex, endocrine reaction, nutrition, socioeconomic condition and secular factor. The aims of the present study were to determine the mean age of root resorption of deciduous teeth in contemporary Korean children and to compare the mean age of root resorption time of deciduous teeth between early 1990s and early 2000s. The study population was made up of Korean children attending the pediatric dentistry ward of Kyungpook National University Hospital. One thousand thirty seven children's panoramic radiograph (girls: 528 persons, boys: 509 persons) in $1990{\sim}1992$ and one thousand sixty five children's panoramic radiograph (girls: 394 persons, boys: 671 persons) in $2001{\sim}2003$ were examined. This study utilized a cross-sectional design. Due to the problems of imaging in the maxillary region and the mandibular incisor region, the mandibular deciduous canine, the mandibular deciduous first molar and the mandibular deciduous second molar were chosen for examination. The results were as follows. 1. There is a tendency for the teeth to resort earlier in the early 2000s group than the in early 1990s group. At the Res c stage, the difference of the mean age was 0.4 years. 2. At the Res c stage, the order of difference of the mean age from smaller to larger for the girls was the mandibular deciduous canine, the mandibular first deciduous molar, and the mandibular second deciduous molar. On the other hand, for the boys, the order was the mandibular second deciduous molar, the mandibular first deciduous molar, and the mandibular deciduous canine. The difference was larger with aging in girls and smaller with aging in boys. 3. There is a tendency for the teeth to resort earlier in girls in both the early 1990s group and the early 2000s group. The difference of the mean age between girls and boys was 0.3 year. 4. The commencement of root resorption of the mandibular deciduous canine was slower than that of the mandibular first deciduous molar. However, the completion of root resorption of the mandibular deciduous canine is faster than that of the mandibular first deciduous molar. The total elapsed time of root resorption from commencement to completion was shortest in the mandibular deciduous canine. 5. For each of the teeth in the early 1990s and the early 2000s groups, the speed of root resorption was in the later stage faster than in the earlier stage. In order to know about the exact timing of root resorption of deciduous teeth, periodic and longitudinal studies preferably covering the entire period of growth, is required.

  • The purpose of this study was to analyze the parental consultation. Also, it was to investigate the tendency divided into different subjects. The subjects were 2142 questions on korean academy of pediatric dentistry homepage. The questions were categorized into 7 subjects, 37 subdivided subjects and children age. The results were as follows: 1. Age group was divided into $0{\sim}6months$, $7{\sim}12months$, $13{\sim}24months$, $25{\sim}36months$, $3{\sim}6years$, $7{\sim}12years$ 12years and over 13 years. $13{\sim}24months$ and $3{\sim}6years$ age group had the largest number of questions. 2. The subject were growth & development, development disturbance & oral disease, behavior management, operative & endodontic treatment, dentition & occlusal guidance, traumatic injury & surgery and etc. Development disturbance & oral disease had the largest number of questions. 3. In subdivided subjects, operative & endodontic treatment showed the most, followed by development & eruption of teeth, traumatic injury, cross-bite, tooth brushing methods in sequence. 4. In age group within subdivided subjects, the most frequent question was neonatal & natal tooth in $0{\sim}6months$, development & eruption of teeth in $7{\sim}12months$, $13{\sim}24months$, operative & endodontic treatment in $25{\sim}36months$, $3{\sim}6years$, time & method of orthodontics in $7{\sim}12years$ and operative & endodontic treatment in over 13years. 5. The questions about cross-bite, traumatic injury and soft tissue disease were distributed evenly in age group.

AN ANALYSIS OF THE GROWTH PATTERN OF MAXILLARY ALVEOLAR BONE OF THE KOREAN GIRLS (한국인 여학생 상악 치조골의 성장 양상 분석)

  • Longitudinal serial cephalometric films in the every second year from fifty girls were used for the analysis of growth pattern of maxillary alveolar bone (MAE). Six parameters regarding MAB height were measured and were compared with stature and chronological age. Descriptive analysis. correlation coefficient test and linear regression analysis were done for the statistical analyses. 1. The height of MAE increased until 14 years although growth velocity was gradually decreased. which was different with stature growth pattern. 2. The correlation coefficients between chronological age and MAB height were high and had no statistical differences with those of stature and MAB height. In addition. the correlation coefficients between chronological age groups within each parameter of MAB were very high. 3. The growth amount of MAB could be predicted using linear regression analysis on the basis of chronological age data. The growth pattern between MAE and statue was quite different because of the growth pattern difference between skeletal and neuronal system. Therefore. growth pattern of Korean MAB must be considered in dental treatment of youth. Also. close relationship between chronological age and MAB height would be useful when clinicians want to predict growth amount of MAE for the dental implant treatment.

