Purpose: This study aimed to clarify the concept of pediatric hospice and palliative care through conceptual analysis. It also sought to identify the differences between related concepts such as pediatric death care and pediatric spiritual care, in order to provide foundational data for the development of nursing theory and knowledge. Methods: A conceptual analysis of pediatric hospice and palliative care was conducted using Rodgers' evolutionary method. Out of 5,013 papers identified, 28 were selected for detailed reading and analysis. Results: Pediatric hospice and palliative care encompasses physical, psychological, social, mental, spiritual, and family care for children with acute and chronic diseases with uncertain prognoses ahead of death, as well as their families. Effective pediatric hospice and palliative care will require multidisciplinary team nursing, effective communication, and supportive policies. Conclusion: The findings of this study suggest that providing pediatric hospice and palliative care will lead to improvements in pain relief for children and families, the efficiency of responses to death in children, and the quality of life for children and families. The significance of this study is that it clearly clarifies the concept by analyzing pediatric hospice and palliative care using an evolutionary method.
Purpose: This study examined the knowledge and perception of hospice-palliative care and terminal care stress among pediatric nurses, and the relationships among these variables. Methods: In this descriptive research study, 154 pediatric nurses who experienced terminal care at least once were surveyed. This study used three scales, including the Palliative Care Quiz for Nursing (PCQN), Perception of Hospice-Palliative Care, and Terminal care stress. Data analyses using SPSS 22.0 included descriptive statistics, independent t-test, one-way ANOVA, Mann-Whitney U test, Pearson's correlation coefficient, and stepwise multiple linear regression. Results: Terminal care stress experienced by the pediatric nurses was significantly related to the perception of hospice-palliative care; the hospice-palliative care education program enhanced the knowledge and perception of hospice-palliative care. Conclusion: Hospice-palliative care education programs should be developed and provided for pediatric nurses to improve pediatric hospice-palliative care. Additionally, further research on this topic is required because the present results are inconsistent with previous and current researches.
Purpose: This phenomenological analysis was conducted to explore the experiences of college student volunteers in the field of pediatric palliative care. Methods: In-depth interviews were conducted with nine study participants who had experience volunteering with children receiving pediatric palliative care. The study analysis was conducted using the Colaizzi method, a rigorous qualitative research method, to develop "intra-group" comparisons among the study participants. Results: Through an analysis of the data, 51 themes, 18 theme clusters, and five main categories were derived. The results confirmed that the experience of university student volunteers in pediatric palliative care can be summarized as "awareness of the role of a volunteer," "difficulties encountered while volunteering," "efforts to overcome the difficulties of volunteer work," "personal reflection," and "awareness of life and death." Conclusion: The results of this study can be used as a practical guide for the effective management of volunteers in the pediatric palliative care setting.
Purpose: We evaluated the status of patients enrolled in South Korea's pediatric palliative care pilot project based on the experience of a single center. This study examined factors related to end-of-life services and differences in medical costs. Methods: The medical records of 120 patients referred by a pediatric palliative care team were analyzed retrospectively. Data from July 1 to February 28, 2022 were collected and analyzed using the chi-square test and the Mann-Whitney U test. Results: Volunteer programs and psychological support (100%), family support and education (99.2%), and financial support through institutional linkage (62.5%) were provided to the participants. In the deceased group, there were no significant differences in general characteristics, which included age, gender, primary disease, religion, duration of hospitalization in an intensive care unit (ICU) and non-intensive care unit (non-ICU). However, the ICU group had fewer opportunities to access individual pain and physical symptom management than the non-ICU group and there were limitations in linking with external resources. Medical expenses were significantly different for the ICU group, with a 3-times higher average cost than the non-ICU group. Conclusion: Although an individualized approach is needed for each patient in pediatric palliative care, psychosocial care is essential. In addition, if early intervention for end-of-life pediatric patients is available from a palliative care team, the cost burden of medical care for patients and their families should be minimal.
