Development of Health Promotion Program for Individuals With Arthritis -Application of Holistic Model- (관절염 환자를 위한 건강증진 프로그램의 개발 -총체적 모델의 적용-)

  • In this study, domains, contents, and effects of pre-existed intervention programs for individuals with arthritis were meta-analyzed to develop arthritis health promotion program based on Holistic Model. The developed program includes strategies of cognition, environment, and behavior. and also generates positive changes in the physical, psychological, and social demensions. Then needs assessment on conveniently selected 153 women who visited a university hospital in Seoul or in Inchon are conducted to identify the objective domains of arthritis health promotion program According to the study results. target health problems of the arthritis health promotion program were shown as pain, disability, depression, and role impediment in social domain. These objectives could be achieved by including the strategies of changing cognition, the strategies of changing behavior through learning the skill related to the health promoting behavior. and the strategies of changing environment in the health promotion program. That is, it is analyzed that the contents of program are not exclusive one another in physical. psychological. and social demensions, and also are not exclusive one another in aspect of cognition, behavior, and environment. The necessary methods to achieve the desired objectives for the developed arthritis health promotion program and evaluation subjects are as follows : (1) In the arthritis health promotion program, knowledge on management of arthritis, efficacy related to arthritis management, skill for pain management, skill for exercise, establishment of positive self-concept, enhancement of positive thinking, stress management. skill for problem solving, skill for setting goals. skill for requesting help, and skill for communication are all included. Through the improvement of all those strategies, intermediate objectives, such as “joint protection, and maintenance of pain management behavior”, “maintenance of regular exercise”, and “promotion of coping skill in psychosocial dimension” are achieved. (2) These intermediate objectives are also the methods for achieving objectives in next stage. It implies that through the intermediate objectives. the final objectives such as “minimization of physical symptoms and signs”, “maximization of psychological function”, and “maximazation of role performance in social domain” could be achieved. Each of these final objectives reflects the different dimension of quality of life, respectively. When these objectives are achieved, the quality of life that client perceives is improved. Therefore, through evaluation of these final objectives, the level of achieving final outcome of arthritis health promotion such as quality of life is determined.

Predictors of Participation Restriction in Community-dwelling Persons With Spinal Cord Injury (지역사회 거주 척수장애인의 참여 제약에 영향을 미치는 예측요인)

  • Objective: The purpose of this study was to suggest predictors of participation restriction in community-dwelling Spinal Cord Injury (SCI) based on environmental, functional, injury-related, psychosocial, and sociodemographic factors. Methods: The participants were 82 community-dwelling SCI adults over the age of 18. They were all included in the American Spinal Injury Association's (ASIA) Levels A, B, and C. This study surveyed participants' participation using the CHART-K-SF, environmental factors using the CHIEF-25 Korean version, and functional factors using the SCIM III. Further, pain and distress were surveyed using Zung's Self-rating Pain and the PAD, self-esteem was assessed using the RSES, and self-efficacy using the GSS. Finally, information concerning injury levels, post-injury period, causes of injury-related factors, and sociodemographic factors such as gender, age, marriage, education level, and residence information were collected. The collected data were analyzed using SPSS Version 21.0 Results: Participants' risk factors were environmental and functional. The Nagelkerke's $R^2$ was.737. The OddsRatio(OR) of environmental factors(referent,0-1) was 40.346. Moreover, the OR of the $1^{st}$ quartile functional factor(referent,4thquartile) was 236. 621, and that of the$2^{nd}$ quartile was 21.174. In addition, the occupational predictors included the "policies" subscales in the CHART-K-SF, "physical/structural" in the CHIEF-25, and "mobility" in the SCIM III. Further, the predictors of "physical independence" were "services/assistance" in CHIEF-25, and "respiration and sphincter management' on the SCIM III. Additionally, "mobility" was predicted by "physical/structural" on the CHIEF-25 and "mobility" on the SCIM III. Conclusions: This study multidimensionally confirmed predictors of participation restriction. Through these facts, we investigated occupational therapists' roles in community setting. Therefore, this study's results will provide useful information for occupational therapy services, for which the goal is SCI participation improvement within the community.

Pilot Clinical Trial to Evaluate the Efficacy of Needle-Embedding Acupuncture Therapy at Gokji(LI11) on TEWL (곡지혈 매선침 시술이 경피수분 손실량에 미치는 영향)

  • The purpose of pilot clinical trial is to evaluate the efiicacy and safety of Needle-Embedding acupuncture therapy on TEWL(Transepidermal Water Loss) and Skin hydration. A total of 24 human who visited Dongshin Oriental Medical Center from December 1st, 2016 to July 31st, 2017 were included in the pilot clinical trial. Needle-Embedding acupuncture therapy performed at Gokji(LI11). We observed change of TEWL(Transepidermal Water Loss), Skin hydration before and after Needle-Embedding acupuncture therapy. In the primary endpoint, index of TEWL showed a statistically insignificant decline ($8.5{\pm}2.12{\rightarrow}7.8{\pm}1.53g/h/m^2$). Index of skin hydration showed a statistically significant result ($38.2{\pm}6.77{\rightarrow}36.8{\pm}6.64$). To evaluate the safety, Adverse events and Vital sign check were conducted and there were no problem. And when the physical response, self-awareness, skin system, musculoskeletal pain, and other abnormal responses were assessed 60 minutes and 24 hours and 7 days after Needle-Embedding acupuncture therapy at Gokji(LI11) for stability assessment, Adverse events disappeared during a pilot clinical trial. According to the above pilot clinical trial, it is suggested Needle-Embedding acupuncture therapy were effective for Skin moisturizing and safe.

