Ebstein`S Anomaly: A Case Report of Plication and Tricuspid Valve Replacement (Ebstein 심기형 수술 1례[Plication 및 삼첨판막 이식예])

  • A 8 year old male was admitted to the Department of Thoracic Surgery, Korea University Hospital on June 22, 1978. The chief complaints were cyanosis and exertional dyspnea since at birth. EKG shows BVH and dextrocardia, phonocardiogram revealed the accentuation of second heart sound in aortic area. Echocardiogram from the left ventricle to the base of the heart, there is a discontinuity between the ventricular septum and the anterior aortic margin with a large aortic root & aortic overriding. His cardiac catheterization data and cardiac angiogram shows situs inversus totalis, dextrocardia, right aortic arch, large ventricular septal defect etc., and finally diagnosed Truncus Arteriosus. Edwards type IV with retrograde aortogram and selective bronchial angiogram. This is the first operative case reported as Rastelli operation for Truncus Arteriosus type IV in the literatures in Korea. Authors have experienced I case of Truncus Arteriosus, Edward type IV and Rastelli operation with Dacron Arterial Conduit Graft under cardiopulmonary bypass on July 3, 1978. The procedures were as follows; 2] Cardiopulmonary bypass: Origin of bronchial arteries excised from descending aorta bilaterally; defects in aorta closed. 2] Horizontal incision made high in right ventricle. 2] Ventricular septal defect [Kirklin type I+II] closed with Teflon patch. 4] Bifurcated dacron arterial graft with pericardial monocusp sutured to the bilateral pulmonary arteries. [Diameter 9 mm: Length 7 cm]. 5] Proximal end of the conduit graft anastomosed to right ventricle. [Diameter 19 mm: Length 5 cm]..Total perfusion time was 220 min. The result of operation was poor due to anastomotic leakage and increased pulmonary vascular resistance resulting acute right heart failure. The patient was died on the operation table. Literatures were briefly reviewed.

The Phenomenological Study on the Experiences of Nasogastric Tube Feeding among Cerebral Stoke Patients with Dysphagia (연하곤란 뇌졸중환자의 비위관으로 영양을 공급 받은 경험에 관한 현상학적 연구)

  • Purpose: This study was to provide understanding about the meaning of nasogastric tube feeding experiences by the cerebral stoke patients with dysphagia. Methods: Data were collected through a face to face interview from five cerebral stroke patients who had dysphagia along with a nasogastric tube insertion. The contents of the interviews were analyzed using the hermeneutics phenomenological method developed by van Manen. Results: The experience of having a nasogastric tube was not only limited to physical problems but also was linked to other psychological problems. Many patients reported that they tolerated the nasogastric tube in order to overcome the stroke along with the desire to eventually take food orally. The essential themes associated with the experience of nasogastric tube insertion were: "The loss of pleasure from food", "The loss of human relationship from surrounding table", "Cower and shrink from the different appearance to others" and "Acceptance for life in painful experiences". Conclusion: This study would highlights the importance of not only physical focused care but also psychological and social aspects. Therefore, nurses should consider all factors when providing nursing care to stroke patient with nasogastric tube feeding.

Treatment of a Traumatic Leptomeningeal Cyst in an Adult with Fibrinogen-Based Collagen

  • Reports of traumatic leptomeningeal cysts (TLMC) are rare in adults. The standard treatment approach is craniectomy with careful exposure of the intact dural edges, followed by duroplasty. However, occasionally, the location of the TLMC makes achieving watertight duroplasty impossible. Herein, we report the case of a 28-year-old male who presented with a soft growing mass on the vertex of his head 16 months after the head trauma. Upon enhanced CT examination, a bony defect involving both the inner and outer table of the cranium was observed close to the sagittal sinus, and a well-defined cystic mass, 5 cm in diameter, was nested within the defect. The risks associated with extension craniotomy were high because the lesion was located superficial to the sagittal sinus, we opted to use fibrinogen-based collagen fleece (TachoCombR$^{(R)}$) to repair the dural defect. Two months after surgery, the patient remained asymptomatic with a good cosmetic result. In cases like ours, when the defect is near the major sinuses and the risk of rupturing the sinus during watertight dural closure is high, fibrinogen-based collagen fleece (TachoCombR$^{(R)}$) is an effective alternative approach to standard dural suture techniques.

The Case Report of Chuna Treatment using Drop Table on Neck Pain Patients with Kyphotic Cervical Curvature (경추 후만에 대한 낙차 교정기법을 이용한 추나치료 치험례)

  • Objectives: This study is performed to evaluate the clinical effect of chuna therapy on the neck pain associated with kyphotic cervical curvature. Methods: This study carried out on three patients with neck pain & kyphotic cervical curvature who have received treatment in Depar1ment of Oriental Rehabilitation Medicine, Daejon Oriental Hospital of Daejon University from 28th July 2006 to 3th November 2006. Pre and post treatment, We evaluated the cervical angle, Jackson's angle, Jochumsen method, Ishihara index, VAS and effective score of treatment. Results & Conclusions: Two patients who received Chuna treatment recovered cervical curvature and improved neck pain. But One patient who dosen't received Chuna treatment was no improvement in cervical curvature & neck pain.

