A Study on Satisfaction Evaluation of Korean Medicine Therapy Using Treatment Tool by Psychological Type (심리유형별 진료도구를 이용한 한방치료의 만족도 평가 연구)

  • Objectives : This study was designed to evaluate patient satisfaction of Korean medicine therapy using treatment tool by psychological type. Methods : 48 subjects were participated in this study. Before a treatment, we analyzed a patient's phychological type first by using a questionnaire. And then we divided 3 groups which were called a rational sympathy module group, emotional sympathy module group, control group. Manner of medical consultation according to patient's phychological type was carried out in all groups. After all treatment, we conducted a survey about medical service perception index, patient happiness index, medical satisfaction index. To evaluate a satisfaction degree, we analyzed results of survey statistically. Results : The results of the analysis, both rational sympathy module group and emotional sympathy module group got a higher score than control group statistical significantly on medical service perception index. Emotional sympathy module group got a higher score than control group statistical significantly on patient happiness index and medical satisfaction index. But rational sympathy module group only had a higher tendency than control group on patient happiness index and medical satisfaction index. Emotional sympathy module group got a higher score than rational sympathy module group on medical service perception index, patient happiness index and medical satisfaction index. Conclusions : Korean medicine therapy using treatment tool by psychological type consisted of patient's phychological type questionnaire and manner of medical consultation could improve a patient's satisfaction. It was more effective in emotional sympathy module group than rational sympathy module group.

The Customer Satisfaction Index Model: An Empirical Study of the Private Healthcare Sector in Malaysia

  • The main purpose of this study was to gauge the patient satisfaction index and subsequently discuss the Importance-Performance (IP) matrix analysis of the inpatient services in the context of the private hospital setting. The Malaysian Customer Satisfaction Index Model was employed as the theoretical framework for the above purposes. This study involving 242 patients in Malaysian's private healthcare sector used a Web-based survey as the main method of data collection. Partial least square structural equation modeling (PLS-SEM) was utilized for data analysis. Using Fornell et al. (1996)'s formula, the resulting patient satisfaction index was slightly lower than the "very satisfied" category, the target level required for positioning as one of the world's premier medical tourism players. The IP matrix showed that medical quality is the main competitive advantage of the private hospitals that can propel their growth in the global healthcare marketplace. The results also indicate that outcome quality, patient rights, and privacy, and service quality are the three quality domains that need to be prioritized for further improvement. On the other hand, the servicescape quality domain needs to be strategized as the unique selling proposition as the performance of the private hospitals in this regard is already extremely good.

  • Purpose: Despite growing interest in health information sharing, researchers remain uncertain whether hospitals' sharing of information increases patient satisfaction, one of the key measures of the quality of care. This study evaluates the effect of health information sharing on patient satisfaction and whether the effect varies with context. Methodology: Regression analysis was conducted using a sample consisting of 6,641 year-hospital observations, based on data from the annual and IT surveys of the American Hospital Association (AHA) and Census. Findings: Our results suggest that information sharing using health IT increases patient satisfaction, while the extent (breadth) of information sharing increases patient satisfaction, the level of detail (depth) of information sharing does not. Our results also show that the effects of the breadth and depth of information sharing vary with different contexts, e.g., the percentage of people over 65 and Hirschman-Herfindahl index (HHI). Practical Implications: Policymakers should pay greater attention to realizing the full benefits of hospitals' information sharing based on market characteristics.

The Effect of Patient-Centered Goal Setting on Rehabilitation Outcomes in Patients With Subacute Stroke (환자중심 목표설정이 아급성기 뇌졸중 환자의 재활치료 성과에 미치는 영향)

  • Background: Patient-centered care has recently become highly recommended, because it can improve health outcomes more effectively than problem-oriented care. The goal attainment scale (GAS) is one of the methods used for patient-centered care. It was originally developed as an outcome measurement tool, but it can also be used as a therapeutic intervention when setting quantifiable patient-centered goals. Objects: We sought to identify the effect of setting patient-centered goals on rehabilitation outcomes in patients with subacute stoke using GAS. Methods: Overall, 46 subjects with subacute stroke were divided into experimental ($n_1=23$) and control ($n_2=23$) groups. Subjects in both groups attended physical therapy sessions five times a week for four weeks. Those in the experimental group set goals using goal-attainment scaling. The mobility of each subject was evaluated using the Rivermead mobility index (RMI); daily-living activities, using the K-modified Barthel index (K-MBI); participation, using the Hopkins rehabilitation-engagement rating scale (HRERS); satisfaction, using the patient-satisfaction scale (PSS). Results: Subjects in the experimental group experienced more significant increases in RMI, HRERS, and PSS than those in the control group (p<.05, p<.05 and p<.01, respectively). After four weeks, GAS scores of the experimental group had increased more significantly than those of baseline (p<.01). Conclusion: Setting patient-centered goals is effective in improving the mobility and satisfaction of patients with subacute stoke. Setting patient-centered goals needs to be performed more frequently in clinical settings.

