Whole Spine Scanography의 검사방향에 따른 환자 선량 평가 (Patient Radiation Exposure Dose Evaluation of Whole Spine Scanography Due to Exposure Direction)

  • Whole spine scanography(WSS)는 전신에 X선을 조사하는 검사로 치료기간 동안 빈번한 X선 조사가 이루어지는 검사이다. 일반촬영 분야에서는 많은 X선이 전신에 조사되는 검사이다. 따라서 논문에서는 Auto image pasta기법의 디지털 WSS 검사에서 환자의 검사방향에 따른 유효선량과 장기선량을 전산모사를 통하여 평가하였고, 영상에서 척추의 확대도와 각도의 변화를 평가하였다. 전후면 자세에서의 평균 유효선량은 0.069 mSv였고, 후전면 자세에서 평균 유효선량은 0.0361 mSv로 약 2배의 차이를 보였다. 전후면 자세에서 남성의 평균 유효선량은 0.089 mSv, 여성에 대한 평균 유효선량은 0.431 mSv로 나타났고, 후전면 자세에서 남성의 평균 유효선량은 0.050 mSv, 여성에서는 0.026 mSv로 나타났다. 확대율에서는 후전면 자세에서 전후면 자세에 비해 5%정도 확대 되었으나 각도에는 큰 변화를 보이지 않았다. 따라서 임상 환경에서 동일한 검사조건에서 환자의 자세를 변화시키는 것만으로도 환자의 피폭선량을 감소시킬 수 있음을 확인하였다. 특히 WSS와 같이 치료기간 동안 반복되는 검사에서 환자의 피폭선량 최적화를 위하여 검사 프로토콜의 재정립이 필요함을 확인하였다.

  • Exposure factors in the chest radiographic examination vary with hospital. They include low voltage radiation to high voltage hard radiation quality, which are in wide use. In the present report, exposure factors generally employed are reviewed, and the chest radiograhic techniques performed in our hospital are explained. In addition, the attitude of radiological technologists toward patients and so forth are also discussed.

핵의학과에서 방사선 피폭관리 실태에 대한 조사 연구 (A Study on the Radiation Dose Managements in the Nuclear Medicine Department)

  • 의료기관 핵의학과에서는 진단과 치료를 목적으로 방사성동위원소를 사용하므로 누구나 방사선피폭에 노출 될 위험이 있다. 일반적으로 방사선 작업종사자에 대한 피폭관리는 비교적 철저히 이루어지고 있으나 환자보호자 및 일반인에 대한 피폭관리는 소홀한 경향이 있다. 특히 방사성의약품을 투여한 환자들은 잠재적 선원이 되어 작업종사자외에 환자보호자 및 일반인에 대해 방사선피폭을 초래하므로 이로 인한 방사선피폭을 최소한으로 감소시킬 수 있도록 관리되어야 한다. 따라서 핵의학과 방사선피폭에 대한 관리실태를 파악하기 위하여 전국에 있는 대학병원 중 7개소에 대해 조사한 결과 환자이송요원, 환경미화원 등 수시출입자에 대해서 2 개소의 의료기관에서는 피폭선량평가 및 관리와 안전교육이 없었다. 또한 환자와 동행하는 보호자에 대한 통제와 관리는 7 개소 모두 허술하였는데 대기실에서 검사직전 환자로부터 흡수될 수 있는 평균 방사선량률은 25.60 ${\mu}$Sv/h로서 일반인에 대해 연간 선량한도를 초과하지 아니하는 범위 내에서 허용되는 20 ${\mu}$Sv/h를 초과하였다. 따라서 비록 아주 적은 피폭선량이 예상된다고 하더라도 수시출입자에 대한 철저한 피폭선량의 관리와 교육이 요구되며, 환자보호자 등을 보호하기 위해 환자와 가까이 하는 것을 통제하거나 환자를 격리할 필요가 있었다.

  • As applications of radiation grow wider from use in the early detection of lesions and preventive diagnosis purposes to the treatment of diseases, the possibilities for patients and working professionals to be exposed to radiation are becoming greater than ever. This can not only directly bring about an increase in patient's individual radiation exposure, but also brings about an increase in the annual radiation dose of working professionals. Therefore, research and countermeasures to reduce radiation dosage are required. In this study, space dosimetry has been divided into two separate measuments with an understanding of the increasing number of angiography procedures: front perspective and side perspective. According to the results of the isodose curve, a way to minimize radiation exposure in working professionals has been suggested. This was made possible by workers through awareness of suitable working positions.

전산화단층촬영검사 시 검사실 내에 위치할 수 있는 의료인의 간접 피폭선량에 대한 연구 (A Study on the Indirect Radiation Exposure of the Medical Personnel Who is Responsible for Patient Safety in CT Examination)

  • A medical personnel could be placed beside a patient together in CT room to do Ambu-bag for a seriously ill patients or emergency patient. At this time, the medical personnel can be exposed indirect radiation unnecessarily. In this case, it is necessary to recognize indirect radiation dose levels and methods to reduce them using actual clinical CT protocols such as Chest, Abdomen, and Brain CT. We researched surface radiation dose with or without radiation protectors such as apron and goggles according to different distances far from gantry using two different CT scanners (Fixed MDCT and mobile CT). As a result, for Chest, Abdomen, and Brain CT with Fixed MDCT, indirect radiation dose on thorax portion were 0.047, 0.089, 0.034 mSv without apron. Also, those with apron were 0.007, 0.012, 0.006 mSv. In case of mobile CT, it was 0.014 mSv without apron and 0.005 mSv with apron. By using protectors and increasing the distance, we could reduce it to 97%. Systematic management is necessary based on the measured data in order to minimize radiation damage due to indirect exposure dose.

