삼종(三種)의 복합한약제(複合韓藥劑) 투여(投與)가 요중(尿中) 5-Hydroxyindoleacetic Acid에 미치는 영향에 관(關)한 연구(硏究) (Effect of the Three Recipes Treatment on the Urinary 5-HIAA Excretion)

  • Serotonin was chemically identified as 5-hydrowytryptamin which occurs in plants, animals, and human beings. The end product of metabolism was excreted as 5-HIAA in urine. Many scientists, specially biochemist and psychiatrist reported that the change of serotonin concentration caused mental disorder and pathological condiions such as schizophrenia and carcinoid. This study was carried out to observe the urinary 5-HIAA excretion rates changed by three recipes treatment (Kamisoyosan, Ondamtang, kalgunhaegitang) according to the classification of endogenous, exogenous and non-exo-endogenous, causes of disease. The urinary 5-HIAA excretion rates before and after three recipes treatment on normal groups divided into three groups on the basis of physical constitutional differences and on patients groups, divided into three groups according to the causal factor were measured. The results were as follows; 1. The urinary 5-HIAA volume of patients groups appeared non-exoendogenous, endogenous, exogenous causes of disease in order of three causative classification. 2. The urinary 5-HIAA volume of Normal groups was $2.50{\pm}088mg/24hrs$(range $0.30{\sim}6.90mg/24hrs$) 3. Thr urinary 5-HIAA volume was $3.70{\pm}0.89mg/24hrs$(range $0.90{\sim}6.50mg/24hrs$) before kamisoyosan treatment and $3.20{\pm}0.72mg/24hrs$ (range $0.80{\sim}6.20mg/24hrs$) after the treatment. 4. The value was $2.60{\pm}0.10mg/24hrs$ (range $0.60{\sim}6.50mg/24hrs$) before Ondamtange treatment and $2.00{\pm}0.12mg/24hrs$(range $0.20{\sim}6.10mg/24hrs$) after the treatment. 5. The value was $4.30{\pm}0.75mg/24hrs$(range $0.92{\sim}6.50mg/24hrs$) before kalgunhaegitang treatment and $3.10{\pm}0.10mg/24hrs$(range $0.80{\sim}5.80mg/24hrs$) after the treatment. Considering the above-mentioned the study on the changes of urinary 5-HIAA volume will make a significant contribution to the diagnosis and the evaluation of therapeutic effect successive research and modified application will be in need.

쥐에서 Di-(2-Ethylhexyl) Phthalate로 유발된 정자 형성 장애에 대한 Vitamin E와 Catechin의 예방 효과 (Preventive Effects of Vitamin E and Catechin on Spermatogenic Disturbance Induced by Di-(2-Ethylhexyl) Phthalate in Rat)

  • 본 연구에서는 수컷 랫드에 DEHP를 투여하여 실험적으로 생식 독성을 유발하고, vitamin E와 catechin을 단독 및 병용 투여하여 수컷 랫드에서 정소의 조직학적 변화, 정액 특성의 변화 및 정자의 운동성 변화 등을 조사하여 그 예방 효과를 알아보고자 하였다. DEHP를 투여한 군은 대조군에 비해 정자농도, 정자 생존율, 정상 정자율이 감소하였다. DEHP에 의한 생식 독성을 예방하기 위해 vitamin E와 catechin을 단독 또는 병행 투여한 결과, DEHP를 투여한 군에 비하여 정자 농도, 정자 생존율, 정상 정자율이 증가하였다. 이러한 결과를 종합해 볼 때, catechin과 vitamin E의 투여는 DEHP에 의한 생식 독성을 예방하는 효과가 있는 것으로 사료된다.

