The purpose of this study was to identify the care needs of family-caregivers to the patients with stroke. Subjects were 115 family-caregivers caring for the patients while they were in-patients or out-patients with stroke in two general hospitals and one oriental medicine hospital located in Seoul and Kwang-Ju. The instrument used for this study was made by the researcher on the basis of results of literature review and interviews with family-caregivers, composed of 35 items. Internal validity by calculation of cronbach's alpha with data of respondents was 0.91, which was regarded as high. The Data were analyzed by SAS program, with percentage, mean, t-test, and ANOVA. Factor structures of care needs of family-caregivers were elicited by factor analysis(PCA, Varimax rotation). Datum collection had been from July 1 to August 14, 1997. The results of this study were as follows : 1. The mean score of the sum of the care needs of family-caregivers was 3.96 and the highest-mean item was 'need for immediate care(M=4.77)', and the lowest-mean item was 'need for chaplian's visit (M=2.82)'. 2. Care needs of the family-caregivers were : Need to be informed of the disease, treatment and care ; need of education and assistance related to physical functional level ; need of social support and consultation ; need of management of nursing problem related to immobility ; need of appreciation ; need of the way to communicate with patients ; need of immediate care and help. The highest mean factor was the 'need for immediate care and help(M=4.74)', and the lowest mean factor was the 'need of appreciation(M=3.58)'. 3. The variables influencing the degree of care needs perceived by family-caregivers to the patients with stroke were as follows : There were significant differences between need to be informed of the disease, treatment and care and general characteristic factors, which were family caregiver's sex (p=.0178), caring period(p=.0223) and patient's suffering period(p=.0244). There were significant differences between need of education and assistance related to physical functional level and general characteristic factors, which were patient's paralysis(p=.0177), patient's ADL dependency(p=.0032). There were significant differences between need of social support and consultation and general characteristic factors, which were family caregiver's sex(p=.0055), occupation(p=.0159), religion(p=.0093) and patient's sex(p=.0134). There was significant difference in the degree of need of management of nursing problem related to immobility, according to the patient's ADL dependency(p=.0493). There were significant differences between need of appreciation and general characteristic factors, which were family caregiver's age(p=.0107), sex(p=.0133), and patient's age(p=.0338). There were significant differences between need of the way to communicate with patient and general characteristic factors, which were patient's paralysis(p=.0002) and aphasia(p=.0001). There were significant differences between need of immediate care and help and general characteristic factors, which were family caregiver's caring period(p=.0162) and patient's suffering period(p=.0116). 4. The mean score of patient's ADL dependency was 3. 38 and the highest-mean item was 'ascending and descending stairs(M=4.12)', and the lowest-mean item was 'drinking(M=2.60)'. There was no significant difference in the degrees of care needs related to the patient's ADL dependency. 5. The highest information source of family-caregivers was from the doctors about the disease, treatment and care(26.1%). The second highest one was from mass media(20.8%), and the third one was from the nurses. The above findings may be used as the basic data to seek more efficient way of elevating nursing practice and quality for family-caregivers to the patients with stroke.

Dypepsia is the most common gastrointestinal disease and mainly occurs by stress or food. According to the cause of dyspepsia, the clinical characteristics and the curative process are different in patients. The purpose of this study is to compare acute dyspepsia patients depending on food factor and stress factor. For this study, we analyzed the cause, and the symptoms of 59 acute dyspepsia patients and divided them into two groups: one group is 27 acute dyspepsia patient group caused by stress factor (stress factor group). The other group is 32 acute dyspepsia patient group caused by food factor (food factor group). In addition, we carried out HRV test and compared HRV index between two groups. There was no significant difference in past illness involved in gastrointestinal tract diseases between two groups. There were big differences in the frequency of chest discomfort, thurst, lack of appetite, and exhaustion after meals between two groups. There were many changes of tongue fur in food factor group. But there were many changes of tongue body in stress factor group. The pulse pattern is mostly wiry in stress factor group. But, The pulse pattern is mostly slippery in food factor group. HF, TP were significantly lower than food factor group in stress factor group, it means autonomic nerve system was more suppressd in stress factor group than food factor group. Food factor group improved sooner than stress factor group. This study presented the clinically different characteristics (past medical history, symptoms, tongue diagnosis, pulse pattern, curative process) of acute dyspepsia depending on food factor and stress factor. Thus, it provides the necessity of oriental medical diagnosis and treatment called pattern identification for acute dyspepsia.

