The Impact of Doctors' Communication Styles on Patient Satisfaction: Empirical Examination (의사의 커뮤니케이션 스타일이 환자만족에 미치는 영향에 관한 연구)

  • These days, the environment of hospital marketing is changing rapidly. The level of expectation and demand of patients have become greater and more diversified, and patients have more alternatives in selecting hospitals. The standard of hospital selection and the type of using hospital have been changed, and competition among hospitals has been accelerated due to the opening of the medical market through globalization. Accordingly, differentiation strategies are critical in hospital marketing. The quality of medical service oriented toward patient satisfaction becomes a strong strategic weapon to secure a hospital's competitive advantage. Therefore, marketing and communication strategies should be focused on patient-oriented, rather than hospital-oriented. Considering the changes in the hospital environment and the increase in the patients' expectation level, this study categorizes doctors' communication styles into four different ones: trust-type, professional-type, cooperation-type, and control-type. The effects of these communication styles on patient satisfaction were empirically examined. The moderating roles of the patient's characteristics and clinical characteristics between the doctors' communication styles and patient satisfaction were also investigated to find out managerial implications for hospital management. To achieve such goals, data were collected from patients of 12 general hospitals in Busan. The data were analyzed to test research hypotheses that examine 1) the relationships between doctors' communication styles and patient satisfaction, 2) the moderating roles of the patient characteristics and clinical characteristics in the research model, and 3) the impact of patient satisfaction on positive word-of-mouth and repurchase. The following summarizes the major results of this research. First, the data showed that patient satisfaction varied across doctors' communication styles. Trust-type style had the strongest impact on patient satisfaction while control-type style had the weakest influence on patient satisfaction. Professional-type style and cooperation-type style also had positive effects on patient satisfaction but the impact of the two are not statistically different. Second, significant differences in terms of patient satisfaction were found depending upon demographic variables such as gender, marital status, age, occupation, and education. Patient satisfaction, however, was consistent across varying income groups. Third, patients' medical insurance types were also related to patient satisfaction. It implies that a doctor may need to use different communication styles depending on a patient's medical insurance type. Fourth, out-patient and in-patient showed a different level of satisfaction with varying communication styles. Fifth, highly professional knowledge and strong control can influence patient satisfaction depending on the characteristics of the patient treatment field. Sixth, patient satisfaction were proved to have significantly positive effects on word-of-mouth and repurchase. The implications drawn from this study must be tempered by its limitations. First of all, the subjects used in this study were patients in Busan and small- and medium-size hospitals were excluded from the research. Therefore, future research should examine the research model by using a variety of hospitals and clinics throughout Korea. Another research agenda has to do with finding more determinant and moderating variables which will increase an explanatory power of the model. In short, this study may be the first empirical research that investigates the effects of doctors' communication styles on patient satisfaction. Interestingly enough, the results showed that each communication style had a unique impact on patient satisfaction. The findings from this research can be very useful in developing hospital marketing strategies.

Comparison of Patient Needs, Patient Satisfaction, and Communication Type of Nurses between Integrated Nursing Care Service Ward and General Ward Patients (간호·간병통합서비스병동과 일반병동 환자의 간호요구도, 간호만족도 및 간호사의 의사소통 유형 비교)

  • Purpose: The purpose of this study is to provide patient-centered nursing by comparing the patient needs, patient satisfaction, and communication type of nurses for patients admitted to integrated nursing care service ward and general ward. Methods: Data were collected by self-report questionnaires between September 7 and October 9, 2021. Patients were patients recruited from a hospital in D city, 100 each from comprehensive nursing care service ward and general ward. The data were analyzed using descriptive statistics, 𝜒2-test, t-test, ANOVA, and Pearson's correlation. Results: There was no significant difference in patient needs between the patients of integrated nursing care service ward and the general ward (t=-1.12, p=.263). However, patient satisfaction was higher among the patients in integrated nursing care service ward than in the general ward (t=-3.55, p<.001). Pertaining to the communication type of nurses, the informational communication type (t=-4.02, p<.001) and the friendly communication type (t=-3.92, p<.001) were stronger in the integrated nursing care service ward than the general ward. It was, hence, confirmed that the educational requirements were positively correlated with patient satisfaction in the reliability domain and technical-professional domain, and that the nurse informational communication type and friendly communication type had a significantly positive correlation with patient satisfaction. Conclusion: The expansion of the integrated nursing care service ward may be considered in the future, with an informational and friendly communication method as an effective response that can be adapted in practice as a reflection the type of communication among nurses.