  • through a literal study upon Arrhythmia between east and west medicine, the results were as follows 1. Arrhythmia is not only irregular cardiac beat but disorder of cardiac impulse making and conduction disturbance. it means almost irregularity of interval between two beat 2. In the east medicine, arrhythmia is represented various category as palpitation, continuous palpitation, dizziness, consumption which give the first consideration by the accompanied symptoms. A separate way in a diagnostics it become a base of diagnosis of diseases and decision of prognosis by the examination of pulse 3. In the west medicine, Arrhythmia is classified as disorder of heart rate, rhythm, conduction disturbance or tachycardia, bradycardia and it is concomitant with fatigue, palpitation, dyspnoea, syncope, chest discomfort 4. The diagnostic study of pulse condition which represent arrhythmia was started from $\mathbb{<}$Nae-Kyung$\mathbb{>}$ and it was revealed as pulse condition of rapid pulse, slow pulse, swift pulse, running pulse, knotted pulse, intermittent pulse etc. Out of them running pulse, knotted pulse, intermittent pulse which obviously are concomitant with irregularity of interval are clinically meaningful in a conditions of disease and decision of prognosis. and the significance of these pulse condition are transformed through the changes of the times 5. According to cause of disease it is classified by Arrhythmia(running pulse, knotted pulse, intermittent pulse) due to heat, cold, phlegm, deficiency(or insufficiency) and There are three categories of etiological factor that is, endogenous, exogenous and non-exo-endogenous factor. the endogenous factor is insufficiency of the heart Ki, deficiency of both Ki and blood, intemal stagnation of phlegm and fluid, stagnation of seven emotions. the exogenous factor is caused by stagnation of Ki and blood by six exogenous pathogenic factor and the non-exo-endgenous factors are improper diet, overstrain, traumatic injury. A cause of arrhythmia in western medicine are a organic and pathological change of the heart itself and malfunction of the autonomic nervous system.

위암환자의 위장조영검사 영상분석 (Image Analysis on Upper Gastrointestinal(UGI) Series of Gastric Cancer)

  • 위절제술을 시행한 위암환자를 대상으로 수술 전 위치확인을 위한 위장조영검사의 영상을 분석하여 위암묘출의 정확도 및 한계성을 알아보고, 이에 따른 대응책을 알아보고자 하였다. 2007년 10월부터 2009년 4월까지 본원에 내원하여 병리조직학적으로 위암으로 확진되어 위절제술을 시행한 조기위암 환자 88명과 조기위암과 비슷한 진행성위암 환자 14명을 대상으로 하여, 수술 전 시행했던 위장조영검사와 수술 후 병리학적 소견을 비교분석하였으며, 영상분석은 10년 이상 근무한 방사선사 2명이 분석하였다. 위암환자 102명의 위절제 후 병리학적 위암위치와 비교를 통한 위장조영검사에서 위암의 발견율은 "확실" 81 예(79.4%), "실패" 21예(20.6%)로 판정되었다. "실패"로 판정된 21예의 위암형태는 IIb+IIc형이 2예(100%), IIb형이 5예(83.3%)로 발견율이 매우 낮았다. 위암의 전구병변인 장형화생은 위장조영검사에서 92예 (90.2%)의 발견율을 보였다. 위장조영검사의 한계점은 IIb형 및 1.0cm이하 미소위암의 발견이 어려웠다. 위암과 동반된 장형화생 92예(90.2%)의 발견은 위장조영검사에서 위암의 발견에 중요한 기준으로 평가된 다. 위장조영검사의 한계점을 극복하기 위해서는 부위에 맞는 정확한 촬영법 및 풍부한 방사선학적 경험을 통한 미세한 점막의 변화를 확인하는 촬영습관이 위암을 발견하는데 기여할 것으로 사료된다.

이락촌폐흡충에 감염된 흰쥐의 폐 병변에 대한 프라지콴텔의 투여 효과 (Effect of praziquantel treatment on pulmonary lesions of rots infected with Payagonimus iloktsuenensis)