전산화단층촬영 검사 시 인체에 미치는 방사선피폭선량 분석연구 (Study of radiation exposure on human body using of Computed Tomography)

  • This study analyzed the total number of 19,636 patients and radiation technologists, 11,433 of male and 8,203 of female by examined body parts, age, types of detectors, the using contrast enhancement and working condition of the technologists, regular staffs or rotation-duty staffs, based on the K-DOS program distributed by FDA with the DLP value of diagnostic evaluation. The result shows that the effective radiation dose was 0.7mSv~41.7mSv for each region and male patients had more radiation exposure than females. And the amount of exposure was also affected by the types and the method of detectors. Furthermore, the regular staffs took the role of helping the patient to get reduced amount of radiation exposure than rotation duty-staffs. Computed tomography (CT) use has increased dramatically over the past several decades. In this reason, to support the patients and the workers' health in the field, the hospitals should apply specialized regular working radiation technologist system and manufacturing companies of those CTs should develop low medical radiation exposure devices.

중증외상환자 CT 검사 시 검사보조자의 방사선피폭 경감을 위한 융합적 망토 차폐체의 유용성 연구 (A study on the usefulness of a fusion model designed cloak shield to reduce the radiation exposure of the assistant during CT of severely injured patient)

  • 중증외상 환자 CT(Computed Tomography)검사 시 검사 보조자의 방사선 피폭경감을 위하여 새로운 융합적 모델의 망토 차폐체를 제작하여 그 유용성을 평가하고자 한다. 실험 방법으로 뇌혈관 CT 검사와 동일한 조건에서 두부 팬텀과 인체 팬텀을 이용하여 기존 차폐체와 새롭게 개발된 망토 차폐체를 이용하여 심장, 양쪽 액와부 그리고 갑상샘 부위를 유리선량계로 피폭선량을 측정하였다. 새롭게 개발된 차폐체가 기존의 차폐체 보다 방사선 차폐율이 심장 61.9 %, 좌측액와부 46.2 %, 우측액와부 69.8 %, 그리고 갑상샘 71.1 % 로 각각 높게 측정되었다. 새롭게 개발된 망토 차폐체가 방사선 피폭을 감소시키는데 매우 유용하며 향후 새롭게 개발된 융합적 모델의 망토 차폐차가 방사선 피폭을 감소시키는데 있어 큰 기여를 할 것으로 판단된다.

치과방사선영상 검사는 위험한 술식인가? (Are dental radiographs dangerous?)

  • Radiographs can help in the diagnosis and treatment planning, but the exposure to ionizing radiation may elevate the risk of developing cancer in a person's lifetime. The objective of this review is to briefly summarize 1) radiation risk, especially cancer risks associated with diagnostic imaging, 2) linear, non-threshold (LNT) hypothesis, 3) the risks of radiation exposure to a fetus, and 4) the campaign of Image Gently. The individual risk of radiation-related cancer from any single medical imaging procedure is extremely small and it is not likely to be cancer risk at doses lower than 100 mGy, but patients may be harmed by avoiding diagnostic imaging due to fear of radiation hazard. Dentists need to understand the radiation doses delivered by various radiographic techniques and the acceptable exposure thresholds to effectively advise the patient and to reduce the unnecessary radiation

  • X-ray high-voltage generator is the most important part that can decide the radiation exposure dose affecting a patient or operator according to the characteristic. If decrease of X-ray radiation exposure dose and output characteristic of high-voltage generator is unstable, a patient or operator must be exposed to more radiation. This study measures and analyzes the exposure dose reproducibility and output characteristic according to a change of tube current on the various rectification methods of diagnostic X-ray equipment. It can find that quality bastardize and output is increased if voltage of X-ray tube is increased. Exposure dose reproducibility according to output of X-ray equipment is extremely excellent in inverter type, and is stable in order of following three-phase, a single-phase and condenser method. This study can find that the reply incidence of high-voltage generator is generated due to difference in rectification method, noise occurs in X-ray due to that, quality of an image is decreased due to that, and medical diagnosis can be failed due to that.

두부 CT 검사 시 테이블 높이에 따른 선량과 화질에 관한 연구 (A Study on Radiation Dose and Image Quality according to CT Table Height in Brain CT)

  • The height of the table should be considered important during computed tomography (CT) examination, but according to previous studies, not all radiology technologists set the table at the patient's center at the examination, which affects the exposure dose and image quality received by the patient. Therefore, this study intends to study the image quality exposure dose according to the height of the table to realize the optimal image quality and dose during the brain CT scan. The head phantom images were acquired using Philips Brilliance iCT 256. When the image was acquired, the table height was adjusted to 815, 865, 915, 965, 1015, and 1030 mm, respectively, and each scan was performed 3 times for each height. For the exposure dose measurement, optically stimulated luminescence dosimeter (OSLD) was attached to the front, side, eye, and thyroid gland of the head phantom. In the signal to noise ratio (SNR) measurement result, The SNR values for each table height were all lower than 915 mm. As a result of exposure dose, the exposure dose on each area increased as the table height decreased. The height of the table has a close relationship with the patient's radiation exposure dose in the CT scan.