A Study on Satisfaction Evaluation of Korean Medicine Therapy Using Treatment Tool by Psychological Type (심리유형별 진료도구를 이용한 한방치료의 만족도 평가 연구)

  • Objectives : This study was designed to evaluate patient satisfaction of Korean medicine therapy using treatment tool by psychological type. Methods : 48 subjects were participated in this study. Before a treatment, we analyzed a patient's phychological type first by using a questionnaire. And then we divided 3 groups which were called a rational sympathy module group, emotional sympathy module group, control group. Manner of medical consultation according to patient's phychological type was carried out in all groups. After all treatment, we conducted a survey about medical service perception index, patient happiness index, medical satisfaction index. To evaluate a satisfaction degree, we analyzed results of survey statistically. Results : The results of the analysis, both rational sympathy module group and emotional sympathy module group got a higher score than control group statistical significantly on medical service perception index. Emotional sympathy module group got a higher score than control group statistical significantly on patient happiness index and medical satisfaction index. But rational sympathy module group only had a higher tendency than control group on patient happiness index and medical satisfaction index. Emotional sympathy module group got a higher score than rational sympathy module group on medical service perception index, patient happiness index and medical satisfaction index. Conclusions : Korean medicine therapy using treatment tool by psychological type consisted of patient's phychological type questionnaire and manner of medical consultation could improve a patient's satisfaction. It was more effective in emotional sympathy module group than rational sympathy module group.

Subjectivity on Patient Safety Recognized by Inpatients

  • The purpose of this study is to identify the subjectivity of patient and describe the characteristics of each type to understand the categorization of types on patient safety. Q methodology was applied to the study. A total of 40 patients admitted to the S hospital in Seoul were asked to categorize 33 statements on patient safety. The collected data was analyzed using QUANL PC Program. After analysis, patient safety as perceived by the admitted patient was categorized into 6 types. The types were as follows: Type 1 'Those who note adequate patient safety', Type 2 'Those who consider preventive safety to be lacking' Type 3 'Those who see the evaluation criteria to be met', Type 4 'Those who see the facility safety to be lacking', Type 5 'Those who find the patient and facility safety to be adequate', Type 6 'Those who see patient support to be lacking'. The study provides a basic set of data for developing mediation measures needed to identify the direction in which patient safety should be promoted.

  • Complex regional pain syndrome Type II(CRPS) can be diagnosed by new IASP criteria in 1994. Sympathetically maintained pain may or may not be present in a patient with complex regional pain syndrome. We experienced a CRPS Type II patient who has sympathetically maintained pain as a major painful nature developed after right multiple iliac bone fracture, right femoral artery thrombosis and lumbosacral plexus injury. Combination treatment with L2, L3, L4 sympathetic ganglion block and continuous lower thoracic epidural block for 30 days were tried to get long term effect. The patient had signs of successful. sympathetic denervation of the right foot. After that pain relief was sustained until three month later.

Use of Single Bedrooms versus Multiple Bedrooms: Selection Factors and Comparison of Patient Satisfaction (1인실과 다인실 입원 병상 이용에 관한 병상 선택 요인과 환자 만족도 분석)

  • Background: It is widely acknowledged that single bedrooms have many potential advantages compared to multiple bedrooms. However, Korea has a reimbursement system that patients have to pay the additional fee if they will use single bedroom or pay-bed (1-3 bedroom). There is little research on patients' bed selection and relationship between patient satisfaction and bed type in the Korean setting. Methods: Using the 2017 Korea Health Panel (KHP) Survey data, we modified bed type by two dichotomous variables: single bedroom vs. multiple bedroom (2+ bedrooms) and pay-bed (1-3 bedroom) vs. reimbursed bed (4+ bedroom). Multivariate logistic regression is performed to determine the factors affecting the patient's choice of room types. Multivariate regression analysis was conducted to examine how hospital room types are affecting patient satisfaction. Results: Single room and pay-bed (1-3 bedroom) use was influenced by patient age (19- years old), male, a person at work, hospitalizing in a clinic, and birth. After controlling variables of the behavioral model of medical utilization, the association between patient satisfaction and use of single bedroom & pay-bed (1-3 bedroom). Cause of hospitalization, major treatment, and recognition of unnecessary care are statistically significant variables on patient satisfaction. Conclusion: Although the single room is about the standard for newly built hospitals in western countries, it remains unclear that single room or pay-bed (1-3 bedroom) has positive effects on patient's outcomes and satisfaction. In this empirical study, the difference in patient satisfaction by bedroom type was not noticeable. In terms of bed management by hospital staff, securing patient credibility in hospital care is an important task. Patients' perception of whether medical staffs have encouraged unnecessary treatment or tests has a significant impact on patient's satisfaction.