  • 흰쥐에 이락촌폐흡충(Paragonimus iloktsnenensis)을 감염시키고 폐흡충중의 특성과 진행 과정을 조사한 다음 praziquantel 투여가 폐 병변에 미치는 대과를 관찰하였다. 이락촌폐흡충 피낭 유충을 흰쥐에 감염시킨 바 전례에서 폐장에 병변을 유발하였고 육안적으로 폐 표면으로 돌출하는 출혈성 및 결절성 충낭들이 관찰되었으며, 현미경적으로 충낭과 아울러 폐실질 및 폐포 내에 충란에 의한 육아종 형성, 국소적 무기폐 및 염증 반응을 나타내었고, 병변 주위에는 육아조직과 결체조직의 증가가 동반되었다. 이락촌폐흡충 감염 5주 후에 prasiquantel을 투여한 바, 3일 후부터 이미 충체가 손상을 받아 지행성 변화를 보이기 시작하였고, 그 후로 충체가 점차 사멸, 소실되어 21일 후에 는 충체를 찾아볼 수 없었다 일부 흰쥐에서는 투약 후 14일까지 충체가 살아있는 경우도 관찰되었다. 대부분의 흰쥐에서는 충체가 사멸됨에 따라 폐장의 병변도 회복되기 시작하였는데 충남의 크기가 줄어들고, 충체가 흡수되며, 주위의 염증 반응과 무기폐 현상도 감소되었고, 산란양이 적어 충란 육아종 형성도 감염 대조군에 비해 경미하였다. 이상의 결과로 볼 때 흰쥐에 이락촌폐흡충을 감염시키면 인체의 폐흡충증과 거의 동일한 병변을 일으키며, praziquantel을 투여하면 1차적으로 충체가 손상, 사멸되면서 2차적으로는 폐 병변이 빠른 시간에 잘 복구되므로 결국 폐흡충증을 치유시키는데에 탁월한 효과가 있음을 알 수 있었다.

Beyond Clot Dissolution; Role of Tissue Plasminogen Activator in Central Nervous System

  • Tissue plasminogen activator (tPA) is a serine protease catalyzing the proteolytic conversion of plasminogen into plasmin, which is involved in thrombolysis. During last two decades, the role of tPA in brain physiology and pathology has been extensively investigated. tPA is expressed in brain regions such as cortex, hippocampus, amygdala and cerebellum, and major neural cell types such as neuron, astrocyte, microglia and endothelial cells express tPA in basal status. After strong neural stimulation such as seizure, tPA behaves as an immediate early gene increasing the expression level within an hour. Neural activity and/or postsynaptic stimulation increased the release of tPA from axonal terminal and presumably from dendritic compartment. Neuronal tPA regulates plastic changes in neuronal function and structure mediating key neurologic processes such as visual cortex plasticity, seizure spreading, cerebellar motor learning, long term potentiation and addictive or withdrawal behavior after morphine discontinuance. In addition to these physiological roles, tPA mediates excitotoxicity leading to the neurodegeneration in several pathological conditions including ischemic stroke. Increasing amount of evidence also suggest the role of tPA in neurodegenerative diseases such as Alzheimer's disease and multiple sclerosis even though beneficial effects was also reported in case of Alzheimer's disease based on the observation of tPA-induced degradation of $A{\beta}$ aggregates. Target proteins of tPA action include extracellular matrix protein laminin, proteoglycans and NMDA receptor. In addition, several receptors (or binding partners) for tPA has been reported such as low-density lipoprotein receptor-related protein (LRP) and annexin II, even though intracellular signaling mechanism underlying tPA action is not clear yet. Interestingly, the action of tPA comprises both proteolytic and non-proteolytic mechanism. In case of microglial activation, tPA showed non-proteolytic cytokine-like function. The search for exact target proteins and receptor molecules for tPA along with the identification of the mechanism regulating tPA expression and release in the nervous system will enable us to better understand several key neurological processes like teaming and memory as well as to obtain therapeutic tools against neurodegenerative diseases.

비스테로이드성 항염증제와 효소 억제제에 의한 사람 중성구 Elastase의 활성도 억제 및 분자약리학적 기전 (Inhibition of Human Neutrophil Elastase by NSAIDs and Inhibitors, and Molecular Pharmacological Mechanism of the Inhibition)

