Relief of Chronic Posterior Neck Pain Depending on the Type of Forest Therapy: Comparison of the Therapeutic Effect of Forest Bathing Alone Versus Forest Bathing With Exercise

  • Objective To compare the pain-reducing effect of forest bathing alone versus forest bathing in combination with stretching and strengthening exercises in patients with chronic posterior neck pain. Methods Sixty-four subjects with posterior neck pain that had lasted more than 3 months were enrolled. They were randomly divided into a forest bathing alone (FBA) group and a forest bathing with exercise (FBE) group; each group included 32 subjects. All subjects from both groups walked every morning in the forest for about 2 hours for 5 days. In the afternoon, the FBE group did a stretching and strengthening exercise for about 4 hours; the FBA group had free time in the woods. Visual analog scale (VAS) on one day, VAS over the previous week, neck disability index (NDI), EuroQol 5D-3L VAS (EQ VAS) and index (EQ index), McGill pain questionnaire (MPQ), the number of trigger points in the posterior neck region (TRPs), and the range of motion of the cervical spine were evaluated on the first and last day of the program and compared between the two groups. Results The number of TRPs were significantly reduced in the FBE group compared with the FBA group (p=0.013). However, the other scales showed no significant difference between the two groups. Conclusion When patients with chronic posterior neck pain underwent a short-term forest bathing (less than 7 days) program, FBE was more effective in the reduction of the number of TRPs than FBA. However, all other pain measurement scales we evaluated showed no statistically significant difference between the two protocols.

  • Purpose : The purpose of this study is to know the therapeutic effect of herbal-acupuncture on shoulder pain from calcific tendinitis. Methods : The patient who have the calcific tendinitis was treated by herbal-acupunture on 6 acupoints around right shoulder. The shoulder pain and dysfunction was assessed by visual analogue scale(VAS). UCLA Shoulder rating Scale. and SPAI scale. Results : After treatment. all scales showed slightly improvement.. Conclusion : Herbal-acupuncture on shoulder joint was effective in short term study. We need more further study.

The Validity of a Single Item Scale - Health Related Quality of Life in Heart Failure Patients - (단일 문항 척도의 타당도 조사 - 심부전 환자의 건강관련 삶의 질을 중심으로 -)

  • Purpose: The purpose of this study was to examine the validity of a single item scale by comparing a single item scale of health related quality of life to subscopes of the multi-item scales in the Medical Outcome Study Short Form-36 (SF-36), and Minnesota Living with Heart Failure Questionnaire (MLHFQ). Method: The data from 103 patients with heart failure were analyzed. The statistics program SPSS 12.0 used for descriptive statistics. Results: Significant correlations were found between the single item scale and subscopes of SF-36, for physical functioning (r= .42), role physical (r= .22), general health perception (r= .46), vitality (r= .40), social functioning (r= .20) and mental health (r= .51), but not for body pain (r= .12) and role emotional (r= .06). Physical (r= -.41), emotional (r= -.49) and total scores (r= -.49) of MLHFQ were significantly related to the single item scale. Conclusion: The single item scale of health related quality of life showed low to moderate correlation with multi-item scales. Even though the single item scale was correlated with several subscopes of multi-item scales, the correlation was not high, so we have to use caution when using the single item scale instead of multi-item scales.

A Symptom Cluster Analysis of Breast Cancer Patients Using a Mediation Model (매개모델을 이용한 유방암 환자의 증상클러스터 검증)

  • Purpose: This study was conducted to test whether sleep disturbance mediates the effect of pain on fatigue, whether fatigue mediates the effect of pain on depression, and whether fatigue mediates the effect of sleep disturbance on depression. Methods: This study was conducted from September 12th to December 20th, 2011. One hundred eighty-eight breast cancer patients were recruited from D city and S city in Korea. The instruments used in this study were the pain, sleep disturbance, fatigue, and depression scales. Data were analyzed using descriptive statistics, Pearson correlation coefficients, and mediation pathway with the SPSS 12.0 and AMOS 20.0 programs. Results: There was correlation among pain, fatigue, sleep disturbance, and depression. Mediation analyses indicated that pain influences fatigue. sleep disturbance, and depression directly as well as indirectly. Sleep disturbance influences fatigue and depression directly, also fatigue influences depression directly. But sleep disturbance had no direct effect on depression. Conclusion: Significant correlations among the four symptoms supported the existence of the symptom cluster. Effects of pain on fatigue was partially mediated by sleep disturbance, also effects of pain on depression was partially mediated by fatigue. However, the effects of sleep disturbance on depression was fully mediated by fatigue.

