Background: In this study, we aimed to investigate the benefits of 18F-deoxyglucose positron emission tomography/computed tomography (FGD-PET/CT) imaging for staging and radiotherapy planning in patients with head and neck cancer undergoing definitive radiotherapy. Materials and Methods: Thirty-seven head and neck cancer patients who had undergone definitive radiotherapy and PET/CT at the Uludag University Medical Faculty Department of Radiation Oncology were investigated in order to determine the role of PET/CT in staging and radiotherapy planning. Results: The median age of this patient group of 32 males and 5 females was 57 years (13-84years). The stage remained the same in 18 cases, decreased in 5 cases and increased in 14 cases with PET/CT imaging. Total gross tumor volume (GTV) determined by CT (GTVCT-Total) was increased in 32 cases (86.5%) when compared to total GTV determined by PET/CT (GTVPET/CT-Total). The GTV of the primary tumor determined by PET/CT (GTVPET/CT) was larger in 3 cases and smaller in 34 cases compared to that determined by CT (GTVCT). The GTV of lymph nodes determined by PET/CT (GTVLNPET/CT) was larger in 20 cases (54%) and smaller in 12 cases (32.5%) when compared to GTV values determined by CT (GTVLNCT). No pathological lymph nodes were observed in the remaining five cases with both CT and PET/CT. Conclusions: We can conclude that PET/CT can significantly affect both pretreatment staging and assessed target tumor volume in patients with head and neck cancer. We therefore recommend examining such cases with PEC/CT before treatment.
Mucosal melanoma of the head and neck is a rare and usually lethal disease. Primary laryngeal malignant melanoma(LMM) are exceedingly rare tumors that morphologically are readily confused with more common types of laryngeal cancer. Treatment of choice for LMM is complete surgical excision and elective lymph node dissection is usually not recommended. The use of radiation or chemotherapy is generally thought to have no effect on local or distant disease and currently used as adjuvant therapy. The prognosis is extremely poor. We have experienced a 61 year old male patient with symptoms of foreign body and lump sense in throat. A dark pigmented polypoid mass was found on the right aryepiglottic folds with normal mobility of vocal cord. Total laryngectomy was performed under the diagnosis of malignant melanoma. Bone scan revealed multiple bony metastasis on ribs and lumbar vertebrae after 5 months of operation. There have been no evidence of recurrence at primary area. The patient died after 8 months of operation.

Head-and-neck cancer is often treated with intensive irradiation focused on the tumor, while delivering the minimum amount of irradiation to normal cells. Since a course of radiotherapy can take 5-6 weeks or more, the repeatability of the patient posture and the fastening method during treatment are important determinants of the success of radiotherapy. Many devices have been developed to minimize positional discrepancies, but all of the commercial devices used in clinical practice are operated manually and require customized fixtures for each patient. This is inefficient and the performance of the fixture device depends on the operator's skill. Therefore, this study developed an automated head-and-neck immobilizer that can be used during radiotherapy and evaluated the positioning reproducibility in a phantom experiment. To eliminate interference caused by the magnetic field from computed tomography hardware, Ni-Ti shape-memory alloy wires were used as the actuating elements of the fixtures. The resulting positional discrepancy was less than 5 mm for all positions, which is acceptable for radiotherapy.

Background: Teeth requiring extraction before radiotherapy in head & neck cancer patients should be removed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it was not uncommon for the radiotherapist and cancer patient to feel an urgent need to process with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy, were considered and applied based on the experimental study Materials and Methods: Eighteen dogs were processed for histopathologic wound healing. The effect of the primary endodontic treatment and extraction before early radiotherapy was examined. Results: No specific complication, such as, post-extraction wound infection, radiation osteitis and osteoradionecrosis, were encountered despite the early radiotherapy. Conclusion: Based on the experimental study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for the early radiotherapy in head and neck cancer patients.