  • 염증 질환의 원인이 되는 사람 중성구 elastases는 혈액에 존재하는 ${\alpha}_1-PI$${\alpha}_2-macroglobulin$과 같은 단백질 분해효소 억제제에 의하여 조절되어진다. 그러나 특수한 병리적 상황에서 과다하게 분비되는 효소나 또는 단백질 분해효소 억제제의 비정상적 작용으로 말미암아 다양한 염증질환이 유발된다. 비스테로이드성 항염증제는 염증 질환을 치료하기 위하여 이미 임상에 적용 하고 있으며, prostaglandin 합성하는 효소인 cyclooxygenases의 활성도를 억제하는 것이 그 작용 기전으로 잘 알려져 왔다. 사람 중성구 elastase의 활성도는 naproxen, phenylbutazone, oxyphenbutazone 등에 의하여 억제되었으나, ibuprofen, ketoprofen, aspirin, salicylic acid, tolmetin 등에 의하여서는 억제되지 않았다. 또한 사람 중성구 elastase의 활성도는 EDTA, EGTA, tetracycline 등에 의하여서도 억제되었다. EDTA에 의하여 2가 이온 $Ca^{++}$$Zn^{++}$등을 elastase 분자로부터 일부 제거함으로 효소 활성도가 억제되었고 Raman spectra의 변화도 강하게 일어났으며, 금속이온 $Zn^{++}$를 새로 충진함으로 그 활성도는 원래대로 회복되고 Raman spectrum도 원래 상태와 유사한 상태로 회복되었다. 이런 현상은 chelator나 chelator-like agents가 효소분자안에 존재하는 $Zn^{++}$ 이온을 제거하거나 chelation함으로 활성 부위나 그 인접 부위의 4차원 구조의 변화를 일으켰음에 기인하며 특히 -C=O나 -COOH기의 관여에 의한 것으로 생각된다.

유방의 섬유선종에서 발생한 관상피내암 (Ductal carcinoma in situ arising within a fibroadenoma of breast)

  • 섬유선종은 여성 유방의 가장 흔한 양성 종양이고, 대부분 악성병소로 변하지 않는 것으로 알려져 있다. 일상생활에 큰 지장을 주지 않는다면 주기적인 관찰만 하는 경우가 많다. 일반적으로 섬유선종에서의 악성화 병변이 발생하지 않는다고 알려져 있다. 또한 섬유선종 내에서 악성병변이 발생한다면 단지 영상 검사만으로 진단하기 힘들뿐 아니라, 치료를 위해 외과 의사가 수술 방향을 설정하는 것 역시 쉽지 않다. 저자는 19세와 51세의 여자 환자의 두 예에서 유방의 섬유선종에서 발생한 관상피내암을 경험하였다. 19세 여자환자는 만져지는 우측 유방 종괴를 주소로, 타병원에서 절제생검으로 진단하였고, 본원에서 주변부의 재절제술을 하였다. 또한, 면역검사결과 에스트로겐 수용체 및 프로게스테론 수용체 양성 소견을 보여 내분비 치료를 하였다. 51세 여자환자는 좌측 유방의 종괴를 주소로, 유방촬영술 검사에서 내부에 미세석회화 소견을 보였고, 이에 초음파를 이용한 조직검사로 진단하였으며, 자기공명영상 촬영 후 보존적 유방 절제술을 하였다. 유방의 섬유선종에서 발생한 악성병변에 대한 보고는 거의 없다. 또한 이에 대한 특성이나 치료에 대한 지침도 없다. 따라서 이러한 유형의 병변은 드물고 경험이 거의 없기 때문에 유방의 섬유선종에서 발생한 악성병변의 모든 예에 대한 영상검사 및 병리학적 특징을 정확히 이해한다면 유방의 섬유선종의 다양한 형태의 병변에 대한 보다 정확한 진단 및 치료를 시행하는데 많은 도움을 줄 것으로 생각된다. 이에 저자는 유방의 섬유선종에서 발생한 관상피내암의 2예를 바탕으로 섬유선종 내에서 발생한 악성병변의 특성 및 치료에 대해 문헌고찰과 함께 보고하는 바이다.

실험적 외상성 뇌손상모델에서 외상 후 저체온과 MgSO4의 효과 (Effects of Posttraumatic MgSO4 Injection and Hypothermia an Animal Model of Traumatic Brain Injury(TBI))