Purpose: The purpose of this study was to investigate provide basic data for improving patient safety in nursing homes in Korea by measuring the patient safety culture of nursing homes and understanding its influencing factors. Methods: This study is a secondary analysis of a descriptive research study using data from development and validation of the Korean patient safety culture scale for nursing homes. A total of 982 cases were analyzed using the SPSS Statistics 20 program. Results: For the safety culture of the patient, there was a significant difference based on the size and location of the facility. For the degree of patient safety, age, education, occupation, marital status, and the size of the nursing home were significant factors. Patient safety culture and the degree of patient safety had a positive correlation. The regression model of the degree of patient safety was significant (F=20.73, p<.001) and the explanatory power of the model was 27.4%. Conclusion: The study results indicate that patient safety culture is a factor influencing safety of elders in nursing homes. To improve patient safety for nursing homes in Korea, continuous evaluation and improvement projects need to be done at a national level.

Purpose: The study was conducted to investigate the correlation between perception of patient safety risk factors, patient safety competency, and safety nursing activities of nurses in hemodialysis units and identify factors affecting patient safety activities. Methods: The participants were 146 nurses from 16 hemodialysis units located in Gyeongsangnam-do. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis using the SPSS, version 24.0. Results: The mean safety nursing activity score was 3.47±0.38. safety nursing activities of the participants were significantly correlated with patient safety competency. The characteristics showing significant differences in safety nursing activities were educational level, hospital type, hospital work experience, number of hemodialysis treatment per day, number of hemodialysis treatment per nurse, educational experience of patient safety, presence of a patient safety incident report registration system, and direct registration of patient safety incident report. The multiple regression analysis revealed that the factors influencing safety nursing activities were patient safety incident report, patient safety competency, and number of daily hemodialysis treatment (<5~7 times/day) per nurse (R2=.34). Conclusion: The results of this study suggest that the safety nursing activities of hemodialysis unit nurses should be intensified. In addition, the registration system of patient safety incident report and nurses' competency on patient safety should be improved, and the number of hemodialysis per nurse should be fewer than 7 times per day.

