Effects of job satisfaction and patients satisfaction on medical profit at public hospitals (공공병원 직원의 직무만족도 및 환자 만족도가 의료수익에 미치는 영향)

  • Purpose: This study was conducted to examine the factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Method: A cross-sectional survey was conducted through staff and patients in 39 hospitals (34 local province hospitals, and 5 red-cross hospitals) from September to October in 2011. A questionnaire was designed to collect information on job satisfaction, inpatients and outpatients satisfaction. The data of medical profit per bed was collected in 2011 annual financial reports from each 39 hospitals. Finally, data from 5,521 staff, 1,730 in-patients and 1,730 out-patients were analyzed. Multiple logistic regression analysis was performed to determine factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Results: The medical profit was positively correlated with job satisfaction of staff. As the results of multiple logistic regression, the significant variables of medical profit per bed, were communication satisfaction of staff(OR=0.48, 95% CI=0.25-0.91), hospital environment satisfaction of in-patients(OR=1.53, 95% CI=1.05-2.22). Conclusion: The satisfaction of patient and staff were significantly effect to medical profit in public hospitals. The Government and administrators have make an effort to improve satisfaction of staff and patients in public hospitals.

Hospital Selection Factors and Patient Satisfaction of Refractive Surgery Patients (시력교정수술환자의 병원선택요인과 만족도)

  • The study compared refractive surgery patients' hospital selection factors and satisfaction, through this to see the factors influencing patients' satisfaction, aiming at 235 patients visiting two Eye hospitals. The research results are followed. First, target group's hospital selection factors appeared facilities and environment factor, reliability factor, personal service factor, and accessibility factor in the order. Second, patients' satisfaction appeared reliability factor, facilities and environment factor, and personal factor in the order. Fourth, in the result comparing patients' expectation on hospital selection and satisfaction score, all factors of facilities and environment, personal service, and reliability had high satisfaction score than expectation. Third, in the result of factors influencing refractive surgery patients' satisfaction, in surgery determination period, surgery expenditure, and hospital selection factors among characteristics relating to refractive surgery, facilities and environment, personal service, and reliability appeared as satisfaction influencing factors. In conclusion, the study found that hospital selection factors and satisfaction influencing factors by refractive surgery patients' surgery type are considered to be able to be used to establish marketing strategy in the fields excluded from insurance application, the researches to improve patients' satisfaction in patient-oriented medical market should be continuously proceeded.

The Study of Satisfaction of Voice Therapy in Patients with Voice Disorders (음성장애환자의 음성치료 후 만족도 연구)

  • Background and Objectives : Voice therapy is proven to be effective, but patients are reluctant to receive the therapy when physicians recommend it. This phenomenon may due to patients'lack of trust in or low satisfaction level of the therapy. This study aimed to evaluate patients'satisfaction level after voice therapy and identify factors that could increase the satisfaction. Materials and Methods : First, the fraction of patients who were recommended voice therapy ahead and completed it was studied, and survey was conducted on those who finished ten voice therapy sessions. The patients'1) satisfaction level during the therapy, 2) satisfaction level about physician's explanation about the therapy, 3) willingness to recommend, 4) satisfaction level about the results were assessed, and was correlated with overall satisfaction level. In each category, patients' gender-, age-, and disease type-related differences were analyzed. Results : Patients under 19 years old were most satisfied during the voice therapy; patients above 40 years old showed statistically significantly higher satisfaction level regarding satisfaction with physician explanation about treatments and with treatment results compared to other ages groups. Patients above 40 also showed the highest willingness to recommend. 26.5% of patients either refused to or discontinued voice therapy. 84.3% were satisfied with treatment results. Considering variabilities among factors, patients'satisfaction with physician explanation about treatment and willingness to recommend had positive correlation. Conclusion : Though 26.3% of patients received voice therapy when physicians recommended, patients who completed the therapy were overall satisfied.

Patients Satisfaction with Nursing Care in the Emergency Department (응급실 이용현황과 환자의 간호만족도 조사연구)

  • The purpose of this study was to describe patients satisfaction with nursing care in the emergency department and factors influencing satisfaction rates. 1. The mean score of patient satisfaction with nursing care in the emergency departments was 2.70. The mean scores of patient satisfaction with different aspects of nursing care services were 3.05 for nursing services for psychological safety: 2.67 for technical competence: 2.49 for information giving: and 1.35 for discharge teaching. 2. Patient satisfaction with nursing care services provided. according to the general characteristics of the patients. was revealed as being significantly high for the groups of patients made up of those who were male. those over 51years of age. married subjects. those with an education level of high school or below level. housewives. Buddhists. and those in the middle economic level. Patient satisfaction with nursing care according to their service utilization in the emergency departments revealed that satisfaction was significantly high for patients who were in internal medicine. neurology or psychiatry. those admitted for the first time. those for whom the distance from the department to home was less than 30 minutes. patients who visited between monday and Friday and between 8AM and 4PM. patients who did not have to wait to be seen and those who stayed in the department less than 24 hours. 3. The correlation between the patients' general characteristics. nursing service utilization of the patients in the department and their satisfaction with nursing care showed that patients with longer waiting time had lower satisfactory scores for nursing service. 4. The registered nurses working in the emergency department reported that the main reasons for low satisfaction rates included 'over worked nurses' and 'too many patients' in the emergency department. In conclusion. the results of this study suggest the necessity of developing relevant nursing interventions for discharge teaching to increase patient satisfaction with nursing care services in emergency departments: and the need for adequate support from hospital administrators to improve patients' service utilization.

