Implant-supported prosthetic rehabilitation for the edentulous maxilla using the additive manufacturing technology: A case report (레이저 적층 제조 기술을 이용한 상악 무치악 환자의 임플란트 고정성 보철 수복 증례)

  • The direct metal laser sintering (DMLS) technique would be promising for the full-arch implant-supported restorations due to reduced cost and manufacturing time without potential human errors and casting defects. The aims of this case report were to describe the successful outcome of an implant-supported fixed dental prosthesis in the edentulous maxilla by using the DMLS technology and computer-aided design and computer-aided manufacturing (CAD/CAM) monolithic zirconia crowns, and to describe its clinical implications. A healthy 51-year-old Korean woman visited Seoul National University Dental Hospital and she was in need of a rehabilitation of her entire maxilla due to severe tooth mobility. In this case, all maxillary teeth were extracted and an implant-supported fixed dental prosthesis was fabricated that involved a cobalt-chromium (Co-Cr) framework with the DMLS technique and CAD/CAM monolithic zirconia crowns. Six months after delivery, no distinct mechanical and biological complications were detected and the prosthesis exhibited satisfactory esthetics and function. In this case report, with the DMLS system, the three-dimensional printed prosthesis was created without additional manual tooling and thus, reliable accuracy and passive fit were obtained.

The Effect of Corporate Social Responsibility on Corporate Image and Corporate Performance (기업의 사회적 책임활동이 기업 이미지 형성과 기업 성과에 미치는 영향에 관한 연구: 공유가치창출 인지정도에 따른 차이비교)

  • Purpose - Recently, although corporate social responsibility activities have been increasing in size, they do not have to achieve qualitative improvements and can be passive and cost consuming. Therefore, companies should make quantitative as well as qualitative improvements in their efforts in corporate social responsibility activities. In this study, the classification of social responsibility activities in a variety of studies was analyzed through a more specific path than in previous studies. Corporate behavior image, social behavior image, and corporate contributions image were analyzed through a more detailed analysis of performance. This study suggests that more detailed and concentrated social responsibility activities be pursued by forming companies. Research design, data, and methodology - The purpose of study is to gauge the corporate need for a more intensive, specific area of CSR activities. For this purpose, the sample of consumers that were targeted for CSR activities, recognized as 261 persons, have been investigated. Through a theoretical discussion on previous research, nine hypotheses were established on corporate image, the influence of corporate performance on CSR, and the CSV regulation effect. In order to test the hypothesis, a survey was conducted on 261 male and female consumers who were targeted for CSR, being persons in their 20s to 40s. PASW Statistics 18.0 and AMOS 18.0 were used for statistical analysis. Results - Corporate behavior image was formed through legal responsibility activities and economic responsibility activities. In addition to economic responsibilities, ethical responsibilities and environmental responsibilities were confirmed to have influence on social behavior image. Corporate social responsibility and philanthropic responsibility were confirmed to have influence on economic contribution image. Corporate image has positive effects on brand attitude, corporate reputation, and corporate competition. In addition, when CSV awareness is high, consumers perceive corporate image only through economic responsibility. However, when CSV awareness is low, economic responsibility as well as legal responsibility through charitable activities form the corporate image that influences the brand attitude and corporate reputation, as well as corporate competitiveness. It would appear that the area of corporate social responsibility needs more intensive management for corporate image and corporate competitive advantage. Conclusion - First, the findings of this study show that each CSR activity has a different effect on corporate image and thus, the corporate image influences corporate performance in distinct ways, depending on the CSR activity. This implies that reactive strategies should be tailored to the required image. Second, there is a difference in CSV awareness between groups. When the CSV awareness is low, we can confirm that legal responsibility activities have an especially significant effect on corporate image, implying that corporations should pursue their economic objectives within legal regulations and need to invest significant time and effort for this. This study has limited generalization potential because the result of the model fit has insufficient reference value. In future research, we need to approach various dimensions of corporate performance.

