혈액 은행 전산 처리 프로그램의 개발 (The Development of Blood Bank Management Program)

  • The blood bank is a field of clinical pathology which requires the most accuratemaintenanceofrecording. Because the mistake in it is directly related to a patient's life. So, the computerization of the blood bank is urgent to maintain a log blook arid to compare the patient's current data with past result. We developed the blood bank management program using 32 bit minicomputer. This is composed of 4 parts; a management of routine test result, special test result, the blood issue and statistics. The management of routine test result handles the patient's information and blood typing and compares above results with the past one of same patient. The management of special test result are for special immunohematologic tests like an irregular antibody, Coombs' test, and etc. Blood issue part records the type of the blood bag, component, and the name of issuer. Statistic part are made to get statistics of each day and each month by the blood type, and the type the blood component. The program is secured by the maintenance of operator's operation history and thu provision of the security code to each operator, without which no one can enter the system and after the content. So the stability and reliability of the data is obtained. This program will be upgraded for bar-code using system in the near future.

진료 품질 향상을 위한 환자 데이터 맞춤형 분석 프로세스 개발: 외국인 환자를 중심으로 (Development of customized patient data analysis process for quality of care improvement : focused on foreign patients)

  • Purpose: The purpose of this study was to find meaningful patient groups of disease using foreign patients data and analyze implemented test of the patient groups. Methods: The data was collected by foreign patients' EMR data of K university hospital. The author proposed tree-form patients' characteristic diagram through statistical methods that association rule, proportion test, clustering using prescription information and questionnaire information. Results: This study's analysis process was applied high blood data and diabetes data. Analysis showed other characteristic of meaningful patient groups in high blood and diabetes. In high blood, test implementation rate of patient group showed the differences. And in diabetes, test implementation rate of patient group and implemented test list showed differences. Conclusion: The result of this study can play a role as basic data that can be clinical testing standard in preventive aspect. Eventually, 5 dimensions of SERVQUAL will be improved by this study's process.

스마트 모바일 기기를 이용한 만성질환 관리 (Chronic Disease Management using Smart Mobile Device)

  • 최근 노인인구의 증가하는 추세에 따라 만성질환자의 수도 증가하고 있으며 이에 따라 만성질환자들에 대한 건강 관리문제가 중요하게 대두되고 있다. 본 논문에서는 스마트 모바일 기기를 이용한 만성질환 관리 시스템을 구현하고자 한다. 제안한 만성질환 관리 시스템은 생체정보를 검출하는 생체계측센서와 그 센서로부터 정보를 수신하고 환자관리 서버로 정보를 전송하는 스마트 모바일 기기, 그리고 무선 통신망을 통해 전달받은 데이터를 해석하고 처리하기 위한 환자관리 서버, 환자관리 DB, 그리고 환자 증상분석 전문가 에이전트로 구성된다. 생체 신호는 심전도, 혈압, 혈당 및 PPG 등의 모듈로 구성하였다. 만성질환자에 대한 건강관리 시스템을 구현함으로서 측정되어진 생체 데이터를 모니터링하여 현재의 건강상태를 확인할 수 있었으며, 모바일 환경에서 환자군에게 맞는 개별적인 서비스를 제공할 수 있다는 점에서 의의가 있다.

안전한 수혈을 지원하는 모바일 헬스케어시스템의 설계 및 구현 (Design and Implementation of Mobile Healthcare System supporting Safe Transfusion)

  • 바코드 시스템과 전자태그(RFID)부터 저 전력 블루투스 기술을 활용한 비콘(Beacon)까지 진료 및 검사 단계를 자동화하고 스마트 기기와 연계된 진료 정보를 이용하는 등 정보기술과 보건‧의료 분야의 용합이 널리 확산되고 있다. 본 논문에서는 환자에게 혈액을 수혈하는 과정에서 다른 혈액형이 수혈되는 사고를 예방하기 위해 안전한 수혈관리를 위한 모바일 헬스케어 시스템을 설계하고 구현하였다. 환자 팔찌, 채혈 용기, 혈액백 및 의료진 신분증의 바코드에 저장된 정보를 스마트 기기로 인식하여 환자의 정보와 매칭시켜 안전하고 효율적인 수혈이 가능하도록 구현하였다. 본 논문에서 제안하여 구현한 수혈관리 모바일 헬스케어 시스템을 종합병원의 수혈 과정에 적용한 결과, 안전하고 효율적인 수혈이 가능하여 수혈 과정에서 발생하는 사고의 예방이 가능한 것으로 확인되었다.