The Effects of the Inpatients' Stress and Privacy for the Satisfaction with Patients' Room (입원환자의 프라이버시와 스트레스가 병실 만족도에 미치는 영향)

  • This study proposes the structural model of inpatient's satisfaction with their room. Relationship among patients' privacy, patients' stress, and patients' satisfaction were evaluated and were discussed. Survey research followed the literature review, in which the subjects answered questions of the 12 items for the patients' privacy requirements, 10 items selected from the HSRS, and an item of patients' satisfaction. A total of 120 copies were analyzed through the statistical process using the SPSS Win Program 20.0 and SEM by the AMOS 20.0. Results and conclusions are as follows: (1) the inpatients' privacy requirements was subdivided into 'the reserve factor', 'the territoriality factor', and 'the solitude factor'. (2) The inpatients' stress level was subdivided into 'the relationship factor', 'the unfamiliarity factor', and 'the control factor'. (3) The model of relationship among the subjects' privacy requirements, stress level and their satisfaction with the patients' room showed that the subjects' stress level affected their satisfaction with the patients' room directly and the subjects' privacy requirements gave an direct and indirect effect on their' stress level and an indirect effect on their satisfaction with patients' room.

Informational Justice, Cognitive Trust, and Satisfaction: Purchasers' Perspective of Healthcare Distribution Market

  • Purpose: We examined informational justice, cognitive trust, and satisfaction in healthcare distribution market and their associations within the physician-patient (provider-purchaser) relationship. Methodology: 253 valid survey samples collected from patients and used structural equation modelling for analysis. Findings: We postulated that (1) physicians' informational justice has a positive impact on patients' cognitive trust, (2) patients' cognitive trust has a positive impact on satisfaction, and (3) patients' perceived informational justice has a positive impact on satisfaction. Participants were 253 people who had visited a hospital in South Korea in the past year. Results confirmed that the presence of informational justice has a positive impact on patients' cognitive trust and satisfaction in the physician-patient relationship. Additionally, once cognitive trust was built, it positively influenced patients' satisfaction. We discussed the concept and the impacts of informational justice in light of our analyses regarding patients' perceived cognitive trust and their satisfaction in the physician-patient relationship. Implications: These results emphasize the importance of ethics in healthcare, particularly physicians' frankness and honesty when providing information to patients. Further, these findings present implications for physician education, as part of their training must involve building their patients' cognitive trust as a prerequisite for developing patient satisfaction.

FACTORS INFLUENCING PATIENT SATISFACTION WITH COMPLETE DENTURES (총의치 환자 만족도의 영향요인)

  • Statement of problem: In spite of the progress in techniques and materials in complete denture prosthodontics, patients still complain of discomfort after the insertion of complete dentures. For the last several decades many prosthodontists tried to find factors influencing patient complete denture satisfaction, however the reported results became a controversy. Purpose: The purpose of the present study was to verify the factors influencing patient satisfaction with complete dentures using multiple regression analysis. Materials and methods: 33 patients who visited the department of prosthodontics, dental hospital of Yonsei University, 4 to 6 weeks after the complete denture delivery, were asked to complete the questionnaires on complete denture satisfaction, social variables and psychological variables. The Prosthodontists who treated the patients with complete dentures were also asked to complete the questionnaires on evaluation of patients' oral condition and technical quality of dentures. The factors influencing patients' satisfaction with their complete dentures were analyzed using multiple regression analysis. Results: Among the patients' sociodemographic variables. the variables of relationship with children, economic status, housing condition, other people's opinions of dentures and gender were the influential factors on patients' satisfaction with complete dentures. Patients showing the symptoms of depression, one of the psychological variables, were dissatisfied with their complete dentures. In spite of the good oral condition, patients were dissatisfied with complete dentures, where-as the technical quality of dentures did not influence patients' complete denture satisfaction. Conclusion : According to the results above, patients' sociodemographic and psychological variables rather than clinical variables including oral condition and technical quality of dentures were the influential factors on complete denture satisfaction. The results of this study may not only enable prosthodontists to predict the success and failure of complete denture treatment, but also help both prosthodontists and patients be informed of the essentials of increasing satisfaction with complete dentures.

