Laryngotracheal Separation for Chronic Intractable Aspiration (만성 흡인에 대한 후두기관 분리술의 유용성)

  • Background and Objectives: Intractable aspiration in patients with impaired protective function of the larynx often results in multiple episode of aspiration pneumonia, repeated hospitalizations and expensive nursing care. The purpose of this study was to review the authors’experience and Patient outcome with the laryngotracheal separation (LTS) procedure. Materials and Methods A retrospective review of 9 patients who underwent LTS between 1996 and 2001 was conducted. Ages ranged from 3 to 72 years. Results : Seven patients were expected to have morbid aspiration as a consequence of acquired neurologic injuries and two were congenital neurologic injuries. Two patients had a postoperative fistula, which was well controlled with local wound care and minor procedure. Following LTS, aspiration was effectively controlled in all patients and four were able to tolerate a regular diet. Conclusion : LTS is a low-risk, successful. definitive procedure which decreases the potential for aspiration, pulmonary complication, hospitalizations and increases quality of life, especially in patent with irreversible upper airway dysfunction and Poor speech potential.

이용자 인터페이스에 관한 연구-인터넷 특허정보 데이터베이스를 중심으로-

  • The purpose of this study is to provide more effective utilization of Internet patent information databases and to help develop an user-friendly interface design. Search functions, level of user-friendliness and user-suport have been closely analyzed, using USPTO, QPAT-US, IBM as well as the relatively well-known domestically-developed Patrom. The factors used in this quality evaluation are those resulting from the combination of CD-ROM databases and traditional databases. The followings are the results. Patrom most effectively utilizes the characteristics of databases. USPTO provides the most adequate Interface for a novice user. QPAT-US employs various auxilary services while IBM uses an assortment of search methods and images. In addition, it is expected that the findings will contribute to the development of user-friendly interface databases.

Ultimate strength of long-span buildings with P.E.B (Pre-Engineered Building) system

  • With the improvement of the quality of construction materials and the development of construction technologies, large-scale long-span steel frame buildings have been built recently. The P.E.B system using tapered members is being employed as an economically-efficient long-span structure owing to its advantage of being able to distribute stress appropriately depending on the size of sectional areas of members. However, in December 2005 and in February 2014, P.E.B buildings collapsed due to sudden loads such as snow loads and wind gusts. In this study, the design and construction of the P.E.B system in Korea were analyzed and its structural safety was evaluated using the finite element analysis program to suggest how to improve the P.E.B system in order to promote the efficient and rational application of the system.

  • Presently, the semiconductor industry has the chronic problem. In the semiconductor industry, it has the semiconductor wafer, a package, the optical filter cut by using the saw blade, the mold, a laser etc. The cutting technique has the difficulty due to the rising of the production cost by the wearing of mold, the poor quality problem due to generated heat at the moment of cutting procedure and curve cutting etc. The goal of this time of national research and development project is develop the apparatus for solving the problem that the existing cutting technique has. The technology is so called waterjet abrasive method. This technology will be mainly applied to cut a semiconductor package and a wafer. Two important things to be considered are ripple effect(in other words, the scale of a market) and simplicity of an application.

  • The keen global competition is expected among foreign advanced nations for source, key technologies required by unmanned combat ground vehicle. Therefore, the trend of technologies for unmanned ground combat vehicle was analyzed in this research. It was based on the submitted patents from 1988 to 2008 in Korea, U.S.A, Japan, Europe, Canada and Israel. This analysis was focused on finding the technical level, the challenge area and breakthrough technologies and the growth of technologies of each nations considering opinions of experts. The Effects of Treatment with Ethylene-Producing Tablets on the Quality and Storability of Banana (Musa sp.)

  • This study was conducted to assess the effect of ethylene producing tablet (patent no.10-1574011) and treatment period on quality and storability of banana (AAA - Cavendish group) fruit imported into Korea from the Philippines. Three and five days of treatment periods, and different concentrations of ethylene tablet (50 ppmv, 100 ppmv) and control (standard ethylene gas treatment at a concentration of 100 ppmv) were used for the study. Slightly higher respiration rate was observed in banana fruits treated with 50 ppmv in both 3 and 5 days after treatments as compared to the control. Better storability was observed with banana fruits treated with 50 ppmv and 100 ppmv as compared to the control. All the treatments were shown a decrease in firmness as the storage days proceeded. The highest firmness was recorded from 50 ppmv on the initial day of storage while the lowest was recorded from 100 ppmv on 9 day storage. It was observed that banana fruits could be stored for up to 12 days without losing their color quality in all treatments except for the control. However, the quality of fruits at the control rapidly decreased (lost marketability) after 9 days of storage. Banana fruits treated with ethylene tablet with 100 ppmv for 5 days recorded the highest soluble solids content (SSC) at the beginning of storage period which was similar with the control. However, banana fruits treated with ethylene tablet with 100 ppmv for 3 days showed better storability than the control. On the other hand, ethylene tablet with 50 ppmv for 5 days of treatment has extended banana shelf life without affecting peel color, firmness and SSC content. Hence, these results indicate that banana fruits treated with ethylene tablet with 100 ppmv for 3 days or /and 50 ppmv for 5 days are an optimum for ripening of banana to be used for market and ultimate consumption.