  • This study examined the intention to use a nursing process application on a smart phone by nursing students who undertake clinical practices, and evaluated the application effect of this program. As research subjects, the study assessed juniors in the college of nursing in Daegu, who had undertaken field training in adult nursing in surgical wards from September 2012 to September 2013. The results showed that the intention to use nursing process application has a significant relationship with effort expectancy, social influence, and self-efficacy. In addition, the nursing process application applied to the research subjects has the largest list of diagnoses, interventions and outcomes regarding acute pain. In addition, the satisfaction with the use of the program was 4.35 points. Through this study, the application of the nursing process presents more opportunities for its use and training in the clinical practice field, and measures that can be applied easily regarding its utilization should be provided.

The efficacy of mobile application use on recall of surgical risks in nasal bone fracture reduction surgery

  • Background: The number of surgical risks recalled by a patient after surgery can be used as a parameter for assessing how well the patient has understood the informed consent process. No study has investigated the usefulness of a self-developed mobile application in the traditional informed consent process in patients with a nasal bone fracture. This study aimed to investigate whether delivery of information, such as surgical risks, through a mobile application is more effective than delivery of information through only verbal means and a paper. Methods: This prospective, randomized study included 60 patients with a nasal bone fracture. The experimental group (n=30) received preoperative explanation with the traditional informed consent process in addition to a mobile application, while the control group (n=30) received preoperative explanation with only the traditional informed consent process. Four weeks after surgery, the number of recalled surgical risks was compared for analysis. The following six surgical risks were explained: pain, bleeding, nasal deformity, numbness, nasal obstruction, and nasal cartilage necrosis. Results: The mean number of recalled surgical risks among all patients was $1.58{\pm}0.56$. The most frequently recalled surgical risk was nasal deformity in both groups. The mean number of recalled surgical risks was $1.72{\pm}0.52$ in the experimental group and $1.49{\pm}0.57$ in the control group. There was a significant association between mobile application use and the mean number of recalled surgical risks (p=0.047). Age, sex, and the level of education were not significantly associated with the mean number of recalled surgical risks. Conclusion: This study found that a mobile application could contribute to the efficient delivery of information during the informed consent process. With further improvement, it could be used in other plastic surgeries and other surgeries, and such an application can potentially be used for explaining risks as well as delivering other types of information.

Effects of Aquatic Exercise on Skinfold Thickness and Circumference of Upper and Lower Extremities in Patients with Rheumatoid Arthritis (수중운동 프로그램이 류마티스 관절염 환자의 사지 피부두겹 두께와 둘레에 미치는 영향)

  • Many arthritis patients experience weakness of muscles in extremities mainly due to disuse atrophy and weight gain because of the limited activities and exercises. This study examines the effects of the 6-week aquatic-exercise program on the body fat and the muscle of the rheumatoid arthritis patients. Seventeen patients in the experimental group and 18 in the control group were assigned depending on their preference and physical condition. These patients had more than 3 points of pain out of 10, deformities in knee, wrist and ankle joints. The amount of aquatic exercise increases from 35 minutes in the first week to 60 minutes in the 6th week. In the resting period they discussed their own experiences about exercise, personal and family affairs, and performed some recreation programs in order to increase the self-efficacy and promote the relationship with other patients by the group activities. Skinfold thickness and circumferences of both extremities were measured before and after experiment to compare the difference. For testing the body fat Saham Model was used. Prior to the experiment two group's body weight, skinfold thickness and skin circumferences were not significantly different which indicates the homogeneity of two groups. Body weight and most parts of skinfold thickness of the experimental group were significantly lower than the control group after 6-week aquatic exercise program. Circumference was not significantly lower than the control after the program. These findings indicate the in-crease of muscle sizes and the reduction of the body fat. Therefore a more active application of aquatic exercise into a variety of clients is strongly suggested.

  • Purpose: The purpose of this study was to analyze the effect of aquatic exercise and to identify directions for future research. Methods: Twenty one articles from 2 sites of article database (1990-2010) and 6 from Korean nursing journals were reviewed. Results: There were 20 quasi experimental design and 1 randomized controlled design. Major subjects of research were older women with arthritis. The most common type of exercise was Kim (1994)'s form. The most frequent duration of exercise was over 6 weeks. Outcome measurement was classified to physical and psychosocial variables. For physical outcomes, pain release was found in 13 articles among 15 articles, muscle strength improvement in 5 articles among 10 articles, flexibility improvement in 5 articles among 9 articles, and balance improvement in 3 articles among 6 articles. For psychosocial outcomes, improvement of quality of life was found in 3 articles among 7 articles and self-efficacy improvement in 5 articles among 7 articles. Conclusion : The effect of aquatic exercise over 6 weeks improves the physical and psychosocial functions. It is suggested that meta analysis needs to be done to integrate the various results.