목적: 본 연구는 아동병동 간호사들의 아동 호스피스 완화의료에 대한 인식과 지식을 알아보기 위해 수행되었다. 방법: 일개 대학병원의 어린이병원 내과계 병동과 중환자실에서 근무하는 간호사를 대상으로 인식 관련 23문항, 지식 관련 20문항으로 구성된 설문조사를 시행하고 총 132명의 자료를 분석하였다. 결과: 대상자의 아동 호스피스 완화의료에 대한 인식의 하위영역 중 '아동 호스피스 완화의료의 어려움'에 대한 인식이 가장 높았으며 특히 의사소통과 관련된 어려움을 크게 인식하고 있었다. 대상자의 일반적 특성이 아닌 아동 호스피스 완화의료 경험에 따라 인식의 차이를 보였다. 아동 호스피스 완화의료의 지식은 20점 만점에 $8.83{\pm}2.64$점으로 '통증과 증상관리', '철학과 원칙', '돌봄의 정신 사회적 측면'의 순으로 점수가 낮아졌다. 현재 근무지가 병동인 경우가 중환자실에 근무하는 경우보다 지식이 높은 것으로 나타났으며, 아동 호스피스 완화의료와 관련된 경험에 따른 지식의 차이는 보이지 않았다. 결론: 아동병동 간호사의 아동 호스피스 완화의료의 인식과 지식의 향상을 위해 간호사들이 어려움을 느끼는 부분과 부족한 지식을 보완할 수 있는 교육프로그램의 개발하여 아동병동 간호사들에게 지속적으로 제공하는 것이 필요하다.
목적: 본 연구는 소아완화의료 대상 환자와 가족을 돌보는 전문기관 종사자의 환자진료 및 시스템 연계에 대한 인식을 알아보기 위해 수행된 서술적 조사 연구이다. 방법: 자료 수집은 호스피스 완화의료 전문기관(이하 전문기관)에 근로하는 전문가를 대상으로 2017년 9월부터 10월까지 진행되어 총 61건의 자료를 수거하였다. 결과: 소아완화의료를 입원형, 자문형, 가정형 중 한가지 형태로라도 제공하고 있는 기관은 11기관(18.0%)이었으며, 지역별 분포는 서울, 경기, 인천, 경상 지역에 집중되어 있어 기타 지역에서는 활용 가능한 자원이 전무한 상황이다. 개입 시 장애요인은 '훈련된 전문인력을 확보하는 것의 어려움'이며, 비암성 소아청소년 환자의 경우 '예후 및 경과 예측의 어려움 47.5% (n=28)' 으로 확인되었다. 소아완화의료 구성형태는 '성인과 다른 소아청소년의 특성, 소아청소년 전문인력 필요, 관리 및 제도보완 필요'를 이유로 응답자 중 73.8% (n=45)이 별도의 소아완화의료팀 구성이 필요하다는 의사를 밝혔다. 개입시점은 '완치가능성이 적은 소아암 진단 시'부터 진행해야 하는 것이 33.7% (n=33)로 가장 높았으나 의뢰시점은 '사망시점을 예상하기 어려우나 지속적 악화상태인 경우'가 38.2% (n=39)로 가장 높아, 전문기관으로 의뢰 전 기존 치료 병원에서 개입을 진행되는 것을 선호하였다. 응답기관 중 14.8% (n=9)만이 추후 소아완화의료 제공에 참여할 의사가 있다고 밝혔다. 결론: 2018년 두 곳으로 시작한 소아청소년 완화의료 시범사업기관에 더해 지역별로 배치되어 있는 전문기관에서 소아완화의료를 제공할 수 있도록 하여 자원의 접근성을 높일 필요가 있다. 별도 소아완화의료 구성의 필요성을 인정함에도 소아완화의료를 제공하는 것에 대한 부담감으로 실제적 서비스를 제공하는 데 어려움이 있기에 우선적으로 소아완화의료 전문가를 양성하고 교육 개발 및 의뢰 시점에 대한 논의 등을 통하여 소아완화의료 확산에 대한 현실적 논의를 진행해야 할 것이다.