  • Objective : Traumatic brain injury including diffuse axonal injury has been shown to result in a decrease in brainfree magnesium concentration, an endogenous inhibitor of calcium entry into neuron, that is associated with the development of neurological motor deficits. The goal of this study is to establish the therapeutic window during which the therapy with $MgSO_4$ and/or hypothermia improve damaged neurons by TUNEL stain. Method : Moderate brain injury was induced in 64 adult Sprague-Dawley rats, weighing 350 to 450gm each, by using a simple weight-drop device(Marmarou model). The animals were randomly assigned to four groups(sixteen rats each, a control group, a group treated with $MgSO_4$, a group treated with hypothermia, and a group treated with $MgSO_4$ and hypothermia) and the rats in each group were sacrificed and studied after 12 hrs, 24 hrs, 1 wk, and 2 wks after insult. In hypothermic group, these rats were subjected to hypothermia after injury, with their rectal temperatures maintained at $32^{\circ}C$ for 1 hour. After 1-hour period of hypothermia, rewarming to normothermic level was accomplished over 30-minute period. In the groups treated $MgSO_4$, hypothermia and $MgSO_4$ were subsequently treated with $MgSO_4$($750{\mu}moles/kg$) infused intra-muscularly at 30 minutes after trauma. Result : In all treated groups, a significant reduction in TUNEL positive cells was found in comparison with the control group each time(p<0.001). Between treatment groups, No differnce was seen 12hrs, 24hrs, and 1wk. However, hypothermic group treated with or without $MgSO_4$ showed more significant reduction in apoptotic cells than group treated with $MgSO_4$ 2 weeks after trauma(p<0.05). However, hypothermic group treated with $MgSO_4$ showed no significant reduction in apoptotic cells compared with hypothermic group(p>0.05). Conclusion : These findings suggest that both hypothermia and $MgSO_4$ significantly improve pathological changes. Otherwise simultaneously $MgSO_4$ and hypothermia treatment groups is failed to provide additional neuroprotection. These results may be relevant to the design of future clinical trials of therapeutic hypothermia and $MgSO_4$ for traumatic brain injury.

Comparative Performance of Broilers Fed Diets Containing Processed Meals of BT, Parental Non-BT Line or Commercial Cotton Seeds

  • An effort was made to assess comparative production performance in broiler chickens fed diets containing solvent extracted cottonseed meal (CSM) processed from BT and Parental Non-BT lines. Processed meal of national check and commercial produce cottonseeds were also used for comparison. The free gossypol contents were 0.02, 0.02, 0.44 and 0.03% in meals of BT, Parental Non-BT, national check and commercial produce cottonseeds, respectively. Day-old broiler chicks (n=243) were divided to 27 groups of 9 each. Nine dietary treatments (iso-nitrogenous, 23% CP and iso-caloric, 2,800 kcal ME/kg) were formulated viz., D1 (control, soybean meal-SBM based), D2 and D3 (BT CSM at 10% of diet with and without additional iron), D4 and D5 (non-BT CSM with and without additional iron), D6 and D7 (national check CSM with or without additional iron), and D8 and D9 (commercial produce CSM with or without additional iron at 2 ppm for every 1 ppm of free gossypol, respectively). Each dietary treatment was offered to three replicated groups up to 6 weeks of age. At the end of 6 weeks of age, 10 birds were taken out randomly from each treatment and were sacrificed to study carcass traits, organs' yield and histo-pathological changes in vital organs. The broiler chickens received CSM processed from BT (D2, 1,753 g and D3, 1,638 g) and Parental Non-BT (D4, 1,653 g and D5, 1,687 g) with or without additional Fe grew at same rate as observed in soybean meal (solvent ext.) based diet (D1, 1,676 g). The feed intake and feed conversion efficiency (feed: gain) in these dietary treatments (BT, non-BT line based diets) also did not differ significantly (p>0.05) from control diet. Similar observation was also observed in dietary treatments (D8 and D9) containing solvent extracted cottonseed meal of commercial produce origin. However, a decrease (p<0.05) in body weight gain and feed intake was observed in D6 containing national check CSM with high gossypol content. Addition of Fe in the diet (D7) improved (p<0.05) feed intake and weight gain but not to the extent as observed in diets containing BT, parental non-BT, and commercial produce CSM or control. However, any type of CSM did not affect feed conversion efficiency when fed with or without additional iron. The carcass characteristics in terms of dressing percentage, liver weight and heart weight was not significantly (p>0.05) different between the treatments. The eviscerated yields emanated from diets containing either BT, non-BT or commercial produce were statistically similar to control. However, eviscerated yield of broilers fed national check CSM with or without iron supplementation was lower (p<0.05) than BT cotton with Fe supplementation and commercial produce CSM. The study envisaged that BT, parental non-BT and commercial produce solvent extracted cottonseed meal can be included at 10% in soybean meal based broiler diet replacing soybean meal and rice bran without additional iron.