고혈압 환자들의 관점에서 본 혈압관리에 대한 지식, 태도, 실천: 질적연구 (The Knowledge, Attitude and Practice of Blood Pressure Management from the Patient's Viewpoint: A Qualitative Study)

  • Objectives: This study adopted a qualitative method to explore the layman's beliefs and experience concerning high blood pressure and its management in order to develop a strategy to increase adherence to proper medical treatment. Methods: Semi-structured interviews that focused on personal experiences with hypertension and its management were conducted with 26 hypertensive patients. The participants were selected according to a BP above 140/90 mmHg (hypertension stage 1), based on the seventh report of the Joint National Committee on prevention, detection, evaluation and treatment of high blood pressure (JNC-VII). The interviews lasted for approximately 30 minutes (range: 20-60 minutes). The resulting questions were formulated into open-ended questions. The interview questionnaire was composed 17 items to examine non-adherence to treatment and 19 items to examine adherence to treatment. Results: Most participants recognized that the direct cause of high blood pressure was unhealthy behavior rather than inheritance. Thus, the hypertensive patient believed they could recover their blood pressure to a normal level through removing the direct cause of hypertension (weight reduction, diet, exercise) instead of taking drugs. The reasons for these statements were that the drugs for controlling hypertension are not natural or they are artificial, and they may have side effects, and drugs are not treatment for the root cause of hypertension. Most of the hypertensive patients chose to manage their behaviors as soon as they knew their blood pressure was high. Therefore, we should not divide the subjects into two groups according to their taking drugs or not, but they should be divided into two groups according to their willingness or not to manage their condition. Conclusions: For developing a strategy for an individual approach to hypertension management, we need to develop a client-centered attitude and strategy. That is, we need to tailor our approach to individual cases to avoid generalizations and stereotyping when developing an adherence increasing strategy.

The Effect of Public Health Center-Based Hypertension School on Hypertension-related Knowledge, Self-efficacy, Anthropometric Value and Blood Pressure

  • This study is to identify the effects of hypertension management program at a community health center on the disease-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. This study is a quasi-experimental study using nonequivalence control group no-synchronized design in order to verify the effects of the hypertension management program at a community health center on the hypertension-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. The result indicated a significant difference between the experimental group and control group in the scores of hypertension-related knowledge (t=-4.25, p<.001), self-efficacy (t=-4.20, p<.001), systolic blood pressure (t=7.70, p<.001) and diastolic blood(t=5.91, p<.001), body weight(t=2.32, p=.026) and abdominal circumference(t=2.17, p=.036). The hypertensive patients' knowledge and self-efficacy were improved, and their weight and abdominal circumference as well as systolic blood pressure and diastolic blood pressure were reduced. Therefore, it was confirmed that multilateral approaches in terms of physical and psychosocial aspects only targeting hypertensive patients were required for managing hypertensive patients in local community.

심혈관 수술 신생아의 혈당수준에 따른 환아 결과 (Patient Outcomes according to Blood Glucose Level in Neonates with Cardiovascular Surgery)