Purpose: This study aimed to describe nurses' perceived needs and barriers to pediatric palliative care (PPC). Methods: Mixed methods with an embedded design were applied. An online survey was conducted for nurses who participated in the End-of-Life Nursing Education Consortium- Pediatric Palliative Care (ELNEC-PPC) train-the-trainer program, of whom 63 responded. Quantitative data were collected with a survey questionnaire developed through the Delphi method. The 47 items for needs and 15 items for barriers to PPC were analyzed with descriptive statistics. Qualitative data were collected through open-ended questions and analyzed with topic modeling techniques. Results: The mean scores of most subdomains of the PPC needs were 3.5 or higher out of 4, and those of PPC barriers ranged from 3.22 to 3.56, indicating the items in the questionnaire developed in this study properly reflect each factor. The needs for PPC were divided into 4 categories: "children and adolescents," "families," "PPC management system," and "community-based PPC." Meanwhile, PPC barriers were divided into 3 categories: "healthcare delivery system," "healthcare provider," and "client." The keywords derived from the topic modeling were perception, palliative, children, and education for necessities and lack, perception, medical care, professional care providers, service, and system for barriers to PPC. Conclusion: In this study, by using mixed-methods, items of nurses' perceived needs and barriers to PPC were identified, categorized, and weighted, and their meanings were explored. For the stable establishment of PPC, the priority should be given to improving perceptions of PPC, establishing an appropriate system, and training professional care providers.
목적: 본 연구는 말기아동 간호에 대한 교육자를 위한 교육프로그램인 ELNEC-PPC에 참여한 간호사의 소아청소년 호스피스완화의료(PPC)에 대한 지식 및 태도를 확인하기 위하여 수행되었다. 방법: 2012년 ELNEC-PPC 교육자를 위한 교육과정에 등록한 간호사 중 연구참여에 동의하고 서명날인 후 설문조사에 응답한 총 196명의 자료를 분석하였으며, 소아청소년 호스피스완화의료에 대한 지식과 태도는 20문항의 7점 척도로 측정하였다. 결과: 학력수준과 소아과병동 및 호스피스 근무경험에 따라 PPC에 대한 지식 정도에 차이가 있었다. 소아과 근무경험, PPC 경력 및 교육경험 이수 정도에 따라 소아 호스피스완화의료 태도에 차이가 있었으며, 기혼 간호사는 자녀의 완화의료서비스 결정에 대한 보호자의 권리에 대해 더 긍정적이었으며, 석사 이상군은 PPC에 대한 지식과 태도 정도가 높았다. 결론: PPC에 대한 간호사의 지식 및 태도를 높이기 위해서는 일반간호사를 대상으로 하여 표준화된 교육이 필요하다.
Purpose: This study aimed to provide basic data for developing interventions to relieve the end-of-life care stress experienced by pediatric nurses by examining the relationships of end-of-life care stress with compassionate competence and attitudes toward end-of-life care. Methods: Data were collected via a survey that was conducted from September 10 to September 30, 2018 and administered to 113 nurses who had worked for more than 6 months in a pediatric unit at a tertiary hospital in Seoul, South Korea. The data were analyzed for frequency, percentage, mean, and standard deviation, and the independent t-test, one-way analysis of variance, and Pearson correlation analysis were conducted using SPSS version 25.0. Results: End-of-life care stress among pediatric nurses had a weak positive correlation (r=0.216, P<0.05) with compassionate competence and had no significant correlation with attitudes toward end-of-life care. Among the sub-factors of end-of-life care stress, psychological difficulties had a weak positive correlation with sensitivity (r=0.309, P<0.01) and communication (r=0.230, P<0.05), which are aspects of compassionate competence. Lack of knowledge about end-of-life care had a weak positive correlation with communication (r=0.209, P<0.05) as an aspect of compassionate competence. Conclusion: To improve the quality of end-of-life care provided by pediatric nurses, it is necessary to improve their compassionate competence and reduce their end-of-life care stress by developing education and support programs tailored to the characteristics of children and specific communication methods.