An Observation of the Chief Complaints of Pediatric Outpatients (소아과(小兒科) 외래(外來) 환자(患者)의 주소증(主訴症)에 관(關)한 연구(硏究)(동국분당한방병원 : 1997년 1월 1일 ${\sim}$ 1997년 12월 31일))

  • BACKGROUND : The purpose of this study was to investigate the new effective oriental medical treatments in pediatric diseases and its clinical applicability. METHOD : The study was composed of 1725 new patients who had been treated at the pediatric unit in the Dongguk Bun-Dang Oriental Medicine Hospital for 1 year, from 1 January 1997 to 31 December 1997, and had ages between 0 and 18 years. The chief complaint was mainly categorized by oral examination on patients and their care-givers. RESULT 1. The ratio of male to female was 1.35:1 and the number of each age group and its percentage distribution was 228 patients (13.2%) for the age group between 0 and 1 year, 746 patients (43.2%) between 2 and 6 years, and 751 patients (43.6%) for the age group over 7 years. 2. The chief complaint of the new outpatients was weakness, asthma, common cold, sinustis, skeletal disease, atopic dermatitis, stomach-ache, short stature, diarrhea, nasal blooding, night terror, allergic rhinitis, gastric disorder, enuresis, dermatitis, strabismus in the order of majority.3. The number of patients and its percentage for the most three complaints was 494 patients (28.6%) for weakness, 647 patients (37.4%) for respiratory disease, 144 patients (8.3%) for digestive disease. 4. The digestive disease tended to increase in summer, and appeared mostly in the age group between 0 and 2 years. The respiratory disease increased In inter-season and winter, but decreased significantly in summer. The age group between 1 and 5 years was the largest group of respiratory disease. CONCLUSION 1. The chief complaint in pediatric diseases that needed an oriental medical treatment was mainly the disease that tends to take long time and the weakness, and appeared frequently in digestive and respiratory diseases. 2. The oriental medical treatment was still preferred as a way to improve the weakness by patients, rather than a way to overcome their disease. In particular, the study shows that the oriental medical treatment should be emphasized in terms of preventing the disease. 3. The new diseases, which were developed with the change of human life and environment, (atopic dermatitis, enuresis, short stature, obesity, dysthymic disorder, strabismus), should be investigated as a new field of oriental medical treatment.

Retrospective Study of the Mandibular Condyle Fracture in Children and Young Adolescents (최근 10년간 소아 청소년에서 발생한 하악 과두 골절에 대한 후향적 연구)

  • The aim of this retrospective study was to investigate etiology, clinical features, and treatment modalities of the mandibular condyle fracture in children and young adolescents. This study was conducted based on medical records and radiographic examinations of 44 pediatric patients. Patients received treatment under diagnosis of mandibular condylar fracture. They were divided into 4 groups according to their age. The involvement of condyle in the mandible fracture occurred in higher percentage in younger age groups. Falling was the most common etiologic factor in all age group, especially in children with age under 7. Condyle head was the most frequent site of fracture in age group of 4 - 7 whereas fracture tends to occur in lower condylar areas in older groups. In 54.5% of the mandibular condyle fracture, symphysis fracture was accompanied. Teeth injury occurred in higher incidency when condyle fracture accompanied symphysis fracture. Conservative treatments were applied to 43 out of 44 patients.

CARIES PREVALENCE AND OCCLUSAL VARIATION OF CHILDREN WITH DECIDUOUS DENTITION IN CHONNAM SEASHORE AREA (전남 해안지역 유치열기 아동의 치아우식 및 교합양상에 관한 연구)

  • The purpose of this study was to investigate the dental caries prevalence and occlusion pattern of children with deciduous dentition in Chonnam seashore area. Two thousand two hundred seventy two kindergarten children ranged 3 to 5 years of age were examined. The obtained results were as follows : 1. The rate of children with caries experience(dmf rate) in deciduous teeth was 66.8% at 3 years of age, 77.7% at 4 years of age, and 83.5% at 5 years of age and showed no significant gender differences(p>0.05). 2. The mean number of decayed, missing, filled deciduous teeth(dmft index) was 3.62 at 3 years of age, 4.73 at 4 years of age, and 5.96 at 5 years of age and showed no significant gender differences(p>0.05). 3. Most of the caries-prevailed tooth was mandibular 2nd primary molar and showed 39.48% at 3 years of age, 49.73% at 4 years of age, and 63.85% at 5 years of age. 4. The caries-experienced teeth in order were as follows : mandibular 2nd primary molar with 52.34%, mandibular 1st primary molar with 45.61%, maxillary 2nd primary molar with 38.81%, and maxillary primary central incisor with 38.86%, respectively. 5. In sagittal primary molar relationship, Class 1 occlusion pattern was 53.26%, Class 2 pattern was 1.41% and Class 3 pattern was 30.02%, respectively. Right Class 2 and Left Class 3 pattern was not observed.