Comparison of Differential Analgesic and Adverse Effects of 0.125% and 0.15% Ropivacaine Coadministrated Epidurally with Fentanyl (경막외강으로 Fentanyl을 첨가한 0.125% 및 0.15% Ropivacaine 투여시 술후 진통 효과 및 부작용의 차이 비교)

  • Background: This purpose of this study is to compare the analgesic efficacy, occurrence of motor block and other side effects of 48 hr continuous postoperative epidural infusions with 0.125% ropivacaine with fentanyl or 0.15% ropivacaine with fentanyl. Methods: Forty patients undergoing Cesarean section were randomly allocated into two groups. Both groups received epidural injection of 2.0% lidocaine 19 ml and 0.75% ropivacaine 2-5 ml with fentanyl $50{\mu}g$ for 20 minutes before surgery. Following surgery, a continuous epidural infusion using a two- day infusor was given for 40 minutes for post-operative pain control. Group 1 (n = 20) then received 0.125% ropivacaine and 6 ug/ml of fentanyl at a rate of 2 ml/h. Group 2 (n = 20) received 0.15% ropivacaine and 6 ug/ml of fentanyl at the same rate. Visual analog scales (VAS) for pain during rest and movement, sensory change and motor blockade were assessed for 48 hr after surgery. Results: There were no significant differences in VAS for pain during rest or movement. The incidence of side effects was similar in both groups. Conclusions: Using 0.125% ropivacaine with fentanyl via an epidural route provided similar pain relief and side effects as 0.15% ropivacaine with fentanyl.

The Effect of Progressive Muscle Relaxation Training on Pain, Oswestry Disability Index, and Psychological Levels in Patients with Chronic Low Back Pain (신체화를 동반한 만성요통환자에서 점진적 근육이완 훈련이 통증과 요통기능장애지수, 심리수준에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of Progressive muscle relaxation training on pain, Korean version of Oswestry disability index (ODI) and psychological level in chronic low back pain patients with somatization. Methods: A total of 30 subjects were treated with the experimental group (n=15) and conservative physical therapy (n=15). The experimental group was trained with progressive muscle relaxation (PMR), and the control group was treated with conservative physical therapy. Physical factor treatment was applied for 60 minutes by hot pack, electrotherapy and ultrasound. Both groups performed three times a week for six weeks. VAS, ODI, psychological level measurements were taken before and after intervention. Results: There was a significant difference in VAS (p<.05) and ODI (p<.01) between experimental and control group. At the psychological level, there were significant differences in somatization (p<0.01) and depression scales (p<.01), but not in anxiety. Conclusion: As a result of this study, the degree of pain was decreased, the level of back pain dysfunction was improved, and the somatization scale and depression scale were decreased by gradual muscle relaxation therapy.

Change Patterns on Subjective Distress Level in Gynecological Postoperative Patients with Different Levels of State Anxiety (불안수준에 따른 부인과 수술환자의 불편감지각의 변화 - 통증 자가 조절기 사용 중인 환자를 중심으로 -)

  • Purpose: A descriptive study was conducted to identify the change patterns on the level of perceived distress during the early postoperative period with regard to state anxiety in patients using patient controlled analgesia. Method: One hundred women who underwent elective hysterectomy procedures or other gynecologic surgeries completed a series of questionnaires measuring state anxiety, and subjective distress assessed by visual analog scales at 8, 24, & 48 hours postoperatively. Data were analyzed with frequencies, percentages, means, ANOVA, Repeated Measures ANCOVA, and Scheff'e post test utilizing SPSS WIN 11.0. Result: There was a gradual decrease in levels of total distress and pain over the three points in time after surgery regardless the levels of state anxiety. However, women with higher levels of anxiety their pain curve rose at 48 hours post-op. In addition, over the first two points in time, women in the higher anxiety group showed higher levels of distress than those in the lower anxiety group, but no such group differences were observed in the levels of pain, revealing higher pain levels only at 8 hours post-op in both groups. Conclusion: Irrespective of effective pain management modality, most postoperatively experienced distress in gynecological patients was derived from anxiety and pain. These findings have implications for pain management, especially in patients with emotionally charged surgeries like hysterectomy.