How to Improve Patients' Satisfaction in Healthcare Organization? - Healthcare Service Quality Classification using Kano Model - (의료기관의 소비자 만족도 향상요소 도출 - Kano 모형에 기반한 의료서비스 품질 분류를 중심으로 -)

  • Objective: This research investigates how to increase the quality of medical service and supply high quality of medical service to patients. By using Kano Model theory we examines what medical service attributes the hospital would be conducted preferentially for patient's satisfaction and provides informations of management strategies for hospitals. Method: To study patients' perception of medical service quality, first we performed pilot test to derive 30 medical service attributes. With 30 medical service attributes, we conducted survey of 300 subjects who have experienced medical services in 6 months. To examine patients' conception of medical services, a modified Kano's questionnaire using 5 scale is applied. Finally we calculated SI(Satisfaction index) and DI(Dissatisfaction index) and PCSI(Potential Customer Satisfaction Improvement) index with Kano's Model analysis results. Key Findings: We found that the quality of medical service categorized in 15 one-dimensional elements, 9 must-be elements and 6 indifferent elements. Moreover the attribute of gives prompt services and have patient's best interest at heart scored the highest SI, whereas the attributes of accurate and precise medical service, exact records, enough explanation and polite attitudes are the highest score of DI. And also good explanation of the bill scored the highest PCSI. In this study findings indicate that while medical service providers try to increase patients' satisfaction by improving hospital's environments, patients' perception of trust and good interpersonal relationships with medical service providers have strong and positive impact on patients' satisfaction.

The effects of the quality of dental care service by factors on the patient satisfaction and service value (요인별 치과 의료서비스 질이 환자만족과 서비스 가치에 미치는 영향)

  • The purpose of this study is to forecast patient satisfaction and service value through the staged regression analysis of the relation between each factor and its satisfaction and service value, following the measurement of the quality of dental care service that patients recognize. And a self-administered survey was used for this study. The subjects of this survey were 394 outpatients of dental clinics located in K city in the period from Dec. 5, 2004 to Feb. 19, 2005, the questionnaire was composed of a total of 31 questions to measure the quality of overall dental care service(22 questions), the patient satisfaction(6 questions) and the service value(3 questions). The 7 points Likert scale ranging from "very poor"(1-point) to "very high"(7-point) was used for these questions The results of study could be summarized as follows: 1. The most useful index to predict patient satisfaction turned out to be "internal environment", followed by "treatment process and waiting time", "dentist", "external environment", and "dental hygienist". 2. The service value had a significant effect on the "internal environment", "dentist", "treatment process and waiting time". 3. The most useful index to predict the service value turned out to be "treatment process and waiting time", followed by "internal environment" and "dentist". Today, the quality of dental care service is becoming a prime concern since it is directly connected to a matter of survival of medical service organizations. Dental clinics desperately need patient-oriented marketing strategies in order to actively cope with changing medical environment. They also need to thoroughly examine possible measures to maximize the service value by establishing a variety of service strategies which can promote service quality that patients recognize.

A Research on Trust Realization Strategies for Oriental Medical Quality Improvement (한방의료 품질 향상을 위한 신뢰구현 체계구축 연구)

  • Objectives : This study was designed to establish medical trust realization system by finding factors influential to it using questionnaire. Methods : 277 subjects were participated in this study. After a treatment, we conducted a survey from April 1st to October 31th about medical service perception index, medical trust index, patient satisfaction index, patient reliability index, patient flow degree index, recall intension index, and hospital image index. To evaluate the influence of medical service perception with other 6 indexes, we statistically made regression analysis of the results through the survey. Results : By the results of the analysis, evaluation of hospital image influenced all 6 indexes. The systemicity of treatment process had an effect on 5 indexes except for the flow degree of patients. The humanity of medical team brought out the estimation of 4 indexes except for the patient flow degree and hospital image. The empathic ability of doctor and appropriacy of medical costs hold the next rank influencing 3 indexes. It reached the conclusion that the systemicity of medical team tend to determine the medical trust and patient reliability. The expertise, professional skill of doctor, the fault, commercial application, fame of medical team, the speed of treatment process, the newest and clean medical facility affected each one index. Conclusions : Korean medicine should find a way to consider the mind of patients for improving the medical quality through trust realization system, keeping up with times. As a result of this research, we can find out important causes which influence the trustful medical system. From now on, we should apply this result to actual treatment of psychology customized system. Also, more simple and clear questionnaire was organized through this research, it can be used to forward research to apprehend patient mentality more conveniently.

