엉덩관절 수술 환자에서 스마트폰을 이용한 관절가동범위 측정의 신뢰도 및 타당도 연구 (The Reliability and Validity of Hip Range of Motion Measurement using a Smart phone Operative Patient)

  • PURPOSE: The aims of this study were to assess the intra-tester and inter-tester reliability and validity of hip range of motion using a smart phone in hip operative patients. METHODS: Twenty-five patients (eight total hip arthroplasty and seventeen femur neck fracture) participated in this study. The range of motions in active and passive hip flexion, abduction and external rotation were measured with a goniometer and smart phone over two times by two observers. The intra-tester and inter-tester reliability were evaluated using the intraclass correlation coefficient (ICC2,1). The validity was measured by Pearson's correlation coefficient RESULTS: The intra-observer reliability was good in all measured items (ICC>0.78). The inter-observer reliability was high with ICC (>0.90). All correlation coefficients of smart phone and goniometer was greater than 0.85 and showed a significant positive correlation (p<0.01). CONCLUSION: The range of motion measurement with a smart phone showed acceptable reliability. Therefore, it could be convenient and have economical benefits to measure the range of motion of the hip joint using a smart phone.

Irreducible Elbow Dislocation Associated with Hill-Sachs-like Lesion over the Capitellum

  • Irreducible dislocation of the elbow is an uncommon event. We present the case of a posterolateral elbow dislocation after a fall injury in a 67-year-old woman. A closed reduction performed in the emergency department was unsuccessful since the limited passive range of motion resulted in difficulty to perform longitudinal traction and flexion. Computed tomography images showed that the posterolateral aspect of the capitellum was impacted by the tip of the coronoid process, thus appearing similar to the Hill-Sachs lesion in the humeral head. Subsequent open reduction of the elbow revealed the dislocation to be irreducible since the tip of the coronoid process had wedged into a triangular Hill-Sachs-like lesion in the capitellum. The joint was reduced by providing distal traction on the forearm, and main fragments were disengaged using digital pressure. At the 3-month follow-up, the patient reported no dislocations, and had an acceptable range of motion. Thus, we propose that to avoid iatrogenic injury to the joint or other nearby structures, irreducible dislocations should not be subjected to repeated manipulation.

Immediate Effect of Hip Hinge Exercise Stretching on Flexibility of Lower Limb, Pelvic Tilting Angle, Proprioception and Dynamic Balance in Individual with Hamstring Tightness

  • Objective: The purpose of this study was to measure the immediate effect of hip hinge exercise stretching on hamstring flexibility, pelvic tilting angle, proprioception, and dynamic balance in individual with tightness of the hamstring. Design: A randomized controlled trial. Methods: A total of 35 healthy young adults (27 males, 8 females) volunteered for this study and randomly divided into three groups (Hip hinge exercise stretching group, passive stretching group, and PNF stretching group). The hamstring flexibility, pelvic tilting angle, knee joint proprioception, dynamic balance was conducted for 3 times. In order to evaluate the hamstring flexibility, the active knee extension test was performed. Forward bending test was performed to examine pelvic tilting angle.The proprioception was tested by the joint position sense test and dynamic balance was evaluated by Y balance test. Results: The hamstring flexibility, pelvic tilting angle and dynamic balance were significantly improved between three groups before and after intervention (p<0.05). Dynamic balance was significantly difference between the three groups in the posterolateral direction (p<0.05). Conclusions: This study result showed that hip hinge exercise stretching was the most effective method for increasing hamstring flexibility, pelvic tilting angle and dynamic balance. In addition, it is necessary to study whether hamstring stretching is effective in low back pain patient with hamstrings tightness.

  • This study proposes an image-based Pose Intention Network (PIN) algorithm for rehabilitation via patients' intentions. The purpose of the PIN algorithm is for enabling an active rehabilitation exercise, which is implemented by estimating the patient's motion and classifying the intention. Existing rehabilitation involves the inconvenience of attaching a sensor directly to the patient's skin. In addition, the rehabilitation device moves the patient, which is a passive rehabilitation method. Our algorithm consists of two steps. First, we estimate the user's joint position through the OpenPose algorithm, which is efficient in estimating 2D human pose in an image. Second, an intention classifier is constructed for classifying the motions into three categories, and a sequence of images including joint information is used as input. The intention network also learns correlations between joints and changes in joints over a short period of time, which can be easily used to determine the intention of the motion. To implement the proposed algorithm and conduct real-world experiments, we collected our own dataset, which is composed of videos of three classes. The network is trained using short segment clips of the video. Experimental results demonstrate that the proposed algorithm is effective for classifying intentions based on a short video clip.