  • Purpose: The purpose of this study was to provide basic data for suitable neonate blood glucose maintenance by investigating and analyzing the blood glucose level of post cardiovascular surgery neonates in the ICU for the effect of blood glucose levels on the following outcomes; ICU stay, hospital stay, mechanical ventilation time, morbidity, and mortality. Methods: The participants were 143 neonates in the ICU after having had cardiovascular surgery. The design for this study was an investigation of the blood glucose levels of the neonates and retrospective analysis of patient outcomes according to blood glucose level. Results: The results for the neonate groups showed that the factors of hospital stay, ICU stay, mechanical ventilation time and mortality, for the group with a blood glucose level over 140 mg/dL were longer and higher than for the group with blood glucose of less than 100 mg/dL or the group between 100-139 mg/dL. Conclusion: The results of this study indicate that when caring for neonates after cardiovascular surgery, it is important to recognize the influence of blood glucose levels on patient outcomes like hospital days, ICU stay, length of time on mechanical ventilation and mortality. Further, care guidelines for neonates' glucose level management need to be developed.

보건소 등록 고혈압 환자에게 실시한 사례관리 프로그램의 효과 (The Effect of Case Management Program for Hypertensive Patient)

  • Purpose: The purpose of this study was to investigate the effects of case management service for hypertensive elderly patients through the customized visiting home health care program. Method: Non-equivalent control group pretest-posttest design was employed. Data were collected from April 2007 to January 2009. Among a total of fifty subjects, 25 subjects were belonged to the case management group and the others were belonged to the general management group. Results: 1) In experimental group, significant increasing rate of number of people within normal blood pressure by time, but no significant increasing rate of number of people within normal blood neutral fat, total cholesterol, HDL, & LDL. 2) In experimental group, the score of knowledge about disease and healthy life practice were increased on 8 week period but decreased on follow up period. Number of people within normal blood HDL and mean score of healthy life practice were significantly higher than control group on follow up period. Conclusion: Case management service was effective on controlling blood pressure but partly effective on blood lipid, knowledge about disease, and healthy life practice in hypertensive patients. Further study is needed to conduct a periodic re-evaluation of the effect of case management and to verify an optimal interval to provide the case management service.

RFID와 USN을 이용한 혈액 온도관리시스템의 설계 (Design of blood temperature management system using RFID and ubiquitous sensor network)

  • We focus on the solutions to prevent fatal risk to patient's life caused by transfusing blood which is wrong type or has exceeded norm temperature. Also, this study gives priority to the verification of medical safety of using blood, of which management is applied advanced sensor tag technology adopted RFID(Radio Frequency Identification) temperature sensor and USN(Ubiquitous Sensor Network) for temperature management of blood. Therefore, this study can contribute to protect of health of patients who take blood transfusion through construction of basis of new process of blood management.

Improvement of Transfusion Practice in Cardiothoracic Surgery Through Implementing a Patient Blood Management Program

  • Background: In this study, we examined the impact of a patient blood management (PBM) program on red blood cell (RBC) transfusion practices in cardiothoracic surgery. Methods: The PBM program had 3 components: monitoring transfusions through an order communication system checklist, educating the medical team about PBM, and providing feedback to ordering physicians on the appropriateness of transfusion. The retrospective analysis examined changes in the hemoglobin levels triggering transfusion and the proportions of appropriate RBC transfusions before, during, and after PBM implementation. Further analysis was focused on patients undergoing cardiac surgery, with outcomes including 30-day mortality, durations of intensive care unit and hospital stays, and rates of pneumonia, sepsis, and wound complications. Results: The study included 2,802 patients admitted for cardiothoracic surgery. After the implementation of PBM, a significant decrease was observed in the hemoglobin threshold for RBC transfusion. This threshold dropped from 8.7 g/dL before PBM to 8.3 g/dL during the PBM education phase and 8.0 g/dL during the PBM feedback period. Additionally, the proportion of appropriate RBC transfusions increased markedly, from 23.9% before PBM to 34.9% and 58.2% during the education and feedback phases, respectively. Among the 381 patients who underwent cardiac surgery, a significant reduction was noted in the length of hospitalization over time (p<0.001). However, other clinical outcomes displayed no significant differences. Conclusion: PBM implementation effectively reduced the hemoglobin threshold for RBC transfusion and increased the rate of appropriate transfusion in cardiothoracic surgery. Although transfusion practices improved, clinical outcomes were comparable to those observed before PBM implementation.
