양육행동에 대한 부모와 청소년의 보고 비교 (Agreement between Parent and Adolescent Reports on Parenting Behaviors)

  • The purpose of the study was to examine the agreement between parent and adolescent reports on parenting behaviors. The participants were 196 father-mother-adolescent triads. Results indicated that there were significant differences in reports of parenting behaviors between parents and adolescents. Generally, parents reported higher respect, disciplines and attention than their adolescents did. However, there were low to high agreements between parental report and adolescent report on parenting behaviors. The degree of those agreements varied by adolescent's developmental stage, birth order and mother's education. The findings suggested that differences in agreements between parent and adolescent reports on parenting behaviors might not be random and the agreements should be addressed in terms of specific sample characteristics.

  • Objectives : The aims of this study were to examine agreement between parent and teacher ratings of the Korean version Strengths and Difficulties Questionnaire (SDQ-Kr) for the children's psychopathology and to determine whether it is effective in diagnostics as the Korean version of Childhood Behavior Checklist (K-CBCL). Methods : SDQ-Kr and K-CBCL were completed by parents and teachers of 165 children aged 6-12 years. The clinical diagnosis was made by a child and adolescent psychiatrist who did not have knowledge of the outcome. Spearman's correlations were computed for associations between parents' and teachers' ratings on a subscale level. The Mann-Whitney U test was performed for the influence of child's age and sex on the results. Finally, AUC values were calculated for the diagnostic capacity and the difference between informants was tested using the z test. Results : High correlations were observed between parents' and teachers' SDQ-Kr on every scale. Parents were more sensitive to emotional symptoms of girls, while teachers appeared to be more responsive to externalized symptoms of boys, inattention-hyperactivity of young children, and emotional symptoms of seniors. Parents' SDQ-Kr demonstrated the highest prediction of attention-deficit hyperactivity disorder. Conclusion : Teachers' SDQ-Kr can be used in identification of child psychopathology when parents are absent or when their reliability is considered low.

Development of Parent Guidelines for Parent-Performed Developmental Screening Tests

  • Objectives: Most developmental screening tests have been built as parent-performed questionnaires. However, they often do not guide parents on how to answer the questionnaire. This study aimed to develop easily applicable parent guidelines. Methods: We implemented the Delphi procedure with 20 panelists. The development of the initial questionnaire was based on the results of two surveys of parents and experts provided by a policy research report that investigated the item adequacy of the Korean Developmental Screening Test. Round one included 33 items comprising all possible measurements in six categories that were identified as difficult to understand or confusing. Round two merged and modified some items and included 32 items. We defined consensus as a median agreement value of one or less and convergence and stability values of 0.5 or less. The subjective usefulness of the parent guidelines was examined based on their previous test experiences. Results: Consensus was reached after the second round, reflecting the items with the highest level of accuracy in each category. Of the 167 parents who participated in the survey, 113 (67.7%) affirmed the usefulness of the guidelines, while 10 (6.0%) answered that they were not useful. Items that recommended a different scoring strategy in answering the questionnaire from their previous measurements were found to be more useful by the parents. Conclusion: The parent guidelines, composed of five bullet points, drew on the consensus of the experts. Further studies are required to assess whether these guidelines improve the accuracy of screening tests in clinical settings.

문제행동에 대한 청소년 자신과 부모 평가간의 관계:K-CBCL과 YSR의 하위요인 구조 비교 (PARENT-ADOLESCENT AGREEMENT IN THE ASSESSMENT OF BEHAVIOR PROBLEMS OF ADOLESCENTS:COMPARISON OF FACTOR STRUCTURES OF K-CBCL AND YSR)