Effect of local anesthesia on postoperative pain and hemostasis after dental rehabilitation under general anesthesia in pediatric patients: a randomized control trial

  • Background: This study aimed to investigate the effect of local anesthesia (LA) on postoperative pain and hemostasis after dental rehabilitation under general anesthesia (DRGA) in pediatric patients. Methods: A total of 43 patients, aged 3-7 years and rated ASA I or II, who had a definitely negative rating on Frankel's behavior rating scale, were included in this two-arm, parallel-design, single-blinded, randomized, controlled study. The patients were allocated equally into two main groups receiving both restorative treatments and tooth extractions. Two pain scales and one bleeding scale were used. In Group A, the treatment was done with LA, and in Group B, the treatment was done without LA. Results: The statistical analysis revealed no significant differences in the pain scores between the groups. It also revealed significant differences in the bleeding scores between the groups but no significant differences in the duration of bleeding. Conclusion: Within the limitations of this study, the use of LA in pediatric dental patients undergoing DRGA had no effects on postoperative pain reduction or bleeding duration after teeth extraction. We also observed that the use of LA had an impact on the reduction in the bleeding scores in pediatric dental patients undergoing DRGA.

Prediction of Pain Expression Using the Extended Gate Control Theory of Pain and Fishbein′s Model (관문통제동통이론과 FISHBEIN의 모델을 이용한 동통표현 예견에 대한 연구)

  • The purposes of this study were to(a) develop theoretical modifications of the extended gate control theory of pain using Fishbein's model and(b) test the efficacy of these modifications. Attitude, social subjective norm, personal subjective norm, habit and state anxiety were operationalized to represent internal stimuli for the cognitive-evaluative and motivational-affective dimensions of the theory. Pain expression was operationalized as sensory and affective responses to pain, and pain endurance. Sixty-two female nurses from 20 to 50 years of age participated. A semantic differential scale measured attitude and motivations to comply; a Likerty-type scale measured personal and social norms and habit. Spielberger's STAI measured state anxiety, Pain was produced using a modified submaximum effort tourniquet technique. Pair expression was measured using ratio scales of sensory intensity and unpleasantness developed by Gracely and his associates. Pain endurance was measured by subtracting time of pain threshold from pain tolerance. The first hypothesis examining whether pain endurance would be more significantly related to the affective response than to the sensory response was net rejected. Four remaining hypotheses, testing the ability of the five variables to predict the sensory and affective responses were not rejected. However, the habit of pain expression and the attitude toward pain expression contributed to the prediction of both sensory and affective responses to pain. The interaction between the cognitive-evaluative and the sensory-discriminative dimensions and the interaction between the cognitive-evaluative and motivational-affective dimensions were partially supported by the data from these two variables. The interaction between the motivational-affective and the sensory-discriminative dimensions was also supported by the relationship of sensory to affective responses. The variables which did not significantly predict pain expression appeared to have potential for prediction. Revision and testing of the tools for better reliability, validity, and clinical usuability are needed. The study contributed to theory building. The identification of variables which pre-dict pain behavior must occur before effective nursing interventions can be developed.

A Study of Relationship between Halitosis and Salivary Gland Function (타액선 기능이 구취에 미치는 영향에 관한 연구)

  • The purpose of this study is reveal the relationship between halitosis and personal profile. The personal characteristics of halitosis patient were studied by symptom checklist-90- revision(SCL-90-R). 135 halitosis patients, 118 TMD patients(control torup 1) and 90 normal persons(control group 2) were selected for this study at the halitosis clinic or the Department of Oral Diagnosis and Oral Medicine, Kyung Hee Dental Hospital. T-scores were analyzed with Korean manual of SCL-90-R. A t-test was used to evaluate the overall group effect and the values were significant at the 0.05 level. The obstained results were as follows : 1. Mean value of T-scores on 9 basic scales of SCL-90-R in all groups(Halitosis patients, TMD patients and normal) was within normal range. 2. When compared with normal group, TamD patients gorup were significantly higher in sclaes of Somatization (p<0.01), Obsession-Compulsion(p<0.05), Depresison(p<0.05), Anxiety(p<0.01), Hostility(p<0.05). 3. When compared with normal group, halitosis patients group were significantly higher in scales of Somatization(p<0.05), Obsessoin-Compulsion(p<0.05), Interpersonal Sensitivity (p<0.05), Psyhcoticism(p<0.05). This data suggests that psychopathologic trends may be associated with halitosis and can be estimated by SCL-90-R questionnaire.