A CLINICAL STUDY ON THE SATISFACTION OF COMPLETE DENTURE PATIENTS (총의치 환자의 만족도에 대한 연구)

  • A success of complete denture prosthesis can be determined by the satisfaction degree of the dentist and the patient. There are many factors influencing on the satisfaction of the complete denture patients. The purpose of this study is to analyze the correlation between the masticatory satisfaction and clinical evaluation, and the speech satisfaction and clinical evaluation in complete denture patients. 79 patients(total 119 complete dentures) were examined in this study who were treated in Seoul National University Dental Hospital. Through clinical evaluation, 2 examiners evaluated sex, age, edentulous period, the years of having used present complete denture, the number of the previous dentures, the state of the opposite arch, retention, stability, support, occlusion, vertical dimension of the complete denture and the condition of the residual alveolar ridge. Through questionnaires concerned with mastication and speech, examiners evaluate the satisfaction degree of the patients. This study led to the conclusion that : 1. The satisfaction degree of the mastication was lower in older patients(p<0.001) and in patients whose present complete denture period was shorter(p<0.05). The satisfaction degree of the mastication in male was higher than in female patients(p<0.01). 2. The satisfaction degree of the mastication was high when the retention of the complete denture was good(p<0.05), and when the condition of the residual alveolar ridge was good(p<0.05). 3. The satisfacion degree of the speech was high when the retention of the complete denture was good(p<0.01). 4. The satisfaction degree of the mastication was higher in patients with upper and lower complete denture than in patients with single complete denture whose oppsite arch was unilateral partial denture(p<0.05).

An effect of medical service of the dental hygienist upon patients' satisfaction and the intention of hospital revisit (치과위생사의 의료서비스가 환자만족도 및 병원재이용 의사에 미치는 영향)

  • Objectives : This study aims at analyzing the quality of medical service of the dental hygienist upon patients' satisfaction and the intention of hospital revisit in order to figure out the role of the dental hygienist to improve patients' satisfaction. Methods : Questionnaire survey was carried out to patients in dental hospital and clinics in Seoul and Gyeonggi-do. Total 301 sheets except for insufficient ones were analyzed and the results are as follows. Results : As for the correlation analysis between variables affecting patients' satisfaction, every variables showed significantly positive relation, and the relation between medical treatment level of the dental hygienist and patients' satisfaction had the strongest positive (+) relation (p<0.001). Oral health education satisfaction by the dental hygienist and the medical treatment satisfaction by the dental hygienist had significant influence on patients' satisfaction, a dependent variable (p<0.001). Hospital environment (p<0.000), oral health education satisfaction by the dental hygienist (p<0.05), the satisfaction of medical treatment level by the dental hygienist (p<0.05) and patients' satisfaction (p<0.001) had significant influence on the intention of hospital revisit. Conclusions : In order to improve dental medical service quality and the intention of hospital revisit, the role of the dental hygienist is important. Thus, it seems necessary to prepare specific education plan on the medical service by the dental hygienist.

A Study on Influencing Factors for Sexual Satisfaction in Diabetic Patients (당뇨병환자의 성생활 만족요인에 관한 연구)

  • This study was designed and undertaken to analyse the factors associated with sexual satisfaction in diabetic patients. The data were collected from September to November, 1997. The subjects in this study were 77 diabetic patients who visited to check their blood glucose level to the outpatient department of internal medicine in one of hospitals located in Taejon city, The questionnaires developed by Derogatis L. R. were used. Data were analysed using percentages, means, 1-test, ANOVA and Peason-correlation coefficients, done with the SAS program. The results of this study were as follows ; 1. The mean score of sexual satisfaction in diabetic patients was higher than the mid level as 3.14 points in the 5 points scale and male patients' score was a little higher than female's. 2. The mean score of factors associated with sexual life was 3.54 points in spouses' support, 3.44 points in sexual attitude and 2.60 points in body image in the 5 points scale, and 2.76 points in the sexual act in the 6 points scale. The score of above all factors were higher in male patients than female patients. 3. The significant factors associated with sexual satisfaction were spouses' support, body image and sexual act. That is, the higher spouses' support, the more positive body image and the more frequent sexual act, the higher patients' sexual satisfaction. 4. In the relationships between the sexual satisfaction and the general characteristics of the diabetic patients, only religion and the period of disease were statistically significant differents, but the relationships between the sexual satisfaction and the period of disease showed a significant inverse correlation. This results showed that the degree of sexual satisfaction in diabetic patients was relatively high. The major factors associated with sexual satisfaction were spouses' support, body image and sexual act, religion and the period of disease. So, the above factors have to be considered during the nursing intervention on diabetic patients' sexual life.