  • 본 연구에서는 청소년 자신의 문제행동에 대한 평가와 부모의 평가가 서로 어떻게 다른지와 각자의 평가에 영향을 주는 변인들을 비교하였다. 일반청소년 3271명을 대상으로 청소년 자신이 평가한 YSR과 부모가 평가하는 K-CBCL을 동시에 실시 한 후 두 평가척도에 공통되는 문항들만으로 요인분석을 실시한 결과 K-CBCL에서는 제1요인으로 공격성/비행요인이 전체변량의 14.08%를 설명하였고 이와 함께 신체적 증상, 사회적 위축, 소란/방해, 우울/불안 등 5개의 요인이 추출되었으며, YSR에서는 제1요인으로 우울/불안/위축요인이 전체변량의 14.23%를 설명하였고 이와 함께 공격성, 신체적 증상, 소란/방해, 인정욕구 등 5개의 요인이 추출되었다. K-CBCL과 YSR의 총 문제행동 점수 평가에 영향을 주는 변인들을 살펴보기 위해 회귀분석을 실시한 결과 부모의 평가와 청소년 평가에서 공통적으로 부모, 친구 등 사회적 관계가 나쁠수록, 학업수행이 나쁠수록 총 문제행동 점수가 높았다. 반면에 청소년 자신의 평가에서는 성, 연령이 중요한 기준이 되며 특히 보고하는 문제유형이 성별에 따라 다른 양상을 나타내었다. 이와 같은 결과는 부모가 자녀의 문제행동을 평가하는 경우 공격성이나 비행 등과 같이 외현적으로 드러난 행동문제가 중요한 기준이 되는 반면, 청소년 자신이 평가하는 경우 우울/불안/위축 등 내재화된 문제 정도를 최우선적으로 자각하게 된다는 점을 시사하며 이에 따른 청소년 평가 및 진단에서의 시사점을 논의하였다.

학령기 아동 정서.행동문제에 대한 부모-교사 평가 일치도 (Agreement between Parents and Teachers on School Children's Emotional/Behavioral Problems)

  • Objectives: In assessing behavioral/emotional problems in school-aged children, the importance of multi-informant reporting has been well documented. However, in clinical settings obtaining multiple informants' opinions has proven difficult. For that reason, we researched the agreement and predictive validity of the Child Problem-Behavior Screening Questionnaire (CPSQ) in order to reveal how accurate parents' assessments reflected teachers'opinions. Methods: We conducted the first screening for second- and third-grade children from 3 elementary schools in Seoul from 2003 to 2007 using the CPSQ. There were 1178 children included in the analysis. We then administered the Korean version of the Child Behavior Checklist (K-CBCL) as a second screening tool and subsequently, the ADHD Diagnostic System (ADS) and the Korean Educational Development Institute version of the Wechsler Intelligence Scale for Children (KEDI-WISC) was administered by a psychiatrist. We examined each item on the CPSQ and the subscale's agreement between parent and teacher as well as the predictive validity of the CPSQ in children diagnosed with emotional/behavioral problems. Results: The agreement rates between parents and teachers appeared high for questions 18 (0.433), 1 (0.385), and 2 (0.325). Among the subscales, a relatively high correlation was found for externalizing problems, attention deficit hyperactivity disorder, and cognitive problems. For all diagnosed children, their parents revealed a higher sensitivity and lower specificity than teachers. Conclusion: From these results, we confirmed that the CPSQ can be useful for sorting out externalizing and cognitive problems. There is a need for further study, however, with a larger sample size.

정신과 환자 자녀의 우울, 불안, 자기 개념 그리고 가정환경의 특성에 대한 연구 (IMPACT OF PARENTAL PSYCHIATRIC DISORDER ON OFFSPRING'S DEPRESSION, ANXIETY, SELF CONCEPT AND PERCEPTION OF FAMILIAL RELATIONSHIP)