The Effects of the Improvement of the Food Service Environment on Patients′ Satisfaction with the Hospital Food Service - Focused on Case Studies - (급식환경 개선이 환자식 만족도에 미치는 영향 -사례 연구 중심으로 -)

  • The purpose of this study was to assess how the changes in the food services environment on patients satisfaction with the hospital food service. Statistical data analyses were completed using the SPSS 11.0 program. The results can be summarized as follows: The flow line and environment of the food services in the hospital were improved through remodeling, which included the replacement of all cooking utensils, ventilation facilities and material storages, the purchase of a combi steamer, and the change of meal carts and trays. After the remodeling, the hospital food service was improved so that it provided spoons at each meal, diversified the menu utilizing the combi steamer, served event meals three times a week as well as water boiled with burned rice in the morning twice a week. In addition, various types of tableware were used in the table settings to produce attractive visual effects. Among the 10 items included on the patient satisfaction questionnaire, ″satisfaction with offered menus″ (p < 0.01) showed significantly higher scores before the remodeling. ″cooking/seasoning of food″, ″amount of meals″ and ″taste of meals″ were not statistically significant, but showed increased satisfaction after the remodeling. However ″temperature of food″, ″cleanliness of clothes and features″ and ″satisfaction with meal times″ were not statistically significant, but showed decreased satisfaction after the remodeling. Correlations among the Continence Function, Cognitive Function and Activities of Daily Living in Elderly Male Patients with Dementia in a Geriatric Hospital

  • The purpose of this study was to identify correlations among the continence function, cognitive function, and activities of daily living(ADL) in elderly male patients with dementia in geriatric hospitals. The subjects were 64 patients aged 65 or above who were diagnosed with dementia among the hospitalized male patients in a geriatric hospital. For the subjects' cognitive function, a questionnaire developed for the Korean Mini Mental Status Examination(K-MMSE) was used. For the continence function and ADL, data were collected using a patient evaluation table. As a result, a lower level of cognitive function resulted in corresponding higher levels of dependence in all items of ADL except bathing(p<.05), and a lower level of cognitive function led to corresponding declines in the continence function(bowel control, bladder control)(p<.01). In addition, a higher level of dependence in ADL resulted in corresponding higher levels of difficulty in bowel and bladder control(p<.01). This study showed correlations among the cognitive function, ADL, and continence function of elderly men with dementia. The results of this study may be used as basic data for the management and treatment of hospitalized elderly male patients with dementia in geriatric hospitals.

Patients' Evaluation on Foodservice in University and General Hospitals (대학병원 종합병원 입원환자의 급식에 대한 평가)

  • This study was conducted to investigate and to improve the actual condition of food service for patients in hospitals. For this purpose, questionnaires were distributed to 283 patients admitted to a university hospital and three general hospitals between January 27 and February 15, 1997. The department participated in the study included internal medicine, surgery, obstetrics, orthopedics and so forth. For meal time, 61.1$\%$ of patients wanted to eat breakfast at 8 am, 55.8$\%$ lunch at noon, and 73.5$\%$ dinner at 6 pm. The patients complained about unsatisfactory hospital food itself by 37.8$\%$, about insufficient food amount by 19.6$\%$, about menu with no choice by 41.2$\%$ and about low variety of the meals by 32.7$\%$. Sixty two point nine percent of the subjects enjoyed snacks between meals because of poor appetite at meal time(46.1$\%$), delayed food service(39.9$\%$) and others(11.2$\%$). The types of diet were mainly regular ones(58.6$\%$) with some high protein(12.4%) and diabetic sensitive ones(7.1$\%$). As eating place, the patient's prefered bed(51.9$\%$), room-table(27.2$\%$) and dining room(17.7$\%$). Fifty-five percent of them also wanted hospital foods available to their caring relatives.   • The purpose of this study is to find application of combination of insotonic(CI) in clinical approach. This is a literature study with books and clinical experience. CI was developed after Margaret Knott that is very useful in clinically. CI has two lever system. It has class II lever system for eccentric contraction and class III lever system for concentric contraction. Therefore it make two different contraction that is not only magnitude of motion and speed increase of motion but also biomechanical advantage. Application of CI has much variety by patient position and treatment position. Especially, it is very effectable on mat activities and training for mid stance period. CI apply in supine position on treatment table, mat or high mat activities, bridging activities, all fourth activities and sit on chair activities. CI apply serially with different techniques. These are rhythmic initiation, dynamic reversal, approximation, rhythmic stabilization and replication.

Preliminary Results of 7-Channel Insertional pTx Array Coil for 3T MRI

  • In this research, we report the preliminary results of an insertional type parallel transmission (pTx) array that has 7-elements that are placed in the space above a patient table as a transmit (Tx) coil to give an RF transmission ($B_1{^+}$) field for the body object of a 3 Tesla (T) MRI system. In previous research, we have tried to compare the performances of different coil elements and array geometries for a pTx body image. Based on these results, we attempt to obtain a human image with the proposed pTx array. Through the simulation and experimental results, we introduce a possible structure of multi-channel Tx array and verify the utility of a multi-channel Tx body image using $B_1{^+}$ shimming. The insertional pTx array, combined with a receiver (Rx) array coil, provides an enhanced $B_1{^+}$ field homogeneity in a large ROI image as a result of $B_1{^+}$ shimming applied over the full body size object. Through this research, we hope to determine the usefulness of the proposed insertional type RF coil combination for 3 T body imaging.