Evaluation of Muscle Load and Fatigue According to the Shape of Severe Dementia Patients' Clothing (중증 치매환자복 형태에 따른 근육 부하 및 피로도 평가)

  • The purpose of this study is to obtain information necessary for the development of patient clothes that can reduce physical fatigue of caregivers by quantitatively measuring the muscle load and fatigue. The patient clothes used in this study can be broken down into three types: A type (back center zipper open suit), B type (top-to bottom separated patient clothes), and C type (front zipper open suit). The EMG measurement sites are as follows: hand muscle (brachioradialis), upper arm (biceps, triceps), shoulder (anterior deltoid, medial deltoid, posterior deltoid, upper trapezius), and waist (erector spinae); additionally, the EMG signals were measured. Through this experiment, muscle load, muscle energy consumption, and muscle fatigue generation tendency were analyzed. The results of the study revealed that the C type patient clothes required the most strength in the muscles of the shoulders, upper arms, hands, and back when being put on and taken off compared to other patient clothes. The A type clothes required a relatively large force in opening the zipper. In terms of muscle energy consumption, B type generally called for more strength when it came to the zip-up and putarmsup motions. With regard to the cover the body and put legs/hips up motions, C type used the highest amount of muscle energy, whereas A type used relatively little energy. In terms of the occurrence of muscle fatigue during the putting on and taking off of the patient's clothing, there was a difference in the area and degree of muscle fatigue in the A, B, and C types, and there was also a tendency for muscle fatigue to occur when performing repetitive movements.

A Case Report of Systemic Type Juvenile Rheumatoid Arthritis with Fever and Eruption (고열, 발진을 동반하는 전신형(systemic) 소아기 류마티스 관절염(juvenile rheumatoid arthritis; JRA) 환아 증례 1례 보고)

  • Objectives This study is to report a case that has an important meaning as a result of treating Juvenile Rheumatoid Arthritis. We investigated a patient who had to maintain his life with western medicines such as DMARDs, NSAIDs and steroids for a long time. The patient has recovered from all symptoms and his ESR, CRP has been back to normal range with oriental medicine treatment. Methods The patient had fever, especially repeated fever during the afternoon, pain and swelling of joints, generalized skin eruption, anorexia, delayed growth, weight loss, fatigue. So we treated him with herbal medicine and reduced his western medicine. The aim of treatment was recovery from Juvenile Rheumatoid Arthritis after discontinuance of all western medicine. Results The symptoms of systemic type Juvenile Rheumatoid Arthritis was vanished and the patient maintains his condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. His ESR and CRP levels were back to the normal range. After this treatment the patient's height and weight has been increased which showed a significant meaning in growth to the child. Conclusions This study showed that oriental medicine can elevate the Juvenile Rheumatoid Arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.

Surgical Treatment of Thoracic Aortic Aneurysm - Surgical Approach and Perioperative Problems - (흉부 대동맥류의 외과적 치료외과적 접근 및 술후 문제점)

  • 12 Patients with thoracic aortic aneurysm were operated between May 1985 to Sept. 1987 at the our department, Hanyang University Hospital. We retrospectively evaluated the surgical results and considered diagnosis, surgical approach and perioperative problems of thoracic aortic aneurysm. There are 9 males and 3 females in the patients. The age ranged from 23 to 61 years with the mean age at 40.6 years. The cause of the aneurysm was atherosclerosis in 5, Marfan`s syndrome in 4, syphilis In 1, trauma in 1 and annuloaortic ectasia in 1 case. According to DeBakey`s classification, Type I was 1 case, Type II was 5 cases and Type III was 6 cases. Among 6 patients with ascending aortic aneurysm, Bentall`s operation in 4 cases and ascending aorta reconstruction using to Dacron Tube Graft in 2 cases were performed successfully. 6 cases with descending aortic aneurysm were managed by prosthetic graft replacement. Chylothorax was observed in 1 patient and postoperative hemorrhage necessitating reopening of the chest occurred in 4 of operative survivors. There were 2 hospital deaths; one patient was dead during the operation and one patient was dead during the post-operative course due to low cardiac output syndrome

A Case Report of Taeeumin Patient with Diabetes Treated with Yeoldahansotang-Gamibang (태음인 당뇨 환자의 열다한소탕가미방 치험 1례)

  • Objectives The aim of this study is to report the improvement in Taeeumin's dryness-heat symptom of a 54-year-old man diagnosed with type-II diabetes mellitus after treated by herbal medicine. Methods The Taeeumin patient took Yuldahansotang-gamibang for 9 months to manage Taeeumin's dryness-heat symptom. We examined the changes of Body Mass Index(BMI), Fating Blood Sugar(FBS), Hemoglobin A1c(HbA1c), and ketouria. Results Type-II diabetes mellitus symptoms in the patient with Taeeumin's dryness-heat symptom were thirst, fatigue, and increased body weight. All numerical levels for blood sugar, HbA1c, and ketouria were reduced after taking Yuldahansotang-gamibang. Conclusions The treatment of the patient with Taeeumin's dryness-heat symptom and type-II diabetes mellitus with Yuldahansotang-gamibang was efficient, and further research is needed to prove the effect of Yuldahansotang-gamibang for treating such patients.