Background: This report describes the authors' experience of "melting" septal cartilage after placement of a septal extension graft in a nasopharyngeal cancer patient that had been previously undergone radiation therapy, and provides a review of the literature. Methods: Electronic medical records were used to obtain details of the patient's clinical history. Results: A 32-year-old woman, who had previously undergone radiotherapy for nasopharyngeal cancer, visited our department to for rhinoplasty. Rhinoplasty was performed using a septal extension graft to raise the nasal tip (first operation). Five days after surgery, it was found that the septal extension graft was melting without any signs of infection, that is, the graft had softened, lost elasticity, thinned, and partially disappeared without any sign of infection at 5 days, and thus, the nasal tip was reconstructed with conchal cartilage (second operation). Five months after surgery, it was found that almost all septal cartilage had disappeared without any sign of infection, and thus, the entire nasal septum was reconstructed using 2-mm costal cartilage and an onlay graft was used for tip augmentation (third operation). Conclusions: After cartilage has been exposed to radiotherapy, its patency should be viewed with suspicion. Further studies are needed for determine the mechanism responsible for cartilage damage after radiotherapy.

Late complications of head and neck cancer survivors include neck muscle atrophy and soft-tissue fibrosis. We present an autopsy case of neck muscle atrophy and soft-tissue fibrosis (sternocleidomastoid, omohyoid, digastric, sternohyoid, sternothyroid, and platysma muscles) within the radiation field after modified radical neck dissection type I and postoperative radiotherapy for floor of mouth cancer. A 70-year-old man underwent primary tumor resection of the left floor of mouth, left marginal mandibulectomy, left modified radical neck dissection type I, and reconstruction with a radial forearm free flap. The patient received adjuvant radiotherapy. The dose to the primary tumor bed and involved neck nodes was 63 Gy in 35 fractions over 7 weeks. Areas of subclinical disease (left lower neck) received 50 Gy in 25 fractions over 5 weeks. Adjuvant chemotherapy was not administered.
Background: Platinum based concurrent chemo-radiation is the de-facto standard of care in the non-surgical management of locally-advanced head and neck cancer of squamous origin. Three-weekly single agent cisplatin at 100 $mg/m^2$ concurrent with radical radiotherapy has demonstrated consistent improvement in loco-regional control and survival. This improvement is however at the cost of considerable hematologic toxicity and poor overall compliance. The routine use of this regime is improbable in developing countries with limited resources. We therefore aimed to determine the safety and efficacy of an alternative regime of weekly cisplatin and concurrent radiotherapy in such patients. Materials and Methods: January-05 and April-12, 188 patients of locally-advanced head and neck cancer of squamous origin were treated with concurrent weekly-cisplatin at $35mg/m^2$ and conventional radiotherapy 60-66Gy/30-33 fractions/5days per week. Results: Overall, 95% patients received planned doses of RT while 74% completed within the stipulated overall treatment time of <50 days. Eighty-two percent received at-least 5 weekly cycles. Grade-III/IV mucositis was seen in 58%/9% respectively, which resulted in mean weight loss of 9.2% from a pre-treatment mean of 54.5 kg. Grade-III hematologic toxicity-0.5%; grade II nephrotoxicity-2.5% and grade III emesis-3% were also seen. Grade-III/IV subcutaneous toxicity-10%/1% and grade-III/IV xerostomia-10%/0% were observed. Complete responses at the primary site, regional nodes and overall disease were seen in 86%, 89% and 83% patients respectively. The median and 5-years disease-free survival were 26 months and 39.4% respectively, while the median and overall survival were 27 months and 41.8% respectively. Conclusions: Weekly-cisplatin at 35 $mg/m^2$ when delivered concurrently with conventional radical RT (at-least 66y/33 fractions) in locally-advanced head and neck cancer is well tolerated with minimal hematologic and neprologic toxicity and can be routinely delivered on an out-patient basis. It is an effective alternative to the standard 3-weekly cisplatin especially in the context of developing countries.
목 적: 현대 방사선치료기술에서 종양표적위치 및 정상장기에 정확한 선량을 전달하기 위해 여러 방법의 영상유도방사선치료(Image Guided Radiation Therapy, IGRT)가 사용되고 있으며 그 중 선형가속기에 장착된 CBCT(Cone Beam Computed Tomography, CBCT)와 이외 장치인 ExacTrac(ExacTrac X-ray System)이 있다. 두 시스템을 비교한 이전 연구들에서는 Offline-review 이용하여 후향적으로 팬텀 및 환자의 Set-up 오차를 분석하거나 X, Y, Z 축과 하나의 회전방향(Couch Rotation)으로만 연구되어졌다. 본 연구에서는 Head and Neck Cancer 환자를 대상으로 CBCT와 ExacTrac을 이용하여 한 치료중심센터에서 각각 6 DoF(Degree Of Freedom) IGRT를 시행한 후, 두 IGRT 장비에서 나타난 팬텀 및 환자의 Set-up 오차, 환자 Set-up에 걸리는 시간, 노출 방사선량의 비교를 통해 유용성을 평가하고자 한다. 대상 및 방법: Rando Phantom을 이용하여 환자 움직임을 배제한 상태의 Set-up 오차 평가와 Head and Neck Cancer 환자의 Set-up 오차 값 두 가지 경우로 나누어 획득하였다. 노출 방사선량 평가는 유리선량계로 하였다. 환자 Set-up 후 IGRT 시행하는데 소요되는 시간을 평가하기 위해 Head and Neck Cancer 환자 11명을 대상으로 하였다. 총 치료기간동안 환자 당 평균 10회의 CBCT와 ExacTrac 영상을 동시에 얻었고, 관심영역지정(Region Of Interest, ROI) 설정 후 6D 온라인 자동위치교정(Online Automatching) 값의 차를 6개의 축(Translation group: SI, AP, LR; Rotation group: Pitch, Roll, Rtn)으로 각각 계산하였다. 결 과: Phantom과 환자에서 Set-up 오차는 Translation group에서 1mm 미만, Rotation group에서 1.5° 미만의 차이가 보였으며, Rtn 값을 제외한 다른 모든 축의 RMS 값이 1mm, 1° 미만으로 나타났다. 각 시스템에서 최종적으로 Set-up 오차 교정까지 걸리는 시간은 CBCT를 이용한 IGRT에서는 평균 256±47.6sec, ExacTrac을 이용 시 평균 84±3.5sec로 각각 나타났다. 1회 치료 당 IGRT에 의한 방사선 노출선량은 Head and Neck 부위 7곳의 측정위치 중 Oral Mucosa에서 CBCT와 ExacTrac이 각각 2.468mGy, 0.066mGy로 상대적으로 ExacTrac에 비해 피폭선량이 37배 높게 측정되었다. 결 론: CBCT와 ExacTrac 두 시스템 간의 6D 온라인 자동위치교정을 통해 Set-up 오차는 두 시스템의 자체적인 Systematic error 뿐 아니라, 환자 움직임(Random error)를 포함한 Set-up 오차가 1mm, 1.02° 미만으로 나타났다. 이는 본원에서 Head and Neck IMRT 치료 시 PTV Margin이 3mm이라는 것을 고려했을 때, 이 오차범위는 합리적으로 사료된다. 하지만 치료기간 동안 환자체중변화로 인한 따른 표적, 손상위험장기의 변화를 고려했을 때 CBCT와 적절히 병용하여 사용하는 것이 좋을 것으로 사료된다.