The Trend and Issues of Nursing Research Related to Ischemic Heart Disease Published in Korea (허혈성심질환 관련 국내 간호연구 동향)

  • This study attempted to analyze the trend of studies related to IHD(Ischemic Heart Disease). This article reviewed 102 researches on IHD conducted from 1980 to 2003, by examining them according to their characteristics, such as sources by publication period, research design, nursing intervention, outcome of experimental research, and theme of qualitative research. The results were as follows : 1. The number of studies related to IHD has steadily increased year by year since 1985, and that of the articles was the highest during the post-2000 period. Especially, the majority of researches has published since 1995. 2. The 69 studies were degree of all 102 studies related to IHD, and the majority of articles' authors belonged to graduate school. Thirty three studies were non-degree articles. As for study design, the number of quantitative studies were 99 and that of qualitative studies were 3. Non-experimental research design were largely survey and correlational study. 3. Non-experimental study makes up a high proportion of the quantitative study. The most of experimental studies were accomplished after 2000. The main subjects were patients, chart records and general subjects. The majority number of subjects was like this : while the number of non-experimental study was between 51 and 100, that of experimental study was below 50. The number of subject of qualitative study is like this : the number of phenomenology study was 21 and 11, and that of case study is 14. 4. Nursing interventions used in the experimental study comprised teaching program, cardiac rehabilitation program, massage, relaxation, music therapy and providing of sense information. And the most nursing intervention effects included physiological effect, increased self-efficacy, relieved pain and increased quality of life. Most studies proved to have the affirmative effects. 5. The number of qualitative studies related to IHD is like this : the number of degree articles was 2 and that of non-degree articles was 1. Its themes included resources of hope, the experience of coronary artery angiography and the experience of admission at ICU(Intensive Care Unit).

  • Purpose: This study aimed to analyze recent trends in nursing research published in the Journal of Muscle and Joint Health by focusing on the content of nursing interventions and their quality. Methods: A total of 80 studies published between 2013 and 2015 were reviewed using analysis criteria developed by the researchers. Results: Quantitative studies accounted for 98.7% while qualitative studies accounted for 2.3%. Most frequently used research designs were quasi-experimental (92.9%) for experimental research and survey (89.4%) for non-experimental research. Patients (43.8%) were the most frequent study participants. Elderly was the most frequently used keyword. Most frequent nursing interventions were exercise (35.7%). Self-efficacy (35.7%), flexibility (28.6%) and pain (28.6%) outcomes for dependent variables were used. Quality assessment of experimental studies by SIGN were high quality (25%), acceptable (50%), low quality (25%). Conclusion: The results suggest that further expansion of randomized controlled trial and qualitative studies. To enhance the quality of research, It is needed to try the intention to treat analysis in experimental research.

The Prediction Model of Adaptation in Patients with Rheumatoid Arthritis - Propositional Synthesis of Roy's and Lazarus & Folkman's Theory - (류마티스 관절염 환자의 적응 예측모형 -Roy이론과 Lazarus 및 Folkman 이론의 명제 합성-)

  • The lack of a comprehensive theory describing the mechanism of adaptation scientifically has been one of the limiting factors for the development of nursing intervention of patients with chronic illness. Since Roy's theory provides the general conceptual framework depicting adaptation process with structural stimuli and control mechanism, it is appropriate to understand the process of adaptation. But in Roy's theory, the propositions about cognator and regulator as control mechanisms are not clearly defined. For this reason, most of the previous researches applying the Roy's theory have disregarded the study of cognator and regulator. For the patients with chronic illness such as rheumatoid arthritis, it was reported that adaptation states were different for the same stimuli due to the difference of the control mechanism. Moreover in nursing it is important to identify the control mechanism which can be and must be intervened by nurses. It was the Lazarus and Folkman's theory that proposed the control mechanism. They suggested that individual differences in the reaction against the perceived stress are due to the difference in appraisal and coping. Therefore, the synthesis of Roy's and Lazarus and Folkman's propositions might help to clearly understand the mechanism of adaptation. From this point, a theoretical framework has been developed and tested. The subjects were the 297 patients who had been diagnosed rheumatoid arthritis and attended the outpatient clinic. A hypothetical prediction model of adaptation was tested by the covariance structure analysis with PC-LISREL 7.13. As a result, the overall fit was good($x^2$=78.83, p=0.00 ; GFI=0.96 : AGFI=0.90 ; RMR=0.04) for the hypothetical model. In the final model added GA(5, 1), the overall fit was increased ($x^2$=57.82, p=0.003 ; GFI=0.97 ; AGFI=0.93 : RMR=0.036). Except the fact that illness symptoms affected physical adaptation directly, it was supported that focal and contextual stimuli affected physical and psychosocial adaptation through appraisal and coping. Therefore, it was asserted that the synthesis of two theory's propositions was appropriate. So this model would be useful for the theoretical framework in the nursing practice. And this study synthesizing and testing the theory might contribute to establish nursing's scientific base.