The study aims to analyze the factors that shape patient loyalty, namely, by involving the service quality factor (SERVQUAL), hospital image, patient value, and patient satisfaction in private hospitals. This study was conducted in Makassar City, Indonesia, with a sample of 296 eligible samples from private hospitals. The sample criteria were patients with outpatient and hospitalization status. Then, this study developed 23 hypotheses to test the statistical relationship between direct, intervening and multiple-effect models. Problem-solving and research focus are carried out using a quantitative method approach with a PLS-SEM-based testing tool. The bootstrapping method is being used with the constant bootstrapping step to demonstrate the results of hypothesis testing; we find that the overall hypothesis has a positive and significant effect. The combination of testing models involving several variables shows that a patient's loyalty can be formed if a patient's satisfaction has been realized. Satisfaction can be realized if the value-customer has been felt by the patients. Therefore, the hospital image must be directly proportional to service quality. Service quality is the essence of service that directly affects customers; service quality is also the reason that shapes consumer perceptions in increasing rationalization and solid customer (patient's) decision-making.
Purpose: This study was undertaken to present an effective plan for the development of an educational program and a strategy to promote patient safety management activities for nursing students by identifying factors that affect these activities based on the theory of planned behavior. Methods: A self-report questionnaire was distributed to 300 nursing students who had clinical practice experience at three nursing colleges in Daejeon, Gyeongbuk, and Jeonbuk. The significance of the model fit, and the path effect was confirmed by confirmatory factor analysis. Results: The hypothetical model for patient safety management activities was appropriate. Among the 5 pathways, 4 were significant. It was found that behavioral intention had a direct influence on patient safety management activities, and perceived behavioral control and attitude had an influence on behavioral intention. Conclusion: To strengthen the perceived behavioral control of nursing students' patient safety management activities, it is necessary to analyze and remove obstacles and provide education that reflects the characteristics of the subject's health problems. In addition, through self-directed learning involving simulation practice, nursing students should be exposed to patient safety accidents, so that they can recognize the risks early and solve problems through critical thinking while bringing about the necessary changes in their attitude.
The purpose of this study was to develop a patient safety nursing activities scale for clinical nurses and to verify validity and reliability it. A preliminary scale was developed through various processes and pilot study. Data were collected from 900 nurses at 28 hospitals selected by proportionate stratified sampling with 331 general hospitals nationwide. The final scale was a 4-point scale, consisting of 72 items in 10 domains. The construct validity was found to be appropriate by exploratory factor analysis and 70.3% of the variance in the total scale and over .74 of factor loading. There was a significant correlation between scale and patient safety culture (r=.52, p<.001) and Cronbach's ${\alpha}$ was .99. This scale developed to measure patient safety nursing activities of clinical nurses was found to be reliable and valid and will be a useful tool in practical field.
The purpose of this paper is to identify factors affecting the optimum mix of required inputs and other relevant factors which account for the variation in physician's productivity in general hospitals, and to find out their implications for the efficient health planning and management. An extended version of Cobb-Douglas production function and cross sectional data of one day patient census from all general hospitals in Korea in 1988 were used in the analysis. Main results of the analysis and their implications could be summarized as follows : (1) The production function for physician's inpatient service shows the evidence of economies of scale, but the production function for physician's outpatient and adjusted-patient service, which combines both out- and in-patient service, shows that of dis-economies of scale. (2) The physician's role for production for all service is smaller than auxiliary personnel's, which imply that more intensive utilization of nurses, nursing aides and other auxiliary personnel is desirable for improving general hospital productivity (3) In case of physician's inpatient and adjusted-patient service, nurses' role is greater than nursing aides'. Therefore, more extensive utilization of nurses is recommended for the efficient operation of general hospitals. (4) The factor of hospital beds plays the leading role among required inputs in the production for physician's in- and adjusted-patient service. (5) The physician's productivity of general hospitals in rural area is lower than that in urban area. And the productivity of teaching hospitals is lower than that of the other hospitals. Further analysis was made in physician production function based upon the size of hospitals, namely those hospitals below 250 beds and those above. Explained variances by the factor of hospital beds was significantly increased in the case of those hospitals above 250. A more detailed and thorough investigation is needed for verifying factors influencing physician's productivity in general hospitals in Korea.

This study examines the relationship between internal marketing, market orientation, patient satisfaction, and hospital image, and especially focuses on the effect of CEO's role on internal marketing at a local national university hospital. A survey was conducted using the convenient sampling technique and 222 questionnaires excluding unreliable replies were used in the final analysis for the hypothesis testing. SPSS 21.0 was used for the basic analysis of the collected data, and confirmatory factor analysis was performed for reliability and validity using AMOS 21.0. Path analysis was performed for the hypothesis testing. The results of this study are as follows: First, the role of CEO positively affects internal marketing. Second, internal marketing has a positive effect on market orientation, and leadership is the most influential factor of internal marketing. Third, market orientation has a positive effect on patient satisfaction and hospital image, which are non-financial organizational performance. Therefore, internal marketing plays a major role in improving market orientation, patient satisfaction, and hospital image, and it is identified that the activation of internal marketing depends on the support of CEO in hospitals.
Purpose: The purpose of this study was to identify clinical decision making pattern of pediatric nurses and analyze how it shows the differences in types of decision making pattern by nurses characters. Methods: A self-administered questionnaire was used to pediatric nurses of 4 general hospitals in Seoul from February 2004 to April 2004. The data of 251 nurses was analyzed by varimax rotation factor analysis, t-test, and ANOVA. Results: 6 decision making patterns were identified: Individual Patient-oriented, Pattern-oriented Intuitive, Typical Nursing Knowledge-oriented, Nursing Model-oriented, Medical Knowledge-oriented, and Patient-Family-Nurse Collaborative. Individual Patient-oriented, Pattern-oriented Intuitive, Typical Nursing Knowledge-oriented, and Nursing Model-oriented decision making pattern got meaningful differences in age, marital status, total number of years in nursing practice, and number of years in pediatric nursing practice. Conclusion: We expect the result of this study can be applied for promotion of understanding the decision making of nurses that occurs in pediatric nursing practice and also can be used as foundation data for development and expansion of pediatric nursing practice.