  • Background : Evaluation of patient's satisfaction is one of the most important aspects of quality improvement. If the patient highly satisfies with the medical service provided in the hospital, he/she will be likely to visit the same hospital again. Patient's satisfaction of a particular hospital is directly correlated with hospital profits as well as reputation in the community. For this reason, various kinds of survey measuring satisfaction level have been performed and many kind of QI activities for enhancing the profits as well as reputation in the community. For this reason, various kinds of survey measuring satisfaction level have been performed and many kind of QI activities for enhancing the patient's satisfaction. This study is to find the effect of hospital QI activities on the patent's satisfaction level. Methods: After questionnares were developed, survey of measuring satisfaction level was performed in August, 1998. On the basis of survey results, QI activities were carried out to attain the target point of 4.0 and subsequent survey was done in November, 1998. Results: With three main principles of "problem solving approach with kindness". "helping patient to participate in medical procedure with sufficient information", and "putting employees into practice of attitude with human respect", the average level of satisfaction was enhanced from 3.45 to 3.55 level. Also kindness level of employees was increased from 3.71 to 3.82. Level of dissatisfaction about insufficient explanation and unkind attitude was dropped from 69% to 48% and from 82% to 46% respectively. Conclusion: With the result of this study overall satisfaction level was enhanced. In order to keep these advantages a operation of Quality Improvement Task Force Team in each subject will be required.

The Effect of Patent's Burden and Quality of Life on the Daily Performance Ability of Children With Developmental Disabilities (발달장애아동의 일상생활 수행능력이 부모의 부담감과 삶의 질에 미치는 영향)

  • Objective : In this study, we are going to check the impact of the daily performance of a child with developmental disabilities on the parent's burden and quality of life. Methods : 120 parents of children with developmental disabilities were targeted in Chungcheong and Jeolla provinces. To measure the daily performance ability of children with developmental disabilities, we used evaluative Pediatric Evaluation of Disability Inventory and The burden of parents was measured by Family Burden Questionnaire and the quality of life by Beach Center Family Quality of Life Scale. Based on the data collected, independent t-test, one-way anova, Pearson correlation analysis and regression analysis were conducted to check the impact of a child's daily performance ability of developmental disability on the parent's burden and quality of life. Results : The daily performance ability of a children with developmental disabilities showed a negative correlation with the burden of the parents and a positive correlation with the quality of life. The hygiene and bowel & bladder control of children with developmental disabilities had an important influence on parent's burden, while hygiene, toileting, and bowel & bladder control of children with developmental disabilities had a significant effect on their parent's quality of life. Conclusion : It has been shown that the daily performance ability of a children with developmental disabilities has a major influence on parent's burden and quality of life. In order to reduce the burden and improve quality of life, therapeutic intervention, education and interview programs for daily life of children with developmental disabilities need to be carried out systematically by occupational therapists.

Initial Experience with Epicardial Ultrasound Scanning in Coronary Artery Bypass Grafting

  • Background: The benefits of epicardial ultrasound scanning (EUS) in coronary artery bypass grafting (CABG) have not yet been established. The aim of this study was to evaluate the usefulness of EUS in CABG, including in the assessment of the quality of distal anastomoses, the identification of epicardial target vessels, and the evaluation of any graft issues other than the distal anastomoses. Methods: Fifty-three patients undergoing CABG were enrolled between March 2018 and February 2019. Intraoperative EUS was performed along with transit-time flow measurement (TTFM). Graft evaluations were performed early (shortly after surgery) and 1 year after surgery for 53 (100%) and 47 (88.7%) patients, respectively. Results: EUS was applied to assess the quality of all distal anastomoses, 32 target vessels, and 2 conduit trunks. Insufficient TTFM findings were obtained for 18 grafts. However, graft revision was performed for only 3 distal anastomoses; based on the EUS findings, the remaining 15 sites were not revised. The early and 1-year overall graft patency rates were 100% (141 anastomoses) and 96.1% (122 of 127 anastomoses), respectively. All 15 of the distal anastomoses that were not revised despite insufficient TTFM results were patent at the 1-year mark. Conclusion: The routine application of EUS in CABG could be beneficial by confirming the quality of surgery and reducing unnecessary procedures.

Clustering-based Statistical Machine Translation Using Syntactic Structure and Word Similarity (문장구조 유사도와 단어 유사도를 이용한 클러스터링 기반의 통계기계번역)

  • Clustering method which based on sentence type or document genre is a technique used to improve translation quality of SMT(statistical machine translation) by domain-specific translation. But there is no previous research using sentence type and document genre information simultaneously. In this paper, we suggest an integrated clustering method that classifying sentence type by syntactic structure similarity and document genre by word similarity information. We interpolated domain-specific models from clusters with general models to improve translation quality of SMT system. Kernel function and cosine measures are applied to calculate structural similarity and word similarity. With these similarities, we used machine learning algorithms similar to K-means to clustering. In Japanese-English patent translation corpus, we got 2.5% point relative improvements of translation quality at optimal case.