The Effects of Screw Retained Prosthesis Misfit & Cantilever on Stress Distribution in Bone Around the Implant (나사유지형 임플란트 고정성 보철물의 적합도와 캔틸레버가 지지골조직의 응력분산에 미치는 영향)

  • A passively fitting prosthesis is an essential prerequisite to attain long-lasting success and maintenance of osseointegration. However, true "passive fit" can not be achieved with the present implant-supported prosthesis fabrication protocol. Many clinical situations are suitably treated with cantilevered implant-supported fixed restorations. The purpose of this study was to compare the stress distribution pattern and magnitude in supporting tissues around ITI implants with cantilevered, implant-supported, screw-retained fixed prosthesis according to the fitness of superstructures. Photoelastic model was made with PL-2 resin (Measurements, Raleigh, USA) and three ITI implants (${\phi}4.1{\times}10mm$) were placed in the mandibular posterior edentulous area distal to the canine. Anterior and posterior extended 4-unit cantilevered FPDs were made with different misfit in the superstructures. 4 types of prosthesis were made by placing a $100{\mu}m$ gap between the abutment and the crown on the second premolar and/or the first molar. Photoelastic stress analysis were carried out to measure the fringe order around the implant supporting structure under simulated loading conditions (30 lb).

  • Implant prostheses were classified into screw-retained prosthesis and cement-retained prosthesis by their method of retaining, and there is screw and cement retained implant prosthesis (SCRP) which has been made reflecting the strengths of these two. The advantages of the SCRP technique are easy retrievability and passive fit of implant prostheses. However, the occlusal screw holes of implant prostheses can be thought as a disadvantage with respect to esthetics and occlusion. Inappropriately positioned implants also limited the use of the SCRP technique. The present study is reporting about the case where nine implants (US II, OSSTEM, Seoul, Korea) were placed in maxilla and eight in mandible respectively in fully edentulous patients. Then, the cement-retained prosthesis was applied for the part in which the screw hole positioned improperly, and screw-retained prosthesis for properly positioned implants so that the combined screw-cement prosthesis has been produced where the satisfying result has shown in both function and esthetics. Three-year follow-up has been done for the patient.

A study on the status of the Dental Health of Adults (일 지역 성인의 구강건강실태에 관한 조사연구)

  • The purpose of this study is to investigate the state of the dental health of adults, used self-reporting qestionnaire as objects of 923 residents living in nine districts at random among the sites of eleven town located in a County. The level of knowledge on dental health of adults in a County was 65.6 points out of 100 points, comparatively low. Accodingly, it is necessary for entire adults to have a dental health-related education and get high standard of knowledge. Especially, educational approach should be performed for groups of 40-49 years old, 50-59 years old, above 60 years old, male, no educational background, having only elementary and middle school education, not having any jobs, engaging in agriculture, doing business on their own and so on. When planning the contents of health education, one actually has to include the habit of amalgam, the factor in influencing on dental health as well as show an example such as how to brush teeth, checking point of proper brushing, how to grip toothbrush. The attitude score related to dental heath was 71.2 points out of 100 points, relatively low. Consequently, the change of attitude related the dental health among entire adults is necessary, particularily, the strategical approach is essential to change dental health connected to attitude positively for groups of male, having high school education background, office workers and the civil service. Besides, among dental health related symtoms, more that 30-40% of objects showed negative attitude toward as the following cases; in case that plaque or food debris are attached to the teeth (40.8%), upper and lower teeth do not fit together(40.3%), you cannot sleep well because of toothache(31.0%), more than one tooth fall out(31.0%), you have loosing teeth(30.6%), the approach should be conducted to form attitude that dental care is necessry. The state of dental health through dental health related symtoms was 33.3 points out of 100 points, which was fairly satisfactory. However, dental treatment for the state of dental health should be executed in case of comparison of the dental health state according to general characteristics, the group who are above 60 years old, have elementary school education background, engage in aggriculture who are not good in dental health state as opposed to other groups. Furthermore, there should be dental care needs according to dental health related symtoms, particularly, more than 60-70% of objects have experienced symtoms that plaque or food debris attached to the teeth, tartar is on the teeth so dental treatment should be peformed for a large number of adults. In addition, for the people who have indications that there was a cavity, more than one tooth loss, chilled teeth, toothache when chewing, loose teeth, upper and lower teeth do not fit together, you cannnt sleep well due to the toothach, etc, there should be care through dental treatment. The actual conditions of the hygine of the mouth was relatively good and the difference of the actual state of dental health care in terms of general characteristics showed in only gender; female was more careful in dental health. Comparing the state of oral health synthetically, when they have symtoms, only 34.8% of objects go to a dentist, 60.7% are using passive or negative care such as gargling, tolerating or ignoring. There was many symtoms to care through dental therapy such as plaque or food debris get in between the teeth, tartar on the teeth, teeth are very cold, more than one tooth fallout, loose teeth, there is wrong amalgam, and so on, among symtoms to care passively or negatively. Therefore the education for proper treatment program should be performed. As a result of dental health-related knowlege, attitude, health state, verification of correlation between the actual condition of care, the higher the dental heath-related knowldege becomes, the more positive the dental health-related attitude is, and the state of dental health, that is, the standard of the symtom of dental health diminishes. the care for dental health executed through more active method and the more positive dental health-related attitude is, the more active means they performed. Consequently, the high level of dental health-related knowledge should be necessary, the more positive the dental health-related attitude was, the more active method they adopted, therefore, the program is needed to form attitude related to the dental health positively. As the knowledge on dental health is increasing, the attitude is also positive, after all, the plan to increase the standard of knowledge on dental health should be contrived through education program related to dental health.