Relationships between Intermittent Locking History and Self-Reported Bruxism in Temporomandibular Joint

  • Purpose: To evaluate aggravating factors of intermittent locking among temporomandibular joint using magnetic resonance imaging (MRI) and diagnostic criteria for temporomandibular disorder (DC/TMD) diagnosis. Methods: A retrospective analysis was conducted of 35 patients with intermittent locking history but normal intra-articular findings between September 2012 and June 2015 in Kyung Hee University Dental Hospital. A standardized DC/TMD assessment was performed on subjects with MRI findings. Clinical findings were assessed on the basis of maximum mouth opening (active & passive), self-reported habits, patients' age, gender, systemic diseases at the initial visit. First, chi square test was used to examine differences with variables and then risk factors for intermittent locking were assessed using multivariate logistic regression. Results: Self-reported bruxism was strongly associated with intermittent locking history. Conclusions: The new DC/TMD protocol is intended for use within any clinical setting and supports the full range of diagnostic activities from screening to definitive evaluation and diagnosis. Self-reported sleep bruxism has been associated with a higher likelihood of intermittent locking. Comorbidity is therefore a factor that must be assessed. It is necessary to consider the amount of contact of the teeth and the duration.

  • Background: This study was to investigate the effect of 8-weeks medical exercise therapy on ankle pain, range of motion, stress symptom after traumatic injury, and depression, in a 51 years old stroke patient with right ankle joint inflammation. Method: The 8-weeks medical exercise therapy program was applied to 4 grades of Dosage 1 (1-3 weeks), Dosage 2 (4-5 weeks), Dosage 3 (6-7 weeks), and Dosage 4 (8 weeks) on right ankle joint inflammation in a female with right hemiplegia admitted to D hospital located in Gyeonggi-do. Result: The findings showed that visual analogue scale (VAS) scores improved from 8 to 0-1 scores, passive range of motion (ROM) increased to 5 degrees more than before, Korean-version impact of event scale-revised (IES-R-K) scores increased from 61 to 31 scores, and Korean-version beck depression inventory II (BDI-II-K) scores decreased from 51 to 17 scores. As such, the 8-week medical exercise therapy program may decrease the pain, increase ROM, improve stress after traumatic injury, and improve depression symptom. Conclusion: The presented evidence suggests that exercise and physical activity have beneficial effects on depression symptoms. It is possible to apply the medical exercise therapy for modulating pain experience and treating pain. Also, it may be effective methods to treat the psychological aspects of pain.

만성 편마비 환자의 발목에 적용한 능동운동을 동반한 관절가동술이 하지근력과 보행의 시공간적 변수에 미치는 영향 (Effects of Ankle Joint Mobilization With Movement on Lower Extremity Muscle Strength and Spatiotemporal Gait Parameters in Chronic Hemiplegic Patients)

  • The purpose of this study was to determine the effect of ankle joint mobilization with movement (MWM) on the range of motion (ROM) in the ankle, on the muscle strength of lower extremities, and on spatiotemporal gait parameters in chronic hemiplegic patients. Fifteen subjects with chronic stroke were divided into two groups: an experimental group (8 subjects) and a control group (7 subjects). Both groups attended two or three sessions of physical therapy each week. The experimental group also attended additional MWM training sessions three times a week for five weeks. For both groups, the ROM of the ankle, the muscle strength of the lower extremities, and the spatiotemporal gait parameters in paretic limbs were evaluated before and after the training period. The results showed that the experimental group experienced more significant increases than did the control group in terms of passive (6.10%) and active (21.96%) ROM of the ankle, gait velocity (12.96%), and peak torque, of the knee flexor (81.39%), the knee extensor (24.88%), and the ankle plantar flexor (41.75%)(p<.05). These results suggest that MWM training in patients with chronic stroke may be beneficial in increasing ROM in the ankle, muscle strength in the lower extremities, and gait speed.