Standardizing the evaluation criteria on treatment outcomes of mandibular implant overdentures: a systematic review

  • PURPOSE. The aim of this review was to analyze the evaluation criteria on mandibular implant overdentures through a systematic review and suggest standardized evaluation criteria. MATERIALS AND METHODS. A systematic literature search was conducted by PubMed search strategy and hand-searching of relevant journals from included studies considering inclusion and exclusion criteria. Randomized clinical trials (RCT) and clinical trial studies comparing attachment systems on mandibular implant overdentures until December, 2011 were selected. Twenty nine studies were finally selected and the data about evaluation methods were collected. RESULTS. Evaluation criteria could be classified into 4 groups (implant survival, peri-implant tissue evaluation, prosthetic evaluation, and patient satisfaction). Among 29 studies, 21 studies presented implant survival rate, while any studies reporting implant failure did not present cumulative implant survival rate. Seventeen studies evaluating peri-implant tissue status presented following items as evaluation criteria; marginal bone level (14), plaque Index (13), probing depth (8), bleeding index (8), attachment gingiva level (8), gingival index (6), amount of keratinized gingiva (1). Eighteen studies evaluating prosthetic maintenance and complication also presented following items as evaluation criteria; loose matrix (17), female detachment (15), denture fracture (15), denture relining (14), abutment fracture (14), abutment screw loosening (11), and occlusal adjustment (9). Atypical questionnaire (9), Visual analog scales (VAS) (4), and Oral Health Impact Profile (OHIP) (1) were used as the format of criteria to evaluate patients satisfaction in 14 studies. CONCLUSION. For evaluation of implant overdenture, it is necessary to include cumulative survival rate for implant evaluation. It is suggested that peri-implant tissue evaluation criteria include marginal bone level, plaque index, bleeding index, probing depth, and attached gingiva level. It is also suggested that prosthetic evaluation criteria include loose matrix, female detachment, denture fracture, denture relining, abutment fracture, abutment screw loosening, and occlusal adjustment. Finally standardized criteria like OHIP-EDENT or VAS are required for patient satisfaction.

The Effects of Pitch Increasing Training (PIT) on Voice and Speech of a Patient with Parkinson's Disease: A Pilot Study

  • The primary goal of therapeutic intervention in dysarthric speakers is to increase the speech intelligibility. Decision of critical features to increase the intelligibility is very important in speech therapy. The purpose of this study is to know the effects of pitch increasing training (PIT) on speech of a subject with Parkinson's disease (PD). The PIT program is focused on increasing pitch while a vowel is sustained with the same loudness. The loudness level is somewhat higher than that of the habitual loudness. A 67-year-old female with PD participated in the study. Speech therapy was conducted for 4 sessions (200 minutes) for one week. Before and after the treatment, acoustic, perceptual and speech naturalness evaluation was peformed for data analysis. Speech and voice satisfaction index (SVSI) was obtained after the treatment. Results showed Improvements in voice quality and speech naturalness. In addition, the patient's satisfaction ratings (SVSI) indicated a positive relationship between improved speech production and their (the patient and care-givers) satisfaction.

Development and Implementation of a Critical Pathway in Patient with Osmidrosis (액취증 환자에서 표준 진료지침서의 개발과 적용)

  • The current health care system demands provisions for patient care in perspectives of a cost-effectiveness and patient satisfaction. Critical pathway implementation facilitates optimal sequencing and intervention timing of patient care, and makes medical team and patients participate in a treatment actively. In this study, a critical pathway was developed and implemented to patients with osmidrosis who undertake operation. Sixty patients were included in the study. The critical pathway was implemented for care of 26 patients while the traditional care was implemented for 34 patients. In the critical pathway implemented group, time needed for charting and unessential working was reduced. Mean time amount of time for patient nursing was increased. The critical pathway implementation is an effective method to utilize time of medical team. Also it increases the satisfaction index of patients and medical team simultaneously.