  • 본 연구의 목적은 정신과 환자의 자녀가 정상인의 자녀와 비교하여 불안, 우울 등의 정신병리와 자기 개념, 가정환경에 대한 인식에 있어서 차이가 있는지를 알아봄으로써 부모의 정신병리가 자녀에게 어떤 영향을 주는가를 밝히기 위함이다. 아울러 환자집단 내에서 환자의 성별에 따라, 환자의 발병 당시의 자녀의 나이에 따라, 환자 진단에 따라 자녀의 불안, 우울 등의 정신병리와 자기 개념, 가정환경에 대한 인식에서 차이가 있는가를 알아보고자 하였다. 1997년 6월부터 1998년 4월까지 서울, 경기지역의 신경정신과 성인 외래 환자 39명을 대상으로 하여 이들의 자녀 중 초등학교 4학년부터 고등학교 3학년까지의 남녀 학생 52명에게 한국형 소아 상태-특성불안척도, 한국형 소아 우울 척도, 한국형 소아자기개념척도, 한국형 가정환경척도를 작성하도록 하고 이를 동 수의 정상인 자녀와 비교하였다. 그리고 환자의 자녀 내에서는 환자가 남자인지 혹은 여자인지, 환자의 발병 연령이 자녀가 3세 이전이었는지 이후였는지, 환자의 진단이 어떤 것인지에 따라 척도들을 비교하였다. 결과는 다음과 같다. 1) 정신과 환자의 자녀들이 정상인의 자녀들과 비교하여 가정환경척도 중 표현력 척도에서 유의하게 낮은점수를 보였고 상태 불안 척도에서는 유의하게 높은 점수를 보였다(p<0.05). 그러나 소아 자기개념척도의 지적 및 학업 상태 소척도와 인기도 소척도에서는 오히려 환자의 자녀가 정상인의 자녀보다 유의미하게 높은 점수를 보였다(p<0.05). 2) 환자군 내에서 환자의 남녀 성별에 따른 자녀의 척도들의 차이는 없었다. 3) 환자군 내에서 환자가 자녀 나이 3세 이전에 발병한 경우가 3세 이후에 발병한 경우보다 가정환경척도 중 조절성 소척도 점수는 유의하게 낮고 특성불안척도는 유의하게 높았다(p<0.05). 4) 환자군 내에서 환자의 진단(정신분열병 범주 장애-정동장애-신경증)에 따른 자녀의 척도들의 차이는 없었다. 자기 보고식 설문으로 측정한 우울, 불안 등의 정신병리는 환자의 자녀와 정상인의 자녀간 유의미한 차이를 보이지 않았다. 환자의 진단이나 남녀 성별의 차이 역시 자녀에게 영향을 주지 않는 것으로 밝혀졌으나 자녀가 3세 이전에 부모가 정신질환에 이환 될 경우에는 자녀의 특성불안이 높은 경향을 보였다. 향후 고위험군 자녀의 발달학적 측면에서 영향을 주는 요인을 밝히는 연구를 위해서는 자녀의 자기보고 이외에도 부모, 선생님 등의 관찰과의 일치도에 대한 조사, 직접적인 면담 등이 필요할 것으로 보이며 통제된 환경에서의 전향적 연구가 필요할 것이다.

Exploring the Differences between Adolescents' and Parents' Ratings on Adolescents' Smartphone Addiction

  • Background: Smartphone addiction has recently been highlighted as a major health issue among adolescents. In this study, we assessed the degree of agreement between adolescents' and parents' ratings of adolescents' smartphone addiction. Additionally, we evaluated the psychosocial factors associated with adolescents' and parents' ratings of adolescents' smartphone addiction. Methods: In total, 158 adolescents aged 12-19 years and their parents participated in this study. The adolescents completed the Smartphone Addiction Scale (SAS) and the Isolated Peer Relationship Inventory (IPRI). Their parents also completed the SAS (about their adolescents), SAS-Short Version (SAS-SV; about themselves), Generalized Anxiety Disorder-7 (GAD-7), and Patient Health Questionnaire-9 (PHQ-9). We used the paired t-test, McNemar test, and Pearson's correlation analyses. Results: Percentage of risk users was higher in parents' ratings of adolescents' smartphone addiction than ratings of adolescents themselves. There was disagreement between the SAS and SAS-parent report total scores and subscale scores on positive anticipation, withdrawal, and cyberspace-oriented relationship. SAS scores were positively associated with average minutes of weekday/holiday smartphone use and scores on the IPRI and father's GAD-7 and PHQ-9 scores. Additionally, SAS-parent report scores showed positive associations with average minutes of weekday/holiday smartphone use and each parent's SAS-SV, GAD-7, and PHQ-9 scores. Conclusion: The results suggest that clinicians need to consider both adolescents' and parents' reports when assessing adolescents' smartphone addiction, and be aware of the possibility of under- or overestimation. Our results cannot only be a reference in assessing adolescents' smartphone addiction, but also provide inspiration for future studies.