The limited mouth opening, also known as trismus, can result from temporomandibular joint disorders, infection, neoplasm, trauma, and abnormal anatomic structure like coronoid hyperplasia. Head and neck cancer patients often complain of limited mouth opening, which is usually induced by myofibrotic contracture of masticatory muscle. But clinicians should consider any reasons such as infection or cancer growth and metastasis if trismus gets worse or pain develops. We report the case of the patient, who was diagnosed with myxofibrosarcoma on nasal cavity. He had suffered from trismus after concurrent chemoradiotherapy. However, pain had developed and trismus had worsened. He was diagnosed with infratemporal space abscess and myositis of masticatory muscles.
Caustic bums of the upper aerodigestive tract continue to be a significant clinical problem. Wide -field pharyngoesophagectomy is commonly performed as treatment for malignancies of the hypopharynx. A total laryngectomy is often necessary at the time of this procedure because of the anatomical proximity of the cancer or because of the likely compromise of swallowing postoperatively. When preservation of the larynx is attempted, aspirations after surgery frequently require a second-stage laryngectomy. And various flaps are using for reconstruction of esophageal defect. The choice of reconstruction is depended to the patient's status. A 54-year-old women whose symptom was severe dysphagia and X-ray revealed laryngopharyngeal stricuture. She had attempted suicide by swallowing lye liquids 32 years ago. She has entire laryngopharyngeal and esophageal stricutures. Total laryngectomy was performed and reconstruction of theesophagus was carried out with unusual reversed-gastric tube formation. Hence, we report this case with the review of literaturefor proper management in the future.