Immediate Effect of Intermittent Versus Continuous Hamstring Static Stretching on the Muscle Tone and Range of Motion

  • PURPOSE: This study examined which stretching exercise had the most positive effect on increasing the range of motion (ROM) of the knee joint in healthy people and whether there was a difference between continuous stretching and intermittent stretching. METHODS: This study included 30 healthy university students from OOO University. The subjects were asked to sit on a mat and perform hamstring-stretching exercise during which the ROM and muscle tone were measured with pre and post-tests. Each subject was assigned randomly to a continuous stretching group (stretching without relaxation time group, n=10, G1) or intermittent stretching group (stretching with 10s relaxing time group, n=10, G2; and stretching with 20s relaxing time group, n=10, G3). The participants conducted hamstring stretching exercises with a sit-and-reach box at three different rest times (0s, 10s, and 20s). Subsequently, they underwent passive knee extension (PKE) tests, in which the ROM of the knee joint was measured with a goniometer, and the muscle tone was evaluated using a MyotonPro. RESULTS: Significant differences in muscle tone, stiffness, and ROM were observed between pre-test and post-test in each groups (p<.05). Although the post hoc tests indicated no significant differences in muscle tone and ROM between the continuous stretching group and intermittent stretching group (p>.05), the rate of change of the ROM showed that the intermittent stretching group developed more effective maintenance of the hamstring flexibility. CONCLUSION: No significant differences in the muscle tone of the hamstring and ROM of the knee joint were observed according to the hamstring stretching exercises with three different rest times. On the other hand, the rate of change of the ROM showed that intermittent stretching maintained the hamstring flexibility more effectively.

테이핑이 발목의 관절가동범위와 고유수용성감각에 미치는 영향 (The Effect of Taping on the Range of Motion and Proprioception at the Ankle Joint)

  • This study was designed to determine the effect of ankle taping and short period of walking on the treadmill on the range of motion (ROM) and proprioception at the ankle joint. Twenty healthy male subjects (mean age=24.2 yr) participated in this study. Goniometry and videotape replaying method were used to measure the ankle ROM. Passive sagittal and frontal plane motions were measured. The difference in degree between the stimulus point and the reproduced point was defined as an angular error. The measurements were performed at four different phases: pre-taping (PRT), post-taping immediately (POT), post-5 minute walking with taping (P5M), and post-10 minute walking with taping (P10M). The ankle of dominant limb was taped by a certified athletic trainer using a closed basket weave technique. Participants walked on the treadmill at 2.5 mph. The results showed that the mean of the sagittal plane motion at PRT, POT, P5M, and P10M was 53.0, 30.5, 36.2, and 40.2 degrees, respectively. The frontal plane motion at PRT, POT, P5M, and P10M was 33.6, 13.9, 15.7, and 18.6 degrees, respectively. The angular error at PRT, POT, P5M, and P10M was 5.5, 1.6, 1.8, and 1.9 degrees, respectively. After 10 minutes of walking, the sagittal plane motion and frontal plane motion was increased by 9.7 and 4.7 degrees compared with POT, respectively. The proprioception was significantly improved after the application of ankle taping. Both the restriction of frontal plane motion and proprioception improvement at the ankle joint may contribute to ankle stability during walking.

The Relationship Between the Range of Hip Rotation and the Quadriceps Angle in Subjects With and Without Flat Foot

  • Background: Alignment of the lower limb is an important factor, influencing balance and gait in kinematics and kinetics, in patients with and without a flat arched foot. Flat arched foot are associated with the range of motion (ROM) of the hip and alignments of the knee joints, is strongly influenced. Objects: The purpose of this research was to investigate the relationship between hip joint ROM and quadriceps angle (Q-angle), by dividing them into two groups according to the presence or absence of flat feet, using a navicular drop test (NDT) and resting calcaneal stance position (RCSP). Methods: Forty elderly patients were allocated to the experimental group (flat foot group, n1=20) or the control group (non-flat group, n2=20). Universal and digital goniometer, tractograph and tape measure were used to determine the related changes in the hip ROM, Q-angle, NDT and RCSP. Results: Data were analyzed using the Pearson correlation coefficients. Active internal ROM of the hip joint (right, r=.803, p<.001), (left, r=.951, p<.001) were highly correlated with NDT, and also, was moderately correlated with Q-angle (right, r=.562, p=.019), (left, r=.757, p<.001). Passive internal ROM of the hip joint (right, r=.742, p=.001), (left, r=.922, p<.001) were highly correlated with NDT, and also, was moderately correlated with RCSP (right, r=-.530, p=.029) and with Q-angle (right, r=.710, p=.001), (left, r=.698, p=.002) in the flat foot group. However, no strong correlation among the hip ROM, NDT, RCSP and Q-angle were found in the non-flat foot group. Conclusion: This research may provide evidence of the correlations between hip internal ROM and flat foot.