The effect of screw tightening techniques on the detorque value in internal connection implant superstructure (내부연결 임플란트 상부구조물에서 나사조임술식이 풀림토크값에 미치는 영향)

  • Purpose: This study evaluated the effect of different screw tightening sequences and methods on detorque values in a well-fitting implant superstructure. Materials and methods: A fully edentulous mandibular master model and a metal framework directly connected to four parallel implants (Astra Tech) with a passive fit to each other were fabricated. Six stone casts were made with a splinted impression technique to represent a 'well-fitting' situation with the metal framework. Detorque values were measured twice after screw tightening using 20 Ncm. Detorque values and minimum detorque values for three screw tightening sequences (1-2-3-4, 2-4-3-1, and 2-3-1-4) and for two tightening methods (two-step and one-step) were analyzed using multi-way analysis of variance and two-way analysis of variance, respectively, at a .05 level of significance. Results: The mean detorque values for screw tightening sequences ranged from 12.8 Ncm (2-4-3-1) to 13.1 Ncm (2-3-1-4), and for screw tightening methods were 13.1 Ncm (two-step) and 11.8 Ncm (one-step). The mean of mimimum detorque values for screw tightening sequences were 11.1 Ncm (1-2-3-4) and 11.2 Ncm (2-4-3-1 and 2-3-1-4), and for screw tightening methods were 11.2 Ncm (two-step) and 9.9 Ncm (one-step). No statistically significant differences among three screw tightening sequences were found for detorque values and for mimimum detorque values. But, statistically significant differences between two screw tightening methods were found for two values. Two-step screw tightening method showed higher detorque value (P = .0003) and higher minimum detorque value (P = .0035) than one-step method. Conclusion: Within the limitations of this study, the screw tightening sequence was not a critical factor for the detorque values in a well-fitting implant superstructure by the splinted impression technique. But, two-step screw tightening method showed greater detorque values than one-step method.

Detorque values of abutment screws in a multiple implant-supported prosthesis (다수 임플란트 지지 보철물에서 지대주 나사의 풀림 토크값에 대한 연구)

  • Purpose: This study evaluated the detorque values of screws in a multiple implant-supported superstructure using stone casts made with 2 different impression techniques. Material and methods: A fully edentulous mandibular master model and a metal framework directly connected to four implants (Br${\aa}$nemark $System^{(R)}$; Nobel Biocare AB) with a passive fit to each other were fabricated. Six experimental stone casts (Group 1) were made with 6 non-splinted impressions on a master cast and another 6 experimental casts (Group 2) were made with 6 acrylic resin splinted impressions. The detorque values of screws ($TorqTite^{(R)}$ GoldAdapt Abutment Screw; Nobel Biocare AB) were measured twice after the metal framework was fastened onto each experimental stone cast with 20 Ncm torque. Detorque values were analyzed using the mixed model with the fixed effect of screw and reading and the random effect of model for the repeated measured data at a .05 level of ignificance. Results: The mean detorque values were 7.9 Ncm (Group 1) and 8.1 Ncm (Group 2), and the mean of minimum detorque values were 6.1 Ncm (Group 1) and 6.5 Ncm (Group 2). No statistically significant differences between 2 groups were found and no statistically significant differences among 4 screws were found for detorque values. No statistically significant differences between 2 groups were also found for minimum detorque values. Conclusion: In a multiple external hexagon implant-supported prosthesis, no significant differences between 2 groups were found for detorque values and for minimum detorque values. There seems to be no significant differences in screw joint stability between 2 stone cast groups made with 2 different impression techniques.
